1.Approach and Practice of Pharmaceutical Care in Department of Gastroenterology

OBJECTIVE:Probe into the procedures and contents of pharmaceutical care in department of gastroenterology. METHODS:The feasibility,requirement and our specialty of pharmaceutical care were systematically analyzed in department of gastroenterology through a special - purpose study and discussion, and a detailed design was instituted, then the four faces were established.RESULTS: Clinical pharmacists should bring their pharmaceutical specialty into full play, and worked smoothly with clinicians and nurses by aiming directly at kinds of clinical requirement .An initial effect of pharmaceutical care was shown in department of gastroenterology .CONCLUSION: Young clinical pharmacists should adopt a scientific analysis and normative procedure to work smoothly with clinical partners as quickly as possible at the beginning of their clinical pharmacy work and strive hard to be one of the important person in multidisciplinary treatment teams.
2.Effects of Dengzhan Shengmai capsule on renovascular hypertension-induced β-amyloid protein deposition and cognitive impairment in rats

Objective To observe the effects of Dengzhan Shengmai capsule on β-amyloid protein (Aβ) deposition in the brain and learning and memory function in renovascular hypertensive rats (RHRSP).Methods Male Sprague-Dawley rats were randomly divided into four groups (5 rats per group):normal group,shamoperated group,hypertension with Dengzhan Shengmai capsule treatment group and hypertension with normal saline (NS) treatment group.Renovascular hypertensive models were created by clipping two-kidney.Dengzhan Shengmai capsules were dissolved in sterile 0.9% NS and were administered (20 mg · kg-1 per day) by daily gavage for 4 weeks.In the NS group,hypertensive rats were given saline in the same volume.Immunofluorescent labeling and western blot were used to detect the expression of Aβ,NF-κB,IL-1β,TNF-α in the brain,respectively.Learning and memory function were detected by Morris water maze.Results RHRSP significantly increased Aβ deposition in the cerebral cortex and impaired memory function in rats.Dengzhan Shengmai capsule treatment significantly lowered the blood pressure compared with NS treatment((157.45±11.58) mmHgvs (197.76±10.12) mmHg).In addition,the levels of Aβ,NF-κB p65,IL-1β,TNF-α protein were significantly reduced,by Dengzhan Shengmai caspule treatment.The escape latency was shortened((24.64±4.57) s vs (37.17±3.87)s),while the frequency of passing through the platform quadrant(5.39±0.12 vs 3.05±0.28) and the dwell time((27.34±3.67) s vs (16.83±5.76)s) (all P<0.01) in the platform quadrant were significantly increased by Dengzhan Shengmai capsule treatment.Conclusions Dengzhan Shengmai capsule may reduce Aβ deposition in brain and improve learning and memory function by anti-inflammatory effects in RHRSP.
3.Comparative study of different gastrointestinal motility drugs on capsule endoscopy

Objective To compare the effect of different gastrointestinal motility drugs on capsule endoscopy. Methods Seventy-one patients with suspected small bowel disease were randomly divided into metoclopramide group (24 patients), mosapride group(25 patients) and control group (22 group). The patients in metoclopramide group swallowed capsule endoscopy immediately after intramuscularly injecting 10 mg metoclopramide, the patients in mosapride group swallowed capsule endoscopy 15 min after taking 5 mg mosapride, and the patients in control group did not take any of the gastrointestinal motility drugs. Three groups had the same bowel preparation before checking. The finishing rate of small bowel examinations, stomach and small intestinal transit time, intestinal cleanliness and the detection rates of lesions in three groups were compared. Results The total small bowel examination finishing rate was 94.4%(67/71). The small bowel examination finishing rate in metoclopramide group, mosapride group, and control group was 95.8%(23/24), 96.0%(24/25), and 90.9% (20/22), and there was no significant difference(P>0.05). The stomach transit time in metoclopramide group, mosapride group and control group was(27.5 ± 20.7), (28.1 ± 20.9) and (52.3 ± 33.5) min. The stomach transit time in metoclopramide group and mosapride group had no significant difference (P>0.05), but it was significantly lower than that in control group (P<0.05). The small intestinal transit time in three groups had no significant difference (P>0.05). The image class scores in metoclopramide group, mosapride group and control group was (2.5 ± 0.4), (2.7 ± 0.4) and (1.7 ± 0.3) scores.The scores in metoclopramide group and mosapride group had no significant difference (P>0.05), but they were significantly higher than that in control group (P<0.05). The detection rate of lesions in metoclopramide group, mosapride group and control group was 45.8%(11/24), 56.0%(14/25) and 18.2%(4/22). The detection rate of lesions in metoclopramide group and mosapride group had no significant difference (P>0.05), but it was significantly higher than that in control group (P<0.05). Conclusions The use of gastrointestinal motility drugs before capsule endoscopy can improve the quality of inspection, and metoclopramide and mosapride shows no significant difference.
4.Effect of basic fibroblast growth factor and insulin-like growth factor-1 on proliferation and collagen synthesis of bone marrow mesenchymal stem cells

