1.Surgical treatment strategy for elderly colorectal cancer patients

Objective To study the surgical treatment strategy for elderly(aged at 80 years or over)colorectal cancer patients.Methods There were 65 cases of colorectal cancer undergoing Surgical treatment in Beijing Cancer Hospital from 1999 to 2006,and results were analyzed retrospectively.Results In this group of patients.the postoperative morbidity was 53.8%,but anastomotic leakage rate was only 3.9%,operative mortality was 1.5%.Postoperative 1-,3-and 5-year survival rate was 78.92%,27.79%,and 16.32%respectively.Kaplan-meier analysis and Cox regression analysis showed that:the TNM stage of the tumor and preoperative hemoglobin and WBC level were independent prognostic factors,but patients' age,gender,preoperative serum level of CEA and albumin,tumor differentiation,tumor size were not independent prognosis factors.Conclusion Elderly CRC patients have a higher risk of surgical treatment.But with the improvement of perioperative management,the anastomotic fistula and mortality rate were not increased significantly and the result is satisfactory.
2.Polymorphism analysis of thymidylate synthase in 252 colorectal cancer patients

Objective To analyze the polymorphism of thymidylate synthase(TS)in colorectal cancer patients,further more,to provide guidance for personalized therapy of colorectal cancer.Methods PCR direct sequencing was used to detect the polymor-phism of TS in 252 patients with colorectal cancer.Results TS genotypes of 252 patients with colorectal cancer were detected total-ly,including 137 male and 115 female.3RG/3RC accounted for the largest proportion in both male and female(36.50% and 36.52%respectively).In female,2RC/3RC and 3RG/3RG both accounted for the second largest proportion(both 18.25%).While in fe-male,3RG/3RC accounted for the second largest proportion(26.09%).If patients were divided according to age groups,in youth patients(n=28),3RG/3RC accounted for the largest proportion(42.86%),and the second was 2RC/3RG(21.43%).In the middle aged patients(n=84),3RG/3RC(45.24%)and 2RC/3RC(16.67%)were the major genotypes.For old patients(n=115),the ma-jor genotypes were 3RG/3RC(36.52%)and 2RC/3RG(16.52%).Conclusion The polymorphism of TS are mainly 3RG/3RC in colorectal cancer patients.
3.Angioplasty and stent implantation for treating lower limb atherosclerosis obliterans:report of 34 lesions

30%. For inferior genicular artery lesions,only balloon angioplasty were carried out. All patients were followed up for 6 months and 12 months to assess of restenosis and clinical efficacy. Results All the patients' therapy was successful,their clinical symptoms were improved or disappeared. The 6-month follow-up showed that restenosis rates were 5.5%,16.7% and 25% respectively in the iliac artery,femoral popliteal artery,and inferior genicular artery. The rates in 12-month follow-up were 11.1%,33.3%,and 75% correspondingly. Conclusion Angioplasty and stent implantation are safe and effective in treatment of lower limb atherosclerosis obliterans. Standardized postoperative anti-platelet,anticoagulant,and anti-lipid therapy is essential to maintain blood vessel recanalization.
4.Effect of hedyotisdiffusa on expressions of apoptosis related protein Fas, caspase3 and caspase7 in Renca renal cell carcinoma of model mice

Objective To analyse the effect of hedyotisdiffusa on expressions of apoptosis related protein Fas , caspase3 and caspase7 in Renca renal cell carcinoma of model mice.Methods One hundred and twenty BALB/C male mice were randomly divided into normal control group, model control group, interleukin group, hedyotisdiffusa group, 30 mice in each group, half male and female.Excepted for normal control group , renal cell carcinoma models were established in the other groups , and were given corresponding drug treatment.The tumor size and weight , and the tumor inhibition rate of all mice were observed, and the expression levels of Fas,FasL,caspase3 and caspase7 were detected.Results Compared with model control group, the tumor size and weight were lower , the tumor inhibition rate were higher , the positive expression rates and protein levels of Fas were higher , the positive expression rates and protein levels of FasL were lower, the positive expression rates and expression levels of Caspase3,Caspase7 protein were higher in the mice of interleukin group and hedyotisdiffusa group, all with significant differences ( P<0.05 ).The above indicators in hedyotisdiffusa group were significant improved compared with interleukin group (P<0.05).Conclusion The hedyotisdiffusa can effectively promote the expressions of apoptosis related protein Fas,caspase3 and caspase7 in renal cell carcinoma of model mice, inhibit the expression of FasL protein, improve the tumor suppression rate.
5.Observation of two temporary cardiac pacemaker electrode fixation methods

