1.Children with urinary fistula after hypospadias: causes and repair (report of 33 cases)

Objective To study the causes of congenital hypospadias fistula and the repair methods,summarize surgical experience,and improve the success rate of fistula repair.Method s From January 2001 to February 2011,33 cases of urinary fistula after hypospadias urethroplasty were collected,the average age being 6.2 years (3 to 13 years).A total of 38 fistula were found,17 with fistula diameter of less than 3 mm,16 with fistula diameter of 3 - 10 mm,5 greater than 10 mm.Ligation embedding method were used,Y-V flap coverage method,a continuous inverted fistula suture,Thiersch method,Denis-Browne method and the U shaped flap covering method were used for multi-urinary fistula repair.Among the 33 cases,successful repair was found in 30 cases,with normal urination during flollow- up,good penile appearance,the success rate being 90.9%.Results The causes of hypospadias fistula after surgery in children are:vascularized free when properly drawn,resulting in poor blood supply to the flap; urine drainage was not smooth,resulting in premature suture wounds soaked in urine,causing infection; distal urethral stricture,high pressure within the urethra during urination,causing anastomotic dehiscence; suture materials of poor quality; technical immaturity.Conclusions The repair of urinary fistula after hypospadias in children has a variety of surgical procedures,choice of which should be based on fistula location,size,quantity and local conditions.

The cascade release of cytokine the critical aggravation factor for acute pancreatitis,induces the multi-ple organ dysfunction syndrome (MODS) and death at last. Nuclear factor-kappa B as a number of regulation of the nuclear factor can specific bind with a variety of gene promoter sites κB site, which cause inflammatory reaction, apoptosis, pathophysiologic reaction. The specific inhibitor of nuclear factor-kappa B can inhibit the activity of nu-clear factor-kappaB, inhibit the body's inflammatory response, and improve the prognosis of acute pancreatitis; it may be a potential target site for acute pancreatitis therapy. However, the other researches also found that inhibition of nuclear facter-κB could significantly increase the aggravation of acute pancreatitis. Accordingly, nuclear factor-kappa B research possess the important rank about the body's inflammatory respanse.
4.Neijing’s Viscera-State Theory of Spleen and Its Method of Thinking Construction

Objective] Calrify the inherent laws of the appearance of Neijing’s viscera-state theory of spleen, enhancing the cognition of life view of traditional Chinese medicine’s ability. [Methods]Using the genetic methods to discuss the physiological function, physiological characteristics and physiognomy of spleen, as well as social, cultural, philosophical background and method of thinking construction. [Results] Under the guidance of the overall concept and from the pre-Qin rank system, cultural thoughts of Confucianist and Taoists, Yin-Yang five elements philosophy doctrine,by using the analogism methods and observation methods of “ estimating interior by taking charge of exterior” and the opinion of middlebrow and harmony, construct the basic frame of viscera-state theory of spleen.[Conclusion] The genetic method of TCM is an important way to deepen the level of Chinese medicine theory.
5.Cloning of rat neuroglobulin gene and construction of its eukaryotic expression vector

Objective To clone rat neuroglobulin (NGB) gene and construct its eukaryotic expression vector. Methods The total RNA was extracted from Wistar rat brain and the full length cDNA encoding NGB was obtained by RT-PCR. After the sequence was confirmed by sequencing and BLAST, it was inserted in the eukaryotic expression vector pCDNA3.1 (+), then the sequence and reading frame were confirmed by two restriction endonucleases and sequencing. Results The NGB gene was cloned with four bases mutated and its eukaryotic expression vector was constructed. Conclusion NGB expressed in Wistar rat brain. NGB gene was successfully cloned and inserted in eukaryotic expression vector.
6.Protective role of recombinant plasmid pCDNA3.1(+)/Ngb during focal cerebral ischemia of rat brain

Objective To observe the protective effect of recombinant plasmid pCDNA3.1(+)/Ngb during focal cerebral ischemia in rat brain.Methods Fifty-four male Wistar rats were randomly divided into three groups:normal saline(NS)control group,plasmid control group,and recombinant neuroglobulin group.NS,plasmid pCDNA3.1(+)and recombinant plasmid pCDNA3.1(+)/Ngb were respectively injected into two sites of the rat cerebra1 cortex 24 hours before induction of neocortical focal ischemia by occlusion of the right middle cerebral artery for 24 hours.The condition of local ischemic damage,expression of bcl-2 and the apoptosis in neural cells were confirmed by staining with 2% 2.3.5-triphenyltetrazolium chloride,in-site cell apoptosis detection,indirect immunofluorescent staining and Western blotting,respectively.Results The extent of cerebral infarction tissue and the apoptosis cells in the pCDNA3.1(+)/Ngb group were significantly reduced than those in other control groups(P
7.Comparative analysis of endovascular interventional therapy and simple drug therapy in patients with ischemic cerebrovascular disease

