1.Clinical Analysis of 137 Eyes of Retinal Detachment Surgery with Drainage of Subretinal Fluid.

Subretinal fluid drainage produces the indentation of the sclera by facilitating firm adhesion between the retina and either retinal pigment epithelium or Bruch's membrane and has the advantage that show the state of fundus and degree of indentation of sclera accurately during the operation. Becaulse it has complications such as choroidal bleeding, retinal incarceration, iatrogenic retinal break, choroidal detachment and loss of vitreous, it should be done carefully. The authors analyzed the clinical data of 128 cases(137 eyes) of retinal detachment surgery with subretinal fluid drainage at the Department of Ophthalmology, Pusan Paik Hospital, College of medicine, Inje University from May, 1988 to April, 1990. And the results were as follows: 1. The most common operative method of rhegmatogenous retinal detachment were explant buckling and encirclement with subretinal fluid drainage, and the success rate of first operation was 86.13% and the overall success rate of rhegmatogemous retinal detachment was 91.2% in 137 eyes of operation. 2. In 116 cases(92.8%) who were done subretinal fluid drainage, absorption of residual subretinal fluid took in a day. 3. The most common cause of surgical failure was proliferative vitreoretinopathy. 4. The most common complication was choroidal hemorrhage and it was developed 14 eyes(10.2%) mong 137 eyes. 5. The preoperative duration of symptom and extent of detachment were influenced to the surgical success rate and they had statistical significance(p<0.05). 6. The power of refraction was changed to myopia and ERG b-wave amplitude were increased and 89 eyes out of 137 eyes(69.6%) showed an improvement of vision at the three months after retinal detachment surgery.
2.Comparative Analysis of Primary Retinal Detachment on Pre and Post 40 years.

Authors analysed clinically two groups(group A<40 years: 187 eyes, group B>40 years: 126 eyes) of 313 primary retinal detachment patients who have received operation and subsequent follow-up study for at least 3 months. The frequency by age was shown double peak at twenties and at fifties. Retinal dialysis and inferior temporal breaks in group A was shown more than those in group B, but superior temporal breaks was more occured in group B. The success rates of the first operation in group A were a little higher than those in group B. And in the case of successful eyes by the first operation, residual subretinal fluid were absorbed less than 1 day. Fish mouthing, glaucoma and chroidal detachment as intraoperative and postoperative complication occurred more frequently in group B. After the first operation, inadequate absorption of the subretinal fluid was more frequent in group B, recurrence of hole was more frequent in group A, but there's no statistical significance in above.

3.Hemorrhagic Retinopathy Associated with Sexual Activity.

A 70 year old male patient experienced a sudden visual acuity decrease in his left eye below 0.1 with preretinal, retinal and subretinal hemorrhage after excessive sexual activity. Four months later, his vision improved in the affected eye without any treatment. Eight months later, there was an area of organized sugbretinal hemorrhage remained around vascular arcade. We report this case with case review.

4.Comparision of Clinical Findings between Phakic Retinal and Pseudophakic Retinal Detachment.

We reviewed our medical records of primary retnal detachment(239 patients, 243 eyes) to compare the clincal findings of phakic retinal detachment(206 eyes) and pseudophakic retinal detachment(37 eyes). Pseudophakic retinal detachment was more common in male(73.0%) than in female(27.0%) and occurred more frequently in aged 60 years or more than phakic retinal detachment(10.4%)(P<0.001). Associated ocular finding such as high myopia(35.0%), peripheral retinal degeneration(23.3%0, retinal tear in fellow eye(5.8%) were more commonly seen in phakic retinal retinal detachment. Rupture of posterior capsule(40.5%), high myopia(18.9%), small pupil(13.5%) were associate with pseudophakic retinal detachment. In pseudophakic retinal detachment, more than 3 quadrant retinal detachment(73.0%) and macular involvement(78.4%) were more common than in phakic retinal detachment(P<0.01), but atropic hole(29.7%), or 2 or more retinal break(10.8%) were less common than in phakic retinal detachment(P<0.01) where the success rate of first operation(59.5%) and the overall success rate(81.1%) were lower in paeudophakic retinal detachment than phakic retinal detachment. The degree of visual recovery was nearly equal between two.

