Phototherapy is the most common and effective method to intervene and treat neonatal hyperbilirubinemia in clinic.The application of phototherapy varies from gestational age,birth weight,age and many other factors.Beside the traditional phototherapy instruments,more advanced high-intensive phototherapy instruments are applied to phototherapy.With the increasing application of phototherapy,many problems have showed up.This paper reviews based on the research of the indication,clinical application and adverse reactions on phototherapy.
2.Effect of thymopeptide on LPS-induced release of nitric oxide from mouse peritoneal microphase in vitro

Purpose The aim is to investigate the effects of thymopeptide on LPS-induced release of nitric oxide (NO) from mouse peritoneal microphase in vitro.Methods NO was detected by Griess reagent.Results Thymopeptide 83.3~333.3 μg/ml reduced NO synthesis and secretion from mouse peritoneal microphase in vitro significantly.Conclusion The effects of thymopeptide on LPS-induced release of NO from mouse peritoneal microphase may be one of mechanisms of its pharmacological effect.
3.Preliminary cbservation on the safety of Sumianxin Ⅱ combined with Diazepam for anesthesia in the periodontal surgery in Beagle dogs

Objective:To observe the anesthetic effects of SumianxinⅡ combined with diazepam on the Beagle dogs in experimental periodontal surgery to Provide the reference. Methods:Healthy adult male Beagle dogs were anesthetized with SumianxinⅡat the doses of 0.05、0.1、0.15 ml/kg in the combination group and diazepam(0.1 ml) by hip intramuscular injection simultaniously. Added injection dosage was half of the first dosage. Healthy adult Beagle dogs in the contral group were only anesthetized with SumianxinⅡ at the doses of 0.05、0.1、0.15 ml/kg by hip intramuscular injection. The time of adding anesthetic was not sure in surgery. Anesthesia induction period,initial duration of anesthesia,and adverse reactions after anesthesia were compared between the two groups. Results: The combined treatment group induction dose respectively (11.00?1.41) min,(9.00?0.82) min,(12.75?0.50) min,For the first time maintaining anesthesia(29.75?6.62) min,(46.00?5.29) min,(45.25?6.02) min,Intraoperative additional one times longer cansustain anesthesia for(38.75?3.50) min,(45.25?4.03) min,(37.25?2.75) min. 4.FEASIBILITY OF SCREENING FOR ANTIBODY TO HIV BY POOLING THE SERUM SPECIMENS

The feasibility of screening for antibody to HIV by pooling 10 serum specimens was demonstrated in this experiment. The test was conducted using the reagent developed by Organon Teknika corporation (U. S. A) for testing for antibody to HIV by ELISA. The determination was made by changing only the ratio between the serum specimen volume and the serum specimen diluent volme, with their total volume being constant. When the serum specimen volume was increased from 5to 59?l as a result of pooling 10 serum specimens, while the serum specimen diluent volume was correspondingly decreased, the optical density of the pooled serum specimens was somewhat raised, but it was always below cutoff value, indicating that the specificity of test did not be subjected to any influence. In addition, when one serum specimen estabished as being positive intesting for antibody to HIV, and nine serum specimens from normal donors were pooled, the serum specimen containing the antibody to HIV had exactly been shown through multiple paired tests, indicating that the serumpooling method did not have any influence on the test sensitivity and could reach the level of original method. The serum-pooling method could be easy to spread because of its many advantages, such as saving the expenditure by 10 times, saving the manpower and material resource, and saving the working hours. 9.Meta analysis of BISAP score system in predicting the severity of acute pancreatitis