Objective To observe the effect of two different methods of fixed temporary pacemaker electrode. Methods Fromn April 2015 to December 80 temporary pacemaker patients were randomly divided into control group (n = 40) and the experimental group (n = 40), the control group using gauze cloth tape cross method combined with the traditional fixed, the experimental group was treated with 3M type of comfortable and stable transparent dressing+3M pressure fixing belt fixed, to compare two groups in the skin lesions, electrode dislocation, inflammatory reaction, the switching frequency difference. Results There was no significant difference between the two groups in general data (P>0.05). There was no significant difference between the two groups in the cost of dressing and materials、electrode dislocation (P>0.05), experimental group of the switching frequency is lower than the control group (Z=-2.064, P<0.05), the differences were statistically significant. Conclusions 3M comfortable type solid transparent sticking and 3M pressurized fixed band method in the absence of increased medical costs while ensuring medical safety, save the manpower cost.
8.The relation of aquaporinl gene expression and kidney injury in rats with disseminated intravascular coagulation

Objective To characterize the effects of AQP1 expression on kidney damage in rat disseminated intravascular coagulation(DIC) caused by lipopolysaccharide(LPS) dosing. Method Fifty male Wistar rats (clean grade) were randomly assigned into 5 groups of 10 rats. The 10 control rats were dosed with 10 ml of 0.9%NaCl solution by a drip via the vena caudalis within 4 h, and blood and tissues were obtained after treatment completion. In the DIC groups, the rats were dosed with LPS (30 mg/kg body weight in 10 ml of 0.9% NaCl solution) by a drip via the vena caudalis within 4 h, and blood and tissues were obtained at 4, 6, 8 and 10 h. The blood platelet(PLT) count, prothrombin time(PT) , activated partial thromboplastin time(APTT), fibrin(FIB) and D-dimer(D-D) were detected. Hematoxylin and eosin(HE) staining was used to examine the pathologic changes in the lung and kidney tissues of each group (both hematologic parameters and tissue pathologic changes were used to judge the course of DIC). The AQP1 gene expression levels in the kidney tissues from the groups were evaluated by the mRNA levels using RT-PCR. Statistical analyses were performed by the SNK- q method. Results The PLT count, PT, APTT, FIB and D-D examinations revealed remarkable changes in all DIC groups compared with the control group (P < 0.01). The AQP1 mRNA level was significantly decreased in the DIC group at 4 h compared with the control group (P < 0.01) , and further decreased to the minimum level in the DIC group at 6 h. Moreover, cloudy swelling of renal tubular cells was observed at 6 h and cell degeneration and necrosis were observed at 8 h among the DIC groups. Conclusions Downregulation of AQP1 mRNA expression occurred before damage to the renal tubular cells in DIC, indicating that AQP1 expression may be involved in the kidney damage observed in rat DIC.
9.Philological Study on"Knowledge Attitude Practice"Method

Objective To master the status of"Knowledge Attitude Practice"(KAP)methodology used in China by philological research.Methods Choosing the appropriate key words to retrieve the literatures based on CNKI total library,removing the false one by intensive reading,and analyzing the valid literatures in different ways based on which subject.Results①The amount of related literatures rising steadily from the middle of 1990's and remarkably since 2000.②The over 2000 valid literatures were mostly distributed in the healthcare field,and the most common subjects related in which were disease control,nourishment,smoking control,sex,conception control,drug abuse etc.③Not all of the research reported in the literatures performed the 3 core elements of KAP, and most of the literatures did not mention about the special interference on the targets,let alone the result analysis of the interference.④There were few literature whose main topic is KAP methodology.⑤The terms used for representing the KAP concept are not standardized.Conclusion ①The KAP methodology had been introduced into China and had been widely used in many fields.but the Chinese concept and terms of which should be more standardized,and the usage of which in some special application should be more advanced.②The KAP methodology Can be used and should be useful in TCM healthcare monitoring.
10.Advancement in researches of mechanism of nervous invasion of pancreatic cancer and its countermeasures

Perineural invasion(PNl) is regarded as a factor associated with local recurrence and an important prognostic factor in pancreatic cancer.Little is known about the mechanism of PNI in Pancreatic cancer.This review summarizes the role of innervation, clinicopathologic factors, lymhatic vessels, vascularity, cytokines, adhesion molecules, some immunoglobulin superfamily members and mucin in PNI.Because PNI is often followed by poor survival.An ear-lier diagnosis can improve pancreatic cancer prognosis.IPEUS,CT scan and immunostaining CK-19, K-ras gene a-nalysis can be used to diagnose perineural invasion pre-, in-tra-or postoperatively.A wide surgical exeresis with an enough lymph nodes clearance together with surrounding connective and nervous tissue can get a better long term survival rate.
