1.Expression of interleukin-6 and matrix metalloproteinase-3 in the synovial fluid of osteoarthritis patients

BACKGROUND:It is confirmed that the onset of osteoarthritis is closely related to inflammatory cytokines. OBJECTIVE: To investigate the levels of interleukin-6 and matrix metaloproteinase-3 in the synovial fluid of patients with osteoarthritis and their correlation with the severity of osteoarthritis. METHODS: Synovial fluid specimens of 78 patients were harvested to detect the levels of interleukin-6 and matrix metaloproteinase-3 using ELISA assay. The severity of osteoarthritis was estimated by the Hospital for Special Surgery (HSS) score. Pearson correlation analysis was adopted to explore the pairwise correlations between interleukin-6, matrix metaloproteinase-3 and HSS score. RESULTS AND CONCLUSION:The levels of interleukin-6 and matrix metaloproteinase-3 in the synovial fluid were (13.1±5.7) and (989.3±429.5) μg/L, respectively; the HSS score was (56.4±12.0) points. Pearson correlation analysis showed that the expression levels of interleukin-6 and matrix metaloproteinase-3 had a significant positive correlation (r=0.70,P< 0.001); the levels of interleukin-6 and matrix metaloproteinase-3 had a negative correlation with the HSS scores (r=-0.70,r=-0.75,P < 0.001). Experimental findings show that there is a positive correlation between interleukin-6 and matrix metaloproteinase-3 in the synovial fluids, and these two cytokines also have a positive correlation with the severity of osteoarthritis, indicating that to measure interleukin-6 and matrix metaloproteinase-3 levels in the synovial fluids of osteoarthritis patients is important for early diagnosis and treatment of osteoarthritis.

Objective To screen for nonresponding cases of superficial transitional cell carcinoma of the bladder at an early stage of intravesical bacillus Calmette Guetin(BCG) therapy. 3.Preliminary construction of nursing care quality index in ophthalmology

Objective To explore the preliminary construction of nursing care quality index based on the characteristics of ophthalmic nursing. Methods Through literature review and experts′discussion, the preliminary construction of nursing care quality index were designed. The Delphi method was used to revise the indicators. Results After two rounds of Delphi procedure, the opinion of the experts became consistent. Questionnaire collecting rates of the two rounds were both 100%, and cooperation index were 0.236 and 0.292 respectively (P<0.01), two first level quality indicators were confirmed, namely ophthalmic care operations (five second level indicators) and health education (four second level indicators). Conclusion The ophthalmic nursing care quality indicators were scientific, reliable and feasible, which could provide objective and quantified basis for the management of nursing care quality.
4.Laparoendoscopic examination in ascites of unknown origin

Objective To investigate the diagnostic value of laparoendoscopic exploration and peri-toneum biopsy in patients with ascites of unknown origin and to study the feasibility of replacing hard laparo-endoscope with ultrathin gastroendoscope. Methods A total of 35 patients with ascites of unknown origin underwent laparoendoscopic exploration and biopsies were taken at suspected sites on peritoneum. Results Definite diagnosis was made in 33 patients after laparoendoscopy (94. 3% ), among which there were 10 tuberculosis peritonitis (28. 6% ), 7 metastatic peritoneal carcinoma (20%), 5 liver cancer, 3 liv-er cirrhosis, 3 primary peritoneal carcinoma, 2 ovary cancer, 1 hepatovascular sarcoma, 1 spleen lymphoma and 1 acidophilic enteritis. No procedure related complications were observed. Conclusion Laparoendosco-py is safe and effective in target peritoneal biopsy, and is of great importance in diagnosis of ascites of un-known origin.
5.The role of mu-opioid receptors in seizure susceptibility induced by kainic acid

Objective To investigate the role of hippocampal mu-opioid receptors (MORs) in the formation of seizure susceptibility induced by kainic acid (KA). Methods The SD rats were injected subcutaneously with a convulsive dose of KA (10 mg/kg). Then mini-osmotic pumps were used for intrahippocampal injection with a selective MOR antagonist ?-FNA, and a selective MOR agonist PL017 in these rats. 7 days later, the seizure susceptibility was checked by a subthreshold dose of KA (5 mg/kg). Results All animals showed motor seizures over Stage 4 after a convulsive dose KA injection. 7 days later, 100% animals showed seizures over Stage 4 in KA, CSF + KA and PL017+KA groups after injection of subthreshold dose of KA, but only 28.5% in PL017+KA group. Mean latency of PL017+KA group was (10.1?3.0) minutes, shorter significantly than that of KA and CSF+KA groups (0.010.05). 6.Risk factors for postoperative delirium in patients undergoing spine operation under total intravenous anesthesia

