1.Clinical significance of C-reactive protein level and erythrocyte sedimentation rates before and after total hip and total knee arthroplasty

[Objective] To study the clinical significance of the changes of 100 cases C-reactive protein(CRP)values and erythrocyte sedimentation rates(ESR)in patients with primary osteoarthritis before and after total hip and total knee replacement.[Method] A prospective study was designed to observe the changes of CRP leves and ESR in patients with primary osteoarthritis before and at 1~7 days,2 weeks,1,2,3,6,12 months after total hip and total knee arthroplasty.Ninty-two cases received follow-up.There are 35 males and 57 females with average of 64.7 years,ranging from 37 to 92 years.Sixteen had cementless total hip replacement(THR),32 had hybrid THR,7 had cemented THR and 37 underwent cemented to tal knee replacement(TKR).[Result]In both groups serum CRP level increased rapidly after surgery,peaking at day 2(THR 216.7 mg/L,TKR 257.3 mg/L),and dropping gradually to pre-operative values at the end of the first month in THR patients and at the end of the second month in TKR patients.ESR peaked at day 5 and 6 respectively after operation in THR and TKR patients(THR 93.2 mm/h,TKR 95.7 mm/h),dropping to pre-operative values at the end of the third month in THR patients and at the end of the ninth month in TKR patients.[Conclusion]Serum CRP levels changed more rapidly than ESR and returned to normal more rapidly.CRP and ESR values tended to be higher in TKR than in THR patients.CRP can reflect the status of inflammation.Dynamic observation can help diagnoze early infection.

3.Effects of different concentrations of ropivacaine hydrochloride on lower limb nerve block anesthesia

Objective To investigate the clinical effect of ropivacaine hydrochloride at 0.25%, 0.375% and 0.5% concentration for lower limb nerve block anesthesia.Methods75 cases of lower extremity nerve block anesthesia from Ningbo Zhenhai District Hospital of traditional Chinese Medicine from September 2014 to February 2015 were enrolled in the course of the study, they were divided into three groups: group 0.25%, the equivalent number of 0.375% and 0.5% groups, and three groups of patients were made with a concentration of 0.25%, 0.375%, 0.5% ropivacaine hydrochloride for lower extremity nerve block anesthesia;the clinical data of three groups were analyzed retrospectively, observation of three groups of patients with lower limb nerve block effect.ResultsThe results showed that 0.25% groups of patients in the motor block time was (36.8±5.9) minutes, motor block in patients with a total of 7 cases, with nerve block in patients with a total of 11 cases.0.375% groups of patients in the motor block time was (23.1±4.3) minutes, motor block in patients with a total of 14 cases, with nerve block in patients with a total of 18 cases.0.5% groups of patients in the motor block time was (20.6±5.7) minutes, motor block in patients with a total of 16 cases, with nerve block in patients with a total of 18 cases.Available block time of 0.375% groups of more than 0.5% groups, less than 0.25% groups;0.375% motor block group was 56.0%, higher than the 0.25% group, lower than the 0.5% group;the 0.375% group and 0.5% group of nerve block ratio is same, higher than the 0.25% group, the difference was statistically significant (P<0.05).ConclusionThe effect of anesthesia using concentration of 0.375% ropivacaine hydrochloride is relatively good, can reduce the dosage of anesthetic drugs, but also ensure the anesthetic effect, meet the clinical requirement, this method will be applied to.
4.Observation of the estrous cycle in female NOD/SCID mice

Objective To observe the changes in estrous cycle and vaginal smears in ovarectomized NOD/SCID mice.Methods To continuously observe the estrous cycle time by vaginal smears of NOD/SCID mice in consecutive nine days, twice daily.After ovariectomy, the changes of estrous cycle were observed by vaginal smears for 7 days.Results The estrous cycle in NOD/SCID mice was 4-6 days.Regular estrous mice accounted for 80%.There was no significant correlation between vaginal opening and estrous cycle status.After ovariectomy, the vaginal smears showed characteristics of metestrus or diestrus.Conclusions Vaginal smear cytology can be used to determine the estrous cycle and characteris-tics of NOD/SCID female mice.The ovariectomized operation of NOD/SCID female mice is effective.
5.Analysis of types of coronary thrombosis by optical coherence tomography in patients with acute ST-segment elevation myocardial infarction

Objective To explore the related factors affecting the formation of different types of thrombosis in patients with STEMI. Methods Retrospective data were collected from September 2014 to October 2018 in Henan Province People's Hospital for emergency interventional treatment of patients with STEMI. According to the type of thrombus detected by OCT, they were divided into two groups, which were red thrombus group and white thrombus group. Clinical baseline data, coronary angiographic findings, and OCT results were collected in patients with STEMI. Logistic regression analysis was used to analyze the type of thrombosis in patients with STEMI. Results The rate of thrombus detection was 100% in 92 patients with STEMI. According to OCT diagnostic criteria, 73 (79.3%) patients were classified into red thrombus and 19 (20.7%) were white thrombus. There was no significant difference in infarct-related artery and location between the two groups (P>0.05) , but there was more multiple vascular lesions in the white thrombus group than that in the red thrombus group. There was significant difference in the incidence of plaque rupture and thin fibrous cap in the two groups (P<0.05). The incidence of plaque rupture and thin fibrous cap lipid plaque was higher in the red thrombus group. Plaque rupture (OR = 2.894, 95%CI:2.704-2.956) , thin fibrous cap plaque (OR = 8.033, 95%CI: 7.985-8.283) , single vessel disease (OR = 1.746, 95% CI: 1.659-1.785) are risk factors for red thrombosis formation. Conclusion Single vessel lesion, plaque rupture andthin fibrous cap lipid plaque are associated with red thrombus formation.While, multiple vessel lesions and stable plaque are associated with white thrombus formation.
6.Analysis of coronary angiography and types of intracoronary thrombus in patients with acute myocardial infarction

