7.Clinical analysis for systemic complications of patients in the dental emergency room

Objective:To analyze the clinical features of oral emergency patients in general health, and to guide the oral emergency patients in the treatment, and prevention of accidents.Methods:The retro-spective study was performed on the day visits to the emergency department in Peking University School and Hospital of Stomatology from February to November 2013.The basic information of the newly diag-nosed adult, their overall health status, American society of anesthesiologists(ASA) classification and oral diagnosis information were analyzed and summarized.Results:The cardiovascular and cerebrovascular diseases(6.4%), hypertension(13.4%), endocrine system diseases(4.2%)were most common in the patients.With the increase of age, the associated systemic diseases had increased in number and in ASA classification.Periapical disease ( 24.6%) , dental pulp disease ( 24.4%) , and periodontal disease (18.0%) accounted for about 2/3.Conclusion:The general health status of patients with oral emergen-cy worsened with the increase of age.Oral surgeons should make risk assessment before treatment.ASA classification system can help doctors predict the patient risk, take corresponding measures in advance, and help to reduce the occurrence of accidents.

9.Arthroscopic one-stage reconstruction of anterior and posterior cruciate ligaments with LARS artificial ligaments

Objective To evaluate the efficacy of arthroscopic one-stage reconstruction of anterior and posterior cruciate ligaments with LARS artificial ligaments. Methods From October 2009 to December 2011, 8 patients with combined injury to anterior and posterior cruciate ligaments received arthroscopic one-stage reconstruction with LARS artificial ligaments. They were 7 men and one woman, aged from 26 to 52 years ( average, 35. 3 years ) . The time from injury to operation ranged from 17 to 21 days, averaging 19 days. Pre-operative and intraoperative inspections showed complete tear of both anterior and posterior cruciate ligaments. Inter national Knee Documentation Committee ( IKDC ) scoring was used at 6 months postoperation to grade the knee movement of the patients; Lysholm scoring was used at final follow-ups to evaluate the knee function. Results The operating time for this series ranged from 85 to 102 minutes ( average, 93 minutes ) . All the patients were followed up for 6 to 24 months ( average, 12 months ) . Wound infection at upper tibia occurred in one patient, which responded to debridement and screwing 4 weeks after operation. All the patients got rid of instability of the knee joint, and gained normal knee extension and fine functional recovery. At final follow-ups, the range of motion of the knee ranged from 100° to 125° ( average, 108. 4°) . Follow-ups revealed no breakage or loosening of the ligaments. By the IKDC scoring, of the 8 patients who had been graded all as D before opera-tion, one was improved into A and 7 into B. The Lysholm scores at final follow-ups averaged 86. 1+4. 8, sig-nificantly higher than the postoperative value ( 18. 7+3. 1 ) ( P < 0. 05 ) . Conclusion Arthroscopic one-stage reconstruction of anterior and posterior cruciate ligaments with LARS artificial ligaments can restore the stability of knee joint and obtain satisfactory short-term results.
10.Analysis on levels of serum adiponectin and homocysteine in elderly patients with type 2 diabetes complicating retinopathy

Objective To explore the levels of serum adiponectin(APN)and homocysteine(Hcy)in elderly patients with type 2 diabetes metlitus(T2DM)complicating retinopathy and the relation between these two indicators.Methods Sixt-eight cases of complicating retinopathy(T2DM+DR group)were screened from the elderly inpatients with T2DM in our hospital from January 2012 to December 2014.Their general data were investigated.The routine biochemical indicators were tested.The enzyme-linked immunosorbent assay(ELISA)was employed to measure the serum total APN,high molecular weight APN and Hcy levels.Mean while the correlation among total APN,high molecular weight APN and Hcy was evaluated by adopting the Pearson correlation analysis.Contemporaneous 65 cases of elderly T2DM and 70 elderly people undergoing the healthy physical examination were chosen as the T2DM group and NC group respectively.Results The WHR,FBP,TC,TG,LDL-C,HbA1c,FINS and HOMA-IR in the T2DM+DR group and T2DM group were higher than those in the NC group,while the HDL-C level was lower than that in the NC group(P<0.05);the duration and BMI of the T2DM-4-DR group were higher than those of the T2DM group,the difference was statistically significant(P<0.05);the levels of total APN and high molecular weight APN in the T2DM+DR group and T2DM group were lower than those in the NC group,while the Hcy level was higher than that in the NC group(P<0.05);moreover the levels of total APN and high molecular weight APN in the T2DM+DR group were lower than those in the T2DM group,and Hey level was higher than that in the T2DM group,the difference was statistically significant(P<0.05);the levels of total APN(r=-0.654)and high molecular weight APN(r=-0.562)were negatively correlated with the Hey level in the T2DM+DR group(P< 0.05).Conclusion The serum APN level is decreased and Hcy level is increased in elderly patients with T2DM complicating retinopathy,which may be associated with the progression of retinopathy in T2DM.
