Objective To study the changes of neuron-specific-enolase (NSE), S100B protein and neuropeptide Y (NPY) levels in serum and cerebrospinal fluid of children with viral encephalitis and their clinical significance. Methods The NSE, S100B protein and NPY levels in the serum and cerebrospinal fluid of 50 children with viral encephalitiswere were measured, and another 20 children without central nervous system infection were selected as controls. Results The NSE, S100B protein and NPY levels in the serum and cerebrospinal fluid of children with viral encephalitis[serum: (18.90 ± 5. 50)μg/L, (0. 57 ±0. 26) μg/L, (267. 3 ± 54. 7 ) μg/L; GSF: ( 10. 45 ± 4. 40) μg/L, (0. 93 ± 0. 53 ) μg/L, (347.2 ± 60. 6) μg/L] were higher than those in control group [serum: ( 10. 35 ± 2. 49 ) μg/L, ( 10 ± 0. 06 ) μg/L, ( 67. 8 ±22.5)μg/L;GSF:(3.96 ± 1.57)μg/L,(0. 29 ±0. 18)μg/L,(102.6 ±38.9) μg/L] ( P <0.01). The levels of serum and CSF NSE S100B protein and NPY in critical patient[serum: (21.93 ±5.39)μg/L,(0.71 ±0. 31)μg/L, (32. 5 ± 62. 8) μg/L;GSF: (13.05 ±4.41)μg/L, (1.23 ± 0. 66) μg/L, (407.3 ±68. 1 ) μg/L] were higher than ordinary patients [serum: ( 15.93 ± 4. 02 ) μg/L, ( 0. 42 ± 0. 14 ) μg/L,(234.7 ±51.2)μ.g/L;GSF:(8.05 ± 1.77) μg/L,(0. 63 ±0.26)μg/L, (320.2 ±59.5) μg/L] ( P <0. 01 ). Conclusion NSE, S100B protein and NPY can be used to evaluate encephalitis condition, brain damage degree and prognosis of viral encephalitis.

Objective To understand the epidemiological feature of rotavirus and humancaliciviruses in Miyun county of Bei-jing and provide theoretical basis for prevention and control.Methods Stool specimens were collected from 240 diarrhea pa-tients in 3 hospitals from April 2011 to March 2012.Enzyme linked immunosorbent assay (ELISA)and reverse transcrip-tion-polymerase chain reaction (RT-PCR)were used to detect rotavirus and humancaliciviruses respectively.Data was statis-tically analyzed.Results The positive rate of rotavirus was present in 10.3%.Positive cases mainly fell in age group 0~5. More male diarrhea patients than female were infected by humancaliciviruses (P<0.05).The positive rate of humancalicivir-uses was present 8.8%.Positive cases mainly fell in age group 19~40.Conclusion The humancaliciviruses susceptible pop-ulations were middle-aged youth and male in Miyun,the infection peak of humancaliciviruses is in autumn.The main infection of rotavirus population was babies.

OBJECTIVE:To virtually screen potential α-glycosidase inhibitor ingredients from C. mori and F. mori,and to pro-vide reference for finding out new typeα-glycosidase inhibitor ingredient. METHODS:Surflex-Dock module of Sybyl-x 2.0 molecu-lar simulation software was used to perform the docking of small molecule compound,which was from the ingredients of C. mori and F. mori as ligand stated in literatures,with α-glycosidase. Total score of affinity scoring function was equal to 7 as the thresh-old value,to judge potential α-glycosidase inhibitor ingredient in C. mori and F. mori. RESULTS:After 70 small molecule com-pounds docked with α-glycosidase, 10 compounds showed binding activity (Total score≥7.00). Among them, moracin M-3′-O-β-D-glucopyranoside,5,7,2′-trihydroxyflavanone-4′-O-β-D-glucoside,mulberroside A,resveratrol-4,3′-di-O-β-D-gluco-pyranoside and 1,4-dideoxy-1,4-imino-(2-O-β-D-glucopyranosyl)-D-arabinitol had higher binding activity with α-glycosidase(Total score>8.00). CONCLUSIONS:Multi-constituents of C. mori and F. Mori show potential α-glycosidase inhibitory activity. The method is a kind of highly targeted,rapid and efficient approach to discover α-glycosidase inhibitor from traditional Chinese medi-cine.