6.CHANGES IN CEREBRAL OXYGENATION DURING A SINGLE BOUT OF COMBINED CIRCUIT TRAINING

The purpose of present study was to investigate the cerebral oxygenation and oxygen uptake (VO2) during exercise of a combined circuit training (CCT) and a circuit resistance training (CRT). Nine healthy young male subjects performed the following two trails on separate days: 1) CCT trail (three circuits of aerobic exercise for 5 min at 50%VO2max and 1 set of four resistance exercises at 50% one-repetition maximum) and 2) CRT trail (six circuits of the same resistance exercises and intensity as for CCT without aerobic exercise). Exercise duration of these trails was 30 minutes. Cerebral oxygenation was determined by near infrared spectroscopy, and VO2 was measured by breath by breath methods. Cerebral oxygenation and VO2 were continuously monitored during the exercise. Oxyhemoglobin and total hemoglobin concentration during CCT trail was significantly higher than during CRT trail (p<0.05). Average VO2 and energy expenditure during CCT trail were significantly higher than during CRT trail (p<0.05). These results suggest that a single bout of circuit resistance training combined with aerobic exercise induced greater energy expenditure and cerebral oxygenation than those induced by a resistance training with the same exercise duration.
7.A Case of Uterine Fibroids Necrosis after Transarterial Embolization for Treatment of Uterine Fibroids.

Uterine fibroids are the most common benign tumor of the female genital tract. Traditional therapy for symptomatic fibroids has been either myomectomy or hysterectony, depending on whether futhur fertility is desire. A promised new altermative therapy-embolization of the uterine arteries- is now available. A 29 year-old woman was diagnosed of uterine fibroid by ultrasonogram and other techniques. We have experienced one case of uterine fibroid necrosis after transarterial embolization. Now we report a case of uterine artery embolization for treattement of uterine fibriods with a brief review.

8.The Controlled Study between The Group Used Highly purified FSH and The Group Used FSH/hMG in Controlled Ovarian Hyperstimulation for In Vitro Fertilization and Embryo Transfer.

OBJECTIVE: It is known that normal follicular growth and pregnancy are possible using Follicular stimulating hormon(FSH) alone during controlled ovarian hyperstimulation(COH) for IVF-ET. The purpose of this study was to compare and analize the results of COH for IVF-ET between a group using only FSH-HP and a group using FSH/hMG METHOD: 111 cycle of the total 280 COH for IVF-ET cycles belonged to the FSH-HP group, and the FSH/hMG group accounted for remaining 169 cycles. The amount of gonadotropin administered, the duration of hormonal therapy, the number of embryos transfered, the endometrial thickness, and the pregnancy rate were compared between the two groups and statistically analized using the t-test. p values less than 0.05 deemed statistically significant. RESULT: There were no statistical differences on the amount of gonadotropin administered, the duration of hormone therapy, number of oocytes retrieved, nor the endometrial thickness between the two groups. However,the serum E2 level was significantly lower, and the number of embryos of high quality(GradeIandII) significantly higher in the FSH-HP group. In pregnancy rate, FSH-HP group shows higher than FSH/hMG group(38.8% VS 34.2%). but the difference was not large enough to be considered statistically significant. CONCLUSION: The results of this study show that compared to using FSH/hMG, treatment with FSH-HP alone allowed the retrieval of more embryos of high quality, and although not statistically significant, achieved a higher pregnancy rate, thus indicating that it can be used as an effective method for COH.

9.Clinical analysis of Carpal Scaphoid Fracture

Scaphoid fracture is the most common carpal bone fracture and its treatment is frequently delayed due to difficulties of diagnosis. Nonunions and avascular necrosis are commonly developed due to peculiar blood supply. Though most of the undisplaced fracture, diagnosed early, can be managed easily by plaster of Paris immobilization, many cases of delayed diagnosis, displaced fractures and nonunions are difficult to treat and its outcome is not uniform. Ruse reported good results by iliac bone graft via volar approach and several internal fixation methods are reported such as K-wire, compression screw, Herbert screws etc. Recently, many suthors advocated accurate anatomical reduction in fear of volar and dorsal intercalary segmental instability. We reviewed 30 cases of scaphoid fracture at the department of orthopedic surgery, Kyungpook national university hospital, from June, 1986 to April, 1989 and obtained the following results. l. Among 30 cases of scaphoid fractures, 27 cases(90 %) were males. 2. Twenty one cases(70%) were between second decade and third decade. 3. The fracture mechanism is hyperdorsiflexion of the wrist during slip down or fall down in 21 cases(70%). 4. Right side is 17 cases(57%) and left 13 cases. Middle 1/3 fracture occupies 24 cases(80%) in anatomical location. 5. Among 15 cases of the fresh fracture, 11 cases(93%) attained bone union by plaster pf Paris immobilization. 6. Among 18 cases of operation, 3 cases were acute transscaphoid perilunar dislocation(TSPD), 1 case was delayed TSPD, 5 were delayed unions and 9 were monunions. Operative methods were Herbert screw fixations in 8, Herbert screw fication with iliac bone graft in 4, Russe's bone grafts in 6 cases. 7. The results of the treatment were classified according to the bone union status and range of motion of the wrist. In operative treatment, excellent and good results were obtained in 6 and 11 cases respectively, poor result in 1 case. In conservative treatment, excellent and good results in 6 and 5 cases respectively, fair in 1 case.
