1.Clinical characteristics in twin premature infants

Objective To explore the clinical characteristics in twin premature infants in order to provide some guidance for clinical work in future.Methods The clinical data of 593 premature infants hospitalized in Department of Pediatrics,the Affiliated Hospital of Kunming Medical University were collected from June 2010 to June 2012,in terms of gestational age,birth weight and neonatal complications.A retrospective analysis was performed for the data.The premature infants were divided into 2 groups:study group of 131 twin premature infants and control group of 462 singleton premature infants.The 131 twin premature infants in study group were divided into large double group(n =64) and small double group(n =67) according to delivery time.The clinical data of premature infants in each group were statistically analyzed.Results The gestational age of study group was (34.23 ± 1.90) weeks,which in control group was (33.91 ± 1.82) weeks,and there was no significant difference between the 2 groups(t =1.689,P =0.092).The birth weight in study group [(1 921.64 ± 414.05)g] had statistically significant difference compared with control group [(2 164.98 ± 495.85) g] (t =-5.209,P =0.000).The study group of incidence of premature rupture of membranes was 16.79% (22/131 cases)and which in the control group was 32.68% (151/462 cases),and there was statistically significant difference between the 2 groups (x2 =12.472,P =0.000) ;the incidence of neonatal asphyxia of study group was 9.92% (13/131 cases) and that of the control group was 17.10% (79/462 cases),there was statistically significant difference between the 2 groups (x2 =4.010,P =0.045) ; the incidence of respiratory distress syndrome in study group was 6.87% (9/131 cases) and that in the control group was 3.03 % (14/462 cases),the difference was statistically significant between the 2 groups (x2 =4.037,P =0.045) ; the incidence of apnea in study group was 4.58% (6/131 cases) and that in the control group was 0.65% (3/462 cases),the difference was statistically significant between the 2 groups(x2 =8.085,P =0.004) ; the incidence of meconium aspiration syndrome of study group was 0 and that of the control group was 3.90% (18/462 cases),there was statistically significant difference (P =0.018) ;the incidence of neonatal hypoglycemia of study group was 27.48% (36/131 cases) and that of the control group was 16.67% (77/462 cases),the difference was statistically significant between the 2 groups (x2 =7.738,P =0.005) ;the incidence of sepsis of study group [16.79% (22/131 cases)] was significantly higher than that of the control group [8.44% (39/462 cases)],and the difference was statistically significant between the 2 groups (x2 =7.715,P =0.005) ;the incidence of extrauterine growth retardation of study group was 6.10% (8/131 cases) and that of the control group was 2.38% (11/462 cases),the difference was statistically significant between the both groups (x2 =4.568,P =0.033).In the study group,the incidence of neonatal sepsis in big double group was 29.68% (19/64 cases),and that in small double was 14.93% (10/67 cases),there was statistically significant difference between the 2 groups(x2 =4.138,P =0.042).The other complications between the big double group and small double group had no significant difference.Conclusions The incidence rates of acute respiratory distress syndrome,apnea,neonatal hypoglycemia,sepsis and extrauterine growth retardation of twin premature infants are higher than the singleton premature infants in the neonatal period.But the incidence rate of meconium aspiration syndrome is a higher in singleton premature infants.

Coprinus comatus cultivated on cotton seed hull medium decomposed lignocellulose straggly and was high of absolute biological efficiency. Lignocellulose is the main carbon source for the fruiting stage of the fungus. There existed the positive correlation between the degradation rates of the cellulose and hemicellulose in the medium and the activities of extracellular CMCase (carboxymenthelcel lulase), FPase (filter paper cellulase) and HCase (hemicellu-lase), there also existed the positive correlation between the degradation rate of the lignin in the medium and the activity of extracellular laccase, but no correlation between the degradatio rate of the lignin in the medium and the activity of peroxidase. The activity of extracellular amylase was comparatively high at mycelial growth stage, and the protease activity peek was at teh time when the fruitbody matured.
4.The fabrication and clinical application of semi-fixed mandibular lingual arch expansion appliance.

Semi-fixed mandibular lingual arch expansion appliance is composed of a mandibular molar band, a keyway and arch expansion spring. The arch expansion spring are used to expand maxillary arch symmetrically or asymmetrically when bolts of the two ends are inset into keyways. Dental arch expansion appliance for 25 patients with mandibular arch stenosis showed that semi-fixed mandibular lingual arch expansion had good effect and could be used to expand mandibular arch.

