1.A design and analysis of a monitoring system for muscle relaxation

Objective:To design and analyze a muscle relaxation monitoring system so as to increase the anesthesia efficiency and safety.Methods: The hardware design of system was based on single chip machine (STC89C52RC), and it has the function of LCD real-time display for pressure change and the printing function. The design of software mainly included the system main program design, pressure sensor subroutine, A/D conversion subroutine, LCD real-time display subroutine and printer subroutine, etc.Results:Through the multiple times of test and improvement for the system, the system has achieved stably run, and the pressure value could achieve accurately display between 0-100N.Conclusion: The monitoring system of muscular relaxation has series of advantages, such as simple circuit design, low cost, higher reliability and practicability and so on. It can real-timely and effectively monitor the change of the indexes of muscular relaxation for patients during operation. And the anesthesiologist can effectively control and change medication for patients. In this way, the monitoring system can increase the anesthetic efficiency and decrease the incidence of postoperative residual muscle relaxant.
2.A design of dynamic intelligent management system for patients in mental hospital based on RFID technique

Objective: To implement dynamic intelligent management system for patients in mental hospital based on RFID technique so as to achieve safe management for patients and contribute to grasp the patient's status in real time. Methods: The hardware of the designed dynamic intelligent management system was consisted of patient's wristbands (electronic tags), locator, reader and the server. The architecture, hardware and function modules of the system were procedural designed. Results: The new design has changed the traditional supervised situation for psychopath which need many manpower and material resources, and reduced the burden of paramedical staff, and decreased the pressure of management for psychopath and enhanced the management level of modernization in hospital. Conclusion: The dynamic intelligent management system based on RFID technique can enhance the management level of hospital, and accelerate the construction of informatization for hospital and improve the image of hospital.

4.Effects of "Internet+ nursing" based on B/S intelligent voice follow-up mode management platform in postoperative follow-up of hand and foot trauma patients

Objective:To explore the effect of "Internet+ nursing" based on browser and server (B/S) intelligent voice follow-up mode management platform in the postoperative follow-up of patients with hand and foot traumas.Methods:From November 2020 to April 2021, 200 patients with hand and foot traumas in Yiwu Central Hospital were selected by simple random sampling. The patients were divided into control group and observation group by random number table method, with 100 patients in each group. The control group was given routine follow-up, and the observation group received "Internet+ nursing" based on B/S intelligent voice follow-up mode management platform. The scores of self-care ability and compliance behavior of patients in the two groups before and after the intervention were compared, and the follow-up rate and satisfaction of patients in the two groups after 9 months after discharge were recorded and compared.Results:After intervention, the scores of self-concept, self-care responsibility, self-care knowledge and self-care skills in the observation group were higher than those in the control group with statistical differences ( P<0.05) . The scores of compliance behaviors such as medication, bad behavior control, rehabilitation exercise and daily life in the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.05) . Nine months after discharge, the follow-up rate in the observation group was higher than that in the control group, with a statistically significant difference ( P<0.05) . The satisfaction scores of patients in the observation group on treatment effect, timeliness of drug adjustment, follow-up method, doctor-patient communication, health information were higher than those in the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The "Internet+ nursing" based on the B/S intelligent voice follow-up mode management platform can improve the postoperative follow-up tracking efficiency and satisfaction of patients with hand and foot traumas, and increase the self-care ability and compliance behavior of patients.

Objective: To categorize and summarize the clinical and mechanism studies of the past 30 years on the treatment of Hashimoto’s thyroiditis (HT) with moxibustion, moxibustion plus medication, and acupuncture plus medication, etc., and to analyze the current problems. Methods: The clinical and laboratory studies related to the treatment of HT with acupuncture-moxibustion therapies published before June 2015 were retrieved from MEDLINE, Excerpta Medica Database (EMBASE), China National Knowledge Infrastructure (CNKI), Wanfang Academic Journal Full-text Database (Wanfang) and Chongqing VIP Database (CQVIP). Results:Moxibustion, moxibustion plus medication, and acupuncture plus medication can produce certain therapeutic effects in treating HT. Conclusion:The research on the treatment of HT with acupuncture-moxibustion therapies is rather limited in the amount and content. In the future, standardization should be fortified, specific moxibustion research needs deepening, and the action mechanism of moxibustion should be emphasized.

