1.Breakfast Skipping and Its Associated Factors among Undergraduates in a Public University in Kuala Lumpur

An analytical cross-sectional study was conducted in a public university in Kuala
Lumpur among a random sample of 2665 undergraduates. The objective was to
study the prevalence of breakfast skipping and its associated factors. Data
collection was conducted via a self-administered pre-tested questionnaire. There
were 43.5% male respondents, with Malays being the majority (58.3%). The
prevalence of breakfast skipping was 29.2 (95% CI: 27.3 – 30.3) %. The factors
significantly associated with breakfast skipping (p<0.05) were age, race,
accommodation, faculty and skipping dinner. As the respondents’ age increased,
their risk of breakfast skipping was lower (OR: 0.95; 0.89 – 0.99). Malays (OR:
1.94; 1.48 – 2.54), Indians (OR: 1.70; 1.08 – 2.66), and students from the Sabah and
Sarawak indigenous communities (OR: 2.13; 1.37 – 3.33) were more likely to skip
breakfast compared to their Chinese counterparts. Respondents who stayed in
their own houses were also less likely to skip breakfast compared to those
staying in hostel with meals catered (OR: 2.32; 1.39 – 3.84), hostel with cafeteria
(OR: 2.92; 1.74 – 4.91) or in rented houses (OR: 2.08; 1.25 – 3.46). Respondents
majoring in Arts & Economics had 1.40 (1.07 – 1.82) times risk of breakfast
skipping compared to those majoring in Life Sciences. Those who skipped dinner
too had twice the odds (1.47 – 2.77) of breakfast skipping. In conclusion the
prevalence of breakfast skipping among the undergraduates of this university
was moderately high. Health awareness campaigns or introduction of healthy
eating guidelines should be initiated for the undergraduates as well as food
caterers in campus. The policy and pricing of catered food in campus should also
be reviewed.
2.Drug resistance mutations among virological failure HIV-1 infected patients in Malaysia

The determination of HIV drug resistance mutations (DRMs) towards antiretroviral
(ARV) drugs among HIV-1 treated patients with virological failure is crucial for further
management of the patient. This study aimed to assess the most common genomic mutation
and to analyse subtypes among the HIV-1 patients with viral load level > 1,000 copies/mL. A
total of 101 virological failure HIV-1 patients from four different regions of Peninsular Malaysia
with a viral load measurement facility were included in the study. Majority of patients (89.1%)
have at least 1 mutation associated with clinical resistance to either protease inhibitors
(PIs), nucleoside reverse transcriptase inhibitors (NRTIs) or nonnucleoside reverse
transcriptase inhibitors (NNRTIs). Major resistance mutations among the patients towards
NRTIs and NNRTIs were 70.3% and 18.8%, respectively. The most common mutation for
NRTIs was M184V while K103N mutation was detected in the majority of patients who were
treated with NNRTIs. The most commonly observed mutations for major PI and minor PI seen
among the study population were V82A/T and L10V, respectively. In HIV-1 subtype analysis,
CRF33_01B was the most predominant HIV-1 subtype in this study group. The vast detection
of DRMs in this study emphasized the importance of genotypic resistance test in the
management of HIV patients as DRMs can alter patient’s susceptibility towards ARV drugs.
Further study on larger number of samples is essential for the development of a database on
HIV-1 DRMs among patients that experience virological failure in Malaysia.
3.The study of tooth wear patterns and their associated aetiologies in adults in Kelantan, Malaysia

Tooth wear is the loss of tooth tissue and structures not due to caries. It can occur in various forms either attrition, abrasion, erosion, noncaries
cervical lesion (NCCL) or a combination of two or more forms. The objective of this study was to determine the patterns and associated aetiologies of tooth wear among adults in Kelantan, Malaysia. This crosssectional
study involved 81 adults with tooth wear which was visually
assessed using the Smith and Knight Tooth Wear Index (TWI). A questionnaire was used to seek putative aetiologies of the wear. Data were analyzed and the results were expressed as frequencies and percentages.
Six thousand three hundred and eighty four tooth surfaces were examined in 1596 teeth. 17.4% surfaces had tooth wear; 80% scored 1, 18% scored 2 and 2% scored 3. Among the 81 cases of tooth wear, 29 (35.8%) had
abrasion; 25 (30.9%) had attrition; 1(1.2%) had erosion while 26 (32.1%) had the combined type. Among those with abrasion, majority had the habit of eating freshwater clams (Corbicula fluminea) as local delicacy where
most of the times people use their teeth to pry open the clam shells. Erosion was related to the pooling of carbonated drinks or beverages in the mouth before swallowing. In conclusion, most adults experienced abrasion
and the most common possible associated aetiology is the way of eating clams.
4.Translation And Validation Of The Malay Post Traumatic Stress Disorder Checklist For Civilians

