1.Study on Pathogenesis of Tic Disorders Accompanied by Chronic Rhinitis Among 258 Cases

This study was aimed to explore the tic disorders ( TD ) pathogenesis induced by chronic rhinitis . A total of 400 TD children were all investigated with self-made TD general questionnaire , Yale Global Tic Severity Scale ( YGTSS ) , TD diagnostic criteria by DSM-IV , chronic rhinitis diagnostic criteria by Zhu-Fu-Tang Textbook of Pediatrics ( 7th edition ) . A total of 258 TD children accompanied with chronic rhinitis were screened . Levels of peripheral blood T lymphocyte subsets , immunoglobulins , IL-6 , TNF-α and other related immunological indexes were detected to analyze pathogenesis. The results showed that the nose movement symptom scores of TD children with rhinitis were higher than the score of TD children without rhinitis . The score of arm-lifting and hand movement in TD children with rhinitis was lower than the score in the TD children without rhinitis . And the YGTSS score of TD children with rhinitis was higher than TD children without rhinitis. Compared with TD children without rhinitis, the level of peripheral blood T lymphocyte subsets in TD children with rhinitis was obviously reduced ( P < 0 . 05 ) . Compared with TD children without rhinitis , the level of immunoglobulins in TD children with rhinitis was slightly reduced . The level of IL-6 in TD children with rhinitis tended to increase , but the level of TNF-α tended to reduce . It was concluded that chronic rhinitis is one of the factors to induce TD .
2.The clinical characteristics of the familial early onset type 2 diabetes mellitus

Objective To investigate the clinical characteristics of the familial early onset type 2 diabetes(T2DM),and the factors affecting the age at diagnosis. Methods 190 probands from early onset T2DM pedigrees (diagnosed before 40 years old) and 103 probands from late onset T2DM (pedigrees) (all patients were diagnosed after 40 years old)were included in this study. The homeostasis model assessment of insulin resistance (HOMA-IR)was used to estimate insulin resistance, HOMA-β cell was used to evaluate basal insulin secretion. The ratio of the incremental insulin (ΔI30) over (incremental) glucose(ΔG30) was calculated after intake of a steamed bread made of 100g flour to (evaluate) the early insulin secretion. Body mass index(BMI)and serum lipid levels were also (determined.) (Results) (1)The early onset group had significantly higher fasting serum insulin ((natural) logarithm transformed, 2.6±0.6 vs 2.4±0.5 μU/ml, P<0.05), triglycerid (TG, 1.7±2.0 vs 1.4±2.0 mmol/L,P<0.05),HOMA-IR(natural logarithm transformed, 1.6±0.8 vs 1.3±0.6, P<0.01)and lower high density lipoprotein (HDL-C, 1.2±0.3 vs 1.3±0.3mmol/L,P<0.05) than those in late onset group.(2)Linear regression analyses showed that HOMA-IR, HOMA-β were (independent) risk factors predicting early age of onset. (3)In the early onset type 2 diabetic patients, the subgroup with BMI more than 25 kg/m2 had significant earlier diagnosis age (32±7 vs 34±5 y, P<0.05), lower level of serum HDL-C (1.1±0.3 vs 1.3±0.3 mmol/L, P<0.01), higher FIns ((natural) logarithm transformed, 2.8±0.6 vs 2.4±0.6, P<0.01), higher HOMA-IR (natural (logarithm) transformed, 4.3±0.9 vs 3.9±1.0, P<0.01)and higher TG (1.9±2.0 vs 1.5±2.1 mmol/L, P<0.01)than those with BMI less than 25 kg/m2. Conclusion Compared with late onset familial type 2 diabetic patients,more severe insulin resistance and relative β cell function defect are the important clinical (features) in early onset familial type 2 diabetic patients. The interaction between (insulin resistance) and (β-cell) dysfunction are probably the determinants of onset age of T2DM.
3.Contribution of MODY2 gene to the pathogenesis of Chinese early onset familial type 2 diabetes

T polymorphism and early onset familial type 2 diabetes,the frequency of allele T of early onset familial type 2 diabetes population was significantly lower ,and the frequency of allele C significantly higher, than those of control subjects(PT polymorphism at MODY2 gene, intron 9 or nearby genes was associated with early onset familial type 2 diabetes .
4.MR Diagnosis of Glomus Jugulare Tumor

Objective To evaluate the value of MR in the diagnosis of glomus jugulare tumors.Methods 15 cases (8 in the right and 7 in the left-side) of glomus jugulare tumors were retrospectively analysed, with the emphasis on the locations, MR signal intensities andthe invading appearances to the surrounding structures of tumors. Results The tumors demonstrated as oval or irregular shape. Thesurrounding bones around the jugular fossa were destroyed in 12 masses. The signal intensity of the tumors was heterogeneous and with marked “salt and pepper”appearance in 12 masses(12/15) on plain MR imaging. The masses(13/13)were all markedly enhanced after the injection of Gd-DTPA, in which 10 of them presented with “salt and pepper”appearance. MR angiography was performed in 9 cases, theinternal carotid artery and the internal jugular vein were all displaced and/or encompassed.Conclusion Glomus jugulare tumor can becorrectly diagnosed according to the combination of MR imaging features with the typical localization,“salt and pepper” appearance and bone erosion in the skull base.
5.The Study of Relationships Between Stoma Fistula and Infection of Incervical Part of Esophageal Carcinoma

