1.Short-term Therapeutic Effect of Elemene Combined with Interventional Therapy for Moderate and Advanced Liver Cancer

Objective To evaluate the short-term effect of elemene combined with transcatheter hepatic arterial chemoembolization (TACE) interventional therapy for the treatment of moderate and advanced liver cancer.Methods A total of 63 patients with moderate and advanced liver cancer were randomized into test group(31 cases) and control group(32 cases). Both groups were given TACE, and additionally, the test group was given intra-arterial infusion of Elemene Emulsion(400-600 mg/m2)together with MAO(Mitomycin C 4-6 mg/m2 , adriamycin 20-40 mg/m2, Oxaliplatin 50-85 mg /m2), and the control group was given intra-arterial infusion of MAO. Short-term effect was evaluated by the clinical outcomes of tumor size, progression-free survival(PFS), total survival rate and adverse reactio n. Results(1)The overall response rate(ORR) in the test group was superior to that in the control group, and the difference was statistically significant(P<0.05) .(2)PFS in the test group covered 125 days, and that in the control group covered 118 days, the difference being significant (P<0.05); the median survival time(MST) in the test group was 257 days, and that in the control group was 214 days,the difference being insignificant(P>0.05).(3) No severe toxic or side effect was found in t he twogroups, the difference being not significant(P>0.05). The test group tended to have milder adverse reaction. Conclusion Intra-arterial infusion Elemene Emulsion combined with TACE interventional therapy is effective in increasing ORR, prolonging PFS, promoting adverse reaction endure and relieving symptoms of weakness, abdominal distension and pain.
2.Construction of Prokaryotic Expression Vector of the Fusion Gene IFN-?1b/CSPⅡ and Expression in E.coli

Objective To Construct the prokaryotic expression vector of the fusion gene IFN-?1b/CSPⅡ. MethodsIFN-?1b was amplified from the human genomic DNA by PCR and cloned into prokaryotic expression vector pGEX-4T-1. The recombinant plasmid pGEX-4T-1/IFN-?1b was constructed. Circumsporozoite proteinⅡ(CSPⅡ) was amplified from the Plasmodium falciparum genomic DNA by PCR and was cloned into the prokaryotic expression vector pGEX-4T-1. The recombinant plasmid pGEX-4T-1/CSPⅡ was constructed. IFN-?1b was cut from the recombinant plasmid pGEX-4T-1/IFN-?1b digested with BamHⅠ and EcoRⅠ and ligated with the recombinant plasmid pGEX-4T-1/CSPⅡ also digested with BamHⅠ and EcoRⅠ . The recombinant prokaryotic plasmid pGEX-4T-1/IFN-?1b/CSPⅡ was constructed. The fusion gene IFN-?1b/CSPⅡ was expressed in E.coli by IPTG. Results The prokaryotic expression vector pGEX-4T-1/IFN-?1b, pGEX-4T-1/CSPⅡ and pGEX-4T-1/IFN-?1b/CSPⅡ were identified by PCR, enzyme digestion and gene sequencing. The expressed fusion protein/IFN-?1b/CSPⅡ in E.coli was identified by SDS-PAGE and Western blot. Conclusion The prokaryotic expression vector of the fusion gene IFN-?1b/CSPⅡ was successfully constructed, which was then expressed in E.coli .
3.Qualitative research on self-management in elderly patients with COPD during community-based rehabilitation

Objective To explore the current situation of knowledge,beliefs and behavior of elderly patients with COPD during their community-based rehabilitation.Methods Phenomenological research method of qualitative research was used,14 elderly patients with COPD during their community-based rehabilitation period received in-depth interviews.Results There were three themes and corresponding subthemes sublimated from the patients:self-management knowledge and skills,self-management beliefs and attitudes and self-management behavior.Conclusions It is necessary to pay close attention to spreading disease related health information,especially for patients with low health literacy.During the process of COPD self-management projects,we should focus on negative beliefs and negative attitudes on their selfmanagement behavior choice,which is as important as strengthening the patients' self-efficacy.
4.Preliminary application of multi-slice CT direct venography in lower extremity vein

