3.The application of reengineering of the dispense process of discharge medications for elderly cardiac patients

Objective The study aimed to improve the medication safety of aged patients through the reengineering of the dispense process of discharge medications for elderly cardiac patients.Methods The original dispense process was analyzed to find the existing problems,changing the order of process,increasing or decomposing steps of process in order to redesign,perfect and implement the distribution process of medications.Time and correction rate of drug dispensing for three times a day,mastering degree of related knowledge of oral medication and satisfaction degree with medication instruction were compared before and after reengineering of the process.Results Compared with the original process,patients' ability to understand the drug knowledge,the timing when the patients take three medications per day and the satisfaction degree with the instructions of taking medications all had statistical significance.Conclusions Reengineering of the dispense process of discharge medications for elderly cardiac patients can help to improve patients' understanding of drug knowledge,improve the security of taking medications outside the hospital as well as their satisfaction degree with nursing.
4.Construction of real-time network teaching platform in anesthesia resident standardization training

This research is to explore the feasibility of construction of real-time network teach-ing platform in anesthesia resident standardization training. The real-time network teaching were car-ried out by platformconstruction and teaching organization. The teaching effect was evaluated. Using the real-time network teaching platform with the four functions of learning knowledge, cultivating ability, communication and supervision can achieve good teaching effect. It was a beneficial supplement of traditional teaching.
5.Control analysis of fusion images of dynamic contrast-enhanced MRI with transrectal ultrasound and conventional transcrectal ultrasound in prostate biopsy for prostate cancer

Objective To evaluation the value of fusion images of dynamic contrast enhanced MRI (DCE MRI) with transrectal ultrasound(TRUS) and conventional TRUS in prostate biopsy for prostate cancer.Methods The clinical data of 127 patients suspected as early-stage prostate cancer were retrospectively analyzed.79 patients underwent prostate biopsy guided by conventional TRUS(group A).48 patients underwent prostate biopsy guided by fusion images of DCE MRI with TRUS(group B).Then the diagnostic rates,puncture times and complications in the two groups were compared.Results The cases with confirmed prostate cancer were 31 in group A and 22 in group B.The first time diagnostic rates of biopsy were 20.2%(16/79) in group A,and 39.3% (19/48) in group B respectively(P <0.05).The first time sensivity,specifity,positive predictive value and negative predictive value of biopsy were 51.6 %,100 %,100%,77.4% in group A and 86.3%,100%,100%,89.6% in group B.Puncture times was 15.40 ± 4.67 and 13.01 ± 3.87 respectively.The Gleason score was 6.21 ± 0.91 and 6.35 ± 0.81 respectively.The complication rates were 19.0%(15/79) in group A and 12.5%(6/48)in group B.Conclusions For patients with earlystage prostate cancer,the combination of DCE-MRI and TRUS to guide transrectal prostate biopsy was recommended.It showed an increase in the first time diagnostic rate of biopsy while a reduction in the times and complications of prostated biopsy.
6.X-ray Features of Heart and Lung in Chronic Renal Failure

Objective To improve the knowledge of X-ray findings of heart and lung in chronic renal failure ( CRF ) . Methods X-ray findings of heart and lung in 120 cases with CRF were analysed retrospectively.Results The X-ray appearances of lung and heart in CRF included augment and blur of hilum, increased, tortuous and disorder lung markings, accretion of heart, and the abnormal shadows in lung were irregular, inhomogeneus and changing fast in morphology. Conclusion Radiography can supply valuable messages and is ofconsiderable value in the diagnosis of CRF.
7.Research on the relationship between field cognition style and self-handicapping of medical students

Objective To explore the relationship between field cognitive style and self-handicapping of medical students.Methods 124 medical students were measured by embedded figure test (EFT) and self-handicapping questionnaire (SHS).Results ①The scores of self-handicapping questionnaire were significantly different in whether they like their major or not(33.61±5.21,35.94±5.16,t=-0.218,P=0.031).Differences were also found in whether they could obtain the scholarships (33.20±6.19,35.12±4.12,t=-0.247,P=0.043),and in whether they were in a relationship(35.35±5.48,33.39±5.01,t=2.063,P=0.041).②Compared with field-independent cognitive style students,Field-dependent cognitive style students ' self-handicapping score was higher (35.69±3.91,32.63±6.08,t=3.303,P=0.001).③Field-dependent cognitive style,dislike their major and be in a relationship were the significant predictors of self-handicapping.Conclusion Correlation exists between field cognitive style and self-handicapping of the medical students.Students who belong to field-dependent cognitive style are more prone to self-handicapping than field-independent cognitive style students.
8.Laparoscopic adrenalectomy:a report of 21 cases

Objective To investigate the clinical application of laparoscopic adrenalectomy and summarize the experience in laparoscopie adrenalectomy.Methods From August 2002 to March 2007,21 cases of benign adrenal tumors were treated with retroperitoneal laparoscopy in this hospital.There were 9 cases of adrenocortical adenoma, 7 cases of primary aldosteronism,3 cases of adenocorticol macronodular hyperplasia,1 case of pheochromccytoma,and 1 case of adrenal gangliocytoma.Results Retroperitoneal laparoscopy was successfully applied in 21 cases.Operating time was between 65 and 130min with an average of 95 min.All patients did not receive blood transfusion and had no obvious complications.Conclusion Laparoscopic adrenalectomy had the advantages of minimal morbidity,mini- mal postoperative discomfort and a short hospital stay,whieh had a good prospect for application in the clinical prac- tice.
9.Effectiveness of the Spot Vision Screener on vision screening of children without cycloplegia

AlM: To evaluate the effectiveness of Spot Vision Screener on vision screening of children without cycloplegia.METHODS:A total of 87 children (174 eyes) aged from 2~9 years old were examined with Spot Vision Screener and optometrist before cycloplegia.RESULTS: Statistical analysis demonstrated that the cylinder diopter and axis, the equivalent spherical diopter in both eyes, and the spherical diaopter in left eye had no significant change ( P>0. 05 ). However, the spherical diaopter in right eye had statistical significance. CONCLUSlON: Spot Vision Screener is a suitable instrument in vision screening of children without cycloplegia.
10.The contain of IL-1β in rat brain after subarachnoid hemorrhage and its relation to secondary brain edema

Objective To investigate the changes of interleukin-1β (IL-1β) in rat brain after subarachnoid hemorhage (SAH) and its relation to cerebral edema and vasospasm.Methods Subarachnoid hemorrhage rat model was produced by anterior cerebral artery perforation method.The brain water content was measured by dry-wet weight method.Hematoxylin-eosin (HE) staining was used to assess vasospasm in the basilar artery,and the expression of IL-1β after SAH was examined by Western blot.Results After SAH,the cerebral edema gradually occured from 1 d,reached the peak at 7d.The level of IL-1β protein started to increase in 1 d,reached its peak in 7d,and remained at a relative high level in 14d.There's correlation between the level of IL-1β protein and changes of cerebral edema.Conclusion After SAH,The level of IL-1β gradually rised and fits remarkably well with the time course of cerebral edema,which may indicate that IL-1β leads to the development of cerebral edema after SAH.
