Brucellosis is a zoonosis with worldwide distribution.There is an upward trend in the incidence of this disease in recent years,and multiple systems and organs are affected.Complication of bone and joint is the most commonly seen of Brucellosis.Although cardiovascular complication is rare,it is the major reason for Brucellosis-related mortality.The author summarized domestic and foreign studies on Brucella endocarditis.

Objective To investigate the changes in serum leptin and adiponectin levels and their significance in patients with chronic hepa-titis B virus (HBV)infection.Methods One hundred and twenty -five patients with chronic HBV infection (44 HBeAg -positive cases, 31 HBeAg -negative cases of chronic hepatitis B,23 chronic HBV carriers,and 27 inactive HBsAg carriers)were recruited in the present study.Fifty -five healthy people served as the control group.Blood samples were collected at 8 to 9 AMof the next morning after overnight fasting.Blood glucose,blood lipids,renal and liver function parameters,fasting insulin,serum HBV markers,and HBV DNA concentration were determined,and HOMA -IR was calculated.Serum levels of leptin and adiponectin were measured by ELISA.Continuous data were expressed as mean ±SD,and Student′s t -test,analysis of variance,linear regression analysis,and multiple regression analysis were ap-plied in the analysis of all experimental data.Results There were no significant differences in serum levels of leptin and adiponectin be-tween patients with chronic HBV infection and the control group (P =0.480;P =0.321),even after stratification by gender (males:P =0.210 /0.895;females:P =0.404 /0.066).After patients with chronic HBV infection were divided into subgroups by HBeAg status,HBV DNA,or ALT level,there were no significant differences in serum levels of leptin and adiponectin between groups (leptin:P =0.820 /0.296 /0.212;adiponectin:P =0.268 /0.760 /0.224).Conclusion For patients with chronic HBV infection,infection status and viral replication level are not significantly correlated with serum levels of leptin and adiponectin.

Objective To understand the status of the improper avademic behavior in medical journal,and to explore the prevention measures.Methods Total of 1 321 article in Zhongguo Yishi Jinxiu Zazhi were included in this study.Similarity detection system of Wanfang data and Google and baidu search engine were used to duplicate checking.The suspected existence of contributions for the improper avademic behavior,to check the whole paper of relevant original manuscript,and analyzed the characteristics of improper avademic behavior.Results One hundred and forty-five improper avademic behavior were found in 1 321 articles,accounted for 11.0％.The similarity of 58 paper were higer than 50.00％,accounted for 40.0％.Among them,the similarity of 10 papers were more than 90.00％,8 papers were 80.00％-89.99％,12 papers were 70.00％-79.99％,13 papers were 60.00％-69.99％,15 papers were 50.00％-59.99％,13 papers were 40.00％-49.99％,accounted for 9.0％,24 papers were 30.00％-39.99％,accounted for 16.6％,50 papers were 20.00％-29.99％,accounted for 34.5％.In 145 papers,copying accounted for 35.2％ (51/145),data forge or tamper accounted for 28.3％(41/145),breach of medical ethics and academic moral accounted for 28.3％ (41/145),fraud accounted for 6.9％ (10/145),a draft cast more accounted for 1.4％ (2/145).Original articles in the clinic were 92 papers,accounted for 63.4％;case reports were 28 papers,accounted for 19.3％;review were 25 papers,accounted for 17.2％.76 papers with a single author,accounted for 52.4％;38 papers with two authors,accounted for 26.2％,31 papers with more than three authors,accounted for 21.4％.Conclusions Contributions of suspected improper avademic behavior ratio is higher,and have certin characteristic.Medical journal editors should strengthen the responsibility consciousness,improve the ability to identify misconduct papers.

Objective To investigate the relationship of thyroid hormones and serum lipid profile during Pregnancy. Methods 30 cases of healthy pregnant women were recruited in the study. The thyrotropin (TSH), free triiodothyronine (FT3), free thyroxine (FT4) and serum lipid profile were examined at 9 ~ 12, 14 ~ 17, 23 ~ 26 and 37 ~ 40 weeks of gestation and the correlations between them were analyzed. Results Positive correlation could be found between serum TSH and total cholesterol (CHOL), triglyceride (TRIG), low density lipoprotein cholesterol (LDL-C), ApolipoproteinA-I (APOA-I), ApolipoproteinB (APOB). Negative correlations could be found between serum FT3, FT4 and total cholesterol (CHOL), triglyceride (TRIG), low density lipoprotein cholesterol (LDL-C), ApolipoproteinA-I (APOA-I), ApolipoproteinB (APOB). And no correlation was found between serum thyroid hormones and high density lipoprotein cholesterol (HDL-C). Conclusion The thyroid hormones were closely related to serum lipid profile except of HDL-C.

