1.Integrated Traditional Chinese Medicine and Western Medicine Treat Disease of Cystic Hyperplasia of Breast

[Objective] To observe the curative effect of integrated Traditional Chinese Medicine and western medicine on the disease of cystic hyperplasia of breast.[Method] The 214 cystic hyperplasia of breast cases were randomly divided into two groups.125 cases in the integrated Traditional Chinese Medicine and western medicine group(observed group).Control group(89 cases)were treated in common practiced western medicine.[Result]In observed group,70 cases fully recovered from illness,40 cases were excellent,6 cases were effective,9case were ineffective.Total effective rate was 92.8%.In control group,27 cases were fully recovered from illness,22 cases were excellent,20 cases were effective,9 cases were ineffective.Total effective rate was 77.4%.The differences between two groups were significant(P

Objective To analysis the result of tumor screening for health examination participants.Methods Tumor screening was applied for 15 863 health examination participants without malignant tumor disease history.Clinical examination,laboratory tests and imaging studies were comprehensively analysed combining the results of other medical items.The cancer detection rate and asymptomatic rate were compared among groups of different age,gender and existence of relevant clinical symptoms,respectively.χ2 test and Fisher exact test were adopted for statistical analysis.Results Among 475 tumor patients with a definite diagnosis,116 were malignant tumor;The total detection rate of the group who were younger than 35 year-old(0.617%)was higher than other groups,the detection rate of intracranial neoplasms of the two groups who were younger than 45 year-old (0.206% and 0.132%,respectively)was higher than other groups,the total detection rate of 75-85 group (0.248%)was lower than other groups; The detection rate of lung cancer,liver cancer of the male(0.228% and 0.080%,respectively)was higher than the female,The detection rate of breast cancer and intracranial neoplasms of the female(0.366% and 0.100%,respectively)was higher than the male; There are 67 patients without correlated clinical symptoms,the asymptomatic rate was 57.8%.Conclusions Tumor screening was of importance in health management,which should be carried out actively in health examination.
3.Effect of chemotherapy on function of isletβcells in patients with breast cancer

Objective To study the effect of chemotherapy on function of islet βcells in patients with breast cancer . Methods One hundred and fifty-six patients with breast cancer who had not received chemotherapy were enrolled from Sep 2012 to Aug 2013 at Chongqing Cancer Institute .One hundred and twenty-three cases completed 2 cycles of chemotherapy;one hundred and twelve cases completed 4 cycles;and, eighty-six cases completed 8 cycles.Height, weight, waist circum-ference(WC), hip circumference(HC) and blood pressure of each patient were measured .Waist-high ratio calculation (WHtR),waist-hip ratio(WHR) and body mass index(BMI) were calculated.All subjects underwent 75 g oral glucose tolerance test(OGTT)and insulin release test before chemotherapy,2 cycles,4 cycles and 8 cycles of chemotherapy, respec-tively.Besides, homeostasis model assessment of insulin resistance index ( HOMA-IR), Matsuda index and pancreatic βcells HOME(HOME-β) were calculated.Results Fasting blood glucose before chemotherapy and after 2 cycles, 4 cycles and 8 cycles of chemotherapy was (4.84 ±0.56), (4.78 ±0.61), (5.02 ±0.56) and (5.96 ±0.52) mmol/L, respec-tively.After therapy, the change in fasting blood glucose after 2 cycles and 4 cycles of chemotherapy was not statistically significant(P>0.05), but was significantly higher after 8 cycles than before therapy(P<0.05).2 h blood glucose before chemotherapy and after 2 cycles, 4 cycles and 8 cycles of chemotherapy was (5.43 ±0.50), (5.66 ±0.58), (8.30 ± 0.66) and (9.65 ±0.80) mmol/L, respectively.With chemotherapy treatment increased , the HOME-IR increased gradu-ally and the sharpest one was after 8 cycles, Matsuda index decreased gradually and the biggest drop was after 4 cycles,and HOME-βdecreased gradually after a transient increase .Conclusion Chemotherapy for breast cancer patients may lead to insulin resistance , pancreatic βcell function decline and the emergence of abnormal glucose tolerance .Four cycles of chemotherapy shoucd be followed by hypoglycemic drugs .
4.Experimental study on the chondroyte-like differentiation of marrow mesenchymal stem cell

