1.The analysis of Interfered therapy of peptic ulcer with helicobacter pylori in 184 patients

Objective To evaluate the clinical effects of interfered therapy of peptic ulcer with lip. Methods 184 patients were randomly divided into two groups,only durgs were given for conventional treatment group:pan- toprazole and teprenone; the same drugs, the type of life and individual diets were given for interfered treatment group. Therapy time lasted for 6 weeks in two groups. Radical eradication of Hp: pantoprazole plus clarithromycinand amoxycilLin. Results The time. of complete alleviation of clinical symptoms: most of patients in interfered treat- ment group alleviated after one week, half past patients in conventional treatment group alleviated after one week; there were significant difference in alleviation rate of clinical symptoms between two groups( P < 0.01 ). Healing of ulcer:after the end of treatment, all patients underwent endoscopy, most of ulcers healed in interfered treatment group, but 90.22% of ulcers healed in conventional treatment group, there were significant difference in he.sling rate of ulcer between two groups(P< 0.05). Eradicating of Hp: the eradicating of Hp was similar in two groups,there were no significant difference between the interfered treatment group and the conventional treatment group (P > 0.05). Conclusion Through the interfered treatment(drugs combining the type of life and the habit of diet), we can alleviate clinical symptoms and improve the effects of healing of ulcer.
2.Construction of cDNA Expression Library of Unfed Female Haemaphysalis longicornis and Immuno-Screening

Objective To construct a cDNA expression library from unfed female tick Haemaphysalis longicornis for screening and cloning potential antigenic genes.Methods Total RNA was isolated from unfed female ticks,mRNA was purified and a library of oligo(dT)-primed cDNA with added directional EcoR Ⅰ/Hind Ⅲ linkers was constructed from the purified mRNA.The constructed cDNA was ligated to the EcoRⅠ/HindⅢ arms of the ?SCREEN vector.Pure phage stocks were harvested by plaque purification and converted to plasmid subclones by plating phage on host strain BM25.8.Recombinant plasmids that were subcloned to E.coli BM25.8 were isolated and transformed into E.coli JM109.Recombinant plasmids abstracted from JM109 were identified by PCR and sequencing.Rusults The recombinant phage DNA was packaged by using phage-marker packaging extracts,resulting in a primary cDNA library with a size of 1.8?106 pfu.Data showed 100% of the library were recombinant and the titer of the amplified library was 2.4?109 pfu/ml.Forty-two clones of encoding immunodominant antigens were obtained from the cDNA library.Sequence analysis revealed 12 unique cDNA sequences and the encoded putative proteins showed similarities to H.longicornis tropomyosin mRNA,Rhipicephalus annulatus unknown larval protein mRNA,chromosome 2R of Drosophila melanogaster,mitochondrial DNA of H.flava,clones HqL09 unkown mRNA and Hq05 mRNA of H.qinghaiensis,and myosin alkali light chain protein mRNA.Conclusion The cDNA expression library from unfed female H.longicornis was successfully constructed and screening of protective genes may provide candidate antigens of the tick.
3.Surgical treatment for patients with anastomotic stoma cancer, gastric cardial cancer or esophagus cancer after subtotal gastrectomy

ObjectiveTo discuss the surgical treatment for patients with anastomotic stoma cancer,gastric cardial cancer or esophagus cancer after subtotal gastrectomy.MethodsThe clinical data of 21 patients with anastomofic stoma cancer,gastric cardial cancer or esophagus cancer after subtotal gastrectomy were analyzed retrospectively.There were 4 cases with anastomotic stoma cancer after operation of gastric cardial cancer,2 cases with gastric cardial cancer after subtotal gastrectomy due to gastriculcer,3 cases with upper esophagus cancer after subtotal gastrectomy due to gastric ulcer,6 cases with mid-esophagus cancer and 6 cases with distal esophagus cancer after subtotal gastrectomy due to gastric ulcer.There were 6 cases treated with partial esophagus resection,resection of remaining stomach and jejuna-esophagus anastomosis,3 cases treated with partial esophagus and stomach resection and esophagus-gastric anastomosis,12 cases with subtotal esophagectomy and colon interposition.ResultsOne case with incision infection,1 case with anastomosis leaks,1 case dead of pulmonary infection.In the 20 follow-upcases,4 cases were dead of cardiac and cerebral accidents at the third year after operation,and other 16 cases survived and had normal diet.ConclusionsReoperation is still one of the best choices for patients with anastomotic stoma cancer,gastric cardial cancer and esophagus cancer after subtotal gastrectomy when their body condition are acceptable and without distant metastasis.The organ for digestive tract reconstruction should be decided according to the situation of the first gastrectomy and the proficiency of the operator.
4.Radial scanning endoscopic ultrasonography for preoperative grading of esophageal cancer

