1.Factors affecting the postoperative function of malleolar fracture

Objective To discuss the factors which affect the postoperative functions of the ankle joint. Methods A retrospective study was done of 102 patients who had been diagnosed as malleolar fracture and operated on in our institute between January 2005 and January 2008. We recorded their age, gender, body mass index(BMI), fracture type (AO type), time from injury to operation and presence or ab-sence of cast immobilization. Their ankle functions were evaluated by X-ray and the Baird-Jackson evaluation system in regular follow-up. Relationship between the above-mentioned factors and the postoperative functions of the ankle joint was statistically analyzed, using univariate logistic regression and multiple stepwise logistic regression. Results A total of 102 patients were followed up for 24.7 (11 to 43) months. A negative correlation between the age, fracture type, reduction and postoperative function was found. The gender, body mass index (BMI), time from injury to operation and presence or absence of east immobilization, however, had no association with the postoperative function. The conservative treatment of the deltoid ligament injury complicated with the lateral malleolar fracture and/or improper treatment of the syndesmotic injury led to poor function. Conclusions The older a patient and the more serious a fracture, as well as the more unsat-isfactory the reduction, the poorer the postoperative ankle functions may be. To some extent, rational treat-ment of the deltoid ligament injury complicated with the lateral malleolar fracture and the syndesmotic injury may also determine the postoperative function of the ankle joint.
2.Influence of Severe Chyle Blood Serum to the Three Types of Common Biochemical Detection Method

Objective Aim to explore how severe chyle blood serum effects on the End-point method of colorimetry,velocity method,immunological transmission turbidimetry(TIA)three types of common biochemical tests and their different influ-ences.Methods Collected 20 normal appearance serums,divided each pooled serum into A,B,C,D,E,F and G 7 experimen-tal groups and one control group.Each group contented for 1ml serum.Added intralipid 10,20,40,80,160μl and 320μl to the experimental groups from A to G in turn to prepare into different concentrations of simulate chylous samples.Tested each group by the End-point method of colorimetry,velocity method and TIA which represented by Glucose (Glu),uric acid (UA),Total bilirubin (TBil),aspartate aminotransferase (AST),gamma-glutamyl transpeptidase (GGT),lactate dehydro-genase (LDH),hypersensitive C reactive protein (hs-CRP),Apolipoprotein A1 (apoA1 )andβ2-microglobulin (β2-MG). Used pair T test for both experimental group and their control group.Results In terms of TIA,there were statistical differ-ences between all the experimental groups and their control groups in the three projects (t=-2.842~29.465,P<0.05).In terms of the End-point method of colorimetry,there were statistical differences between all the experimental groups and their control groups (t=-34.942~13.884,P<0.05)except for A group in Glu experiment (t=1.875,P=0.068>0.05).In terms of velocity method,there were no statistical differences between all the experimental groups and their control groups (t=-1.532~1.619,P>0.05)except for the B and C groups in GGT experiment (t=2.234&5.006,P<0.05).Conclusion Severe chyle blood serum had significant influence on the End-point method of colorimetry and TIA,as well as less influ-ence on velocity method.
3.Clinical study of Salviae Miltiorrhizae and Ligustrazine Hydrochloride Injection aided identiifcation and treatment of ischemic stroke