Objective] To explore the clinical experience of Professor Zhang Juan on Elderly Diabetic Constipation and hope to provide a new perspective for future study about Chinese medicine treatment of Diabetes Mellitus(DM).[Method] Reviewed 103 cases from Sep. 2014 till now who were older than 65,diagnosed with DM and had constipation as primary symptom. Analyzed and summarized the syndrome differentiation and treatment of the cases. [Results] According to the etiology and pathogenesis of Elderly Diabetic Constipation ,Professor Zhang deems that its pathogenesis takes place in two stages:one is spleen deficiency,the other is kidney deficiency and proposes the therapy of“treat the middle-jiao if others are not working”,“nourish the natal qi with post-natal qi”,“combine disease,syndrome and symptom”and“treat in stages”.[Conclusion] The treatment on Elderly Diabetic Constipation raised by Professor Zhang is effective and her clinical experience has important clinical value .

Objective To explore the impact of season and latitude on multiple sclerosis by study the onset/relapse season and latitude distribution in multiple sclerosis (MS) patients.Methods A total of 264 MS patients , with 88 males and 176 females, who were hospitalized in Beijing Tiantan Hospital from January 2002 to December 2012, were enrolled in the study and all the clinical data were retrospectively analyzed.The mean age was (33.9 ± 12.3) years old, with the disease duration of (6.3 ±4.5) years and 453 cases of relapse.The recurrence of MS was collected by four seasons, with March to May defined as spring, June to August as summer, September to November as autumn and December to February as winter.MS patients lived in Beijing (39.39° N-41.07° N) were chosen to test the correlation between the incidence/recurrence and monthly mean temperature, sunlight exposure intensity and duration.All the patients were divided into the high latitude group and the low latitude group, taken the latitude median (40.22° N) of Beijing area as the boundary.Gender composition, age of onset, disease duration and recurrence rate were compared between the two groups.Results Most of the onset/ relapse of MS were observed in winter (134 cases), while summer (97 cases) took the least.In the same latitude region (Beijing area), the onset/ relapse of MS was negatively correlated to the mean temperature and sunlight exposure intensity (r =-0.699, P =0.006;r =-0.623, P =0.015).Recurrence was higher in the high latitude group than in the low latitude group [68.7％ (123/179) vs 63.0％ (51/81), P =0.000], while no significant difference was found in gender composition, age of onset and disease duration between the two groups.Conclusion The onset/recurrence of MS has obvious seasonal characteristics.The onset/recurrence of MS is correlated with latitude, temperature and sunlight exposure intensity of the habitation of MS patients.Environmental factors are important cause of the onset/recurrence of MS, with sunshine exposure as the most key factor.

Objective:To study the metabolism of high density lipoprotein cholesterol ( HDL-C ) and apolipoprotein A-Ⅰ( ApoA-Ⅰ) in different thyroid function status during pregnancy. Methods:This study re-cruited thirty cases of euthyroid, with nineteen cases of subclinical hypothyroid and eight cases of subclini-cal hyperthyroid pregnancy. The concentrations of fasting serum HDL-C and ApoA-Ⅰwere detected and ana-lyzed from 9-12, 14-17, 23-26, and 37-40 gestational weeks. Friedman repeated measures ANOVA on ranks was adopted to analyze the changes of serum HDL-C and ApoA-Ⅰat different stages. General line-ar model ( GLM) was adopted to analyze the differences of serum HDL-C and ApoA-Ⅰin different thyroid function status during pregnancy. Results:There were no significant differences of maternal serum HDL-C among different stages (χ2 =5. 428,P=0. 143,χ2 =2. 027,P=0. 567,χ2 =2. 885,P=0. 410). There were significant differences of serum ApoA-Ⅰduring euthyroid and subclinical hypothyroid pregnancies (χ2 =46. 343, P<0. 001,χ2 =35. 984, P<0. 001), and no significant difference during subclinical hyperthy-roid pregnancy (χ2 =6. 750, P=0. 080). There were significant differences of serum HDL-C and ApoA-Ⅰbetween euthyroid and subclinical hyperthyroid pregnancies (P=0. 025,P=0. 027), and no significant differences between euthyroid and subclinical hypothyroid pregnancies (P=0. 378,P=0. 549). Conclu-sion:Subclinical hyperthyroidism affected the metabolism of maternal serum HDL-C and ApoA-Ⅰ, which could affect the fetal growth and development. Subclinical hypothyroidism ( after treatment with drugs) had no obvious effect on the metabolism of maternal serum HDL-C and ApoA-Ⅰ.