Objective To investigate the primary cause of stoma fistula in cervical part of esophageal carcinoma.Methods Improve the treatment methods before operations to prevent the infection of incisional wound in cervical part.Treatments before the improvement was set as pollution group,and the improved treatments as depuratory group.?2 test is used to compare the incidence of stoma fistula between the two groups.Results The incidence of pollution group was 12.5%,and the depuratory group was 2.4%,(?2=9.258,P=0.002).The difference was significant.Conclusion The infection of incisional wound in cervical part is the primary cause of stoma fistula in cervical part of esophageal carcinoma.
6.Explore the method of repairing bone defect after resection of lower extremity long bone structure fiber

Objective To analyze the repair methods of bone defects caused by the harmful removal of lower extremity long bone segmental bone fibrous structure,and to provide reference for clinical treatment.Methods Six cases of patient with adverse fibrous structure,3 cases with the jnjury of backbone of the tibia and 3 cases of femoralshaft damage were conducted as the research objects.Bone defect length after tumor resection was (17.3 ±4.2)cm,inorder to coinside with the peroneal vessels in the other side,fibulas with peroneal vessels from both sides should be cutdown.Dual fibula medial periosteal was stripped from the central longitudinal incision to the bone crest,folding fibulatwo -stage to make the front sides face to face,and then fixed the ends.The peroneal vascular was looped on one sideof graft to form a U shape.The vessels out of damage region was dissected,and was inserted fibula between the ends offracture,as the case wire or screws may be used.The peroneal vessels had to coinside with those in damage region,andimaging assessment should be taken to evaluate the healing of bone joints after surgery,at the same time,assessingpostoperative function by functional scores of Bone Cancer Society of the United States was also neccessary,finallyvascular anastomosis through the bleeding condition of the grafts were detected.Results All patients were followedup for 5 -8 years,the bone healed 1 year later,and the transplanted bone marrow cavity got recanalization 5 years later,at the same time,there was no tumorigenesis in the double fibula,and x -ray showed the good healing.All bonesturned to be normal ones without nonunion,infection and other complications.Final follow -up score of lower limbswas 91.2%.Conclusion It is effective and feasible to use a combination of double fibula to repair lower extremitylong bone defects caused by large bone fibrous dysplasia in the periosteum.
7.Study of retinal structure changes after YAG laser ablation in physiological vitreous floaters

AIM: To observe the structure changes of vitreous and retina after YAG laser ablation in patients with physiological vitreous floaters.METHODS: The selected 40 patients of physiological vitreous floaters,before YAG laser ablation,were checked for best corrected visual acuity,non-contact intraocular pressure,took their anterior segment photos,measured their foveola thickness(FT) and retinal nerve fiber layer(RNFL) by OCT.The YAG laser ablation was completed by one experienced surgeon.After the YAG laser ablation,all patients were checked for non-contact intraocular pressure and gave pranoprofen eye drops tid for 3d.At 2d,1wk,1 and 3mo after the surgery,they were reviewed for best corrected visual acuity,non-contact intraocular pressure.At 3mo later anterior segment photos were taken.At 1wk,1 and 3mo after the surgery,FT and RNFL were measured again.RESULTS: The postoperative 2d,1wk,1 and 3mo,best corrected visual acuity,non-contact intraocular pressure of the patients had no significant difference with preoperative(P>0.05).The preoperative anterior segment photos showed obvious single or sheet opacity of vitreous,the postoperative 3mo photos showed that vitreous opacities decreased or disappeared,no other abnormal changes were found.The preoperative OCT data showed that FT was 214.60±9.35μm,the postoperative 1wk,1 and 3mo FT were 213.75±9.07μm,213.40±8.83μm,213.85±9.22μm.The preoperative RNFL were upper 130.26±14.23μm,lower 133.15±14.46μm,nasal 82.48±13.50μm,temporal 75.40±11.89μm;The postoperative 1wk RNFL were upper 130.02±14.02μm,lower 132.99±14.05μm,nasal 82.35±13.07μm,temporal 75.42±11.66μm.The postoperative 1mo RNFL were upper 130.28±14.43μm,lower 133.08±13.99μm,nasal 82.31±13.72μm,temporal 75.45±12.03μm.The postoperative 3mo RNFL were upper 130.43±14.30μm,lower 133.22±14.20μm,nasal 82.27±13.11μm,temporal 75.46±11.91μm.The differences of preoperative and postoperative 1wk,1 and 3mo FT and RNFL had no statistical significance (P>0.05).CONCLUSION: YAG laser ablation has no adverse effects to vitreous and retinal structure in patients with physiological vitreous floaters,it is effective and safe.
8.Status analysis on visual disability population in Yuzhong district of Chongqing

To investigate the status of visual disability population in Yuzhong district of Chongqing, in order to provide a counter measure for improving their quality of life.METHODS:The data of visual disability population was analyzed, which was registered in federation of the disabled of Yuzhong district of Chongqing from January 21, 2009 to July 17, 2013, collected the information of their gender and age constituent ratio, visual disability level differences, and the differences of their disability pathogeny.RESULTS: There was no significant difference between the total numbers of male and female visual disability, and before the age of 60, the number of male was more than female, but after 60, it was opposite. The grade four of visual disability accounted for the largest proportion, followed by a level of grade one, two, and three. The main causes of visual disability in proper order were:ametropia ( high myopia) , retina and pigment membrane disease, ocular trauma, optic nerve disease, glaucoma, corneal disease, congenital anomaly or eccyliosis, cataract, amblyopia, agnogenic, methysis, trachoma, and the others ( hyperpyrexia) .CONCLUSlON: The diagnosis and treatment of juvenile in Yuzhong district must be strengthened, also include retina and pigment membrane disease, to reduce the visual disability caused by these diseases.
