1.Oncology Nurses' Knowledge of Safety Guidelines and Compliance with Safe Handling of Antineoplastic Agents in a Tertiary Hospitall.

PURPOSE: The purpose of this study was to identify the knowledge and compliance of guidelines for safe handling of antineoplastic agents among the oncology nurses. METHODS: The participants were 186 nurses from oncology-hematology unit, and outpatient's infusion room in a tertiary hospital. The levels of knowledge of safety guidelines and compliance with safe handling of antineoplastic agents of oncology nurses were measured using structured questionnaires from October 1 to November 30, 2014. RESULTS: Oncology nurses' knowledge of guideline for safe handling of antineoplastic agents presented an average 79.7 out of 100 points. Compliance with safe handling of antineoplastic agents presented an average 4.28 (range: 2.9~5) out of 5 points. The knowledge of guidelines for safe handling of antineoplastic agents was related with oncology experience (F=3.78, p=.012). There were positive relationships of knowledge and compliance with safe handling of antineoplastic agents (r=0.23, p=.002). CONCLUSION: The findings showed that there was a need to establish efficient guidelines for the safe handling of antineoplastic agents and develop our structured, continuous educational program for the improvement of safe practices related to the handling of antineoplastic agents. The developed and enhanced that guideline and educational program would contribute to improvement of staff and patient safety related to the handling of antineoplastic agents.

2.Expression of c-Met Is Different along the Location and Associated with Lymph Node Metastasis of Head and Neck Carcinoma.

BACKGROUND: Activation of the c-Met pathway is involved in cancer progression and the prognosis. We aimed to identify any association of c-Met protein expression with a number of clinicopathologic variables including infection of human papillomavirus and Epstein-Barr virus (EBV) in head and neck carcinomas (HNCa). METHODS: Eighty-two cases were enrolled in this study. Expression of c-Met and p16 was investigated immunohistochemically. EBV was detected by in situ hybridization and amplification of the c-Met gene by fluorescence in situ hybridization. RESULTS: The c-Met protein was expressed in 41.5% (34/82), and gene amplification was found in 1.4% (1/71). High expression of c-Met was associated with the primary location of the tumor; the hypopharynx showed the highest expression, followed by the oral cavity, larynx, and nasal cavity. Squamous cell carcinoma expressed c-Met more frequently than undifferentiated carcinoma. Also, p16 immunoreactivity or EBV infection was associated with the tumor location and well-differentiated histologic type, but were not linked to c-Met expression. The patients with positive c-Met expression showed frequent lymph node metastasis. CONCLUSIONS: Activation of the c-Met pathway might be involved in a subset of HNCa. Cases showing positive c-Met expression should be carefully monitored because of the high probability of lymph node metastasis.

3.A Case of Tufted Angioma Showing a Good Response to Interferon-alpha.

A 13-month-old boy had irregularly shaped, erythematous to dusky red, slightly raised patches and plaques on the left antecubital fossa and arm for 7 months. The lesions were firm and tender. Histopathologically the lesion consisted of small, circumscribed, angiomatous tufts and lobules with a cannonball appearance scattered throughout dermis, a characteristic feature of tufted angioma. For 15 weeks, interferon-a intralesional injections have been performed and the lesions showed an improvement.

4.The Effect of Atropine on Myopic Progression in Children.

PURPOSE: To examine the effects of atropine therapy on myopic progression in myopic children. METHODS: Fifty myopic patients, aged from 7 to 14 years and less than -5 diopters, visited our clinic for correction of refractive error from August 2001, and were followed up for at least 1 year. Of these 50 patients, the atropine therapy group was 23 and the control group was 27. Right eye cycloplegic refraction and axial length changes of the two groups were compared and analyzed. RESULTS: There was a mean increase of myopia of 0.25+/-0.43 D/yr in the atropine therapy group and of 0.83+/-0.37 D/yr in the control group (P=0.000). There was a mean increase of axial length of 0.12+/-0.19 mm/yr in the atropine therapy group and 0.39+/-0.21 mm/yr in the control group (P=0.000). In refraction and axial length changes according to age, there was no statistically significant difference within the group, aged from 7 to 9 years. However, there was a statistically significant difference within the group, aged from 10 to 14 years. In refraction and axial length changes according to myopic grade, there were a statistically significant differences between the groups. CONCLUSIONS: Atropine therapy is effective in slowing the myopic progression in myopic children with refractive error less than -5 diopters.

6.Infantile Hemangioendothelioma of the Liver: Brief case report.