Objective To determine the changes in serum and urine vitamin D binding protein ( VDBP) concentrations in type 2 diabetes, and to explore the clinical significance. Methods The serum and urine VDBP concentrations in 102 healthy individuals and 106 type 2 diabetic patients were determined by ELISA. For analysis and comparison, 106 type 2 diabetic patients were divided into imperfect glycemic control subgroup and perfect glycemic control subgroup, microalbuminuria subgroup and normal albuminuria subgroup. Results The cut-off point of serum VDBP concentrations was 60. 6 μg/ ml and the cut-off point of the urine ratio of VDBP and creatinine was 7. 76 mg/ g, and both were determined according to the upper limit of 97. 5 % credit intervals in 110 healthy individuals. Serum VDBP concentration and the urine ratio of VDBP to creatinine in type 2 diabetic patients were significantly higher than those in the healthy individuals ( P < 0. 01 ), the imperfect glycemic control subgroup had higher serum VDBP concentrations and the urine ratio of VDBP to creatinine than those in the perfect glycemic control subgroup ( P <0. 05). The microalbuminuria subgroup had higher urine ratio of VDBP to creatinine than that in the normal albuminuria subgroup ( P<0. 01). Urine ratio of VDBP to creatinine in diagnosing early diabetic nephropathy had sensitivity of 96. 4 % , specificity of 68 % , and concordance of 83% . Conclusion Detection of serum VDBP levels has some reference value in understanding the state of diabetes. Combined determinations of urine ratio of VDBP to creatinine and ratio of albumin to creatinine have significant clinical value in the early diagnosis of diabetic nephropathy.

Objective To observe the change of the serum trough concentration and its pharmacokinetics of vancomycin in patients with severe acute pancreatitis (SAP), and to analyze the factors influencing vancomycin concentration. Methods A retrospective analysis was conducted. Steady-state trough concentrations of vancomycin from patients (18-80 years old) with SAP concomitantly with G+ infection admitted to Intensive Care Unit (ICU) of Ruijin Hospital Affiliated to Shanghai Jiaotong University School of Medicine from January 2010 to December 2016 were enrolled. According to the usage time of vancomycin, the patients with SAP were divided into early group (onset within 21 days), middle group (onset between 21-28 days) and late group (onset over 28 days). The gender, age, body weight, clinical diagnosis, acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ ) score, renal function, and the pharmacokinetic parameters were recorded. Influencing factors of vancomycin was analyzed by multiple linear regression and stepwise regression. Results Fifty-eight patients were enrolled who contained 134 times trough concentrations of vancomycin. There were 41 patients enrolled and 61 times of trough concentrations in the early group, 24 patients enrolled and 33 times of trough concentrations in the middle group, and 28 patients enrolled and 40 times of trough concentrations in the late group. There was no significant difference in gender, age, body weight, serum creatinine, creatinine clearance (CCr), albumin, APACHE Ⅱ score among the three groups. There was significantly difference in the duration from the onset time to vancomycin administration between early, middle groups and late group (days:15.9±3.2, 23.3±2.2 vs. 35.0±6.7, both P < 0.05). The positive liquid balance in early group was lower than that of late group (mL: 1565.2±3132.1 vs. 3675.1±3411.5, P < 0.01), while it was increased in the middle group as compared with that of late group (mL: 5078.7±3892.4 vs. 3675.1±3411.5, P < 0.05). The average daily dose of vancomycin in the early, middle and late groups were (14.7±5.0), (15.0±2.8), (17.0±4.2) mg/kg, respectively, and there was no significant difference (P > 0.05). Compared with the standard concentration (15 mg/L) of vancomycin, the serum trough concentration of vancomycin was significantly reduced in SAP patients [(7.5±4.3) mg/L, P < 0.01]. Apparent volume of distribution (Vd) was (72.4±15.4) L, and clearance rate (CL) was (9.0±2.8) L/h. According to the Bayesian, the serum trough concentration of vancomycin was significantly reduced in early group and middle group compared with late group (mg/L: 5.0±2.1, 7.3±2.5 vs. 11.5±5.1, both P < 0.01), CL was significantly increased (L/h: 10.5±3.0, 8.1±1.9 vs. 7.4±1.9, both P < 0.05), and Vd was significantly increased in early group compared with late group (L: 73.7±15.5 vs. 71.0±12.6, P < 0.05). It was shown by multiple linear regression analysis that there was strong relationship between serum trough concentration and the serum creatinine, CCr, average daily dose and the starting time of vancomycin treatment (r value were 0.449, -0.318, 0.373, 0.763, respectively, all P < 0.05). Conclusions The serum trough concentration of vancomycin was significantly reduced in SAP patients. And the earlier usage of vancomycin, the lower of the trough concentration is. Therefore, higher dosage regimen was needed to ensure the clinical effect, and reduce the bacterial resistance.