Objective To investigate the value of BISAP scoring system in predicting the severity of acute pancreatitis.Methods Medline,EMBASE,Science Direct,Springer link,CBM,Cnki,Wan fang and VIP database were retrieved by computer between January 2000 and March 2013,and articles of BISAP score system in predicting the acute pancreatitis were collected according to strict inclusion criteria.Quality assessment was made by QUADAS scale.Meta-Disc1.4 software was used to analyze the heterogeneity of included articles and perform quantitative synthesis,as well as calculate the pooled sensitivity and specificity,positive likelihood ratio,negative likelihood ratio,and draw the ROC curve,and the results were presented with 95% CI.Results A total of 11 articles were included,and there were 7 Chinese articles and 4 English articles.According to QUADAS scale,there were 4 articles of A grade,5 articles of B grade,2 articles of C grade.There were 6 studies using 2 as BISAP cut-off value,and 9 articles using 3 as cut-off value,while 4 studies using 2 cut-off values.Pooled analysis showed diagnostic odds ratio of 8.03 (95 % CI 5.66-11.38) when cut-off value was 2,and diagnostic odds ratio of 7.49 (95% CI 5.35-10.49) when cut-off value was 3.There were moderate heterogeneity in both groups (I2 =63.3 %,P =0.018; I2 =56.1%,P =0.019).When BISAP cut-off value was 2,the pooled analysis of sensitivity,specificity,positive likelihood ratio,negative likelihood ratio and area under curve (AUC) were 59% (95% CI56%-63%),82% (95% CI 80%-83%),3.50 (95% CI2.96-4.14),0.45 (95% CI0.36-0.56) and0.82; and when BISAP cut-off value was 3,the corresponding values were 44% (95% CI41%-47%),90% (95% CI 89%-91%),4.59 (95% CI 3.31-6.37),0.64 (95% CI 0.61-0.68) and 0.64.The former had a high sensitivity,low specificity and large AUC,while the latter had a low sensitivity,high specificity and small AUC.Conclusions The best cut-off value of BISAP in predicting SAP is 2.When cut-off value is 2,the misdiagnosis rate is low,and the false positive rate is in the acceptable range,which is suitable for clinical application.
10.Clinical study of Dexamethasone in different time for treatment and prevention of bronchopulmonary dysplasia

Objective To study the effect of using Dexamethasone in different time for treatment and prevention of bronchopulmonary dysplasia (BPD), in order to provide the clinical reference as to whether dexamethasone can be used to prevent and treat BPD.Methods A total of 135 very low birth weight infants hospitalized for 1 week who still survived on mechanical ventilation were studied.The total cases were divided into early Dexamethasone group and none Dexamethasone group.Whether early using dexamethasone could prevent BPD or not were observed.The BPD infants were divided into 3 groups, early Dexamethasone group, later Dexamethasone group and none Dexamethasone group.The effect of using Dexamethasone on BPD in different time were observed.Results (1) Among these 135 infants,there were 65 infants developing BPD.The incidence of BPD was 48.15%.There were no significant differences in the prevalence of BPD, the death rate and the disease index of BPD between the early Dexamethasone group and the none Dexamethasone group (all P > 0.05).(2) There were no significant differences in mechanical ventilation time among the 3 BPD groups [(19.81 ± 5.67) d vs (22.32 ± 6.20) d, (21.46 ± 7.02) d, P =0.405].But the average time of inhaling oxygen and hospital stays of later Dexamethasone BPD group were shorter than early Dexamethasone BPD group and none Dexamethasone BPD group.The differences were significant [(37.27 ± 10.14) d vs (45.96 ± 9.91) d,(43.42 ±8.73) d,P=0.012;(64.11 ±8.14) d vs (76.13 ±7.57) d,(68.59 ±8.53) d,P =0.000].(3) The incidence of infection (90.90%, 100.00% vs 81.90%), hyperglycemia (27.27 %, 30.43 % vs 10.00 %), hypertension(9.09% ,13.04% vs 0) and neonatal necrotizing enterocolitis(22.73% ,34.78% vs 15.00%) of all the infants who received Dexamethasone were higher than the infants who did not receive Dexamethasone.But the differences showed no significance(all P > 0.05).(4) There were no significant differences among the 3 BPD groups in growth [weight : (9.16 ± 1.53) kg vs (8.92 ± 1.13) kg, (9.07 ± 1.46) kg;height: (71.26 ± 8.59) cm vs (69.54 ± 9.32) cm, (70.32 ± 9.07) cm] or been in hospital again because of pulmonary infection during one year(75.00% vs 65.00% ,72.22%) (all P > 0.05).The differences of abnormal Chinese children development scale scores among the BPD groups (mental development index < 80:20.00% vs 15.00%, 16.67 %;physical development index < 80:25.00% vs 20.00%, 16.67%) showed no significance(all P > 0.05).Conclusions Early use of Dexamethasone has no effect to prevent BPD.Dexamethasone therapy on BPD should be used in the later stages ,and small doses and short protocol are preferred.
