1.The correlation study between resilience and coping styles of recurrence of patients with nasopharyngeal neoplasms

Objective To investigate the relationship between levels of psychological resilience and coping styles of the patients with recurrence of nasopharyngeal neoplasms during hospitalization,in order to provide scientific data about the clinical psychological intervention.Methods A total of 240 cases of hospitalized patients with recurrence of nasopharyngeal neoplasms were selected and measured with general information questionnaire which was self-designed,resilience scale (CD-RISC) and coping scale.Results Resilience average score of patients with recurrent nasopharyngeal neoplasms was (53.22 ± 12.09) points,the level of resilience was positively correlated with positive coping and avoidance coping,P<0.05,and negatively correlated with the yield coping,P<0.05.Conclusions The resilience levels in patients with recurrence of nasopharyngeal neoplasms is below the medium level,resilience have a significant impact on patients' coping styles,and coping styles is affecting the level of resilience.In clinical work,health care workers should take effective measures to improve the resilience of patients so that patients take positive coping with disease.
2.Prevalence rates of healthcare-associated infection in a county hospital from 2011 to 2015

Objective To investigate the healthcare-associated infection(HAI) status, common infection sites, and antimicrobial use in a county hospital in 2011-2015.Methods With combination of bedside survey and case history reviewing, HAI among all hospitalized patients at 0:00-24:00 on April 26, 2011, April 25, 2012, May 14, 2013, September 9, 2014, and August 12, 2015 were surveyed, survey data were analyzed by software SPSS17.0.Results A total of 5 165 patients participated in 5 surveys of HAI prevalence rate, 132 patients had 138 cases of HAI, prevalence rate and case prevalence rate were 2.56% and 2.67% respectively.Intensive care unit had the highest average HAI prevalence rate(22.58%).Lower respiratory tract was the main HAI site (58.70%), followed by urinary tract (12.32%), upper respiratory tract (10.14%), and skin soft tissue(5.07%).A total of 45 strains of pathogens were isolated, mainly gram-negative bacteria (n=30, 66.67%).The total usage rate of antimicrobial agents was 45.27%, in 5 prevalence surveys were 61.48%, 43.89%, 37.81%, 39.43%, and 44.79% respectively.The main purpose of antimicrobial use was therapy (including therapy + prevention), accounting for 78.27%;monotherapy accounted for 81.27%.Conclusion Through prevalence rate survey for 5 consecutive years, HAI management departments can be more comprehensively understand HAI in key departments, key patients, key sites of HAI, and antimicrobial use in this hospital, and provide basis for the formulation and implementation of HAI prevention and control measures according to the actual situation of the hospital.
3.Effects of Hirsutella Sinensis Strain Fermentation on The Proliferation of T and B Lymphocyte in Mouse

[Objective] To observe the proliferation of T and B Lymphocyte in mouse after treated by Hirsutella sinensis strain fermentation.[Methods] Mice were treated with 1.0g?kg-1,0.1g?kg-1 of Hirsutella sinensis strain fermentation by intragastric administration.The proliferation of T and B Lymphocyte from the spleen of mouse is detected by the MTT assay .[Results] Hirsutella sinensis strain fermentation and Cordyceps Sinensis can distinctly increasse the stimulation index (SI) of T and B lymphocyte transformation.[Conclusion] Hirsutella sinensis strain fermentation and Cordyceps Sinensis are able to enhance immune function of normal mouse.
4.Effect of continous subcutaneous insulin infusion and mutiple subcutaneous insulin infusion on type 2 diabetes mellitus during perioperation

Objective To study the effect of continous subcutaneous insulin infusion (CSII) and mutiple subcutaneous insulin infusion (MSII) on type 2 diabetes mellitus during perioperation. Methods One hundred and eighty surgical patients complicated with Type 2 diabetes were randomly divided into two groups,98 cases in the CSII group (treated with CSII of novolin R) and 82 cases in the MSII group (treated with MSII of novolin R and no-volin N). Blood glucose level,the time to reach normal blood glucose level, the average dosage of insulin, the inci-dence of hypoglycemic,infection rate of incisions and inpatient days were measured in two groups before and after treatment. Results The level of fasting blood glucose after treatment in the CSII group (4.8 mmol/L (SD: 1.6)) was significantly lower than that of the MSII group (6.4 mmol/L(SD :2.1)) (t = 7.74,P < 0.05), and 2-h glucose in the CSII group (7.6 mmol/L(SD :2.3)) was significantly lower than that of the MSII group (9.3 mmol/L(SD: 2.4)) (t = 7.72, P < 0.05). The time to reach normal blood glucose level in the CSII group (4.1 days (SD: 2.9)) was shorter than that of MSII group (6.9 days (SD :2.0)) (t=2.81, P < 0.05). The average dosage of insulin in the CSII group (40.7 U(SD: 10.3)) was lower than that of the MSII group (63.2 U (SD: 17.0)) (t=3.57, P <0.05). The incidence of hypoglycemic in the CSII group (3.05%) was lower than that of the MSII group (9.20%) (χ~2 = 4.92,P < 0.05). The infection rate of incisions in the CSII group (0.0%) was lower than that of the MSII group (10.9%) (χ~2 =4.18, P < 0.05). The inpatient days in the CSII group (15.3 days (SD :7.2)) was shorter than that of the MSII group (22.5 days (SD :9.7)) (t = 3.12, P < 0.05). Conclusions Compared to multiple sub-cutaneous insulin infusion, continuous subcutaneous insulin infusion is more effective in controlling blood glucose, hypoglycemic and incision infection, thus is recommend to perioperative patients complicated with type 2 diabetes mellitus.
5.Using the External Quality Assessment Return Target Value Obtained from Clinical Biochemical Analyzer to Evaluate the Accuracy After the Regression Calibration of Dry-type Biochemical Analyzer

