1.Comparative analyses of advanced gastric cancer on different chemotherapeutic protocols

Objective A total of 86 cases with advanced gastric cancer were randomly divided into XELOX(capecitabine plus oxaliplatin) and FOLFOX4 (5-fluorouracil,leucovorin,and oxaliplatin)groups (n =43 each).The immediate efficacies were evaluated by the criteria of Response Evaluation Criteria In Solid Tumors (RECIST).Adverse reactions were compared between two groups.The overall effective rate were 56% and 51% in XELOX and FOLFOX4 groups.And there was no significant inter-group difference (P > 0.05).The incidences of myelosuppression and nausea,vomiting in XELOX group were lower than those in FOLFOX4 group (P < 0.05).And other adverse reactions had no inter-group statistical difference (P > 0.05).Both chemotherapies were comparable in immediate efficacies for advanced gastric cancer.However,XELOX chemotherapy had a lower incidence of adverse reactions and it is more tolerable and compliant for patients.
2.Efficacy of 308-nm excimer laser combined with topical tacrolimus cream for the treatment of childhood vitiligo on the face or neck

Objective To assess the efficacy and safety of 308-nm excimer laser combined with topical tacrolimus 0.03% cream for the treatment of childhood vitiligo on the face or neck.Methods Sixty-eight children aged < or =14 years with vitiligo on the face or neck were enrolled in this study,and divided into two groups based on lesional sites,i.e.,skin group with lesions on the forehead,cheek,lower mandible,neck or in the periortic region and mucosa group with lesions in the perioral or periorbital region.All the patients received 308-nm excimer laser irradiation once or twice weekly and topical tacrolimus 0.03% cream twice daily.Twenty sessions of irradiation served as a treatment course.Efficacy was evaluated at the end of the treatment course.The effects of erythema reaction and cumulative irradiation dose on efficacy were assessed.Results After 20 times of laser therapy,an excellent response was achieved in 78.4% of the patients in the skin group,and 54.8% in the mucosa group (P < 0.05).During the treatment,the average frequency of erythema reaction lasting 48 hours or longer was significantly lower in the skin group than in the mucosa group (5.84 vs.9.12,P < 0.01).After 10 and 20 times of laser therapy,the cumulative radiation dose was 4215 mJ/cm2 and 10 453 mJ/cm2 in the skin group,respectively,compared to 3364 mJ/cm2 and 7430 mJ/cm2 in the mucosa group,respectively (both P < 0.01).Conclusions The 308-nm excimer laser combined with topical tacrolimus cream is effective and safe for the treatment of childhood vitiligo on the face or neck.Vitiligo on the forehead,cheek,lower mandible,neck and in the periortic region appears to respond better to this therapy with a weaker erythema reaction than that in the perioral or periorbital region.
3.Research on the Relationship Between the Changing Law of the Cytokines and Tumor-related Fatigue in Gastric Carcinoma

Objective To study the relationship between the changing law of cytokine gene expression profile and tumor-related fatigue and to verify the preliminary theory conclusion that the pathogenesis of tumor-related fatigue may be due to the up-regulation of the TNF,TGF-?,etc.Methods The changing characteristics of gene expression profile of cytokines were studied in peripheral blood mononuclear cells samples with gene chip.Results The gene expression level of TNF-? and TGF-?1 was up regulated.Conclusions The experimental results conform to the theory research conclusion and show that the pathogenesis of tumor-related fatigue due to the up-regulation of the gene expression level of TNF and TGF-?.
4.Influence of users' cognition on information retrieval behaviors in ubiquitous knowledge environment

Users' cognition and information retrieval are faced with great challenges in ubiquitous knowledge environ-ment. The influence of cognitive factors such as personality traits, cognitive style and ability, knowledge structure and ascribed methods on users' information retrieval behaviors was thus analyzed. It was pointed out that different cognitive factors could influence the retrieval, judgment and absorption of its results,users' satisfaction and continuous use in ubiquitous knowledge environment,with suggestions put forward from the aspects of information retrieval education, information service and information users for improving the users ' knowledge system in knowledge assimilating process by working out effective retrieval strategies using the advantages of cognition.
5.Self- management behaviors and influencing factors in patients with liver cirrhosis during ;decompensation period

