2.Result of treatment of sternal infection after open heart surgery in 10 years (1996-2006)
Journal of Medical Research 2007;52(5):16-21
Background: In recent years, open heart surgery has developed rapidly in Vietnam. Sternal infection is a very serious complication for this surgery, it lead to increasing treatment costs and higher risk of mortality. Treatment of local infections is always a difficult technique in sternal wound infections after open heart operations. Objectives: (1) To improve and apply two techniques called “demi ?open + irrigation? and “closed ?Redon drainage? (2) To evaluate the effects of these techniques. Subjects and method: 20 cases of sternal infection (at Viet Duc hospital and some other hospitals in Ha Noi) were researched and described prospectively about diagnosis, techniques and treatment results. The subjects were divided in to 2 group based on the infection processing techniques. Results:Diagnosis had to reply on clinical signals (100%). Bacterial culture only provided 45% positive results, Pseudomonas aeruginosa (44.4%) were obtained more than Staphylococcus au’reus (22.2%). Both techniques have their own reasonable creativeness and the standardized process, improving the limitations of ordinary technique, providing high treatment effectiveness ?85% have recovered after only one operation. Conclusion: For treatment of local infections, two techniques are both very efficient and suitable with the conditions in Vietnam, especially “closed ?Redon drainage?technique.

Chronic wounds have always been a tough fight in clinical practice, which can not only make patients suffer from pain physically and mentally but also impose a heavy burden on the society. More than one factor is relevant to each step of the development of chronic wounds. Along with the in-depth research, we have realized that figuring out the pathophysiological mechanism of chronic wounds is the foundation of treatment, while wound infection is the key point concerned. The cause of infection should be identified and prevented promptly once diagnosed. This paper mainly describes the mechanism, diagnosis and therapeutic strategies of chronic wound infection, and will put an emphasis on the principle of debridement.

5.To further enhance the comprehensive prevention and treatment of burn infection.
Chinese Journal of Burns 2015;31(1):9-10
Comprehensive prevention and treatment of burn infection should be further enhanced, as monotonous treatment is prone to fail to get satisfying curative effects. In the articles to be published in this issue, causative factors for burn infection are analyzed in depth and discussed from different angles, and they will lay the foundation for the comprehensive prevention and treatment of burn infection.

7.Therapy experience and pathogeny analysis of intercurrent infection after augmentation mammaplasty.
Chinese Journal of Plastic Surgery 2004;20(6):412-414
OBJECTIVETo investigate the pathogeny diagnosis, therapy and some possible method for prevention of intercurrent infection, after the operation of augmentation mammaplasty.
METHODSComparing and analyzing the clinical representation and therapy of the seventeen intercurrent infecting case that we had, after the operation of the augmentation mammaplasty. The patients were treated by operating with different approaches.
RESULTSAfter the operation, all the infected patients recovered in a short while without back to surgery.
CONCLUSIONSThe infection after augmentation mammaplasty will get an effective controlling result if take a timely operative therapy according to the position and character of the intercurrent infection. But some galactophore will shrink after the cure of infection.

This article reports the treatment of a patient suffered from acute radiation burn covering 41% TBSA, with deep partial-thickness and full-thickness injury, produced by exposure to a large-scale industrial electron accelerator. An open wound began to appear and enlarged gradually 10 weeks after the exposure. Serious wound infection with methicillin-resistant Staphylococcus aureus and Pseudomonas aeruginosa, pneumonia, respiratory failure, systemic inflammatory response syndrome, nephropathy and hypoproteinemia developed successively since 3 weeks after the wound formation. Skin grafts failed to survive, resulting in enlargement of the wound. After being treated with proper measures, including parenteral nutrition, respiratory support with a ventilator, appropriate antibiotics, steroid administration for nephropathy, deep debridement for wounds followed by skin grafting, the patient was cured and discharged after undergoing 15 operations in 500 days. The clinical condition of an extensive acute radiation burn is complicated. We should pay close attention to the changes in functions of organs, and strengthen the therapeutic strategies to support the function of organs to reduce the incidence of systemic complications. The control of the infection and the timely and effective repair of the wound are still the key points of the treatment of an extensive local radiation injury.

9.Current practice in the prevention and management of surgical site infections in gastrointestinal surgery.
Chinese Journal of Gastrointestinal Surgery 2012;15(6):533-536
Surgical site infection(SSI) is one of post-operative complications in gastrointestinal surgery. SSI may increase the morbidity and mortality of surgical patients and increase their hospital stay and expense. The risk factors can come from patients, physicians and hospital environments. Improving patients nutritional status and organ function, appropriate control of blood sugar level and abstinence from smoking can reduce the occurrence of SSI. Compare to current practice in China, the following recommendations have been identified as priorities for implementation: hair removal done immediately before operation; maintenance of normothermia intraoperatively; the abdominal wall should be closed with an absorbable suture and drains should be removed as early as possible. SSI could be diagnosed by symptoms, local signs and lab examinations and confirmed by physician. Source control is the key point in the management of SSI. Ultrasound and CT guided percutaneous abscess drainage is effective in the localized deep space surgical site infection and critically ill patients. Antibiotics should be used following clinical assessment and evidence based on local formulary.

Chinese Journal of Burns 2015;31(5):389-391
The treatment of wound bacterial infection is an extremely difficult problem in clinic, especially in patients with large wounds which are infected by multidrug resistant, pan-resistant or omni-resistant bacteria. In recent years, with a grim prospect of antibiotic resistance, phage therapy is re-valued by researchers after being ignored for nearly half a century. Phage therapy has made great achievements in prevention and control of bacterial infection of open wounds. This review is mainly focused on the latest research progress of phage therapy in wound bacterial infection.
