2.Detection of formaldehyde in textiles.

Formaldehyde is contained in various everyday products such as shampoos, detergents, leathers and papers, and is thought to be one of the important couses of clothing related contact dermatitis(ACD). The results about the minimum sensitizing concentrations of formaldehyde were different, and the concentration of 30 pg per gram of textiles was the least one. We measured the quantity of formaldehyde in 16 different types of textiles which are widely used in Korea to investigate whether the textiles contain an enough amount of formaldehyde to induce ACD and washing can reduce the amount of it. The results were as follows : 1. The quantity of formaldehyde ranged from 3 to 1246 pg per gram of textiles. 2. Three types textiles contained more than 100pg of formaldehyde per gram of textiles and 6 between 50 pg and 100pg. 3. 1.00% cotton, ramie and rayon contained the highest content of formaldehyde per gram fabric. 4. After washing, the average quantity of formaldehyde decreased to 60% of that, of pre-wash measure.

3.TCA chemical peeling.

Tricholroacetic acid (TCA) chemical peeling is an effective, versatle and safe therapeutic method for patients with pigmentary disorders, acne scars and aging skin. With an experienced and skilled operator, the associated complications are rare and tend to be mild. No. ystemic toxicity is known to be induced by TCA. We observed the effect of TCA chemical peeling, evaluated by 242 patients themselves with melasma, freckles, senile lentgines, pigmented nevi, seborrheic keratosis, skin tags or acne scars. The patients evaluated TCA peeling as excellent for the treatment of seboirheic keratosis, pigmented nevi, skin tags and acne scars, and as selectively recommendable for the treatment of melasma, freckles and senile lentigines.

5.Congenital Tracheoesophageal Fistula without Atresia of the Esophagus.

The congenital tracheoesophageal fistula without atresia of the esophagus is considered a rare variant, and the literature concerned to the tracheoesophageal fistula fistula without atresia is little in adult especially. The 22-year-old male was admitted to Yeungnam University Hospital with his chief complaints of weight loss (6 kg/6 months), abdominal discomfort, and intermittent coughing. The diagnosis was made by the endoscopy and esophagography. The fistula was 1.5 cm in diameter, 0.5 cm in length. The level was around second thoracic vertebra. The operation was performed transpleurally through the right third intercostals space and the fistula was secured with interrupted silk suture after division. The fibrotic adhesion was seen around the tracheoesophageal fistula. The postoperative course was uneventful, and postoperative esophagogram revealed no extraluminal leakage. Herewith we report this unusual case of isolated tracheo-esophageal fistula with review of literatures.

6.Effect of UVB Phototherapy on Pruritus of Pityriasis Rosea.

Pityriasis rosea is sometimes accompanied with considerable pruritus. Ultraviolet B irradiation is reported to be beneficial in relieving pruritus. The purpose was to evaluate the effect of UVB phototherapy on severe pruritus of pityriasis rosea. Nine patients with severe pruritus had UVB irradiation three times a week Five patients became symptom free and three showed a marked improvement. One dropped out. UVB phototherapy could be an effective modality for relieving pruritus in pityriasis rosea.

7.A Morphometric Study of Glomerular Dimensions in Relation to Glomerular Location, Age and Sex in Koreans.

As measurement of glomerular size in the assessment of several renal diseases becomes increasingly important, it has become necessary to devise rapid simple methods for the assessment of glomerular size and to have on hand reference ranges. A few reports on glomerular size have been published in Western literature, but their body builds are different from Koreans. In this study, 100 glomeruli(50 glomeruli each from the outer cortical and the juxtamedullary area) were measured in sections taken from 74 kidneys(ages 3 days~73 years) obtained from autopsy utilizing the semi-automatic image analyser. The percentage of glomerular sclerosis was measured based on its location. The sphere diameter, maximum diameter, area and sphere volume of non-sclerotic glomeruli were measured and evaluated with respect to age, sex and the location of the glomeruli. The results were as follows; 1) Mean glomerular dimensions including sphere and maximum diameter, area and sphere volume increased until 40 years of age, then reached a plateau. The percentage of sclerotic glomeruli then increased slowly with age but without statistical significance. 2) The glomerular dimensions and sclerosis showed no significant differences according to sex. 3) Juxtamedullary glomeruli were larger than the outer cortical ones which was statistically significant in age groups of 0~10, 11~20 and 41~50 years. The percentage of sclerotic glomeruli was generally greater in the outer cortex. 4) Differences in the values of glomerular dimensions between outer cortical and juxtamedullary area were similar in all age groups. 5) All parameters of measurement showed consistent and similar trends between the different groups. 6) The measurements of the largest 12 glomeruli out of randomly-selected 50 glomeruli gave similar results when compared with those of 50 glomeruli. It was evident from our results that glomerular size is influenced by age and glomerular location, but not by sex. The method of assessing glomerular size used in this study will not necessarily give the true, absolute value of size but it may be a simple, practical and useful method of comparing glomerular size in different groups of patients.
8.A Prospective Study of Comparison of Misoprostol and Dinoprostone for Cervical Ripening and Labor Induction.