7.Analysis of influencing factors of turnover intention of nurses in Cardiology Department

Objective To explore the influence and relationship of job stress, working-family conflict and job satisfaction on turnover intention of nurses in Cardiology Department. Methods From June 2016 to June 2017, a total of 482 nurses of Cardiology Department of 9 Class Ⅲ Grade A hospitals in Jiangsu province were selected as the study subjects by random cluster sampling method. All the subjects were investigated with Job Stress Scale, Work Family Conflict Scale, Minnesota Job Satisfaction Scale and Turnover Intention Scale. Pearson correlation analysis was used to analyze the relationship between job stress, work-family conflict, job satisfaction and turnover intention of nurses in Cardiology Department. Multiple linear hierarchical regression analysis was used to analyze the factors influencing turnover intention. Results The total score of Turnover Intention Scale for 482 nurses of Cardiology Department was (2.44±0.73). The score of Job Stress Scale, Work Family Conflict Scale and Minnesota Job Satisfaction Scale Job was (86.06±4.87), (44.04±1.85) and (70.01±2.97) respectively. The result of Pearson analysis showed that job stress was positively related with turnover intention (r=0.434, P<0.01), work-family conflict positively related with turnover intention (r=0.423, P< 0.01), and job satisfaction negatively related with turnover intention (r=-0.450, P< 0.01). Multivariate linear hierarchical regression analysis showed that job stress and work-family conflict had a positive impact on turnover intention, and job satisfaction had a negative impact on turnover intention. Conclusions Nurses in the Department of Cardiology have higher job stress, more conflicts between work and family, general job satisfaction and higher turnover intention, which are the main problems found in the current study. Therefore, the hospital should analyze the reasons of high turnover tendency of nurses in Cardiology Department from various angles, strengthen the support of organization, improve the satisfaction and reduce the turnover tendency. 8.Effects of SREBP-1 over-expression on fatty acid metabolism related genes expression in goats.

The aim of the study was to construct a recombinant adenovirus overexpression vector for Sterol Regulatory Element Binding Protein-1 (SREBP-1) of Xinong Saanen dairy goat, and to detect its effect on genes related to fatty acid metabolism in goat mammary epithelial cells, to establish foundation for further study of its roles in metabolism of fatty acid synthesis and lactation. First, we designed primers based on the SREBP-1 gene sequence in GenBank for PCR amplification and inserted the sequence into shuttle vector pAdTrack-CMV. The recombinant plasmid pAdTrack-CMV-SREBP-1 linearized by Pme I was transformed into E. coli BJ5183 competence cell containing the backbone vector pAdEasy-1 to obtain recombinant vector pAd-SREBP-1 by homologous recombination. pAd-SREBP-1 was linearized by Pac I and transfected into HEK 293 cell. Then we infected goat mammary epithelial cells with recombinant adenovirus which was packaged in HEK 293 cell line. The results showed that the recombinant adenovirus vector containing SREBP-1 was successfully constructed, and the titer of virus was 10(9) U/mL. Compared with the control group, mRNA level of SREBP-1 increased by about 15 times after infected for 48 h and 30 times after infected for 72 h. Fatty acid synthase (FASN) and Acetyl-CoA carboxylase (ACC) was upregulated by almost 2 times. The expression level of Peroxisome proliferator activated receptorgamma (PPARgamma) increased by 1.5 times. Liver X receptoralpha (LXRalpha) and Adipose triglyceride lipase (ATGL) upregulated by 1.2 times compared with that of control. But Stearoyl-coenzyme A desaturase (SCD) had no obvious change. In conclusion, SREBP-1 can activate the expression of genes related to fatty acid synthesis in mammary epithelial cells of Xinong Saanen dairy goat, demonstrated a regulatory function on the fatty acid metabolism in goat mammary gland.

9.Prenatal diagnosis of a fetus with chromosome 18p deletion and duplication.