5.Effect of pre-pregnancy body mass index and gestational weight gain on birth weight of infants: a prospective cohort study

Objective To analyze the relationship between pre - pregnancy body mass index ( BMI) ,gestational weight gain ( GWG) and the birth weight of infants,and explore the effect of weight change before and during pregnancy on low birth weight ( LBW) and macrosomia. Methods Women were enrolled by the Chinese Pregnant Women Cohort Study during first trimester. Each respondent's weight before and during pregnancy and the birth weight of infant were collected after fellow up. Prepregnancy BMI was divided into underweight,normal and overweight /obesity groups and GWG was divided into suitable, insufficient and excessive groups. Multivariate Logistic regression was adopted to explore the relationship be- tween pre-pregnancy BMI,GWG and newborn's birth weight. Results Women's prepregnancy BMI and GWG were associated with neonatal birth weight ( all P<0. 05) . Prepregnancy overweight or obesity ( OR=2. 339,95% CI: 1. 674-2. 282,P<0. 001) and excessive GWG ( OR= 1. 398,95% CI: 1. 188-1. 978,P= 0. 048) were shown as risk factors for macrosomia. Insufficient GWG increased LBW risk ( OR = 1. 479, 95% CI: 1. 461-1. 679,P= 0. 035) while excessive GWG declined LBW risk ( OR= 0. 428,95% CI: 0. 225 -0. 817,P= 0. 010) . Under weight-insufficient GWG was risk factor of LBW ( OR= 1. 335,95% CI: 1. 048 -2. 319,P= 0. 048) while normal BMI-excessive GWG ( OR= 1. 088,95% CI: 1. 016-1. 675,P= 0. 038) and overweight /obesity-excessive GWG ( OR= 1. 498,95% CI: 1. 244-2. 017,P= 0. 046) were associated with higher risk of delivering macrosomia. Conclusions Prepregnancy BMI and GWG were associated with infant's birth weight and women were suggested to maintain their weight in recommended range before and during pregnancy.
6.Series orthodontic treatment on teeth transposition of maxillary canine and lateral incisor.

OBJECTIVETo explore the treatment on teeth transposition of maxillary canine and lateral incisor in order to improve the clinical treatment effect.
METHODSEleven patients with transposition maxillary canine and lateral incisor were treated with the method: Expand space, artificial reverse occlusion of transpositional lateral incisor to give way, transpositional lateral canine distalization and controlling root, mesial movement of lateral incisor with tongue arch, interactive controlling roots and retention with tongue fixed retaining appliance.
RESULTSEleven patients had satisfactory treatment effect, with tidy dentition and parallel teeth roots of transpositional canine and lateral incisor.
CONCLUSIONSeries orthodontic treatment on teeth transposition of maxillary canine and lateral incisor can effectively improve the clinical treatment effect and shorten the treatment time.

7.RNA interference of RelB enhances the radiosensitivity of prostate cancer cell line RM-1 in mice.

OBJECTIVETo investigate the effect of RNA interference of the RelB gene on the radiosensitivity of the mouse prostate cancer cell line RM-1 and its mechanism.
METHODSWe constructed RelB siRNA-expressing lentiviral vectors targeting the RelB gene with the molecular biological technique, and determined the expressions of RelB mRNA and protein on radiation after transfection with siRelB mediated by liposome using RT-PCR and Western blot, respectively. We also detected the apoptosis of RM-1 cells by FCM assay and their radiosensitivity by clonogenic assay.
RESULTSThe expressions of RelB mRNA and protein were significantly lower in the RM-1 cells than in the control and negative interference groups after transfection with RelB siRNA (P < 0.05), while the apoptosis of RM-1 cells remarkably higher in the siRelB-RM-1 than in the control group after radiation treatment (P < 0.05). The activity of MnSOD was markedly decreased (P < 0.05), and the radiosensitization rate of the RM-1 cells in the RelB-RM-1 group was 5.13 after radiation treatment.
CONCLUSIONRNA interference of the RelB gene could enhance the radiosensitivity of the mouse prostate cancer cell line RM-1, which might be associated with its inhibition of Mn-SOD expression and induction of cell apoptosis.

8.Dynamic expression of toll like receptor 2 and 4 in a rat model of myocardial ischemia/reperfusion injury.