Objective To evaluate the serum trough concentration and the pharmacokinetics/pharmacodynamics (PK/PD)of vancomycin in patients with severe acute pancreatitis (SAP), and analyze the effect of vancomycin continuous infusion for optimizing the characteristics of its PK/PD.Methods The inhospital patients with SAP received vancomycin treatment and admitted to emergency intensive care unit (EICU) of Ruijin Hospital Affiliated to Shanghai Jiaotong University School of Medicine from January 2011 to December 2016 were enrolled. Steady-state trough concentrations of vancomycin from patients were collected retrospectively. The SAP patients were divided into augmented renal clearance (ARC) and non-ARC groups, as well as systemic inflammatory response syndrome (SIRS) and non-SIRS groups according to the patients with or without symptom above. Adjustments of increased dosage or 24-hour continuous infusion or increase vancomycin dose were made for patients if the steady-state trough concentrations fell below the target level. Steady state trough concentration for vancomycin intermittent infusion or steady state concentration for vancomycin continuous infusion was determined by the fluorescence polarization immunoassay method. PK parameters of vancomycin were calculated using the Bayesian estimator and the area under the serum drug concentration-time curve (AUCc-t), the minimum inhibitory concentration (MIC) and AUCc-t/MIC was recorded and calculated.Results The steady state trough concentration or steady state concentration from 61 patients with SAP were collected with mean steady state trough concentration of vancomycin of (7.7±4.4) mg/L, which was significantly lower than standard concentration (15 mg/L,P < 0.001). Apparent volume of distribution (Vd) and clearance of vancomycin was (1.06±0.26) L/kg and (8.9±2.8) L/h. The serum steady state trough concentration of vancomycin in ARC group (n = 33) was significantly lower than that in non-ARC group (n = 28; mg/L: 6.7±3.5 vs. 8.2±4.1, P < 0.01), clearance was significantly increased (L/h: 9.8±2.9 vs. 7.7±2.2,P < 0.01). Compared with non-SIRS group (n = 31), the serum steady state trough concentration of vancomycin in SIRS group (n= 30) was significantly lowered (mg/L: 6.1±3.2 vs. 13.0±4.2,P < 0.01), and clearance was significantly increased (L/h: 9.4±2.0 vs. 7.1±2.1,P < 0.05). Compared with the only increasing vancomycin dose group (n = 29), vancomycin continuous infusion for 24 hours (n = 21) could significantly reduce daily dosage (mg/kg: 13.6±3.9 vs. 19.1±3.5,P < 0.01), increase the serum trough concentration (mg/L: 18.1±7.0 vs. 12.6±5.3,P < 0.01), and improve the AUCc-t/MIC.Conclusions The serum trough concentration of vancomycin was significantly reduced in SAP patients with ARC. The more serious of the SIRS is, the lower the vancomycin trough concentration is. Vancomycin 24-hour continuous infusion could optimize the PK/PD parameters, decrease the daily dose, increase the clinical effect, and reduce the bacterial resistance.

Objective To explore the occurrence and change rules of symptom distress in breast cancer patients before and one year after surgery. Methods From June 2016 to July 2016, this study selected 80 breast cancer patients at initial diagnosis by and with surgery in a Class Ⅲ Grade A cancer hospital of Guangzhou as subjects by pathobiology with the method of convenience sampling. The symptom distress was investigated with the M.D.Anderson symptom inventory (MDASI) before, three months, six months and one year after surgery. Results The scores of symptom sub-scale of breast cancer patients before, three months, six months and one year after surgery were (1.23±1.04), (3.20±1.54), (3.09±1.27) and (1.10±0.69) respectively with statistical differences (F=97.70,P < 0.01). The scores of distress sub-scale of breast cancer patients before, three months, six months and one year after surgery were (1.49±1.63), (4.24±2.30), (3.08±1.86) and (0.98±0.89) respectively with statistical differences (F=89.01,P < 0.01). The level of symptom distress of breast cancer patients was the highest three months after surgery and decreased gradually after that, and was lower one year after surgery than that before surgery. Conclusions Breast cancer patients had the mild to moderate levels of symptom distress from preoperation to one year after surgery. The level of symptom distress of patients is the highest three months after surgery and decreases gradually after that, and is lower one year after surgery than that before surgery. Patients are with varying degrees of symptom distress in each period which points out that we should provide targeted management for adverse reactions caused by disease or treatments at different periods.