Objective To discuss the injective method, contrast agent concentration and delay time of MSCT direct venography in lower extremity vein, in order to make optimal venographic schedule. Methods Thirty-two patients with lower extremity vein diseases underwent MSCT direct venography in using different concentration and injective rate of contrast agent, and the images were analyzed. The displaying ability of MSCT direct venography in lower extremity vein segment was compared with that of X-ray venography in 22 patients. Results Blood flow correlation constructed defect was the main factor impacting image quality of CT venography in lower extremity. The image scores between the groups of injective rate 1.5 ml/s and the group of 2.5 ml/s were statistically different (χ~2=5.495, P=0.019), whereas no difference of image score was found between the group of concentration of 25% and 15% (χ~2=3.333, P=0.068). The displaying ability of CT and X-ray venography in anterior tibial vein, posterior tibial vein, popliteal vein and femoral vein was not statistically different (χ~2=2.095, P=0.148). For iliac vein, the displaying ability of CT outstripped that of X-ray venography (χ~2=8.324, P=0.004). Conclusion With the schedule of delay time 50 s, injective rate 2.5 ml/s and concentration 45 mgI/ml (nonionic 300 mgI/ml and diluted 15%), MSCT direct venography in lower extremity can acquire satisfactory image, and its displaying ability of iliac vein is better than that of X-ray venography.
5.Direct Electrochemistry of Horseradish Peroxidase in Surfactant Films

The oxidation-reduction properties,relative to the Fe(Ⅲ)/Fe(Ⅱ) conversion,of horseradish peroxidase(HRP)in surfactant films at edge-plane pyrolytic graphite(EPG)electrode have been investigated by using direct electrochemical methods.Cyclic voltammograms for DDAB/HRP/CME(chemically modified eelctrode),DMPC/HRP/CME,DHP/HRP/CME at 2V/s in phosnhate buffer solution (pH 7.O) show one pair of reversible,reproducible redox peaks. The redox potentials (E1/2) in DDAB/HRP/CME,DMPC/HRP/CME and DHP/HRP/CME are-0.212V,-0.330V,-0.381V respectively. Experimental result indicated that the membranes,especially the positively charged one,providing special film environment,might interfere in the protein-electrode electron transfer process. The electron transfer processes of HRP were described in the absence and in the presence of hydrogen peroxide.There are good linear relationships between the currents and the H2 O2 concentration using all the three CMEs.The results obtained are of potential value for basic and applied bioelectrochemistry and represent an opportunity to build the third generation of biosensors.
6.Research in correlation of hardiness personality and professional commitment among nursing students

Objective To explore the correlation between nursing students' hardiness personality and professional commitment.Methods 1 115 undergraduate nursing students from 3 medical universities in Guangzhou were investigated with the scales of hardiness personality and nursing professional commitment.Results The scores of hardiness personality and professional commitment were (2.55 ± 0.44) and (2.70 ± 0.43),respectively,and both of them were at the middle level.Correlation analysis revealed that hardiness personality was positively related to professional commitment (P<0.01),and the challenge dimension of hardiness personality had positive predictive effect on professional commitment.Conclusions Nursing educators should take measures to improve students' hardiness personality and professional commitment.It would help maintain the stability of the whole nursing team.
7.A transcranial magnetic stimulation study on Alzheimer's disease and behavioral variant frontotemporal dementia

Objective To investigate the changes in cortical excitability and inhibitory circuits of patients with Alzheimer's disease (AD) or behavioral variant frontotemporal dementia (bvFTD) using transcranial magnetic stimulation (TMS).Methods Forty-four patients with AD,30 patients with bvFTD and 44 healthy controls were enrolled in the study.The epidemiological data of all subjects were collected.Each participant received a neurological evaluation and neuropsychologic tests which included Mini-Mental State Examination (MMSE),Activities of Daily Living (ADL) and Hamilton Rating Scale for Depression (HAMD).Neurophysiological evaluations including resting motor threshold (rMT),active motor threshold (aMT) and cortical silent period (CSP) were conducted by means of TMS.Neurophysiological parameters were compared among groups.Results There were significant differences in MMSE,ADL and HAMD assessments among 3 groups,but no significant differences between AD and bvFTD groups.There were significant changes in left rMT(46.52% ± 8.77%,52.00% ± 7.30% and 52.14% ± 8.27%,F =6.295,P=0.003),left aMT(29.68% ±7.01%,35.13% ±8.46% and 35.39% ±7.24%,F=7.735,P=0.001) and right rMT(47.82% ±7.94%,52.07% ±8.77% and 53.12% ±8.61%,F=4.772,P=0.010) among 3 groups.AD patients showed significantly reduced left rMT and aMT comparing to bvFTD patients (P =0.017 and 0.008 respectively) and controls (P =0.005 and 0.002 respectively).In addition,AD patients had a significant decrease in right rMT comparing to controls (P =0.011).There were no statistically significant differences in TMS parameters between bvFTD patients and controls.Conclusions AD is associated with hyperexcitability of the motor cortex,whereas the lack of changes in motor cortical excitability is found in bvFTD.TMS technique may be helpful in differential diagnosis between AD and bvFTD.
8.Detection of CTP gene expression in the asexual erythrocytic stages of Plasmodium falciparum (FCC1 /HN) by reverse transcriptase- polymerase chain reaction and construction of eukaryotic expression vector of CTP gene