Objective To investigate the risk factors for postoperative delirium in patients undergoing spine operation under total intravenous anesthesia. Methods Two hundred patients undergoing elective spine operation were included in this study. Patients' perioperative informations were recorded. The diagnosis of delirium was based on Confusion Assessment Method (CAM). The patients were divided into 2 groups according to the occurrence of delirium within 3 days after operation: postoperative delirium group and no postoperative delirium group. Multivariate logistic regression was used to analyze the risk factors for postoperative delirium. Results Postoperative delirium developed in 39 patients during the first three postoperative days. The incidence of postoperative delirium was 19.5%. The logistic stepwise regression analysis indicated that the risk factors for delirium included age, intraoperative blood loss and alcohol abuse. Conclusion Age, large amount of intraoperative blood loss and alcohol abuse are risk factors for postoperative delirium in patients undergoing spine operation under total intravenous anesthesia.
7.Paeoniflorin effects onCandida albicans biofilms

BACKGROUND:Studies have confirmed that the active ingredients ofPaeonia lactiflora Pal. have better inhibitory effects onCandida albicans, but its monomer paeoniflorin has not been reported whether it can inhibit Candida albicans biofilm. OBJECTIVE:To observe the effect of paeoniflorin onCandida albicans biofilm in vitro. METHODS:Paeoniflorin solution at different concentrations of 4, 2, 1, 0.5, 0.25 g/L was prepared using RPMI-1640 according to 2-fold dilution method. Chlorhexidine was diluted with RPMI-1640 to different concentrations, including 2%, 1%, 0.5%, 0.25%, 0.125%. We compared the effects of different concentrations of chlorhexidine and paeoniflorin on diameter ofCandida albicans by agar diffusion method. MTT assay was used to detect the effect of different concentrations of chlorhexidine or paeoniflorin on the celladhesion of Candida albicans as wel as their inhibitory effects onCandida albicans biofilms. Confocal laser scanning microscope and LIVE/DEAD BacLight Bacterial Viability Kits were combined to observe the changes ofCandida albicans biofilms under normal or intervention conditions. RESULTS AND CONCLUSION:Both chlorhexidine and paeoniflorin possessed bacteriostatic ability, and their bacteriostatic ring diameters were positively correlated with drug concentrations. Significant differences in the bacteriostatic ring diameter were observed between chlorhexidine and paeoniflorin, except between 2 g/L paeoniflorin and 1%, 2% chlorhexidine. Paeoniflorin at different concentration could inhibit celladhesion of Candida albicans as wel as inhibitCandida albicans biofilm. The inhibition rate was also positively correlated with drug concentrations. Under normal conditions, most of bacteria in the biofilms were alive, and there was a smal amount of dead bacteria after 48 hours. After intervention with paeoniflorin, the proportion of dead bacteria in the biofilms was increasing along with the concentrations of paeoniflorin. Compared with the chlorhexidine, paeoniflorin showed a lower bacteriostatic activity. These findings indicate that paeoniflorin has an obvious inhibitory action in Candida albicans biofilms in vitro.

Objective To evaluate the performance of high-fluorescence body fluid cell (HF-BF)mode on the platform Sysmex XE-5000 automated blood analyzer,and analyse its clinical application value in diagnosis of meningeal carcinomatosis.Methods E-valuated the performance of HF-BF by using precision test and methodology comparison test.Retrospectively analyzed 295 test re-sults of cerebrospinal fluid in Zhongshan Hospital Affiliated to Xiamen University from June 2010 to September 2012.Results HF-BF on the platform Sysmex XE-5000 automated blood analyzer had high precision,and exhibited a good consistency with cytolgical examination.The percentage of high-fluorescence body fluid cell(HF-BF%)in the meningeal carcinomatosis group was higher than that in other cerebral diseases groups,had statistically significant differences (P < 0.05 ).The cut-off value for HF-BF% was 4.3%,while the area under a receiver operating characteristic (ROC)curve (AUC)was 0.933,the sensitivity was 95.2%,and the specificity was 92.7%.When HF-BF% was over 4.3%,it was more likely to detect tumor cell in cerebrospinal fluid cytology.Con-clusion HF-BF is an effective reference index for the early diagnosis of meningeal carcinomatosis and has significant clinical appli-cation value.
10.Analysis of pathogenic factors for C5 palsy after open-door laminoplasty

Objective To discuss the risk factors of C5 never palsy incidence after cervical open-door laminoplasty. Methods This study involved 254 patients with cervical spondylotic myelopathy, who underwent expansive open-door lam?inoplasty in our hospital between January 2009 and December 2012. Patients were divided into two groups according to the incidence of C5 palsy after operation. Group A included 20 patients with C5 palsy and group B included 234 cases without C5 palsy. The risk factors for postoperative C5 palsy were compared between two groups. The factors may affect the inci?dence of C5 palsy were analyze. Results All cases received follow-up with an average follow-up time (24.9±2.6) months. Values of cervical curvature index (CCI), lamina open angles and spinal cord shift were significantly greater in group A than those of group B (P<0.05). There were no significant differences in preoperative intramedullary high signal, ossification of posterior longitudinal ligament and intervertebral foramen stenosis between two groups (P>0.05). Results of Logistic regres?sion analysis of multiple factors showed that lamina open angles, the pathogenic factors of C5 palsy included the spinal cord shift and postoperative CCI [OR(95%CI):2.035(1.684-2.359), 3.542 (2.465-9.254) and 9.126(4.425-17.273). Conclusion The angle of open door too large, CCI and spinal cord shift are risk factors for postoperative C5 palsy.
