1.The role of a multidisciplinary team in diagnosing and treating esophageal cancer

Esophageal cancer treatment mode has currently been transformed from surgery alone to multidisciplinary treatment mo-dalities based on the standardization of operation in combination with the reasonable application of perioperative radiotherapy, che-motherapy, and other comprehensive treatment. Although controversies concerning accurate preoperative staging, surgical approach, extent of lymph node dissection, and perioperative chemotherapy regimens still exist, a multidisciplinary team can provide an optimal mode of diagnosis and treatment. A multidisciplinary approach embodies individualized tumor treatment and standardization princi-ple to the maximum extent, as well as prolongs patients' survival time and improves the quality of life.
2.Variety and significance of Intermedin on hypoxic pulmonary arterial hypertension in rats

Objective To observe and compare the changes of Intermedin's contents in plasma and bron-choalveolar lavage fluid (BALF) in rats with hypoxic pulmonary hypertension. Methods male Wistar rats were ran-domly divided into control group and 1,2 and 3 weeks hypoxia group. Intermedin's contents in plasma and bronchoal-veolar lavage fluid were measured by radioimmunoassay. Results Intermedin's contents in plasma and bronchoalveo-lar lavage fluid were markedly increaseded compared with control group. Meanwhile transformational trend is consist-ent between plasma and bronchoalveolar lavage. Conclusions Intermedin's contents in plasma and bronchoalveolar lavage increase ,which shows that there is protection and moderation to hypoxic pulmonary hypertension.
3.Study on the clinical and coronary features of patients with premature coronary heart disease

Objective To discuss the risk factors and clinical manifestation of coronary lesions in patients with premature coronary heart disease. Methods Coronary angiography was done in 415 coronary heart disease (CHD) patients. Patients were devided into two groups by age. One hundred and eighty patients with premature CHD (male≤55 years-old,female≤65 years-old) were compared with 235 patients with mature CHD to illustrate the risk factors of premature CHD and the clinical features of coronary lesions. 4.THE INFLUENCE OF JINLANBAO ON IMMUNOLOGICAL FUNCTION IN MICE

JINLANBAO (JLB), a preparation extracted from the Pecten Argopecten irradiaus and some Chinese medicinal herbs,has been reported to have several immuno-mod-ulatory effects in mice In this paper,the influence of JLB on the immuivlogcai functions in mice is presented. The results showed that JLB could enhance the phagocytosis of intraperi-toneal macrophages (Mo) inhibited by methyl-prednisone,increase the IgM level in CTX-de-pressed cells in a dose-dependent manner in the medium-dose and high-dose group. JLB has been found to have a two-way regulatory effect on ConA-induced T-lyrophocytes prolifera tion, tea promoting effect in low dose group,and an inhibiting effect in high dose group. As an immunopotentiator, the potential values and prospects for the developing of JLB should be paid attention to in pharmaceutical sciences.
6.Effects of propofol and etomidate on inflammation and oxidative stress in septic mice

Objective To observe the effects of propofol and etomidate on inflammation and ox-idative stress in septic mice.Methods Sixty-four male adult BALB/c mice were randomly divided into four groups:normal control group (N),sepsis group (S),propofol treatment group (P)and etomid-ate treatment group (E).The septic mice model was established by lipopolysaccharide (LPS,20 mg/kg)intraperitoneal injection,and propofol (60 mg/kg)or etomidate (10 mg/kg)was injected in the abdominal cavity at 0.5 h after LPS injection.Serum interleukin-6 (IL-6)concentrations and in-terleukin-10 (IL-10)concentrations were measured at 2 h and 6 h after LPS injection;malondialde-hyde (MDA)content of lung,liver and kidney tissue was measured at 6 h after LPS injection. Results Compared with group N,serum IL-6 concentrations increased significantly (P <0.05),and IL-10 concentrations decreased significantly (P <0.05)at 2 h and 6 h after LPS injection in group S;MDA content of lung,liver,kidney increased significantly (P <0.05 )at 6 h after LPS injection in group S;Compared with group S,serum IL-6 concentrations decreased significantly (P <0.05),and IL-10 concentrations increased significantly (P < 0.05 )at 2 h and 6 h after LPS injection in both group P and E;MDA content of lung,liver,kidney decreased significantly (P <0.05 )at 6 h after LPS injection in group E,but only MDA content of lung decreased significantly (P <0.05)at 6 h af-ter LPS injection in group P;Compared with group P,serum IL-6 concentrations was significantly lower (P <0.05),and IL-10 concentrations was significantly higher (P <0.05)at 2 h and 6 h after LPS injection in group E;MDA content of lung,liver,kidney was significantly lower (P <0.05)at 6 h after LPS injection in group E.Conclusion Both propofol and etomidate injected in the abdominal cavity can reduce injury of inflammatory and oxidative stress in septic mice induced by LPS,and the effect of etomidate is more significant.
7.Practices and experiences of traditional Chinese medicine knowledge and skills training on non traditional Chinese medicine community physicians