Objective To evaluate mechanical radial scanning endoscopic ultrasonography (EUS) for preoperative tumor and lymph node ( TN ) staging of esophageal cancer. Methods From January 2010 to June 2010, a total of 60 patients with esophageal cancer underwent preoperative staging with mechanical radial scanning EUS. The findings of EUS were compared with postoperative pathological outcomes. Results EUS accurately predicted T stage in 80 % of cases and N stage in 71% cases. Sensitivities to T1 , T2 , T3 and T4 were 75% , 100% , 96% and 50% , respectively, and those to N0 and N1 were 55% and 100% , respectively. With exclusion of 11 patients with un-passable lesions, the accurate rate of EUS in T staging of focal and advanced cancers was 90% ( 44/49 ). Conclusion Mechanical radial scanning EUS can accurately predict T and N stages in preoperative patients with esophageal cancer, which also exhibits high differential accuracy in focal and advanced esophageal cancer.
5.Clinical effects of continuous sequential therapy of femoston in 121 cases of meno-pausal syndrome

Objective To explore the clinical efficacy and safety of continuous sequential hormone replacement therapy of femoston for menopausal syndrome .Methods 121 cases of menopausal syndrome at expert outpatient service in our hospital from January 2014 to October 2015 received hormone replacement therapy of femoston for 24 weeks .Before and after treat-ment ,Kupperman score ,blood follicle stimulating hormone (FSH) ,estradiol (E2 ) ,luteinizing hormone (LH) and endometri-al thickness were determined .Results With the prolonging of treatment time ,patient's menopausal symptoms gradually re-lieved .Kupperman scores were declining .After 6 courses of treatment ,patient's menopausal symptoms completely eased . Kupperman scores decreased by 95 .0% (P<0 .01) ,serum FSH and LH decreased significantly (P<0.05 and P<0 .01) and serum E2 increased notably (P<0 .01) without endometrial thickness changes (P>0.05) .Conclusion Continuous sequential therapy of femoston can effectively relieve menopausal syndrome and control the menstrual cycle and the endocrine level without stimulating the endometrium excessively .
6.Matched comparison of low kVp imaging and CT spectral imaging in image quality and radiation dose at abdominal CT

Objective To investigate the image quality and radiation dose of spectral CT with automatic spectral imaging mode selection (ASIS) and adaptive statistical iterative reconstruction (ASIR) at abdominal CT as compared with low-kVp CT with ASIR.Methods Two hundred patients underwent the plain and arterial-phase (AP) and portal venous-phase (PVP) contrast-enhanced abdominal CT scan were analyzed prospectively.The patients were randomly assigned to the control group (low-kVp CT scan) and study group (spectral CT scan),if BMI ≤ 23.9 kg/m2,patients were assigned into group A (80 kVp) or group B (spectral CT with ASIS);if 24.0 kg/m2 ≤ BMI<28.9 kg/m2,patients were assigned into group C (100 kVp) or group D (spectral CT with ASIS)(n=50 each).Groups A and B,groups C and D were matched by gender,age,body mass index,cross sectional area and contrast agent dose respectively.Low-kVp images and monochromatic images (40 to 60 keV) were all reconstructed by using ASIR.Radiation dose and quantitative parameters (image noise in HU and contrast-to-noise ratio of the liver,aorta and portal vein) were compared by using t test while overall image quality was assessed by Mann-Whitney U test between the matched groups.Results Radiation dose in the group B was significantly higher than group A (increased by 10%,P<0.05) but there was no significant differences between groups C and D (P>0.05).Compared to the control group,image noises in the study group were higher at 40 keV and 50 keV (P<0.05),but similar at 40 keV (P>0.05).At the energy level of 40 keV,the study group showed higher CNRs,but lower overall image quality scores than the control group except for the similar image quality scores between groups A and B during AP (P<0.05).At the energy level of 50 keV,the CNRs and overall image quality scores in the study group were higher than or similar to the control group.At the energy level of 60 keV,the study group showed lower or similar CNRs,but higher or similar overall image quality scores compared with the control group.Conclusions The radiation dose of spectral CT with ASIS technique was slightly higher than 80 kVp CT but similar to 100 kVp CT.By combining ASIR technique,monochromatic image at 50 keV can maintain or improve CNR and overall image quality as compared with low-kVp images.
7.Risk factors of drug resistance in diabetes mellitus complicated by pulmonary tuberculosis

Objective:To investigate the risk factors of diabetes mellitus complicated by pulmonary tuberculosis.Methods:The clinical data of 83 patients with diabetes mellitus complicated by pulmonary tuberculosis who received treatment in Taiyuan Fourth People's Hospital from March 2020 to March 2022 were collected. These patients were divided into sensitive group ( n = 45) and resistant group ( n = 38 ) according to the results of drug sensitivity test. Univariate and multivariate non-conditional logistic regression was performed to analyze the influential factors of drug resistance. Results:Univariate logistic regression results revealed that there were significant differences in blood CD4 +T lymphocyte count ( χ2 = 11.73, P = 0.001) and diabetic complications ( χ2 = 4.94, P = 0.026). Multivariate non-conditional logistic regression analysis was performed taking whether blood CD4 +T lymphocyte count was lower than the average level and whether patients with diabetes mellitus had complications as independent variables, and taking whether drug resistance was a dependent variable. The results showed that the OR (95% CI) value of the decreased blood CD4 +T lymphocyte count was 4.909 (1.926-12.514). It is a risk factor for drug resistance of diabetes mellitus complicated by pulmonary tuberculosis. Conclusion:The decrease of blood CD4 +T lymphocyte count is a risk factor of drug resistance in diabetes mellitus complicated by pulmonary tuberculosis, and it should be intervened early in the clinic.
