2.Coach-led transitional care of elderlies in the United States:Intervention model

This paper detailed the Coleman concept and practice of coach-led transitional care of elderlies,team members and their responsibilities,intervention aspects and main tasks,as well as the intervention program and its effect analysis.Then the authors analyzed the present transitional care development in China,and recommended on development of a likewise care model for senior chronic disease patients in the country,such as building multi-disciplinary transitional care teams to provide uninterrupted care and service,encourage patients and caregivers to embrace self-management of diseases,and build an appraisal mechanism to ensure quality of care.

5.Relationship of vascular calcification and cardiovascular disease in maintenance hemodialysis patients

Objective To observe the occurrence of vascular calcification in maintenance hemedialysis (MHD) patients. Method Sixty-nine MHD patients were enrolled in this study. Vascular calcification was evaluated by plain X-my films of chest, upper and lower extremites. Analyzed the relationship of vascular calcification and cardiovascular disease. Results Vascular calcification on X-ray films presented in 38 cases (55%). Of the 23 cases with calcification on arteries of extremites, 14 cases were found to have both intimal calcification and medial calcification. Patients with vascular calcification were older. Ischemic heart disease and cardiac failure appeared more frequently in the patients with vascular calcification. Conclusions Both intimal calcification and medial calcification are frequently found in the MHD patients. Vascular calcification is probablly associated with cardiovascular disease in MHD patients.
6.Determination of Total Selenium in Salt by HG-AFS

Objective To establish a sensitive ,reliable and convenient method for determination of total selenium(Se) in salt by HG-AFS. Methods The salt samples were dissolved and added hydrochloric acid (HCl),after heating by boiling water ,used HG-AFS to determinate Se. Results The linear range of the method was 0.5-400 ?g/L, The correlation coefficient was larger than 0.999 5, the recovery rates were 95.7%-102.8%. If 1 ml sample was collected, the detection limit was 0.08 ?g/L. Conclusion The method is sensitive, reliable, simple, less sample usage, and is an ideal method for determination of total selenium in salt.
7.Current status of self-management action of patients with chronic kidney disease during early-mid stage and correlative research of its influencing factors

Objective To investigate the current status of self-management action of patients with chronic kidney disease (CKD) during early-mid stage and its influencing factors. Methods 80 patients were recruited in a tertiary hospital in Beijing from March to June 2011.They were surveyed with a self-designed self-management action questionnaire of patients with CKD. Results The total score of self-management action in patients with CKD was(69.10±5.63 ).1.25% patients with CKD had bad self-management ability and 88.75% patients had moderate self-management ability,only 10.00% patients had good self-management ability.Patients of younger age,with higher education level,ever attending lectures or having inpatient experience had better self-management ability.And the methods of payment also had significant influence on the self-management of patients with CKD. Conclusions When taking care of patients with CKD,health care providers should be more individual case-based,especially paying more attention to those who are older,with low education degree,never have attended health education lectures and do not have inpatient experience,in order to increase patients' self-management ability and help them improve their health status.
8.Effect of different physical activities on blood pressure in elderly patients with hypertension

Objectives To investigate the effect of different physical activities on the blood pressure in elderly patients with hypertension,and to provide evidence for the prevention and treatment of hypertension.Methods 256 elderly patients with grade 1,2 hypertension and no other complications,and 256 healthy elderly people were selected as the control subjects.According to the blood pressure and the level of physical activity,the patients were divided into hypertension combined with low physical activity group(HL group),hypertension combined with middle physical activity group(HM group),normal blood pressure combined with low physical activity group(NL group)and normal blood pressure combined with middle physical activity group (NM group).The changes of blood pressure were compared in quiet,during and after physical activity.Results The diastolic blood pressure(DBP) and systolic blood pressure(SBP) measured at rest,peak exercise and 5 minutes after exercise were significantly higher in group HL and HM group than in NL group and NM group,and the differences between groups were statistically significant(SBP:F=11.846、18.337、10.976,all P< 0.001 for all;DBP:F=13.182、11.694、11.453,all P<0.05).The DBP and SBP in HL group at rest and 5 minutes after exercise were significantly higher than those in the HM group,and the difference was statistically significant(P<0.05).The change magnitudes of DBP and SBP were significantly lower in HL group than in the HM group,and the differences were statistically significant (P < 0.05).Conclusions Taking part in an appropriate exercise can improve blood pressure level and a recovery capability of post-exercise blood pressure in elderly patients with hypertension.
9.Relationship of Elderly Loneliness,Filial Expectations,and Intergenerational Social Support

Objective: To investigate the loneliless situation of the elderly, and explore the relationship between loneliness,filial expectations and intergenerational social support among the elderly. Methods: Using UCLA Loneliness Scale, Intergenerational Social Support and Fealty Expectation Questionnaire, we investigated 423 samples of old people aged 60 ~94 of Changsha City. Results: The mean score of loneliness was 39.8 (s =7.3). Ordinal regression analysis indicated that the loneliness of the elderly was significantly related to received adult children social support (OR=2.208, P=0.006),afforded adult children social support(OR=2.056,P=0.01) and filial expectation(OR=1.872,P=0.027),controlled by age, sex, level of education, marriage, level of family income, and residence status. Filial expectation directly affected loneliness and indirectly affected loneliness through received adult children social support and afforded adult children social support. Conclusion: The loneliness of the elderly are significantly related to filial expectation and intergenerational support.
10.Effects of Hedysari radix polysaccharide on the behavior and central neurotransmitters in SAMP8

Objective To investigate the improving effect of Hedysari radix polysaccharide on the behavior and its influence on cerebral central neurotransmitters in senescence accelerated mouse prone 8 (SAMP 8).Methods The senescence accelerated mice 8 were randomly divided into a model group,a Hedysari radix polysaccharide (HRP) group and a aricept group.SAMRl were served as the control group.After intragastrical treatment for 3 months,the behavior changes and the levels of norepinephrine (NE),dopamine (DA),5-hydroxytryptamine (5-HT) and 5-hydroxyindoleacetic acid (5-HIAA) were observed respectively by applying Morris water maze and high performance liquid chromatography (HPLC).Results In the SAMP8 model group,the hidden platform test showed that the latency to find the hidden platform was remarkably prolonged,the latency range from third day to fifth day was (55.31 ±9.62) s,(58.67± 10.89) s,(58.45± 10.53) s,respectively.The level ofNE、DA、5-HT and 5-HIAA was (4.97± 1.74) mmol/L,(1.63±0.57)mmol/L,(4.29±0.94)mmol/L,(2.91±0.85)mmol/L respectively,significantly reduced (P<0.01).Compared with model group,the latency of hidden platform test was (44.73 ± 9.83) s,(40.53±8.37)s,(44.75±9.16)s respectively,which were remarkably shortened.The expression level ofNE,DA,5-HT and 5-HIAA in the HRP treatment group was (7.58± 1.62) mmol/L,(3.25 ±0.70) mmol/L,(7.04±0.95) mmol/L,(4.62±0.79) mmol/L respectively,which obviously increased (P< 0.05).Conclusion HRP could improve learning and memory ability of SAMP8 and the effect may related to up-regulate the expression level of cerebral Central neurotransmitters.
