1.The effect analysis of clinical pathway management for capillary bronchitis

Objective To analyze the effect of clinical pathway management for capillary bronchitis in Department of Pediatrics.Methods 75 children with capillary bronchitis after the implementation of clinical pathway management were selected as the observation group,and 68 children with capillary bronchitis before the implementa-tion of clinical pathway management were selected as the control group.The hospitalization cost,average hospital stay, antibiotic use rate,cure rate,nonsocomial infection rate,rehospitalization rate within two weeks,service satisfaction and other comprehensive indices were compared.Result The total cost of drug of the observation group[(1 198.49 ± 397.48)yuan]was lower than (1 324.05 ±376.57)yuan of the control group(Z =2.00,P <0.05).The average hospital stay of the observation group[(6.99 ±2.39)d]was shorter than (8.01 ±2.22)d of the control group(Z =2.62,P <0.05).The service satisfaction rate of the observation group(95%)was higher than 82% of the control group(χ2 =5.44,P <0.05).The antimicrobial use rate of the observation group(49%)was apparently lower than 95% of the control group (χ2 =27.59,P <0.01).There were no statistically significant differences between the two groups in cure rate(hospital discharge rate),nosocomial infection rate and rehospitalization rate within two weeks(all P >0.05).Conclusion Clinical pathway management for capillary bronchitis in Department of Pediatrics of primary hospital can play a role in standardizing medical practice,reducing average hospitalization days,controlling medical cost,and improving service satisfaction under the precondition of ensuring medical quality.
2.Effects of hypertriglyceridemic-waist phenotype on cognitive function in patients with type 2 diabetes

Objective To investigate the effect of hypertriglyceridemic waist(HTWC)phenotype on cognitive function in patients with type 2 diabetes.Methods According to the standard of plasma triglycerides concentration≥1.7 mmoL/L,waist circumference(WC)≥ 90 cm in men or ≥80 cm in women,304 patients with type 2 diabetes were divided into four groups:normal triglycerides and waist circumference group(group A,n =65),normal waist circumference and hypertriglyceridemia group (group B,n=53),abdominal obesity and normal triglycerides group(group C,n=114),and HTWC group(group D,n =72)for prospective studies.Patients in four groups were surveyed with Mini-mental state examination (MMSE)and Montreal cognitive assessment (MOCA).And changes of cognitive function among the four groups were compared.Results Total score for the MMSE was significantly lower in group D than in group A(27.1±1.9 vs.29.0±1.3,F=2.869,P=0.019).The subscales of attention and calculation(4.0± 1.4 vs.4.6±0.9,F=1.605,P=0.047)and recall(2.2± 0.9 vs.2.6±0.6,F=1.959,P=0.043) were significantly lower in group D than in group A.Total score for the MOCA was significantly lower in group D than in group A(23.4±3.9 vs.25.9±3.6,F =1.975,P=0.031).The subscales of visuospatial and executive(3.5 ± 1.4 vs.4.1 ± 0.9,F=1.537,P =0.048),attention(5.1±1.4 vs.5.7±0.9,F=1.660,P=0.048)and orientation(5.6±1.0 vs.6.0± 0.0,F=2.362,P=0.030)were significantly lower in group D than in group A.Conclusions There is a statistically more significant decline in cognitive function in patients with HTWC phenotype and the effective intervention and treatment are needed.
3.The Effect of "Xingnao Kaiqiao" Acupuncture on the Expression of Peroxiredoxin VI in the Ischemin Cortex of Rats

Objective To investigate and observe the expression of Peroxiredoxin Ⅵ in iechemia cortex of rats and the intervention effect of Xingnao Kaiqiao Acupuncture(XKA) therapy. Methods Through western blotting,we detected the expression of Peroxiredoxin Ⅵ in xingnao kaiqiao group , comparison group, model group and sham operation group in middle cerebral artery occlusion (MCAO)model of rats. Results At 6h, the expression of Peroxiredoxin Ⅵ in xingnao kaiqiao group , comparison group and sham operation group was increased than that in moddle group,of which Xingnao Kaiqiao group increased obviously(P < 0.05). At 24h, as compared with modlegroup, the expression of Peroxiredoxin Ⅵ in xingnao kaiqiao group and comparison group increased significantly (P < 0.05). Conclusion Xingnao Kaiqiao Acupuncture therapy could raise the expression of Peroxiredoxin Ⅵ in focal cerebral ischemia of rats, and the effect is better that of traditional acupuncture therapy in the earlier period.It is benefit to elevating the ability of antioxygen of focal cerebral ischemia of rats.