OBJECTIVE:
To assess the value of chromosomal microarray analysis (CMA) to verify a fetus with partial 18p deletion signaled by non-invasive prenatal testing.
METHODS:
G-banding chromosomal karyotyping analysis was carried out on amniotic fluid sample of the fetus and peripheral blood samples from the parents. Amniotic DNA was also subjected to CMA analysis. The fetus was also subjected to systematic ultrasound scan.
RESULTS:
The fetus was found to have a karyotype of 46,XX,18p+. CMA has revealed a 5 Mb deletion at 18p11.32-p11.31, a 2.9 Mb duplication at 18p11.31-p11.23, and a 2.5 Mb duplication at 18p11.23-p11.22. No chromosomal aberration or microdeletion/microduplication was detected in either parent.
CONCLUSION
Non-invasive prenatal testing and CMA are both sensitive for the detection of chromosomal microdeletions and microduplications. CMA can help with clarification of genotype-phenotype correlation and facilitate prenatal diagnosis and genetic counseling for the family.

10.Levels of phthalate internal exposure levels in pregnant women and influencing factors.

OBJECTIVETo investigate the levels and influencing factors of phthalate internal exposure in pregnant women (gestation age ≤ 16 weeks).
METHODSDuring April to June in 2013, 1 020 pregnant women (gestation age ≤ 16 weeks) who had established the maternal care manual were recruited in maternal and child health hospital of Siming District, Xiamen city. Participators were asked to complete a questionnaire to obtain information on socio-demographic characteristics, lifestyle behaviors, and antenatal examination and to provide a urine sample. Finally, 998 pregnant women who provided a urine sample and completed the questionnaire were enrolled. Adopting systematic sampling method, 100 ones were selected randomly among 998 pregnant women. High performance liquid chromatography-electrospray ionization-tandern mass was used to determine the concentration of five phthalate monoesters in each urine, including mono-n-methyl phthalate (MMP), mono-ethyl phthalate (MEP), mono-butyl phthalate (MBP), mono-benzyl phthalate (MBzP), mono-ethylhexyl phthalate (MEHP). Based on the measurements and questionnaire data, multivariate logistic regression was used to analyze the association between the phthalate monoester levels and potential influential factors.
RESULTSThe detection rates of MMP, MEP, MBP, MBzP and MEHP in 100 pregnant urine samples were 94%, 93%, 87%, 83%, 99%, respectively. And the urinary median uncorrected concentrations of MMP, MEP, MBP, MBzP and MEHP in 100 urine samples were 20.56, 17.62, 10.15, 2.03, and 5.12 ng/ml, respectively. Specific gravity-corrected concentration were 20.81, 20.36, 12.88, 2.58, 5.00 ng/ml, respectively. The results of multivariate logistic regression analysis indicated that: education degree was negatively associated with urinary concentration of MMP, MEP, MBP, MBzP and MEHP, OR (95% CI) were 0.495 (0.253-0.966), 0.380 (0.191-0.755), 0.379 (0.186-0.774), 0.401 (0.196-0.819), 0.373(0.183-0.762), respectively. Participants who had hair permed and dyed during pregnancy had higher urinary level of MBP and MBzP, OR (95% CI) were 12.867 (1.240-133.525), 15.982 (1.367-186.911), respectively; Participants who use cosmetics during pregnancy had higher urinary level of MEP and MBP, OR (95% CI) were 2.977 (1.012-8.757), 4.440 (1.485-13.272), respectively; plastic bottled water consumption was positively associated with urinary concentrations of MEP and MEHP, OR (95% CI) were 3.780 (1.417-10.083), 2.699 (1.039-7.010), respectively; annual household income was negatively associated with urinary concentration of MMP, OR (95% CI) was 0.597 (0.372-0.959); individuals who took medications during pregnancy had higher urinary level of MEHP than non-takers, OR (95% CI) was 4.853 (1.084-21.732).
CONCLUSIONPregnant women whose gestation age was less than 16 weeks are generally exposed to phthalate. Phthalate internal exposure levels are significantly associated with most measured factors and the influencing factors with different phthalates internal exposure levels are different.