Objective To observe the effect of Salviae Miltiorrhizae and Ligustrazine Hydrochloride Injection in inauxiliary treatment of ischemic stroke and its clinical efficacy. Methods 142 cases of patients with ischemic stroke, were randomly divided into experimental group and control group. Control group(n=71) were treated with conventional therapy. Test group(n=71) were treated with Salviae Miltiorrhizae and Ligustrazine Hydrochloride Injection on the basis of conventional therapy. Hemorheological changes, nerve function improvement and treatment efifcacy in two groups before and after treatment were observed and analyzed. Results After treatment, the plasma viscosity and hematocrit in control group were 1.91±0.35, (49.58±1.04)%; while experimental group were 1.17±0.28, (38.97±1.31)%, the differences between two groups were signiifcant(P<0.05). Before treatment, the neurological function score in control group was 22.49±1.21, while experimental group was 23.18±1.74, there was no signiifcant difference between two groups. After treatment, the neurological function score in control group was 10.58±0.94, while experimental group was 4.62±2.05, the difference was signiifcant(P<0.05). The total effective rate was 80.3%in control group ,while 88.7% in experimental group, the difference was signiifcant(P<0.05). Conclusion The clinical efifcacy of Salviae Miltiorrhizae and Ligustrazine Hydrochloride Injection in auxiliary treatment of ischemic stroke was accurate, and it can alleviate the symptom of patients.
4.Expression of inflammatory factors in coronary atherosclerosis and the relationship between the factors and coronary risk score

Objective To investingate the role of inflammation in coronary atherosclerosis and the relationship between the inflammatory factors and coronary risk score. Methods 56 patients with acute coronary syndrome were studied, among them 26 cases were diagnosed as acute myocardial infarction (AMI) and 30 unstable angina pectoris (UAP). The study group was compared with a control group of 30 cases who were identified as normal by coronary angiography. The concentrations of serum sICAM-1 and hsCRP were determined by ELISA assay. The coronary risk score was recorded in patients with UAP. Results Serum sICAM-1 levels were significantly elevated in patients with AMI or UAP compared with that of control group, while higher hsCRP level was observed only in the patients with AMI compared with those with UAP and the control group. 5.Comparisons on the therapeutic efficacy for upper limb pseudoaneurysm by ultrasound guided thrombin injection and ultrasound guided compression repair

Objective To compare the therapeutic efficacy and safety of ultrasound-guided thrombin injection (UGTI) combined with saline injection and ultrasound-guided compression repair (UGCR) for upper limb pseudoaneurysm (PSA). Methods The clinical features and therapeutic efficacy of UGCR and UGTI for PSA of the upper extremities were retrospectively assessed after catheterization through radial artery. When UGCR was conducted, PSA, its communicating emissaries with other arteries were first identified with color Doppler, and then direct manual compression over the aneurysm for 30 minutes until complete thrombosis was observed. When UGCR failed, a 20-22G needle was inserted into PSA and saline was injected with continuous color Doppler guidance to confirm that the tip of the needle was in the aneurysm. Then thrombin was injected into the aneurysmal sac. All the patients were followed-up clinically and with ultrasound 24h or 14-30 days after the treatments. Results For the 14 patients with upper limb PSA who received UGCR for the first time, 5 were cured with a success rate of 35.7%. Nine cases underwent repeated UGCR treatment, and it was successful in four patients. The overall success rate for UGCR was 64.3% (9/14). All the five patients in whom UGCR failed were successfully cured when UGTI was conducted. In four cases it was successful after the first UGTI, and only one patient received repeated UGTI treatment. The total success rate for UGTI was 100% (5/5). No obvious adverse effect and recurrence were found after follow-up for 14-30 days in all patients. Conclusions Both UGCR and UGTI are safe, effective, and expedient treatment for patients with PSA in the upper extremity. UGTI is the preferred method for its simplicity, high success rate and avoidance of possible distal arterial embolism.

7.Relationship between clopidogrel resistance and long-term outcome of patients implanted with drug-eluting stents

Objective To evaluate the effects of delayed transplantation of bone marrow mononuclear cells (BM-MNCs) after myocardial infarction (MI) on the expressions of bcl-2/bax mRNA and proteins in myocardial cells, and to explore the possible mechanism of cardiomyocyte apoptosis. Methods The MI rat model was reproduced by ligating the left anterior descending coronary artery. Two weeks later 5?106 of BM-MNCs were injected into the infarct zone and the peri-infarct zone (BMT group). 9.Therapeutic effect of Hcy-lowering therapy on patients with coronary heart disease after PCI