Objectives: There are a number of validated questionnaires available for the
screening of Post-Traumatic Stress Disorder (PTSD), and the PTSD Checklist
for Civilians is one of them. However, none was translated into the Malay
Language and validated for use in the Malaysian population. The aim of this
study is to translate and validate the Malay PTSD Checklist for Civilians
(MPCL-C). Methods: The PCL-C was translated into the Malay Language and
back-translated. The reliability and validity of the MPCL-C were then
determined by administering them to those who presented at the emergency
department for motor vehicle accident at least one month before. Results: The
MPCL-C has good face and content validity. In terms of reliability, it is also
good, with Chronbach’s alpha values of 0.90, 0.77, 0.75 and 0.74 for the full
scale, re-experiencing, avoidance and arousal domains respectively. Conclusions:
The MPCL-C is a valid and reliable instrument to screen for PTSD in motor
vehicle accident victims for the studied population.
5.Variation in Sensitivity Among Some Isolates of Macrophomina phaseolina Isolated from Cotton Roots to Flutolanil Fungicide.

Toxicity of the fungicide Flutolanil was in vitro tested against 20 isolates of Macrophomina phaseolina and cotton seedlings of ten commercial cotton cultivars. The isolates were recovered from roots of cotton plants obtained from different cotton-growing areas in Egypt. Most of the tested isolates were sensitive to Flutolanil; however, they varied in sensitivity. Twenty-five percent of the isolates were highly sensitive where IC50 ranged from < 1 to 5.1 microg/ml, 20% of the isolates were sensitive where IC50 ranged from 15 to 30 microg/ml, 45% of the isolates were moderately sensitive where IC50 ranged from 46 to 58.5 microg/ml, and 10% of the isolates were not much sensitive (tolerant) where IC50 was > 100 microg/ml. Flutolanil was very safe on both shoots and roots of the tested cultivars (IC50 > 100 microg/ml). Treating cotton seeds with Flutolanil resulted in highly significant (P < 0.01) reductions in pathogenicity of 18 isolates and a significant reduction (P < 0.05) in pathogenicity of isolate M29. M1 was the only isolate, which was insensitive to the application of Flutolanil. In vivo toxicity to Flutolanil was not correlated with its in vitro toxicity. However, a highly significant correlation (r = 0.60, P < 0.01) was observed between pathogenicity of isolates and the in vivo toxicity of the fungicide.

6.Knowledge, Attitude, Practices Related To Dengue Fever Among Rural Population In Terengganu, Malaysia

Aedes mosquito-borne Dengue morbidity is predominantly high in the tropics and subtropics regions. Dengue is also a
public health problem in Malaysia since the first epidemic in 1973. Reducing the vector population and personal
protection still plays an important role in dengue prevention and control. With the information of community’s dengue
knowledge, attitude and practices (KAP), the authorities could construct evidence-based, community- empowered vector
control program. Upon the understanding of the value of baseline data, a cross-sectional study was carried out in dengue
hotspot areas in Seberang Takir using universal sampling. The study results showed that 54.6% of the population had high
level of knowledge, 18.6% had good attitude and 91.7% were performing good practices against Dengue infection. After
adjusting confounding variables, age and educational levels of respondents, knowledge as well as attitude were found to
be significant associated factors for having good practice against Dengue. The study findings provide the need for further
information to undertake a holistic approach which is in need of community participation and cooperation.
7.Clinical features and prognostic factors of cutaneous vasculitis among dermatology patients in Johor Bahru, Malaysia

Background: Cutaneous vasculitis is common, yet the riskfactors for its chronicity have not been established.Objective: To describe the clinical spectrum and identify riskfactors for chronicity of cutaneous vasculitis.Methods: Retrospective data analysis of 275 patientsdiagnosed with cutaneous vasculitis from January 2008 toDecember 2013.Results: The mean age was 33.7 (±17.89) years, with femalepredominance. The majority of patients were Malays (67.3%).Skin biopsy was performed in 110 (40%) patients. Thecommonest sign was palpable purpura (30.6%). Theaetiology remained elusive in 51.3% of patients. Commonidentifiable causes include infection (19.7%) and connectivetissue disease (10.2%). Extracutaneous features were notedin 46.5% of patients. Erythrocyte sedimentation rate andantinuclear antibody were raised in 124 of 170 and 27 of 175patients with documented results respectively. Cutaneousvasculitis was the presenting symptom in seven patientswith newly diagnosed systemic lupus erythematosus. AntiStreptolysin O Titre was positive in 82 of 156 patients withdocumented results. Despite antibiotics, 31.7% of them hadchronic lesions. Prednisolone alone was used in 20% ofpatients while 16.4% needed steroid-sparing agents. Mostpatients who needed systemic therapy (62%) hadunidentifiable aetiology. Among the 155 patients whoremained under follow up, 36.4% had chronic disease, onepatient succumbed due to septicaemia, and the rest fullyrecovered within three months. The presence of ulcerativelesion was significantly associated with developing chronicvasculitis (p=0.003).Conclusion: The clinical spectrum of cutaneous vasculitis inour population was similar to other studies. 8.Perceptions Of Dental Stress And Social Support Among Malaysian Dental Students

