1.Effects of comprehensive training on mild cognitive impairment in patients with stroke

Objective To explore the comprehensive training effects of mild cognitive impairment (MCI) in patients with cerebral apoplexy.Methods Selected 70 cases of cerebral apoplexy patients with MCI were randomly divided into control group(n=35) and intervention group (n=35).The patients in control group maintained the original treatment and nursing,and the comprehensive training was used in intervention group.Before and after the intervention Montreal cognitive assessment scale (MoCA),daily life ability Barthel index (MBI) were used to evaluate the change of MCI patients.Results After training,the score of MBI in the intervention group(55.71±5.08) was significantly higher than that of before training (52.59±5.62) and the control group(53.65±5.78).After training,the score of MoCA in the intervention group(23.37±2.40)was significantly higher than that of before training (21.77±2.53)and the control group(21.73±2.43).Conclusion Comprehensive training can improve cognitive function in patients with cerebral apoplexy,ease the burden on society and the family,and improve the quality of life.
2.Research progress of autoimmune gastritis

Autoimmune gastritis(AIG) is a CD4 + T cell-mediated organ specific autoimmune disease of the stomach, characterized by the inherent gland atrophy in the stomach and intestinal metaplasia and/or pseudopyloric gland metaplasia. The diversity and nonspecificity of clinical features make the diagnosis difficult. In the past, AIG was thought to occur in the West, but in recent years, more and more studies have shown that the prevalence rate of AIG in China is not low. However, the disease has not attracted the attention of domestic clinical workers. Therefore, this paper reviews the latest research progress in the etiology, clinical characteristics, relationship with other diseases, diagnosis and treatment of AIG, so as to improve the understanding of clinical workers and improve the treatment rate and prognosis of AIG.

5.Investigation and Analysis of Satisfaction Degree of Hospitalized patients on Nursing in Primary Hospitals

Objective:To comprehensively improve the quality of clinical nursing, survey the hospitalized pa-tient′s satisfaction and analyze the low satisfaction issues.Methods:Adopt the self-made questionnaire in four pri-mary hospitals.Results:Through the questionnaire analysis, understand the hospitalized patients to improve grass-roots hospital trusteeship overall satisfaction, but satisfaction to nursing technical operation, the life care is low. Logistic stepwise regression analysis, according to the results of nursing technical operation and health education are the main factors influencing the patients′satisfaction.Conclusion:Nursing managers should pay attention to the factors influencing patient satisfaction, and take corresponding corrective measures, improve the patient′s satisfac-tion, to further improve the reform of hospital custody.
6.Expression of CXC chemokine receptor 5 in skin lesions of bullous pemphigoid and its significance

Objective To study the role of CXC chemokine receptor 5 (CXCR5) in the pathogenesis of bullous pemphigoid (BP) by detecting the expression of CXCR5 on follicular helper T (Tfh) cells in skin lesions of BP.Methods An immunohistochemical procedure was used to detect CXCR5 expression in biopsy specimens from skin lesions of 16 patients with BP and normal skin of 10 healthy individuals.Results CXCR5 was mainly expressed in the cytoplasm and membrane of Tfh cells in the basal cell layer of normal skin as well as in both basal cell layer and prickle cell layer of BP lesions.The number of CXCR5-positive ceils per high power field (× 400) was 11.16 ± 4.47 in normal skin specimens,significantly lower than that in lesional skin specimens (35.70 ± 12.20,t =6.07,P < 0.01).Conclusion CXCR5 may be involved in the pathogenesis of BP.
7.The diagnostic value of double-balloon enteroscopy in 67 cases with obscure abdominal pain

Objective To evaluate the diagnostic valve of double balloon enteroscopy in patients with obscure abdominal pain and analyze the etiology of chronic abdominal pain resulted from enteral diseases. Methods Sixty-seven cases with chronic abdominal pain underwent a previous negative gastroscopy, colonoscopy, gastrointestinal barium, B ultrasound and electrocardiogram were received double balloon enteroscopy during June 2005 to June 2008. Results Thirty-six of 67 patients was done by enteroscopy via anus, and 19 cases via oral, and 12 cases via both anus and oral. The lesions were found in 41 of the 67 patients, with overall diagnostic yield of 61.19%. Among 41 cases of abdominal pain resulted from small bowel diseases, Crohn's disease were found in 15 cases (36.59%), non-specific small enteritis in 10 cases (24.39%), tumors in 8 cases (19.51%),other enteral diseases in 8 cases (19.51%). Conclusions Double balloon enteroscopy was a diagnostic modality with a high diagnostic value for obscure abdominal pain resulted from small bowel diseases. The most common causes of obscure abdominal pain were Crohn's disease, non-specific small enteritis and tumors.
8.Isolation and gene modification of amniotic fluid derived progenitor cells.