BACKGROUND:How to control the orderly formation of colage in skin repair and scarring process is worthy of attention. OBJECTIVE: To investigate the effect of basic fibroblast growth factor (bFGF) combined with insulin-like growth factor 1 (IGF-1) on the proliferation and colagen synthesis of rat bone marrow mesenchymal stem celsin vitro. METHODS:Rat bone marrow mesenchymal stem cels were isolated and cultured to induce adipogenic differentiation assessed by oil red O staining and osteogenic differentiation identified by alizarin red stainingin vitro. Passage 3 cels were cultured in the medium containing bFGF, IGF-1, combination of them or the control fluid, respectively. MTT assay was used to detect cel proliferation at 12, 24, 48, 72 and 96 hours of culture. The expression of type I colagen and type III colagen were detected by RT-PCR and western blot after 10 days of incubation. RESULTS AND CONCLUSION:Compared with the control group, bFGF or IGF-1 alone significantly promoted the proliferation of bone marrow mesenchymal stem cels, and inhibited the expression of type I colagen and type III colagen. After combined use of bFGF and IGF-1, the proliferation of bone marrow mesenchymal stem cels was improved more significantly, and the expression of type I colagen and type III colagen returned to normal levels. These findings indicate that the combination of IGF-1 and bFGF can promote proliferation of bone marrow mesenchymal stem cels and restrain the expression of type I colagen and type III colagen, which may be helpful for control and repair of scar formation during wound healing.
5.THE NORMAL VALUE AND THE WAVE FORM OF OPTOKINETIC NYSTAGMUS OF AIRCREW

A revolving drum, about 40cm in diameter, with vertical stripes on its surface, was used in examination. The drum was held 50cm in front of the eyes of aircrew receiving examination. The horizontal optokinetic nystagmus was elicited by vertical black stripes moving horizontally. The examinee was instructed to gaze straight ahead at the black stripes, without deliberate fixation. The drum speed was kept constant at 20?, 40?,60?,80?,100?,120?/s. The velocity of the slow component of nystagmus, its frequency, amplitude and symmetry were recorded when the reaction reached its peak.The slow component velocity and the frequency were increased with the drum speed, but the amplitude was decreased with the drum speed. The wave forms of optokinetic nystagmus were usually of five types.
6.Dissolution property of home-made and imported ambroxol hydrochloride tablets determined by HPLC and analyzed by Weibull's model

Objective: To evaluate dissolution properties of 2 kinds of ambroxol hydrochloride tablets.Methods: A HPLC method was used.The column was Hypersil C 18 (10 ?m,150 mm?4.6 mm).The mobile phase was methnol water tetrahydrofuran triethylamine (71∶29∶2.5∶0.25).The wavelength of detector was 242 nm.Data were analyzed by Weibull's distribution model.Results: Both tablets dissolved rapidly and their dissolution rate reached 85% in the first 8 min.But there was significant statistic differences between the 2 tablets as for Weibull's parameters.Conclusion: Although the dissolution rate of 2 tablets is similar,there are still some differences between their dissolution parameters.The dissolution of the imported tablets is more rapid than the home made ones.
7.Lesion detection value of Cine sequence in the small intestinal Crohn’s disease:a preliminary study

Objective To evaluate the detection rate of lesions of patients with Crohn’s Disease(CD)tusing combining the con-ventional MR and Cine MRE(MR enterography).Methods MRI images of twenty-nine patients with CD confirmed by clinic were analyzed retrospectively.Scan sequences include coronal Cine MRE,and standard MR protocol (FIESTA,SS-FSE,LAVA C+). Two radiologists analyzed the images with double blind method to explored whether there were significant differences existing be-tween having or having not Cine MRE on lesion detection rate (wall thickness,stenosis,comb sign,adhesions,fistulas).Results The number of lesions detected by standard MR combined with Cine MRE compared with standard MR alone were 82/80 (P =0.1 6) for wall thickening,52/43 (P =0.03)for stenosis,6/7 (P =0.36)for the comb sign,5/7(P =0.66)for adhesions,2/2for fistulas re-spectivily.Conclusion It is meaningless to combine Cine MRE with standard MR in lesions detection in patients with CD,Cine ima-ges can accurately determine diseased bowel stenosis,and show the adhesive location.On the basis of standard MR,Cine MRE can be added in patients with CD when necessary.
8.Clinical analysis of 502 cases of subarachnoid hemorrhage

Objective To summarize the clinical and imaging features of subarachnoid hemorrhage(SAH) and its complications and prognosis. Methods The clinical data of 502 cases of SAH were retrospectively analyzed and summarized. Results There were 84.3 % (423/496) patients sufferring from intense headache, dizziness or vertigo (13. 1%, 66/496 ), disturbance of consciousness ( 27. 5%, 138/496 ), partial neurologic impairment (58.2% ,292/496) ,meningeal irritation sign(72.5% ,364/496). The positive rate of digital subtraction angiogra-phy was 54.1% (93/172). Aneurysm was the common reason of SAH (20. 1%, 101/496). Prognosis in most pa-tients was better,but the old patients suffered from high mortality. Conclusions The cause and clinical manifesta-tion of SAH varies,cerebral angiography often could help find the cause of SAH. Most SAH patients recover well.

After removal of unilateral molars in the rats,the changes in the articularcartilage and articular disc of the temporomandibular joint and HRP uptake of thelining cells of synovial membrane were observed with light,transmission and scanningmicroscopy.The HRP uptake activity of the lining cells was lowered and degenerative chan-ges were presented in ths articular cartilages following the removal of the molars.It is suggested that the temporomandibular joint is intimately integrated with theocclusion of the upper and lower teeth.Defect of the teeth on one side mayinduce occlusion disorders.These will certainly cause some changes of the structureand function of the temporomandibular joint.