Objective: To explore the association between socialsupport and stress levels in preclinical and clinical dentalstudents in Malaysia. Method: A cross sectional surveyof dental undergraduate students was conducted at theFaculty of Dentistry, University of Malaya, UniversitiKebangsaan Malaysia and Universiti Sains Malaysia.Stress was measured using the Dental Environment Stress(DES) questionnaire. A DES-32 item was used to measurestress for the clinical students and DES-16 item for thepreclinical students. Four questions were used to measuresocial support. The total stress scores were standardizedfor comparison purposes. Results: A total of 357 (79.7%)preclinical and 417 (71.8%) clinical undergraduate dentalstudents responded to the questionnaires. The clinicalstudents experienced higher stress [mean standardizedDES score = 72.63, SD = 10.64] than preclinical students[mean standardized DES score = 70.19, SD=12.01]. Thetwo most stressful items reported by preclinical studentswere “fear of failing” and “examination and grades”.Among clinical students, the two most stressful items relatedto academic were “completing course requirement” and“fear of failing course” and items related to clinical sessionwere “fear of being barred due to the clinical schedule”and “patients late or absent”. Multiple regression analysesrevealed that low stress levels among preclinical studentswere significantly associated to a lot of contact withstudents of the same course. Conclusion: To some extent,social support does play a role in explaining differences inperceived stress, in particular among preclinical students.
9.The Impact of Dry Needling on Patients with Neck Pain in a Tertiary Hospital

Neck pain presents as a symptom of dull pain or discomfort mainly along the trapezius muscle. Dry needling is an
invasive procedure which uses acupuncture needle directed at myofascial trigger points. The aim of the study was to
evaluate the effectiveness of dry needling in managing patients with neck pain. A pre-test-post-test interventional
study design was used. Patient education package was provided to 32 respondents who fulfilled the inclusion criteria.
A 13-item Pain Catastrophizing Scale (PCS) questionnaire was used to assess Rumination, Magnification and
Helplessness. Subjective pain intensity was measured by Visual Analogue Scale (VAS). These questionnaires were
given before and after the dry needling intervention. The findings reported that respondents scored high in pre-test
total PCS score (27.41±13.652). Post-test result revealed a significant improvement in total PCS score
(23.06±13.938) (p = 0.000). Post-test VAS score (4.78±1.237) was also significantly better than pre-test
(6.47±1.414) (p = 0.000). There was no significant difference in pre-test PCS in terms of marital status (p > 0.05)
whereas there was significant difference between marital status and rumination in post-test (Z = -2.303, p = 0.021).
There was significant difference between pre-test magnification in terms of respondents’ occupation (p = 0.008) and
race (p = 0.035) but no significant difference in post-test. Respondents’ age group showed no significant differences
between pre-test and post-test PCS and VAS (p > 0.05). In conclusion, patients who received dry needling showed
improvement in pain intensity and catastrophizing towards neck pain.

10.Pre-osteoblastic MC3T3-E1 cells promote breast cancer growth in bone in a murine xenograft model.

The bones are the most common sites of breast cancer metastasis. Upon arrival within the bone microenvironment, breast cancer cells coordinate the activities of stromal cells, resulting in an increase in osteoclast activity and bone matrix degradation. In late stages of bone metastasis, breast cancer cells induce apoptosis in osteoblasts, which further exacerbates bone loss. However, in early stages, breast cancer cells induce osteoblasts to secrete inflammatory cytokines purported to drive tumor progression. To more thoroughly evaluate the role of osteoblasts in early stages of breast cancer metastasis to the bones, we used green fluorescent protein-labeled human breast cancer cell lines MDA-MB-231 and MDA-MB-435, which both induce osteolysis after intra-femoral injection in athymic mice, and the murine pre-osteoblastic cell line MC3T3-E1 to modulate osteoblast populations at the sites of breast cancer metastasis. Breast cancer cells were injected directly into the femur with or without equal numbers of MC3T3-E1 cells. Tumors grew significantly larger when co-injected with breast cancer cells and MC3T3-E1 cells than injected with breast cancer cells alone. Osteolysis was induced in both groups, indicating that MC3T3-E1 cells did not block the ability of breast cancer cells to cause bone destruction. MC3T3-E1 cells promoted tumor growth out of the bone into the extraosseous stroma. These data suggest that breast cancer cells and osteoblasts communicate during early stages of bone metastasis and promote tumor growth.