We established methods to isolate human amniotic fluid-derived progenitor cells (hAFPCs), and analyze the ability of hAFPCs to secrete human coagulation factor IX (hFIX) after gene modification. The hAFPCs were manually isolated by selection for attachment to gelatin coated culture dish. hFIX cDNA was transfected into hAPFCs by using a lentiviral vector. The hFIX protein concentration and activity produced from hAFPCs were determined by enzyme-linked immunosorbent assay (ELISA) and clotting assay. The isolated spindle-shaped cells showed fibroblastoid morphology after three culture passages. The doubling time in culture was 39.05 hours. Immunocytochemistry staining of the fibroblast-like cells from amniotic fluid detected expression of stem cell markers such as SSEA4 and TRA1-60. Quantitative PCR analysis demonstrated the expression of NANOG, OCT4 and SOX2 mRNAs. Transfected hAFPCs could produce and secrete hFIX into the culture medium. The observed concentration of secreted hFIX was 20.37% +/- 2.77% two days after passage, with clotting activity of 16.42% +/- 1.78%. The amount of hFIX:Ag reached a plateau of 50.35% +/- 5.42%, with clotting activity 45.34% +/- 4.67%. In conclusion, this study established method to isolate and culture amniotic fluid progenitor cells. Transfected hAFPCs can produce hFIX at stable levels in vitro, and clotting activity increases with higher hFIX concentration. Genetically engineered hAFPC are a potential method for prenatal treatment of hemophilia B.

9.Effect of interferon-α on rat liver fibrosis induced by CCl(4).

OBJECTIVE:
To explore the curative effect and the underlying mechanism of interferon-α (IFN-α) on rat liver fibrosis induced by CCl(4).
METHODS:
Forty-five Wistar male rats were randomly divided into 3 groups: a normal control group (n=15), a liver fibrosis group (n=15) and a IFN-α treatment group (n=15). The rats of the control group and the liver fibrosis group received peanut oil (0.2 mL/100 g body weight), twice a week for 8 weeks. The rats of the liver fibrosis group and the IFN-α treatment group were received intraperitoneal injection of 50% CCl(4) (CCl(4):peanut oil=1:1, 0.2 mL/100 g body weight, ip) or IFN-α (CCl(4):peanut oil=1:1, 0.2 mL/100 g body weight, ip), twice a week for 8 weeks. In the 9th week, the rats of IFN-α treatment group were switch to receive IFN-α at 100 000 units (s.c.) per day for 3 weeks. The rats were all sacrificed in the 11th week. Pathological changes of liver, semi-quantitative scoring of rat liver was observed. Tissue hydroxyproline, the mRNA expression of Collagen I, Collagen III, transforming growth factor-beta 1 (TGF-β1), connective tissue growth factor (CTGF), a-smooth muscle actin (α-SMA), matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1) of rat liver was detected. The protein expression of MMP-9 and TIMP-1 also was detected.
RESULTS:
Semi-quantitative scoring of inflammation,semi-quantitative scoring of liver fibrosis, hydroxyproline and collagen in the IFN-α treatment group were significantly lower than those in the liver fibrosis group (all P<0.05). The mRNA expression of TGF-β1, CTGF, α-SMA and TIMP-1, and the protein expression of TIMP-1 in the IFN-α treatment group were significantly lower than those in the liver fibrosis group (all P<0.05). But there was no significant difference in MMP-9 between the IFN-α treatment group and the liver fibrosis group.
CONCLUSION
IFN-α can decrease the liver fibrogenesis induced by CCl(4) in rats and reduce liver inflammation response. The anti-fibrosis effect of IFN-α may be related to decrease in TGF-β1, CTGF and TIMP-1 expression and to inhibiting of the hepatic satellite cells' activation and extracellular matrix synthesis.

10.Progress in the treatment of Helicobacter pylori infection.

Helicobacter pylori (H. pylori) infection is highly associated with the occurrence of gastrointestinal diseases, including gastric inflammation, peptic ulcer, gastric mucosa-associated lymphoid-tissue lymphoma and gastric cancer. It is a key strategy to prevent and treat these diseases by eradicating H. pylori. Due to the increasing rates of antimicrobial resistance of clarithromycin and metronidazole, the eradication rate of standard triple therapy is less than 80% in recent years. On the basis of traditional triple therapy, combination with some treatments, such as probiotics, mucoprotective agents, Chinese medicine and oral clean, can improve the H. pylori eradication rate.