BACKGROUND: Tissue engineering method has been employed to recover cartilage defect and overcome many traditional shortages.OBJECTIVE: In vitro marrow mesenchymal stem cells (MSCs) of adult rat was induced to differentiate into chondrocyte phenotype so as to probe into the feasibility of MSCs to be cartilage seed cell in tissue engineering.DESIGN: Completely randomized design and controlled experimental study.SETTING: Department of Histology and Embryology, and Department of Neurobiology, Harbin Medical University MATERIALS: Six Wistar rats of either gender, cleanness grade, were provided by Experimental Animal Center, Affiliated Second Hospital, Harbin Medical University. Permission number of experimental animal production was SCXK(black) 20020002.METHODS: The MSCs of the second generation adult rat was taken and divided into test group and control group. The test group was induced with serum free and control group was induced with 10% fetal calf serum. Collagen type Ⅱ immunohistochemical staining, toluidine blue staining was performed to detect differentiation.MAIN OUTCOME MEASURES: Identification of chondrocyte, comparison of the positive rate of Collagen type Ⅱ immunohistochemical staining at different induction time point.RESULTS: Induced MSCs had identical characteristic to the chondrocyte.CONCLUSION: In vitro culture can induce MSCs to differentiate directly to chondrocyte-like cell. The results suggest that it is feasible to use MSCs as seed cells in the cartilage tissue engineering.

Patient satisfaction is an important index of hospital quality evaluation and performance evaluation.This study established multi-attribute patient satisfaction index system based on the theory of customer satisfaction index,determined the index weight by combination weighting approach,and graded the hospitals and adjusted the weights by the RSR method.A set of comprehensive evaluation scheme is initially formed,which is suitable for patient satisfaction evaluation and performance evaluation.

Objective Examination and evaluation of employee satisfaction index in the hospital performance evaluation.Methods Stratified sampling,field survey and telephone survey were used in customizing a questionnaire for two surveys in July and December 201 5 respectively.Results The standardized score of employee satisfaction was 86.252±1 5.1 53,and the lowest score was found in the canteen environment and food quality.Conclusion Employee satisfaction is found as good overall,and targeted measures are recommended to improve insufficiencies for better employee satisfaction.
7.How do medical students understand disease behaviors?Evidence from event-related potentials

Objective:To investigate how medical students process Western medicine(WM)terms vs.traditional Chinese medicine(TCM)terms and how this is impacted by analogical priming from the perspective of cognitive neuroscience.Methods:The experiments were designed as a 2(TCM terms vs.WM terms)x 2(correct terms vs.incorrect terms)x 2(analogical priming task vs.non-priming task)scheme.A total of 26 medical stu-dents completed a non-priming judgment task and an analogical priming judgment task on medical terms.During the tasks,the participants were asked to make correct/incorrect judgments on WM terms and TCM terms,and their behavioral data and event-related potentials(ERPs)were recorded.Results:Behaviorally,the response speed and accuracy of WM terms were higher than those of TCM terms(both P<.001),indicating a prominent concreteness effect.Analogical priming shortened the response time to medical terms(P<.001),and the response time to TCM terms was shortened more significantly(P=.001).For ERPs,WM terms induced a larger P200(an early positive ERP component),a smaller N400(a negative ERP component),and a higher late positive ERP component,indicating supe-riority of attention adjustment,smaller-scale semantic activation,and a higher cost of late semantic analysis and integration.However,the analogical priming eliminated the difference between WM and TCM terms in P200 and N400 while maintaining it in the LPC.This suggests that WM terms are sensitive to analogical attention adjustment,and TCM terms are susceptible to analogical semantic integration.Conclusion:Our findings suggest that WM and TCM disease behavior terms may initially differ in concreteness or the use of a verbal-linguistic system.Analogy is more conducive to understanding TCM terms.This research provides important neuroscientific evidence of the difference in thinking between TCM and WM.
8.Study of TCM thinking based on TCM language:a novel way from cogni-tive neuroscience

