Objective To observe the clinical efficacy of point injection of spleen polypeptides in the treat-ment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Totally 60 patients with AECOPD were randomly divided into the observation group and the control group,30 cases in each group.Based on conventional treatment,the observation group was treated by point injection of spleen polypeptides,4ml for each time, 1 time a day;and the control group was given equivalent volume of spleen polypeptides by intramuscular injection.The treatment lasted two weeks.The clinical efficacy,PCT,inflammatory and ADRs of patients in the two groups were observed.Results In 7 days of treatment,the clinical efficacy was 87% in the observation group,which was better than 60% of the control group (χ2 =5.45,P=0.019).PCT,interleukin-6(IL-6),interleukin-8(IL-8),tumor necrosis factor-α(TNF -α)of the observation group and the control group were (1.84 ±1.48)ng/mL and (2.21 ±1.75)ng/mL,(32.54 ±6.62)ng/L and (38.94 ±5.69)ng/L,(31.51 ±6.03)ng/L and (36.75 ± 7.95)ng/L,(45.90 ±4.40)ng/L and (51.53 ±2.21)ng/L respectively,which were lower than before treatment (t=12.76 and 15.25,13.23 and -8.42,-10.43 and 5.78,25.04 and 26.95,all P<0.01),which of the obser-vation group were lower than the control group(t=-2.84,4.01,-2.74,-6.27,all P<0.01).After treatment, PCT,IL-6,IL-8,TNF-αof the observation group and the control group were (1.05 ±0.74)ng/mL and (1.38 ± 0.87)ng/mL,(26.89 ±3.57)ng/L and (32.44 ±5.96)ng/L,(21.40 ±3.41)ng/L and (27.71 ±5.50)ng/L, (32.29 ±2.40)ng/L and (39.77 ±2.57)ng/L respectively,which were lower than before treatment(t=22.40 and 22.44,23.10 and -11.96,21.40 and 12.66,48.94 and 38.81,all P<0.01),which of the observation group were lower than the control group(t=-4.09,-4.26,-5.29,-11.68,all P<0.01).No obvious side effect was found in the two groups in the treatment and followed-up period.Conclusion Point injection of spleen polypeptides can effectively reduce the inflammatory response in patients with AECOPD,improve clinical outcomes,shorten course of treatment,has better clinical value.

Objective:To induce neural stem cells(NSCs)differentiating into oligodendrocytes in vitro and provide experiment foundation for NSCs-derived oligodendrocytes.Methods:The primary NSCs were isolated from endbrain and midbrain of neonatal rats,and then the cells were cultured in vitro.NSCs were induced by thyroid hormone(T3)and(or)insulin,and the effect of T3 and insulin was analysed by comparing the proliferation and the differentiation rate of NSCs and by measuring the prominency of GalC-positive cells.Results:NSCs were induced into oligodendrocytes by thyroid hormone and/or insulin,and different concentration of T3 and/or insulin had different effects on differentiation.Conclusion:Thyroid hormone and insulin can induce NSCs to differentiate mainly into oligodendrocyte and promote the proliferation and maturation of oligodendrocytes.The effect is more outstanding when 40ng/ml T3 is used in combination with 25?g/ml insulin.

Objective To study the effect of different types of dyslipidemia on heart rate variability in patients with type 2 diabetes.Methods Cohort study was carried out at Renmin Hospital of Wuhan University during January 2014 to May 2016.Total 725 patients with type 2 diabetes were recruited,including 231 patients without dyslipidemia and 494 patients with dyslipidemia.Clinical parameters were collected from electronic medical records system.Using variance analysis,chi square test,Pearson correlation analysis and partial correlation analysis,we explored an association of different types of dyslipidemia and heart rate variability in patients with type 2 diabetes.Results TG,LDL-C,HDL-C,TG + HDL-C group significantly decreased heart rate variability compared group without dyslipidemia (P ＜ 0.05).But the partial correlation analysis showed that only atherogenic index of plasma and Triglyceride (TG) were negatively correlated with heart rate variability,HDL cholesterol was positively correlated with heart rate variability,and the difference was statistically significant (P ＜ 0.05).Conclusion Triglyceride (TG),atherogenic index of plasma(AIP),HDL cholesterol were related to the reduction heart rate variability in patents with type 2 diabetes.