Objective Using Hitachi 7170 external quality assessment return target value to evaluate the accuracy of 10 items after Regression calibration of the Vistros 350 dry-type Biochemical Analyzer.Methods The same quality control samples were separately tested on two instruments,and results were reported to the clinical National Center for Clinical Laborato-ries.Substituted the external quality assessment return target value result from the National Center for Clinical Laboratories by using Vitros350 into regression calibration equation,then the getting data were compared with the external quality assess-ment return target value obtained from Hitachi 7170,and the deviation analysis was processed.The total error range from the America Clinical Laboratory Amended Bill was used as the standard.For the results within the reference range,error less than 1/2CLIA’88 total error,taken as the comparable judging standard,as it satisfied the requirement.For the results out off the reference range,error less than CLIA’88 total error,those still satisfied the requirement.For those items not meet the requirements,it must to do the regression calibration for Vitros350,using Hitachi 7170 as the standard instrument.Results The deviations of 7 items were all less than 1/2CLIA’88 allowed total error,with LDH was 0.16~-9.89,CK 2.92~6.25, ALT -4.64~-8.07,TBIL 0.08~2.67,TP -0.37~4.41,ALB 2.74~4.77 and URIC 1.04~3.0 respectively,and did not need re-calibration.For GLU and CREA,only one out of the reference range sample,the error range was >1/2CLIA’ 88,but
6.Clinical Application of Immunofixtion Electrophoresis, Serum Protein Electrophoresis and Immunoglobulins and Light Chain Quantitative Analysis in the Diagnosis of Multiple Myeloma

Objective To study the effectiveness of the laboratory diagnosis of multiple myeloma(MM)patients with immun-ofixtion electrophoresis (IFE),protein electrophoresis (SPE)and immunoglobulins and light chain quantitative analysis. Methods Selected 192 MM patients and 30 healthy controls during June 2012 to December 2013 and analyzed the results of IFE,SPE and immunoglobulins,and light chain quantitative analysis in MM patients.Results M protein bands were seen in 120 cases (62.5%)by using SPE and M protein were positive in 174 cases (90.6%)among the 192 MM patients by using IFE.IFE showed that IgG were maximum type of the M protein (106 of 192,55.2%).There were IgG-λ type 66 cases (34.4%),IgA type 36 cases (18.8%)and free light chain type 24 cases (12.5%).Immunoglobulins of different immuno-phenotypes had higher than the nomal group with serum immunoglobulin and light chain quantitative analysis (P <0.05). The detection rate was 67.7% (130/192).Whateverκ-type M protein orλ-type M protein,the ratio ofκ/λwas significantly abnormal (P <0.05).The detection rate was 85.4% (164/192).Conclusion The better detection rate of immunological techniques such as immunofixtion electrophoresis and immunoglobulins quantitative analysis might provide valuable basis for the diagnosis and treatment of MM clinically and prevent misdiagnosis.
7.Application of the Requirement of Biological Variation Determination in the Quality Management of Blood Cell Analyzer

