1.Value of CRP/PA combined with nCD64 index in assessing the severity and prognosis of mycoplasma pneumonia in children

Objective:To analyze the value of C-reactive protein to prealbumin ratio (CRP/PA) combined with neutrophil CD64 (nCD64) index in evaluating the severity and prognosis of mycoplasma pneumonia (MPP) in children.Methods:The clinical data of 98 children with MPP admitted to Changzhou Maternal and Child Health Hospital from March to December 2023 were retrospectively analyzed. According to the severity of the patients, they were divided into mild children (60 cases) and severe children (38 cases). After 7 days of routine treatment, the patients were divided into good prognosis group (72 cases) and poor prognosis group (26 cases). The CRP/PA and nCD64 indices of children with different severity of disease were compared, and the relationship between CRP/PA and severity of disease was analyzed. The clinical data of children with different prognosis outcomes were compared, and the influencing factors of prognosis were analyzed. The area under receiver operating characteristic (ROC) curve (AUC) was used to analyze the prognostic value of CRP/PA and nCD64 index.Results:CRP/PA[(1.05±0.21) vs (0.77±0.15)] and nCD64 index [(4.11±0.82) vs (2.96±0.59)] in severe children were higher than those in mild children (all P<0.05). Pearson analysis showed that CRP/PA and nCD64 index were positively correlated with clinical pulmonary infection score (CPIS) ( r=0.587, 0.574, all P<0.05). Univariate analysis showed that the age of the poor prognosis group [(4.81±0.96)years vs (6.67±1.33)years] was smaller than that of the good prognosis group. The severity of the disease was severe [65.38%(17/26) vs 29.17%(21/72)], complicated complications [46.15%(12/26) vs 18.06%(13/72)], CRP/PA[(1.23±0.25) vs (0.74±0.15)] and nCD64 index [(4.48±0.89) vs (3.02±0.61)] were higher than those in good prognosis group (all P<0.05). Binary logistic regression analysis showed that the severity of disease ( OR=4.251, 95% CI: 1.682-10.742), CRP/PA ( OR=3.857, 95% CI: 1.886-7.887), nCD64 index ( OR=3.161, 95% CI: 1.382-7.228) were prognostic factors (all P<0.05). ROC curve showed that the sensitivity and specificity of CRP/PA, nCD64 index and their combination in predicting prognosis were 76.90%, 73.10% and 88.50%, respectively, and 73.60%, 72.20% and 90.30%, respectively. The AUC values were 0.809, 0.794 and 0.923 (all P<0.001). Conclusions:CRP/PA and nCD64 index are significantly positively correlated with the severity of MPP in children, and they are the influencing factors for the prognosis of children. The combined detection of CRP/PA and nCD64 index has good predictive value for the prognosis of children.

Single-cell or low-input multi-omics techniques have revolutionized the study of pre-implantation embryo development.However,the single-cell or low-input proteomic research in this field is rela-tively underdeveloped because of the higher threshold of the starting material for mammalian embryo samples and the lack of hypersensitive proteome technology.In this study,a comprehensive solution of ultrasensitive proteome technology(CS-UPT)was developed for single-cell or low-input mouse oocyte/embryo samples.The deep coverage and high-throughput routes significantly reduced the starting material and were selected by investigators based on their demands.Using the deep coverage route,we provided the first large-scale snapshot of the very early stage of mouse maternal-to-zygotic transition,including almost 5,500 protein groups from 20 mouse oocytes or zygotes for each sample.Moreover,significant protein regulatory networks centered on transcription factors and kinases between the MII oocyte and 1-cell embryo provided rich insights into minor zygotic genome activation.
3.Identification of Long Non-Coding RNAs and Their Target Genes from Mycelium and Primordium in Model Mushroom Schizophyllum commune

Schizophyllum commune has emerged as the most promising model mushroom to study developmental stages (mycelium, primordium), which are two primary processes of fruit body development. Long non-coding RNA (lncRNA) has been proved to participate in fruit development and sex differentiation in fungi. However, potential lncRNAs have not been identified in S. commune from mycelium to primordium developmental stages. In this study, lncRNA-seq was performed in S. commune and 61.56 Gb clean data were generated from mycelium and primordium developmental stages. Furthermore, 191 lncRNAs had been obtained and a total of 49 lncRNAs were classified as differently expressed lncRNAs.Additionally, 26 up-regulated differently expressed lncRNAs and 23 down-regulated between mycelium and primordia libraries were detected. Further, Kyoto Encyclopedia of Genes and Genomes (KEGG) analysis showed that differentially expressed lncRNAs target genes from the MAPK pathway, phosphatidylinositol signal, ubiquitin-mediated proteolysis, autophagy, and cell cycle. This study provides a new resource for further research on the relationship between lncRNA and two developmental stages (mycelium, primordium) in S. commune
4.Correlation analysis of physical indicators with fasting plasma glucose and blood pressure indicators in low-aged and high-aged elderly people in Guangxi