Objective To investigate the self-management behaviors in patients with liver cirrhosis and analyze its influencing factors. Methods Totally 103 liver cirrhosis patients were recruited by convenience sampling method. They were investigated by using Self- management Questionnaire for Chinese patients with liver cirrhosis. Results The total score of self-management behaviors was 63.69 ± 13.60. All subscales scoring from high to low in turn were medication, diet management, condition monitoring and daily management. Conclusions The self-management level in patients with liver cirrhosis of decompensation period need to be improved, nursing staff should pay attention to effect evaluation during the hospital health education, to ensure that patients can master disease self-management knowledge, to improve the self-management behavior of patients with liver cirrhosis.
6.Genotyping and Homology Comparison of Meticillinresistant Staphylococcus aureus Isolated from Earthquake Victims

OBJECTIVE To investigate the genetic feature of meticillin-resistant Staphylococcus aures isolated from earthquake victims and compare them with those of common patients.METHODS The MRSA isolates underwent SCCmec(Staphylococcal Cassette Chromosome mec) multi-PCR and PVL gene detection.Diversilab method was used for homology comparison and antibiotic resistance was also analyzed.RESULTS All the MRSA isolates were confirmed to be type Ⅲ SCCmec with PVL gene negative.Homology analysis showed relatively consistent with community-acquired MRSA confirmed to be ST8 by MLST typing.CONCLUSIONS There may be cross-spread of MRSA between hospital and community.We should pay more attation even in emergent time.
7.The status and influencing factors of sub-health in inpatient caregivers

Objective To explore the status and influencing factors of sub-health in inpatient caregivers.Methods A total of 432 caregivers met the inclusion criteria were enrolled.Suboptimal health status questionnaire (SHSQ-25) was used to evaluate the status of sub-health.Sub-health was defined as SHSQ-25 score ≥ 35.100 people undertaking routine physical examinations were enrolled as control group.The incidence and influencing factors of sub-health in caregivers were analyzed.Results (1) The caregivers group valued higher in SHSQ-25score significantly (36.9 ± 8.6 vs.27.5 ± 13.0,P < 0.05).The incidence of sub-health was obviously higher in caregiver group than that in control group (69.9% vs.39.0%,P < 0.05).(2) A significant difference in the SHSQ-25 scores between female caregivers and male caregivers was observed (37.9 ± 9.0 vs.34.6 ± 7.1,P <0.05).The SHSQ-25 score was positively related with age (r =0.578,P < 0.05) and accompanying time (r =0.507,P < 0.05).Parents,children,or spouses of patients got higher scores than other types of caregivers (38.1 ±8.1 vs.31.1 ±8.8,P<0.05).(3) Among the sub-health caregivers,only 10.9% of them were aware of their sub-health status.Conclusion The incidence of sub-health may be higher in inpatient caregivers than the general population.Gender,age,accompanying time,and the relationship between patients and caregivers may be influencing factors for sub-health.Effective action should be taken to improve the health status of caregivers.
8.Preliminary discussion on the effects of guanosine, curcumin on learning and memory in AD double transgenic mice

Objective Using Morris water maze test to evaluate the effects of guanosine and curcumin on cognitive function of APPswe/PS1dE9 double transgenic mice .Methods 3-month old APPswe/PS1dE9 dtg mice were randomly di-vided into model group , donepezil HCL group , guanosine group , curcumin group , curcumin and guanosine group ( n=12), with age-matched Wild C57BL/6J mice of the same genetic background as normal control group .Medication was giv-en once a day for 1 month.Using Morris water maze to test the spatial learning and memory ability of mice .Results Guanosine and curcumin could improve spatial learning and memory disorders of AD mice , particularly in the group of cur-cumin.Conclusion Guanosine and curcumin improve the cognitive ability of APPswe /PS1dE9 double transgenic mice with early cognitive impairment .
9.Comparison of High Volume Zero-Balanced Ultrafiltration With Different Replacement Fluid in Infant Patients