Objective To evaluate the effect of the traditional Chinese medicine (TCM) knowledge and skills training on non TCM community physicians in Shanghai, and provide important basis and feedback for future adjustments and training improvement.Methods The current situation of the TCM knowledge and skills training on non TCM community physicians in Shanghai was reviewed. The community physicians participated in the training courses in 2014 in Jiading district were randomly selected for investigation. The TCM theory and skills scores of all the 65 trainees were compared before and after training, a self-made questionnaire survey on assessment of training was conducted after training, and the influencing factors of training effect were analyzed. Results After training,test results of TCM theory and skills were significantly higher than before(P<0.01). According to the survey of trainees after TCM training, 84.6% thought the training was necessary,95.4% liked the teaching form, 93.9% accredited training content difficulty level,92.3% thought the training was practical, 98.5% were willing to provide Chinese medicine services for community residents, 60.0% thought training to promote the general medical work was very helpful. The existing problems were short training periods(43.1%), only 10.8% could skilled TCM appropriate technologies and 60.0% were basically master. Conclusion The TCM knowledge and skills training on non TCM community physicians in Shanghai had achieved better effect. There still were some deficiencies, which require further exploration in order to better carry out future training.
8.MRI and pathological features of different molecular subtypes of breast cancers

Objective To investigate the MRI and pathological features of different molecular subtypes of breast cancer.Methods The data of 202 patients who underwent primary breast cancer resection were retrospectively reviewed.All of the patients had MRI preoperatively.The molecular subtypes of breast cancer defined by immunohistochemistry were classified as basal-like,luminal and HER-2 overexpression.Morphology (including mass or non-mass like enhancement,shape and margin of masses,unifocal or multifocal masses) and enhancement characteristics on MRI,histologic types and grades of tumors were analyzed with Chi-square test,exact test,Fisher exact test,Kruskal-Wallis H test,and Wilcoxon test.Results Among the 202 patients,34 were basal-like,144 were luminal and 24 were HER-2 overexpression.The number of mass cases in each subtype was 29,133 and 19 respectively,making no significant difference (x2 =4.136,P =0.126).As for the shape of basal-like lesions,8 were round,19 were lobular and 2 were irregular,while this distribution was 23,58,52 in luminal subtype and 1,11,7 in HER-2 overexpression subtype (x2 =13.391,P < 0.05).The margin was also strikingly different among three groups (smooth,spiculated,irregular):20,5,4 respectively in basal-like,27,53,53 respectively in luminal,and 4,7,8 respectively in HER-2 overexpression (x2 =28.515,P < 0.01).52.6% (10/19) of HER-2 overexpression cases were multifocal,while only 6.9% (2/29) of luminal and 8.0% (24/133) of basal-like ones were multifocal (x2 =16.140,P < 0.01).Characteristics in dynamic contrast-enhanced MRI were statistically different,with homogeneous,heterogeneous,and rim enhancement 0,13,16 respectively in basal-like cases,28,93,11 respectively in luminal cases and 2,11,6 respectively in HER-2 overexpression cases (P < 0.01).However,the difference for enhancement curve did not reach significance (P =0.457).Histologic types were significantly different among molecular subtypes (P < 0.01).Luminal breast cancer consisted of various histologic types,32.6% (47/144) of which were mixed type.The majority of the other two molecular subtypes were invasive ductal carcinoma.Furthermore,invasive ductal carcinomas with different molecular subtypes showed different histologic grades (Hc =30.014,P < 0.01).Basal-like breast cancer was more likely associated with a higher grade of malignancy.Conclusions Different molecular subtypes of breast showed distinct MRI features and pathologic characteristics.MRI might be a useful tool for preoperative prediction of molecular subtypes of breast cancer.
9.Practice and thinking of quality management of organ procurement organization