Objective To explore the application effect of chemotherapy management table in breast cancer case management mode. Methods By using the convenient sampling method,breast cancer patients who recruited the breast cancer case management were selected from January 2016 to December 2017 in the Sun Yat-Sen University Cancer Center as the research objects. 182 patients who were admitted from January to December 2016 were included in the control group;183 patients who were admitted from January to December 2017 were included in the intervention group. The control group carried out the breast cancer case management mode,and the intervention group used the chemotherapy management plan table to nurse the patients. The intervention effect of the two groups was compared. Results In the control group,98.35％(179/182)patients completed chemotherapy on time,and in the intervention group,99.45％(182/183)patients completed chemotherapy on time. There was no significant difference in the compliance between the two groups(χ2=0.258,P> 0.05). In the control group,84.06％(153/182)patients completed the auxiliary examination on time,and in the intervention group,98.36％(180/183) patients completed the auxiliary examination on time. There was a significant difference between the two groups (χ2=21.559,P<0.01). The standard accuracy rate of the treatment of abnormal index in the control group was 75.82％(138/182),and that in the intervention group was 92.90％(170/183). There was statistically significant difference between the two groups in the accuracy rate of the treatment of abnormal index in the auxiliary examination of chemotherapy(χ2=20.182,P<0.01). Patients in the intervention group actively reported their physical condition and examination results to the case manager for a total of 1020 times,compared with 210 times in the control group. Conclusions The application of chemotherapy management table in breast cancer case management mode can improve the compliance of postoperative chemotherapy auxiliary examination of breast cancer patients,improve the ability to deal with abnormal examination indicators,improve the enthusiasm of patients' active consultation and the work efficiency of case managers.

Objective To explore the current status of medication literacy among urban elderly patients with chronic diseases in Anhui Province,aiming to reveal the factors influencing their medication literacy,and to propose targeted measures for improvement.Methods This research involved 381 participants aged 60 and above.It was conducted in Anhui province between December,2022 and January,2023,with data collected through face-to-face interviews by pharmacists.Single-factor analysis and ordinal multi-class logistic regression analysis were conducted to determine factors affecting medication literacy.Results Medication literacy cognition and medication literacy behavior were rated as good among urban older adults in Anhui province of the 294 valid questionnaires.Those who did not understood package insert exhibited significantly lower medication literacy behavior than those who fully understood[estimate=-1.224,95%CI=(-2.130,-0.317),P<0.01].Elderly patients with chronic diseases faced issues such as an inability to read or understand drug instructions in the investigation.90.48%of elderly patients with chronic diseases never heard or seldom heard of medication guidance services.Conclusion Medication literacy among urban elderly patients with chronic diseases is generally good in Anhui province.The ability to understood drug instructions significantly influenced the medication literacy of urban elderly patients with chronic diseases.Modifying the drug instructions to meet the reading needs of the elderly patients with chronic diseases and developing pharmaceutical care could effectively enhance rational drug use among this demographic.

Objective To explore the therapeutic mechanism of Modified Lugen Formula(Phragmitis Rhizoma,Cicadae Periostracum,Batryticatus Bombyx,Lonicerae Japonicae Flos,Glycyrrhiza,Menthae Haplocalycis Herba,Notopterygii Rhizoma et Radix,Puerariae Lobatae Radix,Bupleuri Radix)in treating influenza from the virus-host interaction interface.Methods The phytocompounds were first collected from the HERB database,and then potential active compounds were screened out by Lipinski's rules of five.The targets of active compounds were further predicted through the SwissTargetPrediction platform.Differentially expressed genes(DEGs)were determined from the human H1N1 influenza dataset GSE90732 available in the Gene Expression Omnibus database(GEO).H1N1-Homo sapiens-related protein-protein interactions(PPIs)were gathered from the Pathogen-Host Interaction Search Tool(PHISTO).The above mentioned bioinformatic datasets were integrated.Then a PPI network and a Formula-virus-host interaction network were constructed using Cytoscape.Functional enrichment analyses were performed by using R software.Finally,molecular docking was carried out to evaluate the binding activities between the key compounds and targets.Results A total of 1 252 active compounds,1 415 targets,951 influenza-related DEGs,and 10 142 H1N1-Homo sapiens-related PPIs were obtained.There were 72 intersection targets between the Modified Lugen Formula and influenza.Functional enrichment analyses showed that these targets are closely related to host defense and programmed cell death.The network topological analysis showed that active compounds in the Modified Lugen Formula,such as oleanolic acid,γ-undecalactone,and longispinogenin,regulate viral proteins M2,NA,NS1,and HA and/or the host factors HSP90AA1,NRAS,and ITGB1,thus exert therapeutic effect.Molecular docking results confirmed that these compounds had a good binding ability with the targets.Conclusion Multiple active ingredients in Modified Lugen Formula directly target influenza virus proteins and/or host factors,thereby play an anti-influenza role in multiple dimensions,including inhibiting virus replication,regulating host defense and cell death.This study provides a theoretical basis for further experimental analysis of the action mechanism of the Modified Lugen Formula in treating influenza.