Objective:To compare the clinical effects of high volume zero-balanced ultraifltration (ZBUF) with different replacement lfuid in infant patients.
Methods: A total of 40 infant patients who received aortic coarctation with deep hypothermic circulatory arrest operation in our hospital from 2012-11 to 2014-02 were summarized. The patients were randomized into 2 groups, Group A, the patients had ZBUF with plasmalyte A, n=21 and Group B, the patients had ZBUF with modiifed replacement lfuid n=19. All patients received ZBUF (15-20) ml/kg during re-warming period until the temperature reached 34°C at rectum. Blood gas analysis were conducted at 4 time points as T1 (before CPB), T2 (open ascending aorta and 5 min after calcium perfusion), T3 (ifnishing ZBUF) and T4 (end of CPB). The intra-operative CPB time and the post-operative recovery with the complication were recorded in all patients.
Results: For blood gas analysis, the calcium concentration was higher in Group B than that in Group A at T3 time point, P<0.01 and the base excess was higher in Group A than that in Group B at T3 time point, P<0.05. Those indexes were similar at T1, T2 and T4, all P>0.05. The other indexes were similar between 2 groups, P>0.05 and the post-operative recovery was similar between 2 groups, P>0.05.
Conclusion: ZBUF with modiifed replacement lfuid could avoid decreased calcium concentration and acidosis, therefore, provide a relative stable homeostasis in infant patients.
10.Effects of different temperature rewarming on intraoperative brain protection in children with deep hypothermia circulatory arrest

Objective:To observe the effect of different temperature rewarming on intraoperative brain protection in children with deep hypothermia circulatory arrest (DHCA).Methods:Totally 24 cases of children undergoing deep hypothermia circulatory arrest under general anesthesia were selected, and randomly divided into 38 ℃ and 43 ℃ groups including 12 cases respectely. Children patients in each group were given forced air rewarming at 38 ℃ and 43 ℃ respectively during the rewarming phase after cross-clamping remission. The age of patients, weight, operation time, intraoperative blood loss, intraoperative fluid infusion and postoperative length of stay were recorded. The arterial blood was taken to determine the serum neuron-specific enolase (NSE) , Platelet activating factor (PAF), and blood lactic acid immediately when the patients were entering into operating room (T 0), before Extracorporeal Circulation (T 1), before aortic cross-clamping (T 2), before DHCA (T 3), rewarming (T 4), temperature to 30 ℃ (T 5), temperature to 33 ℃ (T 6), temperature to 35 ℃ (T 7), CPB was terminated (T 8) and at the end of operation (T 9). And the body temperature were recorded at T 0, T 1, T 3 and T 8. Results:At T 0, T 1, T 3, T 8 and T 9, the temperature difference between the two groups was not statistically significant ( P>0.05). The body temperature first decreased and then increased in the group of 38℃ and 43℃ ( P<0.01). And the serum PAF and NSE significantly increased during the operation ( F values were 5 172.087, 3 535.935, P<0.01). The content of PAF, NSE and lactate in 38 ℃ group and 43 ℃ group increased with time from T 0 to T 9 ( P<0.01).The PAF content of children in the 43℃ group at T 8 and T 9 were (20.34±4.70) μg/L and (25.46±7.75) μg/L respectively, lower than that in the 38℃ group (25.20±3.80) μg/L and (34.97±8.52) μg/L, the differences were statistically significant ( F value was 7.769, P=0.011; F value was 8.193, P=0.009). At T 8 and T 9, NSE content of children in the 43℃ group were (22.13±5.13) μg/L and (28.15±7.90) μg/L respectively, lower than those in the 38℃ group (26.92±3.89) μg/L and (36.57±8.90) μg/L, the differences were statistically significant ( F value was 6.656, P=0.017; F value was 6.012, P=0.023). Blood lactic acid content of children in the 43℃ group at T 4-T 9 respectively (2.77±0.70), (2.93±0.69), (3.13±0.77), (3.39±0.64), (3.77±0.86), (3.83±0.62) mmol/L, lower than 38 ℃ group of children (3.57±0.87), (3.82±0.80), (4.35±0.89), (4.60±0.95), (4.79±0.94), (4.92±0.92) mmol/L, the differences were statistically significant ( F values were 6.125-11.551, P values were 0.022-0.003). Conclusion:Both 38 ℃ and 43 ℃ can ensure that the body temperature of the children increases steadily during the intraoperative rewarming phase in deep hypothermic circulatory arrest operation,but 43℃ can effectively reduce the serum PAF and NSE in children with brain protection, its mechanism may be associated with reduced blood lactic acid.