5.Degradation kinetics of larotaxel and identification of its degradation products in alkaline condition

Larotaxel, a new taxane compound prepared by partial synthesis from 10-deacetyl baccatin Ⅲ, is active against tumors. In this research, a selective LC–MS method was developed and validated for the study of degradation kinetics of larotaxel, which was carried out in aqueous solutions with different pH (1.5, 3.0, 5.0, 6.5, 7.4, 9.0, 10 and 11.0) and temperature (0, 25, 37 and 45 °C). The linear range was 0.5–25μg/mL, the intra-and inter-day precisions were less than 7.0%, and accuracy ranged from 97.4–104.5% for each analyte. The observed rate obtained by measuring the remaining intact larotaxel was shown to follow first-order kinetics. The activation energies for degradation were 126.7 and 87.01 kJ/mol at pH 1.5 and 11, respectively. Although larotaxel was stable in pH 5, 6.5 and 7.4 buffers at 37 °C for 24 h during our study, increasing or decreasing the pH of the solutions would decrease its stabilities. Moreover, three main degradation products in alkaline condition were separated by HPLC and identified by Q–TOF–MS. The three degradation products were confirmed as 10-deacetyl larotaxel, 7, 8-cyclopropyl baccatin Ⅲ and 10-deacetyl-7, 8-cyclopropyl baccatin Ⅲ.
6.Efficacy and safety evaluation for 28 Chinese non-Hodgkin lymphoma patients treated with Hyper-CVAD/MA regimen

Objective To evaluate the efficacy and safety of the hyper-CVAD/MA regimen as an intensified treatment option for 28 T cell and aggressive/highly aggressive B cell NHL in Chinese patients. Methods Clinical data of 28 NHL patients treated with hyper-CVAD/MA regimen from Jan 2005 to Sep 2008 were retrospectively analyzed. Results 27 NHL patients were available for the efficacy analysis, with a response rate of 70.4 %. For the 13 B cell lymphoma cases, the response rate was 84.6 %. The main toxicity was Grade Ⅲ or Grade Ⅳ myelosuppression in all cases and 2 treatment related deaths. Conclusion Hyper-CVAD/MA regimen had a high response rate in T cell and aggressive /highly aggressive B cell NHL lymphoma, companied by significant toxicity when treating Chinese patients. Further clinical practices are needed to pick up a suitable dose which can balance efficacy and safety.
7.Structural changes of gut microbiota in patients with Parkinson's disease

Objective To investigate the structural changes of gut microbiota in patients with Parkinson's disease ( PD).Methods Twenty-four PD patients and 14 healthy controls from Beijing Hospital in 2015 were recruited in this cross-sectional study.The general clinical information was collected and all subjects were assessed with Parkinson's disease related scales.The gut microbiota status between two groups was analyzed after extracting feces'DNA and carrying out high-throughput sequencing of bacterial 16S rRNA.Results At the phylum level, actinobacteria (0.76%(0.13%, 1.85%) vs 0.14%(0.07%, 0.30%), Z=2.784, P<0.01) were significantly increased and bacteroidetes (57.28%(48.75%, 64.95%) vs 63.78%(56.72%, 68.21%), Z=-4.963, P<0.01) were significantly decreased in PD patients compared to healthy controls.At the class level, bacilli (0.52%(0.11%, 2.10%) vs 0.13%(0.05%, 0.16%), Z=2.693, P<0.01), negativicutes (5.04%(2.93%, 14.02%) vs 2.87%(1.46%, 4.43%), Z=2.360, P=0.018), actinobacteria (0.60%(0.10%, 1.59%) vs 0.12%(0.04%, 0.20%), Z=2.512, P=0.011 ), gammaproteobacteria ( 1.72%( 0.58%, 5.46%) vs 0.43%(0.24%, 2.19%), Z=2.179, P=0.029) were significantly increased in PD patients compared to healthy controls.At the family level, veillonellaceae (3.78%(0.53%, 13.82%) vs 0.49%(0.08%, 3.14%), Z=2.754, P<0.01), streptococcaceae (0.33%(0.09%, 0.69%) vs 0.19%(0.14%, 0.24%), Z=1.770, P=0.004), enterobacteriaceae (1.04%(0.40%, 4.95%) vs 0.20%(0.10%, 0.45%), Z=2.784, P<0.01 ), lactobacillaceae ( 0.079%( 0.014%, 0.575%) vs 0.003%(0.002%, 0.028%), Z=3.119, P<0.01), bifidobacteriaceae (0.60%(0.09%, 1.57%) vs 0.11%(0.03%, 0.19%), Z=2.481, P=0.012) were significantly increased and pasteurellaceae (0.009%(0.002%, 0.047%) vs 0.110%(0.022%, 0.898%), Z=-2.545, P=0.010) were significantly decreased in PD patients compared to healthy controls.Conclusions The structures of gut microbiota in PD patients and healthy controls were significantly different at the levels of phylum , class, and family.All these changes are potentially associated with the development of PD pathology.
8.The effect of miR⁃9 on the proliferation of neural stem cells by regulating β⁃tubulin Ⅲ and GFAP

