The pathogenesis and clinical manifestation of histiocytosis are veriant, and misdiagnosis is relatively high. We reviewed a total of cases of this disease and related literature, and the possibility pathogenesis and its clinical features. were discussed.

Objective Studying the related factors of left ventricular hypertrophy for hypertensive disease, to provide the basis for clinical treatment. HZ Methods 500 hypertensive disease patients were performed dynamic monitoring of blood pressure and UCG. Then 130 patients with left ventricular hypertrophy were compared the dynamic blood pressure, age, sex and diseases course with 130 patients who had not left ventricular hypertrophy. Results (1) Systolic pressure, the load and the night descent rate of systolic pressure were significantly related to left ventricular hypertrophy, there was obviously difference in two groups. To the patients with systolic pressure and diastolic pressure were all increased, the incidence rate of left ventricular hypertrophy was reduced. (2) The age of left ventricular hypertrophy group was higher than no left ventricular hypertrophy group. (3) The female's incidence rate of hypertensive disease in left ventricular hypertrophy group was higher than no left ventricular hypertrophy group. (4) The course of diseases in left ventricular hypertrophy group was extended than no left ventricular hypertrophy group. The incidence rate of left ventricular hypertrophy was obviously higher in the patients whose blood pressure was increased in the daytime. Conclusion (1) The pulse pressure is a important forecast factor in numerous risk factor. (2) The advanced age is a risk factor to hypertensive disease. (3) The female hypertensive disease patients take place left ventricular hypertrophy is easier than the male. (4) Left ventricular hypertrophy is significantly related to hypertensive disease course, and isn't significantly related to blood pressure's durative. The clinic must think highly of observing and evaluating the pulse pressure, the long disease course of old female patients. There is important clinical significance to reinforce monitoring the risk factor of left ventricular hypertrophy, and to perform pertinent treatment and prevention for the high risk patients.

Objective To evaluate the therapeutic effect of ureteroscopy with holmium laser on ureteral stricture associated with calculi.Methods The clinical data of 36 cases of ureteral stricture associated with calculi undergoing ureteroscopy with holmium laser treatment were analyzed retrospectively with 26 cases of male patients and 10 cases of female patients.The age was 26-65 years.There were upper and middle ureteral stricture associated with calculi in 8 cases,and lower segment of ureteral stricture associated with calculi in 28 cases.All the patients were treated with ureteroscopy with holmium laser.All cases were followed up for 6-12 months.In the follow-up of 36 cases B ultrasound and urinary tract flat piece or intravenous urinary tract imaging were performed to evaluate the therapeutic effect.Results Thirty-six patients underwent ureteroscopy with holmium laser successfully.No conversion to open surgery occurred.Mean operating time was 30 (20-45) min.Mean hospital stay postoperatively was 3-5 d.Postoperative 8 cases with low thermal,alleviated by the anti-infection treatment,no ureteral avulsion,leakage and severe complications such as hemorrhage.Postoperative follow-up,32 cases (89％,32/36) were cured and 4 cases (11％,4/36) were improved.Conclusion Ureteroscopy with holmium laser in treatment of ureteral stricture associated with calculi is a simple and effective minimally invasive method.

Objective To investigate antimicrobial use in 33 township central hospitals,and improve the rational use of antimicrobial agents. Methods Thirty-three township central hospitals in 1 1 counties were randomly select-ed,7 920 outpatient prescriptions,medical records of 465 non-surgery patients and 213 surgery patients were inves-tigated and analyzed. Results Of 33 hospitals,antimicrobial usage rate in outpatients and inpatients was 56.60%and 89.68% respectively,combined antimicrobial usage rate was 24.16% and 43.58% respectively. Antimicrobial use density in inpatients was 147.25DDDs. Antimicrobial usage rate in surgery patients was 97.18% ,combined an-timicrobial usage rate was 59.90% ,the percentage of one drug,two-drug combination and three-drug combination was 40.10% ,47.82% ,and 12.08% respectively. The percentage of antimicrobial use in patients of type Ⅰ,Ⅱ, and Ⅲincision was 97.56% (40/41),96.93% (158/163)and 100.00% (9/9)respectively,combined antimicrobial us-age rate was 30.00% ,67.72% ,and 44.44% respectively.Conclusion The overuse of antimicrobial agents exists in 33 township central hospitals,antimicrobial usage rate,combined usage rate,antimicrobial use density and antimi-crobial prophylaxis in typeⅠincision operations are all high.