Objectve To detect whether the CTP(phosphocholine cytidylyltransferase) gene was expressed in the asexual erythrocytic stages of Plasmodium falciparum (FCC 1/HN )by using the RT - PCR and to construct eukaryotic expression vector of CTP. Method The erythrocytic stage parasites of Plasmodium falciparum were cultured as described by Trager and Jensen. RNA from erythrocytic stage parasite was extracted by using Trizol reagent. The complete genes coding for CTP gene isolates FCCI/HN were amplified by reverse transcriptase -polymerase chain reaction(RT- PCR). CTP gene was cloned into eukaryotic expression vector pcDNA3. Results CTP encoding gene was amplified from the erythrocytic stages of Plasmodiumfalciparum (FCC 1/HN) and eukaryotic expression vector of CTP was constructed. Conclusion CTP gene was expressed in the erythrocytic stages of Plasmodium falciparum (FCC 1/HN) and eukaryotic expression vector of CTP was successfully constructed.
9.The negative emotion and self-management in elderly patients with COPD during community based rehabilitation

Objective To explore the negative emotion and self-management of elderly patients with COPD during their community-based rehabilitation.Methods Phenomenological research method of qualitative research was used,12 elderly patients with COPD during their community-based rehabilitation period received in-depth interviews.Results There were three themes sublimated from the patients negative emotions:aloneness and inferiority feeling,self-abasement and guilty feeling,sense of helplessness.Self-management of negative emotions also had three themes:gratitude mood,complying with the disease and distracting attention.Conclusions It is necessary to pay close attention to the negative emotions of elderly patients with COPD during their community-based rehabilitation; the community healthcare services should bear the responsibility of COPD patients' rehabilitation; we should explore practical intervention plan work on gratitude mood of elderly patients with COPD ; mobilize all social resources to do a good job on social support in elderly patients with COPD.
10.Prognosis analysis of repeat hepatectomy for recurrent hepatocellular carcinoma with Cox proportional hazards model

Objective This retrospective study was to explore the efficacy and determine the risk factors of survival for recurrent hepatocellular carcinoma ( HCC) treated by repeat hepatectomy. Methods From January 1995 till December 2010, 60 patients with recurrent HCCs, were treated by repeat hepatectomy.The significance of seventeen clinical or pathological variables in the risk factors of overall survival were assessed. Results The overall survival 1,3, and 5-year survival rates were 76. 3% , 40.7% and 25. 0% (from repeat hepatectomy), and 95. 0% , 62. 6% and 43. 3% ( from initial hepatectomy) , respectively.Univariate analysis indicated that tumor size at initial hepatectomy, recurrence interval from initial hepatectomy, serum albumin(ALB) level, resection margin, diameter of largest recurrence tumor and rumor vascular invasion were significant prognostic factors(P <0. 05, Kaplan-Meier Log-rank test). Multivariate analysis showed recurrence interval from initial hepatectomy, resection margin, diameter of largest recurrence tumor and rumor vascular invasion were significant prognostic factors(P<0.05, Cox proportional hazards model).Conclusion Repeat hepatectomy is effective for recurrent HCC. Recurrence interval from initial hepatectomy, resection margin, diameter of largest recurrence tumor and rumor vascular invasion were significant prognostic factors.
