1.High Insulin-induced Insulin Resistance Dependent on the Increasing Glucose Concentration Surrounding the Cells

Objective To explore the effects of high insulin and high glucose concentrations on glucose transport activity,the expression of insulin signaling peptides and glucose transporter 4(GLUT4) translocation in primary cultured rat adipocytes.Methods Isolated rat adipocytes were cultured for 24h at insulin(10 4?U/ml) and different concentration glucose(5,10,15 and 25mmol).Then the glucose uptake,cellular contents of insulin receptor substrate(IRS) 1/2,phosphatidylinositol 3-kinase 85 subunit(p85),protein kinase B(PKB) and GLUT4 were measured by Western blotting method.Results These adipocytes treated with insulin and different high concentration glucose had shown to impair glucose uptake in a dose-dependent manner,inhibited cellular IRS1 expression,impaired IRS2 protein expression independent on glucose concentration in the medium,it did not influence the contents of p85,PKB and GLUT4,but decreased GLUT4 translocation.Conclusions Chronic high insulin induced insulin resistance may be caused by the increasing glucose concentration surrounding the cells.The mechanism may be involved in affecting IRSs protein expression and GLUT4 translocation.
2.Nursing Intervention on Visual Quality after Excimer Laser Refractive Correction Operation

Objective To explore the impact of nursing intervention on the visual quality of patient after excimer laser refractive correction operation. Methods 400 patients, underwent excimer laser refractive correction operation and regularly followed up more than 6 months, were selected in our hospital from January to December in 2010. The clinical data of 200 patients without nursing intervention were analyzed as control group compared with those of 200 patients receiving nursing intervention as intervention group. Both groups were conducted routine nursing instruction before the operation. The intervention group was given systematic and normative nursing intervention besides routine nursing instructions, including psychological intervention, behavioral intervention, close observation, discovery of adverse reaction in time and giving early disposal. The cooperation reaction of the patients during the operation, and incidence of surgical complications during and post surgery were observed. Results Compared with the control group, the incidence of adverse reactions of visual influence such as incomplete valve during surgery, eccentric cutting after surgery, overcorrection, glare, and refractive regression were significantly lower (P<0.05). Conclusion The implementation of nursing intervention can reduce the adverse reaction of excimer laser refractive correction operation, alleviate the suffering of patients and promote visual quality after surgery. 3.Comparison of therapeutic effect and safety between atorvastatin and simvastatin in patients with angi-na pectoris of coronary heart disease

Objective:To compare therapeutic effect and safety between atorvastatin and simvastatin treating angina pectoris of coronary heart disease (CHD).Methods:A total of 150 patients with chronic stable angina pectoris (SAP)of CHD were selected and randomly and equally divided into atorvastatin group and simvastatin group accord-ing to number table method.Changes of angina pectoris attack frequency,duration,degree of pain,nitroglycerin dosage and blood lipids were compared between two groups before and after treatment.All patients were followed up six months.Results:Compared with before treatment,there were significant reductions in angina pectoris attack frequency,duration,pain degree and nitroglycerin dosage after treatment 12 weeks in two groups,P<0.05 all;but there were no significant difference in above indexes between two groups before and after treatment (P>0.05 all). Compared with before treatment,there were significant reductions in serum levels of total cholesterol [ (5.62 ± 0.54)mmol/L vs.(3.68±0.41)mmol/L,(5.78±0.56)mmol/L vs.(3.33±0.43)mmol/L],triglyceride [(2.30 ±0.42)mmol/L vs.(1.62±0.34)mmol/L,(2.35±0.41)mmol/L vs.(1.68±0.69)mmol/L]and low density lipoprotein cholesterol [(3.56±1.22)mmol/L vs.(2.76±0.83)mmol/L,(3.48±1.21)mmol/L vs.(2.43± 0.72)mmol/L],and significant increase in level of high density lipoprotein cholesterol [(1.32±0.35)mmol/L vs. (1.58±0.34)mmol/L,(1.30±0.46)mmol/L vs.(1.60±0.45)mmol/L]in atorvastatin group and simvastatin group after treatment (P<0.05 all),but there were no significant difference in blood lipid levels between two groups after treatment (P>0.05);there was no significant difference in incidence rate of cardiovascular events (28.00% vs.25.33%,P>0.05)between two groups during follow-up.Conclusion:Both atorvastatin and simvasta-tin can effectively regulate the state of high blood lipid,and possess good effect and safety for angina pectoris of coronary heart disease.
4.Significance of thyroid hormone level change in patients with acute ST elevation myocardial infarction