Objective:To investigate the characteristics of coronary angiography and types of intracoronary thrombus in patients with acute myocardial infarction (AMI) who were classified according to changes of ST segment in electrocardiogram (ECG).Methods:A total of 232 consecutive AMI patients within 24 h of symptom onset undergoing primary percutaneous coronary intervention (PCI) in Henan Provincial People’s Hospital from September 2016 to August 2018 were included. According to the changes of ST segment, patients were divided into three groups: ST-elevated group (161 cases), ST-unoffset group (28 cases) and ST-depression group (43 cases). Optical coherence tomography (OCT) during primary PCI were performed and intracoronary thrombus types were differentiated according to OCT. One-way analysis of variance, Chi-square test and Fisher exact probability test were used to analyze the clinical baselines, angiographic characteristics and intracoronary thrombus types among the three groups. A P<0.05 was considered statistically significant. Results:Left anterior descending coronary artery and right coronary artery were the most common infarct related arteries in the ST-elevated group and ST-depression group, while left circumflex artery was more common in the ST-unoffset group ( P<0.001). The infarct sites in the ST-elevated group and ST-depression group were mostly located in the proximal and middLe segments, while those in the ST-unoffset group were mostly located in the middLe and distal segments ( P=0.008). The proportion of occlusive lesion in the ST-elevated group and ST-unoffset group was higher than that in the ST-depression group (68.3% vs. 30.2%, P<0.05; 67.9% vs. 30.2%, P<0.05). The proportion of single vessel in the ST-elevated group and ST-unoffset group was higher than that in the ST-depression group (36.0% vs. 4.7%, P<0.05; 39.3% vs. 4.7%, P<0.05). The proportion of collateral circulation of infarct related artery in the ST-unoffset group and ST-depression group was higher than that in the ST-elevated group (35.7% vs. 16.1%, P<0.05; 58.1% vs. 16.1%, P<0.05). The incidence of red thrombus in the ST-elevated group and ST-unoffset group was significantly higher than that in ST-depression group (76.4% vs. 34.9%, P<0.05; 64.3% vs. 34.9%, P<0.05). Conclusions:Compared with the ST-depression group, the proportions of single-vessel, occlusive lesion and red thrombus are higher in the ST-unoffset group and ST-elevated group. Therefore, in acute non ST-segment elevated myocardial infarction, those without ST-segment deviation are different from those with ST-segment depression. It is recommended to further divide acute myocardial infarction into ST segment elevation type, ST segment unoffset type and ST segment depression type according to ECG.
7.Factors associated with smoking cessation attempts in male current smokers in rural area of Shandong province

Objective To explore the influencing factors for the smoking cessation attempts in male current smokers.Methods This cross-sectional study was conducted in nine villages selected from three counties (Pingyin,Junan and Liangshan) of Shandong province in August,2010 through household questionnaire survey in villagers aged ≥ 15 years to collect the information about current smokers demographic characteristics and smoking-related behaviors.A hurdle count data model was used to assess factors associated with the times of past smoking cessation attempts.Results Among 1 798 male current smokers,29.53% had at least one smoking cessation attempt.Smokers who were married (3=0.705,P=0.002),had high educational level (β=0.214,P=0.026) and had higher level of awareness of smoking risks (β=0.237,P=0.009) were more likely to have smoking cessation attempt.Young age at smoking initiation (3 =-0.167,P=0.035) and higher level of awareness of smoking risks (β =0.146,P=0.020) were associated with increased smoking cessation attempts.Conclusions Less male current smokers had smoking cessation attemps in rural area in Shandong.The factors influencing smoking cessation attempt varied.It is necessary to conduct targeted intervention according to the smokers' smoking cessation experience.
8.Effects of modified electroconvulsive therapy on hippocampal morphology and inferior ventricular vein in patients with major depressive disorder

