We describe a patient with hyponatremia induced by the use of angiotensin-converting enzyme (ACE) inhibitor; imidapril HCl. Although the mechanism of severe hyponatremia due to ACE inhibitor is not clear, it is conceivable that ACE inhibitor therapy may complicate the syndrome of inappropriate secretion of antidiuretic hormone and induce hyponatremia. In addition, the possibility should be considered that hyponatremia in our patient is a presumptive interaction between oxcarbazepine and imidapril HCl.

9.Dilutional Hyponatremia with Generalized Convulsion in Post-anesthctic Period.

Dilutional hyponatremia secondary to fluid retention in the postoperative period may be related to a persistent and inappropriate secretion of ADH, possibly orginated from surgical stress, pain, fear and others. It is also serious and over 50% mortality when it should be combined with neurological feature, especially generalized convulsion. But this phenomenon is transitory and complete curable with active management. So Anesthesiologists should be aware of this symptoms by measure of elctrolytes is postanesthetic period. We experienced 2 cases of dilutional hyponatremia with convulsion in postoperative period.

Post-operative hyponatremia is associated with fluid overload and inappropriate secretion of antidiuretic hormone (ADH), which is related to stress, pain and others. Hyponatremia is a frequently encountered condition in clinical practice but severe post-operative hyponatremia has rarely been reported. Hyponatremia is curable; but severe hyponatremia is a fatal and emergency. Therefore, we should be aware of this phenomenon, diagnosis and management. We experienced one case of severe hyponatremia with generalized convulsion and neurologic symptoms in post-operative period.