Objective:To analyze the relationship between physical indicators and blood pressure or fasting plasma glucose levels in the young-old and oldest-old.Methods:Totally 1 516 subjects from the Guangxi Natural Longevity Cohort were screened in this study and physical examination parameters included body mass index(BMI), waist circumference(WC), waist-to-height ratio(WHtR), fasting plasma glucose(FPG)and blood pressure, and the correlations between them were analyzed.Results:The overweight elderly and overweight young elderly groups had an increased risk of concurrent hypertension and impaired fasting glucose, compared with both elderly people with normal BMI and young elderly people with normal BMI( OR=2.66, 95% CI: 1.90-3.72; OR=3.03, 95% CI: 2.11-4.34). Elderly people with general obesity and young elderly people with general obesity were more likely to have hypertension( OR=5.25, 95% CI: 2.07-13.28; OR=4.75, 95% CI: 1.84-12.21), impaired fasting glucose( OR=2.95, 95% CI: 1.00-8.69; OR=3.06, 95% CI: 1.04-9.02), and concurrent hypertension and impaired fasting glucose( OR=7.94, 95% CI: 3.04-20.72; OR=8.68, 95% CI: 3.28-22.94), whereas underweight young elderly had a reduced risk of hypertension( OR=0.27, 95% CI: 0.09-0.80). Elderly people in the central obesity group(WC)showed increased risk of hypertension( OR=1.39, 95% CI: 1.04-1.84)and concurrent hypertension and impaired fasting glucose( OR=2.39, 95% CI: 1.75-3.27), compared with those in the non-central obesity group.Young elderly people with central obesity had increased risk of hypertension( OR=1.46, 95% CI: 1.07-2.00), impaired fasting glucose( OR=1.62, 95% CI: 1.14-2.28), and concurrent hypertension and impaired fasting glucose( OR=3.03, 95% CI: 2.13-4.32); both elderly people and young elderly people in the central obesity group(WHtR)had increased risk of hypertension( OR=1.35, 95% CI: 1.03-1.76; OR=1.55, 95% CI: 1.13-2.14), impaired fasting glucose( OR=1.42, 95% CI: 1.04-1.94; OR=1.62, 95% CI: 1.13-2.31), and concurrent hypertension and impaired fasting glucose( OR=2.20, 95% CI: 1.60-3.02; OR=3.22, 95% CI: 2.14-4.84). In the elderly group, BMI was correlated with diastolic blood pressure and WHtR was correlated with the fasting blood glucose level. Conclusions:The levels of fasting plasma glucose and blood pressure increase with elevated physical indicator values(BMI, WC, WHtR)in the Guangxi elderly population, and the risk of developing hypertension, impaired fasting glucose, and concurrent hypertension and impaired fasting glucose increases in elderly patients with general obesity and central obesity, with a higher risk in low-aged elderly patients.
5.Clinical advantages of transumbilical single-site laparoscopy in surgery for giant ovariotubal cysts

Objective:To observe the efficacy and safety of single-port laparoscopy for giant ovariotubal cysts.Methods:38 patients with giant ovarian and fallopian tube cysts in Ruijin Hospital Affiliated to Shanghai Jiaotong University Hospital from January 2018 to June 2019 were retrospectively analyzed. They were divided into transumbilical single-port laparoscopy group (18 cases) and traditional laparoscopy group (20 cases). The perioperative related indexes such as operation time, intraoperative bleeding and the number of analgesic cases were compared between the two groups.Results:All 38 patients successfully completed the operation without conversion to laparotomy, and no auxiliary hole was added in the single-port laparoscopy group; There were no postoperative complications and no cystic fluid leakage. In the single-port laparoscopy group, 18 patients were satisfied with the concealment of umbilical scar. There was no statistically significant difference in operation time [(51.67±13.72)min vs (55.50±14.59)min], intraoperative blood loss [(52.22±24.38)ml vs (61.50±28.88)ml] and first postoperative anal exhaust time [(25.77±8.59)h vs (27.60±6.67)h] between single-port laparoscopy group and traditional laparoscopy group (all P>0.05); The number of cases requiring postoperative analgesia in the single-port laparoscopy group (2 cases vs 12 cases) was less than that in the traditional laparoscopy group ( P<0.05); The out of bed activity time [(20.95±3.65)h vs (26.95±5.43)h] and postoperative discharge time [(3.11±0.68)h vs (4.30±1.21)h] were shorter than those in the traditional laparoscopy group (all P<0.05). All 38 patients were followed up in the gynecological clinic for 6-24 months. The incision healed well without recurrence. Conclusions:Transumbilical single-port laparoscopic surgery for giant ovarian and fallopian tube cysts is reliable and safe when the possibility of malignant ovarian and fallopian tube tumors was excluded before operation.
6.Diagnosis and treatment of primary hyperparathyroidism in pregnancy: analysis of six cases