Objective this research mainly analysis in the treatment of patients with ischemic cerebrovascular disease by comparison of clinical therapeutic effect of endovascular treatment and simple drug treatment. Methods 60 cases of patients with ischemic cerebrovascular disease admitted in our hospital from January 2016 to January 2017, and were randomly divided into control group and observation group, 30 cases in each group. The control group were treated by endovascular treatment, the observation group were treated with simple drug treatment, the two groups of patients treated for 6 months, 12 months to improve blood flow, the incidence of cerebrovascular events, mortality, prognosis Quality of life scores as a comparison basis.Results The patients in the observation group were treated for 6 months, 12 months after the blood flow improved superior to the patients in the control group (P<0.05); the patients in the observation group were treated for 6 months, 12 months of cerebrovascular disease incidence rate lower than that of the control group (P<0.05); the observation group after 6 months treatment and mortality of patients in the control group had no significant difference; the patients in the observation group after 12 months of treatment the mortality rate lower than that of the control group (P<0.05). The patients in the observation group survival quality scores were excellent in the control group (P<0.05). Conclusion The study proves that, in the treatment of patients with ischemic cerebrovascular disease in the treatment can effectively improve the treatment effect of endovascular intervention, improve the blood flow status of patients, reduce the events in patients with cerebrovascular disease incidence and mortality.
8.Evaluation of the Role of Multimedia-assisted Teaching in Stomatology Courses

In order to evaluate the effect of multimedia-assisted teaching on stomatology courses and find out the appropriate teaching methods for non-stomatology medical students,we investigated the distribution of examination scores,diagnostic accuracy of ordinary diseases and accuracy of problem-based learning(PBL) results of multimedia-assisted teaching and traditional teaching means.The examination scores were closer to standard distribution,and the diagnostic and PBL results accuracy increased among students with multimedia-assisted teaching.Compared with traditional teaching means,multimedia-assisted teaching may improve the teaching quality in non-stomatology medical courses.
10.Biological effect of antisense transforming growth factor beta 1 in inhibiting hyperplastic scar of rabbit's ears

BACKGROUND: Nowadays, it is thought that transforming growth factorβ (TGFβ) is closely related with cicatrization. TGFβ that is a key active molecule can affect each phase of cicatrization. Theoretically, to inhibit the biological effect of TGF β can reduce cicatrization.OBJECTIVE: To explore the inhibitive effect of antisense TGF β1 deoxy-oligonucleotide on generation of cicatricle in intention of animal models with hyperplastic scars and observe the effective route of administration of using antisense TGF β1.DESIGN: Own control and animal study.SETTING: Department of Plastic Surgery, Anning Hospital of General Hospital, Lanzhou Military Area Command of Chinese PLAMATERIALS: The experiment was performed at the laboratory of anatomy,Lanzhou Medical College from September 2002 to July 2003. Totally 20flap-eared Japanese rabbits were selected.METHODS: Blood vessels could be seen in ventral surface of each rabbits' ear getting out of the way along long axis to establish two 1.0 cm×2.5 cm oblong full-thickness cutaneous deficiency raw surfaces that interval for 1.5 cm, to the surface of cartilage, totally 80, so asto establish ventral surface of rabbits' ear models with hyperplastic scars. After epithelizatio of raw surfaces of rabbits' ear (20 days, averagely), 5μL(1 g/L) antisense TGF β1 deoxy-oligonucleotide was closely injected into local endepidermis of each raw surface of left ear of each rabbit with microinjector, which was regarded as TGF β1 group. 5 μL saline was injected into each raw surface of right ears, which was regarded as saline control group. After injection for3, 7, 11, 20, 30 days, cicatricial tissues were cut, 4 rabbits in each time phase. Hematoxylin-esoin (HE) staining, Masson staining and TGFβ1 mRNA, type Ⅰ and Ⅲ collagen mRNA in situ hybridization histochemistry staining were applied.MAIN OUTCOME MEASURES: Results of HE staining, Masson staining and in situ hybridization histochemistry staining.RESUTLS: A total of 20 animals were included in the result analysis. ①HE staining showed that inflammatory cell infiltration and significant infiltrative zone of leukocytes occurred in hyperplastic scars of right ears in each group. There was inflammatory cell infiltration, but no infiltrative zone of leukocytes in hyperplastic scars of left ears after intervention with antisense TGF β1. ②Masson staining suggested that collagen fibers with deep blue-stain occurred in hyperplastic scars of right ears from the 3rd week after injury, till the 7th week there still was blue-stain collagen fibers,which was bulky (width of about 8-10 μm) and arranged in a great mess.The blue-stain collagenous fibers also appeared in hyperplastic scars of left ears at the 3rd week after injury by the intervention of antisense TGF β1,but till the 6th and 7th weeks the blue-stain became light and thin (width of about 3-5 μm), arranged in order. ③In situ hybridization revealed that expressive rates of TGF β1 mRNA, type Ⅰ collagen mRNA, type Ⅲ collagen mRNA positive cells decreased obviously.CONCLUSION: Antisense TGF β1 can inhibit the proliferation of hyperplastic scars of rabbits' ears and lighten markedly the fibrosis of cicatricial tissue. The local injection with naked DNA is feasible in the treatment of cicatricle.