TCMlanguage is a language based on metaphorical thought,describing a metaphorical world of human physiology,pathology and treatment of diseases.Metaphor is not only a linguistic phenomenon, but more importantly,a cognitive or thinking mode.Various cognitive modes will produce diverse cere-bral electromagnetic and metabolic information.Understanding Chinese medicine language will lead to different brain nerve activities from those Western medicine leading to,if there were differences between the two medicines.Event-related potential (ERPs)technology or the functional magnetic resonance ima-ging (fMRI)technology can be used to capture the different brain activities.This article provided a new way for exploration the proposition“TCM has a unique way of thinking”,from cognitive neuroscience. Consequently,the application of cognitive neuroscience methods and theories in metaphorical cognition study and the research status of TCMlanguage cognition were analyzed.Accordingly,the theoretical hy-pothesis of TCMthinking and the scheme of linguistic contrastive study of Chinese and Western medicine by application of ERPs technology and fMRI technology were put forward.It would be of great signifi-cance that ERPs and fMRI provide a novel way for empirical study of TCMcognition.
9.Eczematoid clear cell acanthoma of the nipple/areola: the first case reported in China and literature analysis

Objective:To investigate clinical manifestations, pathological features and diagnosis of eczematoid clear cell acanthoma of the nipple/areola.Methods:The clinical manifestations, histopathological features, special staining results and immunohistochemical features of a case of eczemtoid clear cell acanthoma of the nipple/areola firstly reported in China were analyzed, and compared with those of similar cases in foreign literature.Results:The female patient presented with recurrent pruritic rashes on the left nipple and areola for over 2 years. Skin examination showed hypertrophic skin on the left nipple and areola, and scattered erythema, hypopigmented macules and hyperpigmented macules on the areola, which were covered with a few crusts and scales. Histopathological examination of the skin lesions showed focal epidermal crusts and scales, focal parakeratosis, extended and fused rete ridges, thickened spinous layer, focal spongiosis, clear cell clumps in the spinous cell layer, telangiectasia in the superficial dermis, with infiltration of a few eosinophils and neutrophils. Periodic acid-Schiff staining showed positive results, and immunohistochemical staining revealed positive reaction for epithelial membrane antigen. Topical treatment with triamcinolone acetonide and econazole cream was effective, and topical application of 3% boric acid solution could alleviate exudation. During the 6-month follow-up, the patient experienced intermittent recurrence twice, and responded well to the above treatment.Conclusions:Eczematoid clear cell acanthoma of the nipple/areola has unique clinical and pathological features, revealing that it′s a new subtype of clear cell acanthoma. Pathological examination is the gold standard for its diagnosis.
10.The sutureless technique for total anomalous pulmonary venous connection

Sutureless technique is a recently developed operation as a first-line therapeutic options for total anomalous pulmonary venous connection (TAPVC). It was originally used to relieve postoperative pulmonary venous obstruction after TAPVC repair and shared a favorable safety and efficacy. Sutureless technique has advantage over conventional repair since the sutures will not touch the intima of pulmonary veins, leading to limited intimal hyperplasia and anastomotic stenosis. In recent years, the sutureless technique has been adopted as a prophylactic method to reduce postoperative obstruction. Compared with the conventional surgery, the indications of the sutureless technique are yet unclear, and a variety of improved methods of sutureless technique with satisfactory outcomes have emerged. This paper review the development of the sutureless technique, introduce some improvements of the surgical technique and summarize indications of suturelss technique.