OBJECTIVETo explore the role of toll-like receptor 2 (TLR2) and toll-like receptor 4 (TLR4) in myocardial ischemia/reperfusion injury (MI/RI) by observing the dynamic expression changes at mRNA and protein levels early after myocardial ischemia/reperfusion (I/ R).
METHODSThe Wistar rats were randomly divided into Sham and I/R group (n = 42), and killed according to different reperfusion time (1, 2, 4, 6, 12, 24 h and 7 d). Structural and morphous changes of myocytes were observed under optical microscope. The mRNA and protein levels of TLR2 and TLR4 were detected using real-time PCR (RT-PCR). Monocyte chemokine protein-1 (MCP-1) and interleukine-6 (IL-6) mRNA levels were measured by reverse transcriptase-polymerase chain reaction (rt-PCR).
RESULTS(1) With the extension of reperfusion time, the myocardial infarct size increased smoothly, and reached the plateau at 4 h, then stayed in the platform. After reperfusion for 7 d, the ventricular had been remodeled. (2) At the beginning of reperfusion, myocardial structure showed no significant change in Sham group, but had different degrees of injury in I/R group. In rats of the group reperfused for 7 d the left ventricular remodeling could be visible. (3) Compared to sham group,TIR2, TLR4, MCP-1, IL-6 mRNA level were increased in myocardium in I/R group. TLR2 and TLR4 both peaked at 4 h of reperfusion, IL6 peaked at 6 h, followed by a gradually decrease. TLR4 and IL-6 mRNA levels rose again at 7 d. MCP-1 level in I/R group remained fairly with sham group at the beginning of reperfusion, and markedly elevated at 7 d.
CONCLUSIONExpression of TLRs mRNA in myocardium during early after myocardial ischemia/reperfusion increased rapidly and activated TLRs might play an important role in MI/RI through promoting the generation of inflammatory factors. At the late reperfusion, TLRs levels raise again and the expression of inflammatory factors increase once again, Those may probably affect the remodeling of ventricular, and injure myocardial structure and function.

9.Association between genetic polymorphisms in folate metabolic enzyme genes and colorectal cancer: a nested case-control study.

OBJECTIVETo investigate the interrelationship of genetic polymorphisms in folate metabolic enzymes (MTHFRC677T, MTHFRA1298C, MTRA2756G and MTRRA66G) and their combinative effects with colorectal cancer (CRC).
METHODSA nested case-control study was designed and carried out. 140 CRC patients and 343 control subjects were included in this study. Polymorphisms of folate metabolic enzyme genes were genotyped by PCR-restriction fragment length polymorphism method. Risk of CRC was estimated by unconditional logistic model, and P value for interaction was calculated by likelihood test.
RESULTSThe allele of MTR2756G showed a positive association with CRC (OR = 2.04, 95% CI = 1.22 - 3.40). Those with MTHFR1298AA and MTR 2756AG/GG genotypes had an elevated risk with CRC (OR = 2.57, 95% CI, 1.42 -4.65), and their combinative effect showed a significant association with CRC (P = 0.04).
CONCLUSIONMTR2756G allele may be a risk factor of CRC, and interaction may exsit between polymorphisms of MTHFRA1298C and MTRA2756G. Further studies with larger sample and in different ethnic groups are needed.

10.Totally robotic surgery for rectal cancer with transanal specimen extraction and robot-sewn anastomosis technique.

OBJECTIVETo summarize the clinical experience of transanal specimen extraction and introduce how to achieve the rectum reconstruction in robotic rectal cancer surgery.
METHODSClinical data of 15 cases undergoing rectal cancer resection with the da Vinci robotic system in Jinglin Hospital between February 2012 and May 2012 were analyzed retrospectively.
RESULTSThere were 9 males and 6 females with a mean age of (61.5±9.2) years old. All the cases underwent robotic surgery successfully without intraoperative conversion. The average operative time was (154.7±10.6) min with minimal blood loss (17.3±6.5) ml. The time to first flatus was (2.3±0.8) days postoperatively. The postoperative hospital stay was (3.3±0.6) days. The mean number of lymph node harvested from the surgical specimen was (15.0±1.2). All the resection margins were negative. All the patients had postoperative follow-up ranging from 4 to 8 weeks. There were no postoperative infection, anastomotic leak, incision hernia, or short-term deaths.
CONCLUSIONda Vinci robotic system greatly reduces the difficulty of the rectal cancer surgery with its unique light source system and flexible equipment operation.
