1.Treatment of NH_3~-burned patients complicated with inhalation injury

Objective: To investigate the management of NH~-_3 burned cases with inhalation injury. Methods:The clinic treatments of 15 NH~-_3 burned cases complicated with inhalation injury in his hospital between January 1998 and April 2004 were analyzed retrospectively.His management included reasonable fluid therapy,prompt and efficient usage of antibiotics ,tracheotomy after diagnosis,mechanical ventilation,bronchoalveolar lavage using fibrobrochoscope after the shock period and short-time usage of glucocorticoid. Results: Of the 15 cases,14 patients were cured and 1 patient was improved without any death.The cure rate was 93.3%. Conclusion: All of the managements should be performed as quickly as possible.Paying much attention to prevent the complications could improve the cure rate.Tracheotomy and bronchoalveolar lavage in the early stage are vital to patients.

4.Laparoscopic hepatic forceps for laparoscopic hepatectomy: An experimental study in pigs

Objective To evaluate the application of a new laparoscopic hepatic forceps for laparoscopic liver resection in pigs.Methods A laparoscopic hepatic forceps was designed by using the CAD software,and was manufactured in coordination with the Hangzhou Kangyou Medical Company for pilot tests.An assessment was made on outcomes of the forceps for blocking hepatic blood flow and controlling cut surface hemorrhage in laparoscopic hepatectomy in 12 pigs.Results The experimental group had a significantly lower intraoperative blood loss than control group(47.8 ?6.9 ml vs 86.3?3.9 ml;t=11.898,P=0.000) and a significantly shorter operation time than control(53.2?7.4 vs 97.5?5.8 min;t=11.541,P=0.000).No significant difference was observed in the weight of resected liver between experimental and control groups(34.7?4.2 g vs 35.3?4.2 g;t=0.245,P=0.812).Conclusions This new laparoscopic hepatic forceps can be used for the occlusion of liver blood flow,offering advantages of less blood loss and shorter operation time and facilitating laparoscopic performance in liver resection.
5.Effect of Simiaoyongantang on Human Umbilical Vein Endothelial Cells Injured by Hypoxia

Objective To study the effect of Simiaoyongantang on human umbilical vein endothelial cells(HUVECs) injury induced by hypoxia.Methods The HUVECs were divided into normal group,model group and Simiaoyongantang group,and the hypoxia model was established at the latter two groups.The livability of HUVECs was measured by MTT assay.The transudation of LDH in the medium was measured by biochemistry and the positive expression of VEGF was detected by immunofuorescence chemistry.Results The level of LDH was reduced significantly and the cell activity indicated by MTT was increased.The number of the cells stained by VEGF increased in the treatment group of Simiaoyongantang.Conclution Simiaoyongantang has protective effect on hypoxia injured HUVECs,which may be related to its effect of increasing the cells livability,reducing the injury induced by hypoxia,up-regulating expressions of VEGF and promoting angiogenesis.
6.Effects of aldosterone on nos-no system activity of cardiac fibroblasts in neonatal rat

Objective To investigate the changes in nitric oxide synthase (NOS) activity in cardiac fibroblasts (CFs) of neonatal rat induced by aldosterone. Method CFs were isolated by trypsin digestion method. Proliferation of CF was assessed by MTT colorimetric assay. Nitric acid reductase method and spectrophotometry were used to determine the NO contents and NOS activity, respectively, at different time after CFs being treated with aldosterone. 7.Inhibitory effects of Arg-Gly-Asp-Ser(RGDS) tetrapeptide on proliferation and collagen synthesis via downregulating focal adhesion kinase in hepatic stellate cell

Objective To investigate the inhibitory effect of Arg-Gly-Asp-Ser (RGDS) on proliferation and collagen synthesis stimulated by fibronectin (FN) and the influence on the expression of focal adhesion kinase of hepatic stellate cell (HSC), in order to provide a theoretical ground for its use in prevention and treatment of hepatic fibrosis in clinic. Methods The adhesion rates were determined by toluidine blue colorimetric assay. The effects of RGDS on the proliferation and collagen synthesis of HSC were assessed by ~3H-TdR and 3H-proline incorporation assays, respectively. The FAK mRNA of HSCs was determined by RT-PCR. Results RGDS was found to inhibit the rates of adhesion, while RGES has no such ability; ②The proliferation and collagen synthesis of HSC were inhibited by RGDS in dose and time-dependent manners; ③The expression of FAK in the (25, 50, 100mg/L) RGDS groups was significantly lower than that in the control group at mRNA level. Conclusions RGDS can inhibit proliferation and collagen synthesis of HSC, and these effects correlate with its anti-adhesion ability. Meanwhile the expression of FAK is also down-regulated significantly. As a signaling molecule, FAK may be involved in the effect of RGDS. This may be one of the underlying mechanisms of anti-fibrosis effect of RGDS.

9.Application of the job embedded scale in the dropout prediction of medical workers at a tertiary hospital

Objective Introducing the job embedded scale to explore the dropout intention of medical workers at tertiary hospitals.Methods Correlation analysis, analysis of variance and regression analysis were used to analyze with a job embedded scale, the dropout intention of medical workers of a Beijing hospital.Results There was a significant negative correlation between job embedding and dropout intention of such people(r=-0.223).The predictive ability of on-the-job embeddedness is better than off-the-job embeddedness.Fit-organizational, sacrifice-organizational, fit-community and sacrifice-community were significantly negatively correlated with dropout intention.There are differences in the relationship between job embedding and dropout intention of medical workers of different individual attributes.Conclusion Job embedded scale can effectively predict the dropout intention of medical workers, offering a usable tool to predict the dropout of these people.