Objective:To evaluate clinical significance of change of serum thyroid hormone level in patients with ST elevation myocardial infarction (STEMI) and old myocardial infarction (OMI) .Methods :Clinical data of 52 STE‐MI patients (STEMI group) and 48 OMI patients (OMI group) hospitalized in our department of cardiology were retrospectively analyzed .Automatic chemiluminescent analyzer was used to quantitatively measure thyroid hormone levels in two groups .Results:Compared with OMI group ,there were significant reductions in serum levels of free triiodothyronine [FT3 ,(4.45 ± 1.09) pmol/L vs .(3.03 ± 0.14) pmol/L] ,free thyroxine [FT4 ,(18.53 ± 3.89) pmol/L vs .(11.83 ± 0.53) pmol/L] and thyrotropic‐stimulating hormone [TSH ,(1.85 ± 1.04)μIU/L vs .(0.26 ± 0.13)μIU/L] in STEMI group ,P<0.01 all .Conclusion:Thyroid hormone levels of STEMI patients are significant‐ly higher than those of OMI patients , serum thyroid hormone levels may reflect severity of coronary vessel lesions in patients with myocardial infarction.
5.Chlamydia pneumoniae infection,inflammatory factor and blood lipid level in patients with coronary heart disease

Objective:To explore the Chlamydia pneumoniae (Cpn) immunoglobulin (Ig) G positive rate , inflamma‐tory factors and blood lipid levels in patients with coronary heart disease (CHD) .Methods :A total of 50 CHD patients hospitalized in our hospital were selected as CHD group ,and 33 non‐CHD patients visited to outpatient clinic in same period were regarded as non‐CHD control group .Blood lipid ,inflammatory factor levels ,and positive rate of Cpn IgG were compared between two groups .Results :Compared with non‐CHD control group ,there were significant rise in levels of TC [ (3.96 ± 1.07) mmol/L vs .(4.75 ± 1.33) mmol/L] ,TG [ (1.05 ± 0.42) mmol/L vs .(1.67 ± 0.82) mmol/L] and LDL‐C [ (2.12 ± 0.47) mmol/L vs .(2.88 ± 0.86) mmol/L] , P < 0.05 all ;significant rise in levels of inflammatory factor :C reactive protein [CRP ,(3.85 ± 0.87) mg/L vs .(5.80 ± 1.60) mg/L] ,interleukin‐ 6 [IL‐6 ,(6.50 ± 1.53) mg/L vs .(15.80 ± 7.62) mg/L] and tumor necrosis factor‐α [TNF‐α ,(5.50 ± 1.60) mg/L vs .(16.10 ± 2.20) mg/L] in CHD group , P < 0.05 all ;positive rate of serum Cpn IgG in CHD group was significantly higher than that of non‐CHD control group (78.0% vs .21.2% ,P < 0.05) .Conclusion : In present study ,positive rate of serum Cpn IgG ,inflammatory factor and blood lipid levels significantly rise in CHD patients ; Chlamydia pneumoniae infection and dyslipidemia may have a synergistic effect ,cause occurrence and development of CHD in common .

