1.Open reduction and internal fixation for intercondylar comminuted fractures of the humerus by AO reconstructive plate

Objective To study the method and outcome for intercondylar comminuted fractures of the humerus through open reduction and internal fixation with AO reconstructive plate. Methods From March 1997 to September 2000, 31 cases of intercondylar comminuted fracture of humerus were reviewed, which involved 24 males and 7 females with an average age of 37.4 years (range, 18-67 years). The fracture was located on left side in 17 and on right side in 14. According to AO/ASIF classification, there were C1 in 11, C2 in 12, and C3 in 8, among them, 9 cases were open fractures including of Cusstilo and Aderson typeⅠin 5 and Ⅱ in 4. Posterior trans-olecranon osteotomy with open reduction and internal fixation by AO reconstructive plate or plus 1/3 tube plate were performed in all the cases within 48 hours after injury. The osteotomy was "V" shape at 2 cm to proximal end of ulna. All the patients began the passive motion exercise within 24-48 h, and active training at one week postoperatively. X-ray was taken in 1, 3, 6 and 12 months after operation to observe the bony union. 5 cases with ulnar nerve injury underwent nerve exploration and anterior transposition. Results All the cases were followed up 1-3.4 years (average, 1.5 years), the fractures healed well, and the elbow function recovered satisfactorily. According to the Cassebaum scoring system, 11 were assessed as excellent 11, good 14 and fair 6, and the excellent-good rate was 80.6%. The complications included 1 infection, 1 heterotopic ossification and 2 injuries of the radial nerve, which recovered with dressings, antibiotics and neural nutrients. Conclusion The technique of AO reconstruction plating for the intercondylar comminuted fractures of the humerus through trans-olecranon approach offers many advantages, such as sufficient exposure, anatomical reduction, stable fixation, earlier exercise and satisfactory elbow function.
2.The ultrasonographic effectiveness of SCSA in the prenatal diagnosis of fetal limb deformities

Objective To explore the value of ultrasonographic on fetal limb deformities in prenatal diagnosis by using systemat-ic continuous sequence approach(SCSA) Methods During the period of January 2008 to April 2012 ,SCSA with color doppler ultra-sonography was used to screen 41 460 pregnant women in order to observe the four limbs of the fetus .And compared with the limbs of the fetus after birth or induction of labor .Results There were 99 cases of fetal limb deformities in 41 ,460 pregnancy ,of which 31 cases of femoral abnormalities were found by routine ultrasound ,92 cases of fetal limb deformities were found by SCSA .68 cases of fetal limb deformities were detected by SCSA in 19- <27 weeks of gestation ,accounting for 73 .91% of detection limb deformi-ties .In the 99 cases of fetal limb deformities ,varus feet were the most was (27 cases) ,followed by short limbs deformity (24 cases) , limbs compound malformations(11 cases) ,limb posture abnormalities(10 cases) ,part of the amputation(10 cases) and multiple fin-gers/toes(7 cases) .Conclusion SCSA with color doppler ultrasonography is an effective method for prenata screening of fetal limb deformities .
3.A model analysis on the knowledge-attitude-practice of children guardians in Jiangxi, Shanghai and Qinghai

Objective:To understand the knowledge attitude and practice (KAP) on vaccination among children's parents in Jiangxi, Shanghai, and Qinghai and explore the factors influencing KAP.Methods:The study selected two counties/districts in Jiangxi, Shanghai, and Qinghai, respectively, by stratified sampling and used a unified questionnaire to investigate the parental KAP of vaccination. A structural equation model (SEM) was used to explore factors influencing parental KAP, as well as the relationship between knowledge and behavior.Results:Of the 760 valid questionnaires, the knowledge of vaccination among children's parents was better, and the vaccination knowledge of parents in Qinghai and Shanghai were slightly better than those in Jiangxi. Parents mainly obtained vaccination knowledge through medical staff and vaccination manuals. The fitting degree of SEM was relatively good; the root mean square error of approximation of the model is 0.033. The higher the parents' education level, the better their knowledge of vaccination ( β?=0.082). Parental vaccination knowledge could influence whether the vaccinated children stay for half an hour in the clinics ( β?=0.541). It could also impact whether parents giving up vaccinating their children in the face of media reports about the adverse effects of vaccinations ( β?=0.515). Conclusions:The knowledge of vaccination among the parents in Jiangxi, Shanghai, and Qinghai was quite good. Moreover, we should pay more attention to the mass media programs and vaccination knowledge among parents with low or middle education backgrounds. Vaccination knowledge can be disseminated through medical staff, vaccination manuals, or mobile applications.