Objective:To analyze therapeutic effect of homocysteine (Hcy)‐lowering therapy on serum levels of Hcy and inflammatory factors in patients with coronary heart disease (CHD ) after percutaneous coronary intervention (PCI) .Methods :A total of 82 CHD patients who received PCI in our hospital were selected .According to random number table ,they were randomly and equally divided into routine treatment group (received routine postoperative therapy) and Hcy‐lowering group .Serum levels of Hcy ,inflammatory factors ,N‐terminal pro‐brain natriuretic pep‐tide (NT‐proBNP) and soluble intercellular adhesion molecule‐1 (sICAM‐1) were compared between two groups . Results :1) On six months and one year after treatment ,compared with routine treatment group ,serum Hcy level significantly reduced [after six months: (15.39 ± 1.83) μmol/L vs . (13.21 ± 1.35) μmol/L ,after one year :(15.61 ± 1.62)μmol/L vs . (8.73 ± 0.72)μmol/L] in Hcy‐lowering group ;2) after six‐month treatment ,compared with routine treatment group ,there were significant reductions in serum levels of CRP [ (67.27 ± 7.51) mg/L vs . (37.11 ± 6.32) mg/L] ,IL‐6 [ (87.58 ± 7.21)μg/L vs . (60.17 ± 5.45)μg/L] ,procalcitonin [PCT , (21.34 ± 3.04) ng/L vs .(15.61 ± 2.32) ng/L] ,NT‐proBNP [ (298.37 ± 53.28) pg/ml vs .(104.28 ± 13.17) pg/ml] and sI‐CAM‐1 [ (391.83 ± 75.04) ng/ml vs .(162.18 ± 30.26) ng/ml] in Hcy‐lowering group , P<0.05 all .Conclusion:Hcy‐lowering therapy is help to reduce serum Hcy level ,relieve systemic inflammatory response and protect myocar‐dial function in CHD patients after PCI .
10.Therapeutic effect and safety of atorvastatin sequential treatment on patients with acute coronary syn- drome undergoing PCI

Objective:To observe therapeutic effect and safety of six‐month atrovastatin sequential treatment on pa‐tients with acute coronary syndrome (ACS) undergoing PCI .Methods :A total of 280 ACS patients were consecu‐tively enrolled ,randomly and equally divided into sequential treatment group (received atorvastatin 80mg 2d before PCI operation ,then 40mg/d after operation ,20mg/d a month later ) and routine treatment group (no taking atorv‐astatin before PCI operation ,then 20mg/d after operation and as a maintenance dose ) .Both groups were observed for six months ,therapeutic effect and safety were compared between two groups .Results:Compared with before PCI ,there were significant rise in levels of creatine kinase isoenzyme (CK‐MB) and cardiac troponin I (cTnI) after PCI 18h~24h in two groups ,and those of sequential treatment group were significantly lower than those in routine treatment group [CK‐MB:(2.72 ± 0.52) μg/L vs .(6.04 ± 0.51) μg/L ,cTnI : (0.28 ± 0.13) μg/L vs .(0.42 ± 0.16) μg/L , P < 0.01 both ];Compared with routine treatment group , the high sensitive C reactive protein (hsCRP) level significantly reduced in sequential treatment group after PCI 2d ,and continued until post‐operation six months [ (2.34 ± 0.51) mg/L vs .(1.49 ± 0.78) mg/L ,P=0.001];the blood lipids levels (except level of high density lipoprotein cholesterol significantly rose ) in sequential treatment group were significant lower than those of routine treatment group , P<0.01 all;there were no significant difference in percentage of transaminase level high‐er than normal three times and incidence rates of major adverse cardiovascular events (MACE) between two groups , P>0.05 all .Conclusion:Preoperative 80mg atrovastatin combined postoperative 40mg atrovastatin treatment can significantly reduce inflammatory reaction ,improve blood lipids ;compared with routine dose ,it doesn't significant‐ly increase adverse drug reactions ,so it's safe and effective .
