1.Study of Outbreak of Infectious Gastroenteritis due to Norovirus

In 2006 there was a multiple outbreak of infectious gastroenteritis due to norovirus in health care facilities, old age nursing homes and other facilities in Hiroshima Prefecture. Also at JA Yoshida General Hospital during the period from October 3 to 17, 2006, symptoms of vomiting and diarrhea were observed in a total of 47 people including 29 in-patients in three out of six wards and 18 hospital staff members.This was reported to the public health center and an emergency meeting of the Infection Control Committee was convened. Under the guidance of the Public Health Center, the Infection Control Team (ICT) immediately had a good control over the situation, took infection preventive measures, informed patients of what had happened, and restricted hospital admissions and hospital visits. The number of new patients dropped to zero on October 17.After re-examining the gastroenteritis outbreak, the ICT reviewed the existing manual and prepared a new operation manual to cope withe future outbreaks of infectious gastroenteritis, including emergency measures to be taken, staff assignments, and improvement of the hospital environment. The ICT considers that there is a need for disseminating knowledge regarding measures against the infection and for making preparations for the future as a vanguard.

2.Cardiac Tamponade due to Detachment of the Aortic Valve Commissure

A 68-year-old man was taken to our hospital by ambulance due to syncope. He was in shock with cardiac tamponade. Pericardial drainage was performed. Aortic valve regurgitation gradually increased and surgery was performed at 25 days after onset. Surgical finding showed that there was a detachment of the commissure between the right and non coronary cusps of the aortic valve. An intimal tear was detected in the same place and aortic root replacement was required. The patient had a good recovery and he was discharged 14 days after surgery.
3.Switching from Conventional Fibrates to Pemafibrate Has Beneficial Effects on the Renal Function of Diabetic Subjects with Chronic Kidney Disease

Background:
Fibrates have renal toxicity limiting their use in subjects with chronic kidney disease (CKD). However, pemafibrate has fewer toxic effects on renal function. In the present analysis, we evaluated the effects of pemafibrate on the renal function of diabetic subjects with or without CKD in a real-world clinical setting.
Methods:
We performed a sub-analysis of data collected during a multi-center, prospective, observational study of the effects of pemafibrate on lipid metabolism in subjects with type 2 diabetes mellitus complicated by hypertriglyceridemia (the PARM-T2D study). The participants were allocated to add pemafibrate to their existing regimen (ADD-ON), switch from their existing fibrate to pemafibrate (SWITCH), or continue conventional therapy (CTRL). The changes in estimated glomerular filtration rate (eGFR) over 52 weeks were compared among these groups as well as among subgroups created according to CKD status.
Results:
Data for 520 participants (ADD-ON, n=166; SWITCH, n=96; CTRL, n=258) were analyzed. Of them, 56.7% had CKD. The eGFR increased only in the SWITCH group, and this trend was also present in the CKD subgroup (P<0.001). On the other hand, eGFR was not affected by switching in participants with severe renal dysfunction (G3b or G4) and/or macroalbuminuria. Multivariate analysis showed that being older and a switch from fenofibrate were associated with elevation in eGFR (both P<0.05).
Conclusion
A switch to pemafibrate may be associated with an elevation in eGFR, but to a lesser extent in patients with poor renal function.