Infantile hemangioendothelioma of the liver is a common vascular tumor in infancy. The tumor is usually multinodular or diffuse and classified into two types. We present a case of infantile hemangioendothelioma of the liver, which predominantly consists of type 2. A 4-month-old female was admitted for an evaulation of an abdominal distension. A CT scan of the liver showed a multinodular mass. The right lobectomy was done. Grossly, the mass consisted of round nodules ranging from 2cm to 5cm in diameter. Microscopically, the tumor revealed proliferation of small vascular channels lined by endothelial cells. Bizarre cells and mitotic cells were frequently noted. Vesicular nuclei and multilayering of the endothelial cells were also noted.

7.Knowledge, Anxiety, and Compliance with Preventive Behavior of Middle East Respiratory Syndrome (MERS) in Liver Transplant Patients.

BACKGROUND: This study was conducted to investigate the knowledge, anxiety and compliance with preventive behavior of Middle East respiratory syndrome (MERS) in liver transplant patients. METHODS: The participants in the study were 100 liver transplant patients at an outpatient clinic. The data were collected from May 20, 2016, to June 20, 2016. RESULTS: The knowledge score was 6.37±1.73 (range; 1~10) and the correct answer rate was 63.7%. The state anxiety score was 41.35±10.08 (range; 20~68) and the trait anxiety score was 39.60±8.85 (range; 22~59). The compliance with preventive behavior score was 35.70±5.23 (range; 21~45). Anxiety differed significantly according to education level (P=0.040) and occupation status (P=0.047). Compliance with preventive behavior differed significantly according to period after surgery (P=0.035). Compliance with preventive behavior and state anxiety (P=0.007) and trait anxiety (P<0.001) were negatively related. The factor that had the greatest effect on compliance with preventive behavior was trait anxiety (P=0.003). CONCLUSIONS: The epidemic of the MERS has already been completed and none of the subjects of this study has been infected with the MERS; however, patients with liver transplantation always have a risk of various opportunistic infections. If new infectious diseases such as MERS occur in the future, increased anxiety may decrease the practice of prevention. Therefore, anxiety experienced by liver transplant patients should be managed.

8.Knowledge and Educational Need of Central Supply Room Nurses for the Sterilization Process.

BACKGROUND: This study aimed to assess central supply room nurses' knowledge on and educational needs for the sterilization process. METHODS: The participants were 88 nurses working at central supply rooms of hospitals in Busan, Ulsan, and Gyeong-nam. From August to September 2014, knowledge and educational needs of the sterilization process were measured by using self-reported questionnaires. Data were analyzed by descriptive statistics, t test or analysis of variance (ANOVA), Scheffe test, and repeated-measures ANOVA with the SPSS 21.0 program. RESULTS: The mean score for knowledge on the sterilization process was 39.58 out of 50 points and was highest in keeping sterilization (8.94 out of 10 points) and lowest in packing sterilization (6.82 out of 10 points). The mean score for knowledge was significantly higher in the nurses with postgraduate studies (F=3.35, P=0.040), in those with internal training (t=2.03, P=0.045), with three or more internal trainings (F=9.72, P<0.001), with external training (t=3.48, P=.001), with two or more external trainings (F=8.40, P=0.001), and in those working in hospitals with 700 beds or more (F=15.88, P<0.001). The mean score for educational need was 4.06 out of 5 points, ranging from 3.99 to 4.12 points. It was significantly higher in nurses aged 40 years and older (F=4.28, P=0.017) and in those working in hospitals with 700 beds or more (F=4.82, P=0.010). CONCLUSION: Nurses working in central supply rooms need education on the sterilization process. We recommend the development of a standardized textbook and an educational program based on the results of this study.

10.Ischemic stroke as an initial presentation of primary bone marrow lymphoma

Various cancer types have been associated with cancer-related cerebral infarction. In this study, we
describe the first case of cancer-related cerebral infarction in which the underlying disease was primary
bone marrow lymphoma (PBML). A 79-year-old man presented with abruptly developed bilateral lower
extremity weakness and confusion. Diffusion-weighted imaging on admission showed multiple cortical
and subcortical embolic infarction lesions in multiple vascular territories. Diagnostic evaluations to
determine the embolic source revealed no abnormalities. Laboratory testing demonstrated elevated
D-dimer (2.59 μg/mL) but no other prothrombotic abnormalities. In suspicion of cancer-related stroke,
we performed chest CT, abdomen CT, and FDG-PET to detect the hidden malignancy. Findings
revealed no evidence of cancer; however, they did reveal signs of anemia (hemoglobin 9.0 g/dL).
Bone marrow aspiration biopsy showed large atypical B cell involvement suggestive of high-grade B
cell lymphoma. The patient was diagnosed with primary bone marrow diffuse large B-cell lymphoma
initially presenting with ischemic stroke. Our case suggests that primary bone marrow cancer may be a
candidate for the differential diagnosis of hidden malignancy in patients with suspected cancer-related
stroke. Bone marrow biopsy may be essential for establishing an appropriate differential diagnosis in
patients with abnormal hematologic findings.
