1."The conjugal airway: yours or mine?" fine tuning of voice during thyroplasty under dexmedetomidine sedation
Philippine Journal of Anesthesiology 2008;20(1):38-44
This is a case of a 37 ear old female who as scheduled to undergo thyroplassty for left vocal cord paralysis due to chronic focal mononeuropathy of the left recurrent laryngeal nerve. Correct placement of the implant would be verified by awakening the patient at a predetermined time and asking her to phonate. This was accomplished under balanced anesthesia using mask inhalation of titrated concentration of sevoflurane and contunous intravenous infusion of dexmedetomidine.

Heterotopic ossification (HO) is the growth of bone in soft
tissue, and can be broadly classified into neurogenic, genetic and traumatic causes. The pathophysiology of HO remains unknown. This disorder is extremely rare in infants and can mimic or coexist with thrombophlebitis, cellulitis or osteomyelitis. Most importantly, HO has to be differentiated from bone-forming tumours such as osteosarcoma and osteochondroma. We report a case of traumatic HO in a fiveday- old newborn following intravenous cannulation of the right wrist and left ankle, with the latter complicated with osteomyelitis. We highlight the clinical and radiological
features of HO and differential diagnoses of soft tissue
ossification in early childhood.

Fresh kiwi fruit freeze-dried juice has been traditionally used for constipation of elderly patients. Its effectiveness and safety was studied in 42 elderly patients (age 60–84: median age 67±6) suffering from mild to moderate constipation, taken as a food supplement of 6 capsules/3 times per day for 4 weeks following a one week control period, after receiving written informed consent. Bowel habits and generic quality of life questionnaires were monitored. Bowel habits such as evacuation time and stool quality improved from the 14th day and continued for 28 days compared to the control period.
Quality of life in terms of external appearance (such as skin and edema), lumbago, and headache improved. In conclusion, freeze-dried fresh kiwi fruit juice had a laxative and QOL-improving effect on the elderly. No apparent adverse effects were observed.

We evaluated the safety of fucoidan from Gagome Kombu (GKF) in genotoxicity tests. In bacterial reverse mutation test, GKF had no reverse mutation inducing activity on five bacterial strains with or without S9 metabolic activation. In chromosome aberration test, GKF had neither structural nor numerical chromosome aberration inducibility with or without S9 metabolic activation. In micronucleus test, neither formation of micronuclei nor decrease of mironucleated reticulocytes was observed in the bone marrow of the mice treated by GKF. These results indicate that GKF has no genotoxic activities under the condition of this study.

Coix lacryma-jobi L. var. ma-yuen Stapf is a grass long been used in traditional medicine as a nourishing food and reported to possess pharmacological effects including anti-tumor, anti-obesity, anti-diabetic, etc. In order to evaluate the possible mutagenicity of the hot water extract of all parts (husks, pellicles, and astringent skin) of the food, we performed a reverse mutation test in bacteria, a mouse lymphoma assay and a mouse micronucleus test. The results of all tests were negative. It was concluded that the extract has no mutagenicity for living bodies.

Coix lacryma-jobi L. var.ma-yuen Stapf (Coix seed) is a grass crop that has long been used in traditional medicine as a nourishing food. However, high-intake safety of the extract of the husks, pellicles and astringent skin of Coix seed has rarely been evaluated. We performed a safety test of hot water extract of all parts of Coix seed (CRD extract) in rats. CRD extract showed no significant toxicity on body weight, blood analyses, urinalysis and histopathological examination in acute toxicity tests.

A cruciate ligament avulsion is a much less common form of injury than a cruciate ligament tear. Simultaneous tibial avulsion fractures of both cruciate ligaments occur even more rarely. Over the last decades, many studies have described arthroscopic fixation of acute cruciate tibial avulsion fractures, but arthroscopic treatment in a late presenting patient has not been reported in the literature. This case report presents a 32-year-old female with a chronic tibial avulsion fracture of both anterior cruciate ligament and posterior cruciate ligament. Simultaneous fixation of both fractures was performed arthroscopically at week four post-injury. At one year of follow-up, the patient had demonstrated full knee range of motion and stable knee with no complaints, and achieved excellent clinical outcomes. Radiographs showed union of both fractures, and the patient had resumed high-impact exercises.