Objective:To study the clinical manifestations, diagnosis and treatment of primary hyperparathyroidism (PHPT) in pregnancy.Methods:This study involved six pregnant patients with PHPT who were admitted to Ruijin Hospital of Shanghai Jiao Tong University from August 2014 to November 2019. Their clinical manifestations, treatment strategies (multidisciplinary consultation with departments including Obstetrics, Endocrinology, Neonatology, General Surgery, Anesthesiology and Intensive Care Unit), maternal complications and maternal and infant outcomes were described and retrospectively analyzed.Results:(1) The median age of the six patients was 34(23-38) years old. PHPT was diagnosed in one case before pregnancy, four in the second or third trimester and one after delivery. The main clinical manifestations of four cases were nausea, vomiting, anorexia and other non-specific symptoms. Anemia was the most common maternal complication (five cases). Other complications included hypercalcemia crisis with renal failure (one case), multiple bone destruction and osteoporosis (one case). The median levels of serum calcium, parathyroid hormone and 25-hydroxyvitamin D were 3.08 (2.84-4.21) mmol/L, 216.7(93.1-2 603.6) ng/L and 29.66 (13.50-90.24) nmol/L, respectively. Results of parathyroid ultrasonography showed all patients had hypoechoic focus. Four cases underwent routine parathyroid radionuclide imaging showing abnormal radioactive concentration areas. (2) Obstetricians initiated the multidisciplinary team (MDT) for all patients to determine the treatment plans after communication with the patients and their families. One case received surgical treatment five months after delivery. Three cases received surgical treatment in the second trimester and among them, one terminated the pregnancy in the second trimester and the other two continued and achieved good pregnancy outcomes. Two cases underwent surgical treatment after giving up pregnancy and induced abortion. Postoperative pathology revealed that five cases were isolated parathyroid adenoma and one was parathyroid carcinoma. All of the patients were followed up and no abnormal serum calcium was reported. (3) One newborn had hypocalcemic convulsions one month after birth and was recovered by intravenous calcium supplementation. The newborn was followed up and the serum calcium level was normal so far. The other two neonates had normal serum calcium during follow-ups, but one of them was diagnosed with autism at the age of four and is now undergoing rehabilitation treatment.Conclusions:Pregnancy complicated by PHPT may lead to serious maternal and infant complications. MDT consultation ensures timely diagnosis, comprehensive treatment for the patients and better pregnancy outcomes.
7.Growth suppression of colorectal cancer expressing S492R EGFR by monoclonal antibody CH12.

Colorectal cancer (CRC) is a common malignant tumor in the digestive tract, and 30%-85% of CRCs express epidermal growth factor receptors (EGFRs). Recently, treatments using cetuximab, also named C225, an anti-EGFR monoclonal antibody, for CRC have been demonstrated to cause an S492R mutation in EGFR. However, little is known about the biological function of S492R EGFR. Therefore, we attempted to elucidate its biological function in CRC cells and explore new treatment strategies for this mutant form. Our study indicated that EGFR and S492R EGFR accelerate the growth of CRC cells in vitro and in vivo and monoclonal antibody CH12, which specifically recognizes an EGFR tumor-specific epitope, can bind efficiently to S492R EGFR. Furthermore, mAb CH12 showed significantly stronger growth suppression activities and induced a more potent antibody-dependent cellular cytotoxicity effect on CRC cells bearing S492R EGFR than mAb C225. mAb CH12 obviously suppressed the growth of CRC xenografts with S492R EGFR mutations in vivo. Thus, mAb CH12 may be a promising therapeutic agent in treating patients with CRC bearing an S492R EGFR mutation.