Objective :
To explore whether miR⁃9 affects the proliferation of neural stem cells (NSCs) by regulating β ⁃tubulin Ⅲ and glial fibrillary acidic protein (GFAP) .
Methods :
NSCs cells were isolated and cultured on the purchased healthy pregnant mice and the isolated cells were divided into NO group ( non⁃transfected NSCs cell line) , NN group (NSCs transfected with miR⁃9 ⁃NC) , NM group (NSCs transfected) transfected with miR⁃9 mimics) , NI group ( transfected with miR⁃9 inhibitor) . Nestin was identified by immunofluorescence , the contents of Mir⁃9 , β ⁃tubulin Ⅲ and GFAP were measured by QRT⁃PCR , the proliferation of NSCs was measured by MTT method , the apoptosis of NSCs was measured by flow cytometry , and the protein expressions of Mir⁃9 , β ⁃tubulin Ⅲ and GFAP were detected by Western blot.
Results :
The detection index Nestin of NSCs for 24 h after cell passage was positive in 90% of the NSCs , and co⁃localized with the nucleus , indicating the successful isolation of NSCs. The expression of miR⁃9 mRNA in NSCs of the NM group was the highest among the four groups , indicating that the transfection was successful. Among the four groups , β ⁃tubulin Ⅲ mRNA and protein expression was the highest in the NM group , and the GFAP mRNA and protein expression was the lowest. The NI group was the opposite (P < 0. 05) . In the four groups after 24 h , the cell activity of NSCs in the NM group was the strongest and showed a trend of gradually increasing with time (P < 0. 05) . The cell activity of NI was significantly weaker than that of the other three groups (P < 0. 05) . Among the four groups , the apoptosis rate of the NM group was lower than that of the other three groups (P < 0. 05) , and the apoptosis rate of NI was higher than that of the other three groups (P < 0. 05) . The difference in the above indicators between the NO group and the NN group was smaller.
Conclusion
Overexpressed miR⁃9 promotes the proliferation of NSCs by promoting the expression of β ⁃tubulin Ⅲ and inhibiting the expression of GFAP.
9.The treatment of refractory wounds with apheresis platelet-rich plasma

【Objective】 To explore the model of treatment of refractory wounds by apheresis platelet-rich plasma (PRP), and evaluate its efficacy and safety. 【Methods】 PRP treatment was carried out for 34 patients with refractory wounds who were hospitalized in our hospital from June 2020 to December 2023. The patient′s autologous PRP was collected by the blood composition separator. PRP point-like injection and/or direct application along the edge of the wound, or platelet gel (PG) to cover the wound and fill the sinus were used to treat different types of wounds. The frequency of treatment was once to twice a week, with each course of treatment consisting of 4 to 5 sessions. The efficacy was observed and recorded, and the safety was evaluated. 【Results】 Before PRP collection, the basal platelet, white blood cell and red blood cell counts were (321.85±114.64) ×109/L, (9.52±3.21) ×109/L and (4.34±0.62) ×1012/L, respectively. The counts of platelets, white blood cells and red blood cells in PRP products were (1 438.53±376.89)×109/L, (1.38±1.03)×109/L and (0.03±0.01)×1012/L, respectively. The platelet concentration of PRP products increased by 4.47 times on average, and the sampled bacterial culture was negative. Out of 34 patients treated with PRP, 33 showed improvement in symptoms, while 1 did not respond well due to long-term bedridden and poor compliance. Four patients had mild adverse reactions during PRP collection or treatment, but all were able to complete PRP collection or treatment after standardized treatment. 【Conclusion】 Apheresis PRP products have the advantages of stable quality, safety, reliability and traceability, and can effectively promote the healing of a variety of refractory wounds. Its treatment process is safe and effective, and is worthy of popularization and application.