Objective To study application of the quality management of Blood Cell Analyzer according to the requirement of biological variation determination.Methods Collected the indoor imprecision value(CV%) from 8 items detected by blood cell analyzer during from April to Nov.of 2016,and the bias (Bias%) of 8 items of two EQA (external quality assessment) from the ministry of health in 2016.Then according to the 3 levels of the minium,appropriate and optimal quality specifications derived from the biological variability the rates of imprecision and bias were culculated.The pass rate of the imprecision and bias was calculated.By using mean bias and mean imprecision and biological variation 3 levels of total error (TEa) crite rion,and to calculate the corresponding σ and QGI value,so as to evaluate the performance of whole blood cell analyzer.Then improved the quality.Results For the imprecision value of 8 items,except the MCHC average value,all others were all 100 % meeting the appropriate level of quality requirements.For the bias value (Bias %) from 8 items,except MCH,all others were over 80 % meeting the appropriate level of quality requirements.While for the calculated σ value,based on the best level of quality requirements,except the σ value of WBC was 4.6,the σ value of all other items were all<3.Based on the appropriate level of quality requirements,except the σvalue of MCHC was 1.9,the value of σ of all other items were all> 3,and based on the minimal requirements,the σ value of all 8 items were all >3.After analysis,this blood cell analyzer,except that MCHC should use the minimal quality standard requirements,all other examination items could used the proper quality standard requirements,and the calculated QGI were all <0.8.Conclusion Based on the biological variation determination requirement and calculated σ and QGI value,this method could be used to more accurate quality evaluation of blood cell ana lyzer.Which is a higher levelof quality management,will be more conducive to quality improvement and better serve the clinical.
8.Efficacy analysis of modified sequential therapy versus different quadruple therapy for eradicating the helicobacter pylori

Objective To evaluate the efficacy and safety of modified sequential therapy versus different quadruple therapy for the helicobacter pylori (H .pylori) eradication .Methods A total of 240 H .pylori infected patients with non atrophic gastritis accompanies erosion and peptic ulcer diagnosed by gastroscopy ,were evenly divided into sequential therapy group (A group) ,con-comitant therapy group(B group) ,7 days bismuth-containing quadruple therapy group(C group) and 10 days bismuth-containing quadruple therapy group(D group) .A group :rabeprazole 10 mg ,amoxicillin 1 000 mg were taken twice daily for 5 days firstly ,then rabeprazole 10 mg ,clarithromycin 500 mg ,furazolidone 100 mg were taken twice daily for 5 days .B group :rabeprazole 10 mg , amoxicillin 1 000 mg ,clarithromycin 500 mg ,furazolidone 100 mg were taken twice daily for 7 days .C group and D group :rabe-prazole 10 mg ,bismuth 220 mg ,amoxicillin 1 000 mg ,clarithromycin 500mg were taken twice daily for 7 and 10 days respectively . H .pylori status was re-assessed with 14C-urea breath test after 4-weeks therapy .Results Among them ,224 cases completed the study .According to the analysis of intention-to-treat (ITT ) ,the H .pylori eradication rates of A ,B ,C ,D group were 88 .33% , 83 .33% ,73 .33% ,81 .67% respectively ,and according to per-protocol (PP )analysis which were 92 .98% ,90 .90% ,78 .57% , 87 .50% .The difference between A and C group was statistically significant (χ2 = 4 .36 ,4 .83 ,P= 0 .037 ,0 .028) .Conclusion Fura-zolidone-containing sequential therapy provide provide high H .pylori eradication rates ,which could be the first-line treatment option .

Salvia miltiorrhiza bunge (Dan shen in Chinese)is extracted from dried roots and rhizomes of Labiatae Salvia.Tan-shinones are the main lipid-soluble components in Salvia,each has specific pharmacological activity.This review focuses on the research progress of Tanshinones on cardiovascular diseases,an-ti-inflammatory and immunomodulatory effects, anti-tumor effect,hepatocyte protection and neuroprotective effects.Thera-peutic effects and mechanisms of Tanshinones on diverse disea-ses are summarized,pharmacokinetics and pharmaceutic evalua-tion were concluded.This review provides a global understand-ing about Tanshinones as a class of effective and promising can-didates for further studies,and lays a foundation for developing new Tanshinone-based agents according to the characteristics of Tanshinones.
10.Expression and significance of changes of TLR4 and IL-6 in hippocampal CA1 area of rats after ischemia adaptation in distance

Objective:To observe the changes of TLR4 and IL-6 expression in rats hippocampus CA1 region in remote ischemic postconditioning(RIP) and explore its significance.Methods: All the 72 male SD rats were divided into Sham group,Contrast group and RIP group randomly.Each group was divided into 4 time points:12h,24 h,48 h and 72 h group.There were 6 rats in each group.Use the cerebral ischemia-reperfusion model which was established with modified Longa method as the contrast group.The method of RIP was to Fasten rats Posterior limbs by a tourniquet for 30 min immediately,then relax them for 30 min, repeat 3 times.To observe the pathological variation of hippocampus CA1 region by HE dyeing;to test the expression of TLR4 and IL-6 by immunohistochemical staining.Results: Compared to contrast group, neuronal loss and swelling reduced significantly in RIP group.Compared to sham group, the TLR4 and IL-6 expression in contrast group and RIP group increased significantly ( P<0.05 ) .Compared to contrast group,the TLR4 and IL-6 expression in RIP group reduced significantly(P<0.05).Conclusion:RIP dose have protective effect on cerebral ischemia.The effect may be associated with the inhibition of TLR4 and IL-6 expression.