BACKGROUND:With the introduction of related policies on organ procurement and distribution, the organ procurement and standards of the organ procurement have a clear operational criteria, as wel as how to al ocate organs. But the work processes and organizational positioning on organ procurement organization are not enough elaborated and normal. OBJECTIVE:To analyze the experience of Guangzhou General Hospital of Guangzhou Military Command as the organ procurement organization units in quality management, timely review working progress and explore areas for improvement, which provide the corresponding countermeasures for enhancing quality management and establishing the efficient and professional organ procurement organization. METHODS:A computer-based online search in CNKI database and PubMed database was performed with the keywords of“organ donation, organ transplantation, Organ Procurement Organization, quality management”in English and Chinese by screening titles and abstracts. RESULTS AND CONCLUSION:According to relevant laws and regulations, Guangzhou General Hospital of Guangzhou Military Command formulates a standard evaluation standard and sets up the team, thus achieving the reunification of the responsibilities and powers. At the same time, they improve the relevant organ donation,acquisition, preservation, registration, and design different quality management plans according to these different aspects. In addition, In addition, the hospital hold regular coordinator training and make it standardization. Through a series of quality management measures, the hospital organ procurement organization has gradual y regularized and achieved some results. It is recommended to further strengthen scientific research, achieve the unification of efficiency and effectiveness of scientific research;improve the awareness of the hospital leaders, consummate the mechanism of organ procurement;focus on personnel training, improve the professional quality of the organization members;enhance information management, achieve multi-disciplinary col aboration;build quality evaluation system, achieve the whole process of quality management;emphasize on medical ethics, and promote the harmony between the hospital and the patient.
10.Cardioprotective effects of Guanxinshutong (GXST) against myocardial ischemia/reperfusion injury in rats

Background The protective effects against reperfusion injury of cardioprotective drugs have recently been evaluated and found to be inadequate. Guanxinshutong (GXST), a combination of the traditional herb and Mongolian medicine, is effective and safe in treating angina pectoris in clinical trials. We assess the cardioprotective effects of GXST against myocardial ischemia and reperfusion (MI/R) injury in rats and explore its possible mechanism. Methods Forty-five male Sprague Dawley rats were randomized into three groups: non-MI/R group (Sham, n = 15), MI/R group treated with vehicle (Control, n = 15) and MI/R group treated with GXST (Drug, n = 15). MI/R was induced by ligation of the left anterior descending coronary artery (LAD) for 30 minutes, followed by 2/24 hour reperfusion in the Control and Drug groups. In the Sham group, the LAD was exposed without occlusion. GXST powder (in the Drug group) or saline (in the Control and Sham groups) were administered via direct gastric gavage from 7 day prior to surgery. Blood samples were collected from the carotid artery (10 rats each group) after 2 hours of reperfusion, to determine the levels of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), interleukin-6 (IL-6) and intercellular adhesion molecule-1 (ICAM-1) using enzyme-linked immunosorbent assays. The animals were then sacrificed and the hearts were harvested for histopathology and western blot analysis. Infarct size was measured in the remaining five rats in each group after 24 hours reperfusion. Results GXST significantly decreased levels of TNF-α, IL-1β, IL-6, ICAM-1, apoptosis index (AI) and infarct size. GXST also obviously inhibited nuclear factor kappa B (NF-κB) activity when compared with the Control group (all P < 0.05). Conclusions GXST is effective in protecting the myocardium against MI/R injury in rats. Its possible cardioprotective mechanism involves inhibition of the inflammatory response and apoptosis following MI/R injury.
