1.Review of cases of hyponatraemia in the Port Moresby General Hospital between August 1993 and June 1995

Hyponatraemia (serum sodium level below 130 mmol/l) is a common electrolyte abnormality in a hospital population. It can be associated with dehydration, overhydration or normal hydration. Clinically, it is important to recognize the common diseases associated with hyponatraemia since correct treatment in terms of fluid replacement is essential in preventing complications of low serum sodium. We have reviewed results of serum sodium tested from patients admitted to the Port Moresby General Hospital between 1993 and 1995. This was aimed at identifying the most common features associated with low sodium. Clinical information and diagnosis were obtained by looking through a series of request forms. Of the approximately 30,000 blood samples taken over 23 months, the percentage of samples with hyponatraemia was about 1%. Hyponatraemia was more common in medical (38%) and paediatric (35%) cases and at the extremes of ages, ie, under the age of 6 years and above 40 years. Over a quarter of the hyponatraemic patients had severe hyponatraemia (serum sodium below 120 mmol/l). Clinical conditions commonly associated with hyponatraemia, in descending order of importance, were diarrhoea and vomiting, renal failure, central nervous system infections and trauma, pulmonary infections, oedematous states (eg, nephrotic syndrome) and diabetes mellitus.

3.Clinical features and prognosis analysis of small-cell lung cancer complicated with hyponatremia.

OBJECTIVE:
To evaluate the clinical features and prognosis of small-cell lung cancer (SCLC) complicated with hyponatremia.
METHODS:
The clinical data of 158 patients with SCLC of the Second Xiangya Hospital from January 2000 to December 2007 were studied retrospectively.
RESULTS:
The incidence rate of hyponatremia in SCLC was 42.4%(67/158). The median survival time was 7.6 months in patients with subnormal serum sodium, and 14.1 months in patients with normal values. There was significantly different between 2 groups (P<0.001). The patients who did not fully regain normal values of serum sodium, had poorer survival (6.2 months) compared with the patients with normal serum sodium (10.3 months, P=0.044).
CONCLUSION
The incidence rate of hyponatremia in SCLC is high. The prognosis of SCLC with hyponatremia is very poor, especially in those who can not regain normal values of serum sodium after the treatment. So diagnosis and treatment at early stage are very important.

4.Influence of preventive use of vasopressin tannate on diabetes insipidus and serum sodium at the early postoperation of craniopharyngioma.

To explore the influence of preventive use of vasopressin tannate on diabetes insipidus and serum sodium at the early postoperation of craniopharyngioma.
Methods: The data of 83 patients, who underwent unilateral sub-frontal approach resection of craniopharyngioma between 2010 and 2014 by the same senior neurosurgeon, were retrospectively analyzed. The patients were divided into a vasopressin tannate group (used group) and a control group. The diabetes insipidus and serum sodium changes were compared between the two groups.
Results: Compared with the control group, the incidence of diabetes insipidus decreased at the early postoperation in the vasopressin tannate group (P<0.05). There was high incidence of diabetes insipidus in patients with pituitary stalk excision and tumor close adhesion to the third ventricle floor at the early postoperation (P<0.05). Under such conditions, the incidence of diabetes insipidus in the vasopressin tannate group was decreased compared with the control group (P<0.05). Postoperative hypernatremia occurred in 37 patients (44.6%), and hyponatremia occurred in 60 patients (72.3%), the average time of the occurrence of hpernatremia and hyponatremia was 1.4 and 3.7 days after surgery. Postoperative high serum sodium and low serum sodium appeared alternately in 19 patients (22.9%). There was significant difference in the serum sodium distribution in the first day after surgery in both groups (P<0.05), and the percent of hpernatremia in the vasopressin tannate group was significantly less than that in the control group (P<0.05).
Conclusion: Preventive use of vasopressin tannate can effectively reduce diabetes insipidus and hypernatremia incidence at the early postoperative stage after microsurgery for craniopharyngioma.

5.Prognostic value of hyponatremia in heart failure patients: an analysis of the Clinical Characteristics and Outcomes in the Relation with Serum Sodium Level in Asian Patients Hospitalized for Heart Failure (COAST) study.

BACKGROUND/AIMS: Hyponatremia is a well-known risk factor for poor outcomes in Western studies of heart failure (HF) patients. We evaluated the predictive value of hyponatremia in hospitalized Asian HF patients. METHODS: The Clinical Characteristics and Outcomes in the Relation with Serum Sodium Level in Asian Patients Hospitalized for Heart Failure (the COAST) study enrolled hospitalized patients with systolic HF (ejection fraction < 45%) at eight centers in South Korea, Taiwan, and China. The relationship between admission sodium level and clinical outcomes was analyzed in 1,470 patients. RESULTS: The mean admission sodium level was 138 +/- 4.7 mmol/L, and 247 patients (16.8%) had hyponatremia defined as Na+ < 135 mmol/L. The 12-month mortality was higher in hyponatremic patients (27.9% vs. 14.6%, p < 0.001), and hyponatremia was an independent predictor of 12-month mortality (hazard ratio, 1.72; 95% confidence interval, 1.12 to 2.65). During hospital admission, 57% of hyponatremic patients showed improvement without improvement in their clinical outcomes (p = 0.620). The proportion of patients with optimal medical treatment was only 26.5% and 44.2% at admission and discharge, respectively, defined as the combined use of angiotensin-converting-enzyme inhibitor/angiotensin receptor blocker and beta-blocker. Underuse of optimal medical treatment was more pronounced in hyponatremic patients. CONCLUSIONS: In hospitalized Asian HF patients, hyponatremia at admission is common and is an independent predictor of poor clinical outcome. Furthermore, hyponatremic patients receive less optimal medical treatment than their counterparts.

6.Analysis of prognostic factors for patients with acute-on-chronic liver failure.

OBJECTIVETo analyze the prognostic factors for patients with acute-on-chronic liver failure, and to build a scoring system for assessment of the prognosis of liver failure.
METHODS480 patients with acute-on-chronic liver failure in our hospital from January 2006 to June 2008 were enrolled in this study. The patients were divided into improved group and deteriorated group. The clinical data were analyzed by using chi square test, independent-Samples T Test and Binary logistic regression.
RESULTSThe factors that significantly affected the prognosis of Acute-on-chronic Liver Failure included age, hepatitis or liver cirrhosis, Staging, Hyponatremias, alpha-fetoprotein (AFP), the prothrombin time activity (PTA), total bilirubin (TBil), creatinine (Cr), albumin (ALB) and Hepatic encephalopathy, ascites, alimentary tract hemorrhage (P less than 0.05, P less than 0.01). PTA, Hyponatremias, hepatitis or liver cirrhosis, Hepatic encephalopathy and alimentary tract hemorrhage were independent risk factors of prognosis.
CONCLUSIONPTA, Hyponatremias, hepatitis or liver cirrhosis, Hepatic encephalopathy and alimentary tract hemorrhage are important to build a scoring system to assess the prognosis of Acute-on-chronic Liver Failure and may be useful to guide clinical treatment.