Objective To investigate the expression and characteristics distribution of ciliary neurotrophic factor (CNTF) and its receptor during the development of retina of healthy Sprague-Dawley(SD)and Royal College of Surgeons (RCS) rats with hereditary retinal degeneration. Methods The expression and distribution of ciliary neurotrophic factor and its receptor were detected by immunohistochemical staining in the retinal paraffin sections of SD and RCS rats from newborn to 12 moths old. Results In the normal retina of SD rats 0-7 days after birth, positive CNTF staining was found in all of the retinal layers and the staining of ganglion cells strengthened and other cells weakened as the age of rats increased; the staining of ganglion cells reached the peak at the 4th week and lasted till the agedness. The same results of the CNTF staining were also found in RCS rats retina. Weak positive staining of CNTFR in all of the retinal layers was seen in the 0-3-day-old SD rats; the ganglion cells were darkly stained and incontinuous positive staining at the site which would develop to be the external segment was found; as the age increased, the positive staining of external segment of photoreceptor enhanced and reached the peak at the 14-28th day after birth. At the 56th day, the staining of ganglion cells in retina of SD rats was strengthened while the staining of external segment weakened till the agedness. The expression of CNTFR in retina of 3-14-day-old RCS rats was the same as which of normal SD rats basically, but the staining of external segment weakened obviously from the 21st day on, and negative staining of external and positive ganglion cells were detected at the 28th day till the agedness. Conclusions Expression of CNTF in normal SD rats and RCS rats with hereditary retinal degeneration is almost the same. The presence of significant difference of expression of CNTFR between normal SD rats retina and RCS rats retina may provide the experimental gist for the CNTF treatment to retinal degeneration.

Objective[WT5”BZ] To evaluate the effect of stump anorectal longitudinal incision and colorectal heart shape anastomosis for the treatment of Hirschsprung′s disease (HD). Methods Ninety eight children with HD underwent colorectal resection and GI reconstruction by our method from April 1991 to April 2000. Results All patients were followed up for 5～96 months. There were no postoprative infection in pelvic and celiac cavities or wound infection,nor anastomotic strictures. One patient had an anastomotic fistula that was treated successfully by early repair. Recurrent constipation in 3 cases was successfully treated by dilatation. One patient experienced occasional fecal seepage. Enterocolitis occured in six patients. The overall complication rate was 11 2%.[WT5”HZ] Conclusion[WT5”BZ] Anorectal longitudinal incision and heart shape colorectal anastomosis has the advantage of satisfactory clinical result and less complication in the treatment of HD.

Objective To investigate the efficacy and safety of oxcarbazepine combined with levetirac-etam on children with epilepsy. Methods According to different treatments, 62 cases of children with epilepsy were divided into the OXC group (treated with oxcarbazepine, 40 cases) and the OXC combined with LEV group (treated with oxcarbazepine and levetiracetam, 20 cases). Then the therapeutic efficacy, EEG changes and ad-verse reactions of patients in the two groups were studied. Results The total efficacy was 87.5% in the OXC group, and was 95.5% of the OXC combined with LEV group, with no significant difference. EEG improvement rate was 52.5% in the OXC group and 81.8% in the OXC combined with LEV group , with significant difference (P < 0.05). The rate of adverse reactions in the OXC combined with LEV group was lower than that in the OXC group (P < 0.05). Conclusion Both levetiracetam and oxcarbazepine has good curative effect on children with epilepsy , but combination with levetiracetam can significantly decrease the epileptic discharges on EEG , with higher security.

Objective To investigate therapeutic effects of endoscopic mucosal resection( EMR) on gastric polyps and colorectal polyps.Methods 172 cases of elderly patients with early gastric and colorectal polyps were col-lected.They were divided into the observation group and control group by random number table,each group 86 cases. The observation group were treated with EMR,the control group were received conventional treatment for surgery.The clinical efficacy and adverse events were compared.Results The total effective rate of the observation group was 96.51%,significantly higher than 70.93% in the control group (χ2 =15.73,P<0.05);the postoperative adverse reactions in the observation group was 22.09%,significantly lower than 47.67% in the control group (χ2 =31.65, P<0.05).Conclusion For patients with pre-senile and senile stomach and colon polyps,the application of endo-scopic mucosal resection in the treatment not only can achieve better clinical efficacy, but also lower incidence of adverse reactions.

Objective To explore the efficacy of four-step method for transurethral enucleation of the prostate . Methods A retrospective analysis was conducted on 120 cases of benign prostate hyperplasia ( BPH) treated with four-step method ( traction, enucleation, resection, and repair ) for transurethral plasmakinetic resection of the prostate in our hospital from January 2012 to February 2014. Results The average operation time was (91.4 ±35.3) min.The intraoperative blood loss was 78.8 ±34.1 ml. The resection of the prostate tissue weight was (32.1 ±15.7) g.The postoperative bladder infusion time was (1.5 ±0.5) days.The postoperative indwelling catheter time was 3.5 ±0.8 days.The postoperative hospital stay was (5.5 ±0.5) days.No death, blood transfusion, or TURS happened.Follow-up reviews at 3 months after surgery found postoperative international prostatic symptom score (IPSS) was significantly less than the preoperative level [4.5 ±2.2 vs.24.5 ±5.1, t=39.445, P=0.000], postoperative Qmax was significantly higher than preoperative level [(21.5 ±10.5) ml/s vs.(6.0 ±1.9) ml/s, t =15.912, P =0.000], and postoperative residual urine was significantly less than the preoperative level [(12.0 ±2.0) ml vs.(145.0 ±33.0) ml;t=44.069, P =0.000].No retrograde ejaculation, sexual dysfunction, or dysuria happened. Conclusion The four-step method for transurethral enucleation of the prostate is radical , safe, and simple.