Objective To compare the efficacy of empagliflozin and DDP4 inhibitors in the treatment of type 2 diabetes mellitus.Methods With Computer,we retrieval PubMed,Cochrane Library,EMbase,CNKI,CBM and Wanfang database to find all of the randomized controlled trials about the effectiveness and safety of empagliflozin and DDP4 inhibitors in the treatment of type 2 diabetes mellitus.The retrieval time was from the establishment of database to March,2016.References screen were performed manually.Meta-analysis was carried out using RevMan 5.2 software.Results A total of five studies included 1935 cases.The Meta analysis results showed that:HbA1c level (MD =-0.07,95％ CI:-0.13--0.01,P ＜0.05),fasting blood glucose (MD =-16.73,95％ CI:-21.02--12.43,P ＜ 0.01),weight loss level (MD =-2.43 95％ CI:-2.68--2.19,P ＜ 0.01) in the two groups showed statistically significant differences.Empagliflozin group was better than DDP4 inhibitors group in reducing HbA1c level,fasting blood glucose,weight loss level.Safety:hypoglycemia between the two groups were similar (OR =0.86,95％ CI:0.46-1.6,P ＞ 0.05).Empagliflozin group's genital tract infection rate was statistically significant higher than that of DDP4 inhibitors.(OR =2.85,95％ CI:1.71-4.76,P ＜ 0.01).Conclusion Empagliflozin can effectively reduce the fasting blood glucose,HbAIc,and body weight,but increase the incidence of genital infection compare with DDP4 inhibitors.

In order to study the factors which affect the synthesis ability of medical students,the investigation was made on 200 junior students and 12 related teachers in Tianjin medical University,the relationship of the factors influencing the cultivation for comprehensive ability of medical students was analyzed by the ISM(Interpretative Structural Modeling),the related countermeasures and suggestions were proposed,and the implementation effect was studied by fuzzy comprehensive evaluation method.The study shows that it is the time to carry out the teaching reform by defining the cultivation goals and perfecting the education systems,and introduce the teaching idea of combing the professional education with professional learning so as to improve the teaching quality,stimulate the students`study and practical enthusiasm for their professional development and attain,the ultimate goals for the effective education of the excellent medical talents and improvement for the students`clinical comprehensive skills and ability.

Objective:To observe the anti inflammatory action of Yangfeihuoxue Decoction and its effects on inflammatory mediums. Methods: The mouse and rat inflammatory models were established by dimethylbenzene, carrageenin and paper strip investing method, and the rat model of pulmonary fibrosis was established by bleomycin. The inhibition of Yangfeihuoxue Decoction on various inflammatory reactions (with swelling rate, swelling degree and height of granuloma as markers) and its effects on levels of histamine (H) and tumor necrosis factor (TNF) in blood were studied. Results: Yanfeihuoxue Decoction can obviously inhibit acute, subacute and chronic inflammatory disease ( P

0.05). There were no adverse events or severe adverse events in all these 72 cases. Conclusion Ozone is effective and safe in the treatment of bacterial vaginosis.

Objective To investigate the changes of plasma fibrinolytic activity and D dimer level in the primary hypertension of the elderly.Methods ELISA was applied to measure blood and urine D dimer.Chromophore substrate method was used to detect PL,Plg,t PA and PAI 1.Results The levels of PAI 1,D dimer and Plg were obviously higher in the elderly primary hypertension and hypertentive kidney disease than those in the control group,while t PA and PL activity were lower than in the control group.Conclusion The changes of PLA and D dimer play an important role in hypertension and hypertentive renal disease of the elderly,on which the early diagnosis can be based.

Objective To study the relationship between carotid artery atherosclerosis and serum C-reactive protein(CRP) in elderly patients with essential hypertension. Methods 100 hypertensive patients(mean age 73?7 years), including 93 males and 7 females were enrolled in this study. Colour Doppler ultrasonography was used to measure intima-media thickness(IMT) and atherosclerotic plaque of carotid arteries. Serum CRP was determined with immunological method. Objective To simply the method of how to obtain cultures rich in astrocytes from SD rat cerebral cortex which could be utilized in vitro experiments.Methods The neonatal rat cerebral cortex was made into suspension by mechanical dissociation,and then reduced other cells by differential velocity adherent technique,shaking in orbital shaker and passage of cultured cells.After purification,the cultured cells were identified by double immunofluorescence staining and SABC.Results We successfully obtain cultures rich in astrocytes and the proportion of astrocytes were more than 95%.Conclusion The method described above was reliable in obtaining the high purity astrocytes from neonatal rat cerebral cortex and double immunofluorescence staining was more vivid and direct.