Objective To study the changes in hippocampal morphology and inferior ventricular vein in patients with major depressive disorder (MDD) before and after modified electroconvulsive therapy (MECT) using morphological measurement and susceptibility weighted imaging (SWI).Methods Recruited for this study were 64 MDD patients who had been admitted to the Affiliated Guangji Hospital to Soochow University from February 2016 through March 2017.They were assigned into 2 equal groups.One group was subjected to MECT and the other to simple drug treatment.All the patients were scanned with T1-MR and SWI one day before and after treatment to observe the changes in hippocampal morphology and inferior ventricular vein.The associations between relative parameters and Hamilton Depression Scale (HAMD) were analyzed.The 2 groups were compared in terms of therapeutic efficacy.Results The volumes of the whole hippocampus and the hippocampal body were increased significantly after MECT (P<0.05) while there were no significant differences in the hippocampal head or tail (P>0.05).Significant increases in the branch number,diameter and length of the inferior ventricular vein were found after MECT (P<0.05).The left and right hippocampal volumes were negatively associated with the HAMD scores (r=-0.442,P=0.011;r=-0.470,P=0.007).Negative correlations were also found between the branch number,diameter and length of the inferior ventricular vein and the HAMD scores (r=-0.419,P=0.017;r=-0.477,P=0.006;r=-0.398,P=0.024).The curative efficacy of the MECT group was significantly higher than that of the drug treatment group (91% versus 72%) (P<0.05).Conclusion Changes in the hippocampal morphology and inferior ventricular vein can be used as one of the objective indicators to evaluate the therapeutic efficacy of MECT in MDD patients.
9.Outcomes of very severe aplastic anemia patients with different absolute neutrophil counts after frontline immnunosuppressive therapy.

OBJECTIVETo analyze early hematopoietic response and long-term survival of very severe aplastic anemia (VSAA) patients with different absolute neutrophil counts (ANC) after frontline immnunosuppressive therapy (IST).
METHODSClinical data and outcome of 145 VSAA patients treated with rabbit antithymocyte globulin combined with cyclosporine were retrospectively analyzed. Hematopoietic responses to IST and long-term survival were statistically analyzed for VSAA patients in different ANC subgroups.
RESULTSPre-IST ANC=0.05×10(9)/L acted as the best cutoff level to predict IST response at 3, 6 months. For 145 VSAA patients, early death rate was 13.4% (11/82) vs 1.6% (1/63), respectively, in the ANC≤0.05×10(9)/L group and ANC>0.05×10(9)/L group (P<0.05). Hematopoietic response rates to IST was 22.0% vs 54.0% (P=0.000) at 3 months, 34.1% vs 63.5% (P=0.000) at 6 months; the overall five-year survival rate was only (62.5±5.4) % vs (91.4±3.7) % (P=0.000) and five-year event-free survival rate was (42.3±5.5) % vs (63.1±6.5) % (P=0.003), respectively, in the ANC≤0.05×10(9)/L group and ANC>0.05×10(9)/L group.
CONCLUSIONVSAA patients with extremely low ANC (≤0.05×10(9)/L) had high early death rate and with very low response rate to frontline IST and poor survival, so it is urgent to seek for the alternative frontline therapy that will bring faster and better outcome for these patients.

10.Effect of excretory/secretory protein of Trichinella spiralis adult worm on CLP-induced sepsis in mice

Objective To observe the effect of excretory/secretory products from Trichinella spiralis adult worms(AES)on cecal ligation and puncture(CLP)?induced sepsis in mice. Methods Forty?eight BALB/c mice were randomly divided into 3 groups:a sham operation group(PBS+sham group,Group A),a CLP?induced sepsis group(PBS+CLP group,Group B)and an AES treatment group(AES+ CLP group,Group C). The mice of each group were intraperitoneally injected with 25 μg of AES or PBS only as a control in a total volume of 200μl. Eight mice from each group were selected randomly for survival analy?sis of 96 hours. The other 8 mice in each group were observed for pathological changes in the lung,liver and kidney tissues by HE staining 12 h after CLP,and then determined for the detection of cytokines including TNF?α,IL?1β,IL?6,IL?10 and TGF? βin the sera by ELISA. Results The difference among the survival rates of mice in the 3 groups was statistically significant (χ2=21.16,P<0.05). Compared to Group A(100%),the survival rate of mice in Group B(0)decreased significantly(P<0.05),and also the pathological damage degrees in the lung,liver and kidney tissues of the mice in Group B increased signifi?cantly after CLP. Compared with the mice in group B,the survival rate of those in Group C(70%)increased significantly(P<0.05),and the pathological damage degrees in the lung,liver and kidney tissues of the mice in Group C decreased significantly after the treatment with AES. The differences among the levels of pro?inflammatory cytokines TNF?α(F=27.11,P<0.05),IL?1β(F=18.75,P<0.05)and IL?6(F=100.93,P<0.05)in the sera of the mice in the three groups were statistically signifi?cant. Compared with the mice in Group A,the levels of the 3 cytokines of those in Group B increased significantly(all P <0.05). However,after the treatment with AES,the levels of the pro?inflammatory cytokines of those in Group C decreased signifi?cantly(all P<0.05). The differences among the levels of immunoregulatory cytokines IL?10(F=10.88,P<0.05)and TGF?β(F=11.37,P<0.05)in the sera of the mice in the three groups were also statistically significant. Compared with the mice in Group B,the levels of IL?10 and TGF?β of those in Group C were higher after treatment with AES(both P<0.05). Conclu?sion T. spiralis AES has a therapeutic potential for alleviating sepsis induced by CLP in mice.