8. A study on urinary iodine levels and influencing factors of urban and rural residents in some areas of Gansu Province

Objective:
To analyze the urinary iodine status of urban and rural residents in some areas of Gansu Province and its related influencing factors, and to provide certain references for scientific iodine intake of urban and rural residents in Gansu Province.
Methods:
Subjects in Gansu Province with normal thyroid function were recruited in the investigation of thyroid diseases and iodine nutritional status, conducted a questionnaire survey and measured height, weight, waist circumference, collected 1 urine sample, and tested their urinary iodine levels. The urinary iodine levels of different groups were compared based on different gender, region, age, body weight, the waist, and salt intaking. The affecting factors of urinary iodine levels were analyzed by multiple linear regression models.
Results:
Totally 1 964 subjects were recruited, including 1 099 males, and 865 females. The age was (41.23 ± 14.75) years old. The median urinary iodine of the selected group was 225.60 (158.80, 311.58) μg/L. The urinary iodine level of rural residents was significantly higher than that of urban residents [μg/L: 249.80 (180.58, 336.88)
9. Efficacy and safety of recombinant human thrombopoietin in adult patients with primary immune thrombocytopenia during the perioperative period

Objective:
To evaluate the efficacy and safety of recombinant human thrombopoietin (rhTPO) treatment for primary immune thrombocytopenia (ITP) patients during the perioperative period.
Methods:
Adult ITP patients who were refractory to first-line glucocorticoid therapy and underwent selective surgery were enrolled to be treated with rhTPO at the dosage of 1.5×104U/d subcutaneously during the perioperative period. rhTPO treatment would not be terminated until one of the following conditions occurred: ①Platelet counts met the requirement of surgery; ②Platelet counts were ≥100×109/L; ③Completed the 14 days of therapy. End points of the study were surgery rate, rhTPO therapy response rate, rescue therapy rate and adverse responses.
Results:
42 patients were enrolled from Jan. 1, 2016 to Jun. 30, 2018. 14 were male and 28 were female. The median age was 60 (25-73) years old. There were no newly diagnosed patients. 5 patients were persistent and 37 were chronic. 27 patients completed selective surgery. The surgery rate was 64.3% (27/42) . Among them, 13 patients were under local anesthesia and 14 under general anesthesia. Of 42 cases receiving rhTPO therapy. 31 patients achieved responses, The overall response rate was of 73.8%. Among them, 24 patients achieved CR. The CR ratio was 77.4% (24/31) . 7 achieved response. The response ratio was 22.6% (7/31) . 11 patients did not respond to rhTPO therapy. The non-response rate was 26.2% (11/42) . The median time to reach CR was 7 (3-16) days. The median time to reach the peak of platelet counts were 10 (3-21) days. rhTPO was used for a median of 7 (3-14) days. The median platelet counts of patients undergoing surgery before rhTPO therapy, before surgery and at day 7 after surgery were 33 (20-89) ×109/L, 125 (78-245) ×109/L and 72 (30-250) ×109/L, respectively. The median peak of platelet counts was 149 (101-466) ×109/L. No infection, bleeding, thromboembolism and therapy-related adverse responses occurred in the patients.
Conclusion
rhTPO for ITP patients during the perioperative period is safe and effective.
10. Evaluation of the role of platelet derived growth factor in papillary thyroid carcinoma by targeted ultrasound contrast agent

Objective:
To explore the role of targeted ultrasound contrast agent in the evaluation of angiogenesis in papillary thyroid carcinoma (PTC) in nude mice by constructing platelet derived growth factor receptor α (PDGFRα)-targeted ultrasound contrast agents, and to explore the effect of platelet derived growth factor (PDGF) on angiogenesis in PTC.
Methods:
PDGFRα-targeted ultrasound contrast agents were constructed through biotin-avidin linkage, and blank micro-bubbles were served as control group. After the siRNA-PDGF BCPAP cell line was established, well prepared BCPAP cells or siRNA-PDGF-BCPAP cells were injected subcutaneously into the back of male BABL/C nude mice. The growth of the tumors was observed closely. All tumors of the normal group and the siRNA-PDGF group were examined by PDGFRα-targeted ultrasound contrast agents or blank micro-bubbles, and the dynamic images were analyzed quantitatively by QontraXt software.All mice were sacrificed after ultrasonography. Microvessels density (MVD) in the tumors was counted by CD31 staining and the expressions of vascular endothelial growth factor(VEGF) and PDGF were detected by Western Blot.
Results:
PTCs in nude mice were enhanced well by PDGFRα-targeted ultrasound contrast agent or blank micro-bubbles. The peak of tumors in siRNA-PDGF group was significantly lower than that in the normal group [(43.085±13.244)% vs (57.428±10.952)%,