Objective:To investigate the inhibitory effect and its possible molecular mechanisms of MicroRNA-34a(miR-34a) on the human nasopharyngeal carcinoma CNE-2 cell line subcutaneous xenograft tumor in nude mice.Methods: The human nasopharyngeal carcinoma CNE-2 cell line was cultured in vitro.miR-34a and Scrambled miRNA recombinant plasmids were successfully established and stably transfected into CNE-2 cells.Fifteen six-week-old male nude mice were divided randomly into three groups:miR-34a group(5 mice) ,Scrambled miRNA group(5 mice) ,Blank control group(5 mice).Different CNE-2 cells were subcuta-neously injected on the back near right lower limb.Tumor volumes were examined every 7 days.Mice were executed on the 35 days,and the eventual average tumor volumes and weights were examined.Total RNA and protein were isolated from tumors,and the expression of miR-34a,CDK6,and Bcl-2 mRNA and protein were determined by qRT-PCR and western blot,respectively.Results: The relative expressions of miR-34a was significantly up-regulated in miR-34a transfected group compared to Scrambled miRNA transfected group (P<0.05).Eight subcutaneous xenograft tumor models were successfully established.The nude mice of miR-34a transfected group appeared a smaller tumor volume compared to the other two groups at the beginning of 21th day( P<0.05).The eventual average tumor volumes in miR-34a group,Scrambled miRNA group and blank control group were(351.37±98.19)mm3,(798.75±91.04)mm3 and
(849.62±101.32) mm3 ,respectively,and the eventual average tumor weights in miR-34a group,Scrambled miRNA group and blank control group were(0.81±0.13)g,(1.47±0.21)g and(1.58±0.37)g,respectively.Both the eventual average tumor volumes and weights in miR-34a group were lower compared to the other two groups(P<0.05).qRT-PCR results revealed that the expression of miR-34a in miR-34a transfected group was significantly higher than in the other two groups,while the mRNA and protein expression of CDK6 and Bcl-2 were lower than the other two groups ( P<0.05 ) .Conclusion: miR-34a may inhibit the growth of human nasopharyngeal carcinoma CNE-2 cell line subcutaneous xenograft tumor in nude mice by down-regulating CDK6 and Bcl-2.

Lp-PLA2 is widely concerned in recent years as a vascular inflammation factor that plays an important role in the development of atherosclerosis .The level of plasma Lp-PLA2 is related to the stability of carotid artery atherosclerotic plaque .it is an independent predictor of coronary heart disease risk . Its specific inhibitor Darapladib also becomes a hotspot of clinical cardiovascular pharmaceutical research . However,two randomized, placebo-controlled, double-blind, international, multicenter, event-driven trials, STABILITY and SOLID-TIMI 52, have shown that Darapladib could not significantly reduce the risk of cardiovascular events.

Five kinds of manipulations of plucking, circular rubbing, pecking, pinching and patting were performed on the channels of Governor Vessel and bilateral Bladder Meridian. According to syndrome differentiation, all the 32 cases were divided into 5 syndromes of the stagnated liver-qi causing fire syndrome, the phlegm-heat attacking upper parts syndrome, the hyperactivity of fire due to yindeficiency syndrome, the deficiency of both heart and spleen syndrome and the timidity due to insufficiency of qi and deficiency of blood of the heart syndrome and were then treated by different manipulations. After 2 courses of treatment, 25 cases were cured, 7 cases got marked effectiveness,no case failed and the effective rate was 100%.