OBJECTIVE: To compare the safety and efficacy of intravaginal misoprostol versus intracervical dinoprostone (prostaglandin E2 gel) for cervical ripening and labor induction. METHODS: 60 patients with indication for labor induction and unfavorable cervices were randomly assigned to receive either intravaginal misoprostol or intracervical dinoprostone. 50 microgram tablets of misoprostol were placed in the posterior vaginal fornix every 4 hours for a maximum of 3 doses or dinoprostone 0.5mg was placed into the endocervix every 6 hours for a maximum of 2doses. No more medication was given after either spontaneous rupture of membranes or beginning of active labor. RESULTS: Among 60 patients enrolled, 30 received misoprostol and 30 received dinoprostone. The average interval from start of induction to active labor was shorter in misoprostol group (6.5+/-3.2 hours) than in the dinoprostone group (10.7+/-7.3 hours) (p<0.05). Oxytocin augmentation of labor occurred more often in the dinoprostone group (36.7%) than in the misoprostol group (10.0%) (p<0.05). There was a higher prevalence of fetal distress (23.3% versus 3.3%) and tachysystole (16.6% versus 6.6%) in the misorprostol group than in the dinoprostone group(p<0.05). CONCLUSIONS: Vaginally administered misoprostol is an effective agent for cervical ripening and induction of labor. Furthermore, the cost of misoprostol ( 360/200microgram) is much less than that of dinoprostone ( 42,000/0.5mg). Cost benefits from administration of misoprostol are evident, especially in clinics under system of diagnosis-related group (DRG) : however when given at this dosage, it is associated with a higher prevalence of fetal distress and tachysystole than dinoprostone. Further studies to compare the safety of misoprostole to that of dinoprostone and to delineate an optimal dosing regimen for misoprostol are needed.

10.Urinary Tract Stone in Patients with Spinal Cord Injury: A Retrospective Radiological Study.

PURPOSE: To compare the incidence between author's first and current report on urinary tract stone in patient with spinal cord injury and to evaluate the effectiveness of recent developed in medical technology and care on in treating the patients. MATERIALS AND METHODS: We reviewed urinary tract stone in 257 patients with paraplegia or quadriplegia after spinal cord injury. These patients were diagnosed retrospectively by KUB and intravenous urography at the Korea Veterans Hospital during 10years from January, 1984 to December, 1993. We evaluated and compared the overall incidence, incidence of specific location of urinary tract, recurrent rate, incidence according to the level of spinal cord injury, and the duration of development in urinary tract stone. RESULTS: Total patients were 257 with 186(72.4%) paraplegia and 71(27.6%)quadriplegia. Overall incidence of the stone was 16.0% in this study and 38.1% in the first study. Incidence of the stone in individual organ ;5.5% in kidney, 1.2% in ureter, and 13.6% in urinary bladder. The recurrent rate was 29.3% in this study and 40.6% in the first study. Incidence of the stone according to the level of spinal cord injury was as follows;15.6% in cervix, 17.1% in upper thorax, 17.9% in lower thorax and 13.9% in lumbar. The stone developed during the first 4 years and between 12 to 16 years following spinal cord injury was 28.3% each. CONCLUSION: Overall incidence and recurrent rate of urinary tract stone was obviously decreased since the first study. Highest incidence of the stone occurred in urinary bladder and in patient with lower thoracic spinal cord injury, which is similar to first report. Peak incidence of the stone was in the first 4years, and another peak was in 12-16years after spinal cord injury. The decreased overall incidence of urinary tract stone maybe attributable to the development in medical technology and care, and active rehabilitation.