7.The Molecular Mechanism of Glucocorticoid-Induced Insulin Resistance

Objective To explore the effect of glucocorticoid on glucose transport activity and the expression of insulin signaling peptides in the primary cultured rat adipocytes .Methods Isolated rat adipocytes were cultured for 2h,8h,16h and 24h respectively at 5 mmol glucose with dexamethasone (Dex) 0 3?mol. Then the glucose uptake, cellular contents of insulin receptor substrate (IRS) 1/2, phosphatidylinositol 3-kinase 85 subunit (p85) and protein kinase B(PKB)were measured by Western blotting.Results These adipocytes treated with Dex have shown to impair the basal and insulin-induced glucose uptake with a maximum inhibition at 2h and 24h respectively; Dex down-regulated IRS1 protein expression in a time-dependent manner and up-regulated IRS2 content. The cellular p85 and PKB contents were also decreased by Dex. Conclusion Chronic exposure to glucocorticoid can inhibit glucose uptake and induce insulin resistance. The mechanism may be involved in affecting the expression of insulin signaling peptides.
8.Effects of high glucose on phosphorylations of insulin signaling proteins in rat adipocytes

Objective To explore the effects of high glucose on glucose transport activity, and phosphorylationandexpressionofinsulinsignalingproteins in primary cultured rat adipocytes. Methods Isolated rat adipocytes were cultured at different glucose concentrations (5, 10, 15, 25 mmol/L) for 24 h. Then the glucose uptake, the phosphorylations of insulin receptor (IR), insulin receptor substrate (IRS) 1 and 2 and protein kinase B (PKB) as well as the protein expressions of IRS1, IRS2, p85 subunit of phosphatitylinositol 3 kinase (p85) and PKB were measured. Results These adipocytes treated with different high glucose showed the impairment of the basal and insulin induced increase in glucose uptake and significant decrease of IR, IRS1 and PKB phosphorylations as well as IRS1 protein expression, but up regulation of IRS2 protein expression. p85 and PKB contents and IRS2 phosphorylation were unaffected. Conclusion The exposure to high glucose inhibits glucose uptake and induces insulin resistance in adipocyte. The mechanism may be involved in affecting the multiple step phosphorylations and the expressions of insulin signaling proteins.
9.Glucocorticoid Aggravates High Glucose-Induced Insulin Resistance

Objective To explore the molecular mechanism of glucocorticoid and high glucose-induced insulin resistance(IR).Methods Isolated rat adipocytes were cultured for 24h at 5 or 25 mM glucose with or without dexamethasone (Dex) 0 3?M. Then the glucose uptake , the phosphorylation of tyrosine of insulin receptor substrate (IRS)1/2 and expression of IRS 1/2 and protein kinase B(PKB) protein were measured by western blot analysis with molecular dynamics. Results These adipocytes treated with 25 mM glucose have shown to impair glucose uptake, IRS1 phosphorylation and the protein expression; Combined treatment with Dex enhanced high glucose-induced the suppression, and inhibited IRS1 tyrosine phosphorylation; High glucose increased IRS2 protein expression, Dex abolished partly its the effect. Conclusions High glucose can induce IR, Dex can aggravate the effect induced with high glucose. The mechanism may be involved in affecting the phosphorylation and expression of insulin signaling proteins.
10.The clinical value of fertility index in patients with infertility for EMT

Objective to investigate the clinical application value of endometriosis fertility index (EFI) in the evaluation of ab‐dominal surgery endometriosis(EMT) combined infertility patient .Methods the date of 105 cases of infertility EMT merge were retrospectively analyzed from January 2009 to January 2013 in the Yuanzhou MCH and the obstetrics and gynecology department of Yuanzhou people′s hospital .All patients were followed up for two years and the rates of cumulative pregnancy were compared .Re‐sults the rates of two year cumulative pregnancy rate were 64 .80% (68 cases) .The two year cumulative pregnancy rates of EFI score ≤ 4 points ,5 to 7 points ,8 to 10 points were 34 .48% ,57 .14% ,92 .68% (P<0 .05) .There were no difference in cumulative pregnancy rate of different types of EMT in patients(P>0 .05) .Conclusion EFI and EMT with infertility pregnancy after conser‐vative treatment has a close relationship ,and EFI score would provide important guidance for follow up treatment .
