Objective To investigate the expression of PPAR-γ in rectal carcinoma and the relationship between the expression of PPAR-γ and apoptosis in rectal carcinoma. Methods Tissue microarray and immunohistochemistry were used to detect the expression of PPAR-γ gene, bcl-2 and bax in 16 cases of the normal rectal tissue, 16 cases of rectal adenoma and 80 cases of the rectal carcinoma. TUNEL was used to detect of the AI in 80 cases of the rectal carcinoma. Results The positive rate of PPAR-γ expression in normal rectal tissue, rectal adenoma and adenocarcinoma were 6.3%, 25.0% and 71.3% respectively. Clinicopathological analysis showed that the in-crease of PPAR-γ expression were not associated with age, sex and histological classification (P > 0.05), but associated with differentiation grades, pathological stages, lymph nodes metastases and 3-year survival rate (P < 0.05). The expression of PPAR-γwas positively correla-ted with bcl - 2 and bax. AI was negative related with PPAR-γ (r = -0.51, P < 0.05). Conclusions PPAR-γ gene may play a role in the oncogenesis and progression of rectal carcinoma. The abnormal regulation of apoptosis may play an important role in the pathogenesis of rectal carcinoma. Detection of PPAR-γ is of great value for diagnosing colorectal carcinoma, and evaluating malignancy extent and prognosis.

Objective How to avoid the rejection of the synthetic patch and human tissue has become an urgent problem to be solved.The article investigated thein vivobiocompatibility of polypropylene mesh scaffold with adipose -derived stem cells(ADSCs) in rabbits. Methods Rabbit ADSC suspension were prepared.ADSCs were seeded onto polypropylene mesh scaffolds after passage and amplification and cultured invitro for 1 week .The polypropylene mesh and ADSC fixed polypropylene mesh were implanted respec-tively into the surface of rectus abdominis in rabbits.4 weeks later, adhesion and erosion of the meshes were evaluated, HE staining was used in histological observation and RT-PCR was applied to detect the dynamic changes of VEGF mRNA level.ADSCs were isola-ted from rabbit subcutaneous adipose tissue after collagenase digesting, filtrating and centrifuging. Results The results of flow cy-tometry showed that the expressions of CD44, CD73, CD90, CD45, CD14 and CD34 were 98.54%, 95.32%, 98.49%, 1.21%, 3.01%, 2.14%, respectively.Polypropylene mesh, ADSC-fixed polypropylene mesh had different degrees of corrosion and adhesion , but polypropylene mesh showed denser adhesion.In comparison with polypropylene,ADSC fixed polypropylene meshes induced a mil -der chronic inflammation response,with lower scores for inflammation (1.1 ±0.2 vs 0.6 ±0.1, P=0.001), higher scores for neovas-cularization (17.0 ±0.0 vs 2.6 ±0.3, P=0.000) and fibroblastic proliferation(0.9 ±0.1 vs 2.2 ±0.2, P=0.001).Relative a-mounts of VEGF mRNA of were significantly lower for ADSC-fixed polypropylene compared with the remaining polypropylene meshes (t=94.6, P<0.05). Conclusi on Polypropylene mesh scaffold with ADSCs exhibits excellent cellular compatibility and have a bright future in clinical practice.

BACKGROUND: At present, osteoarthritis of knee is mainly treated with non-steroid drug in medication, but resulting in the side effects on gastrointestinal tract. The research indicates that bushen yiqi huoxue formula, with litter side effect, improves remarkably pain and motor function of patients with osteoarthritis of knee.OBJECTIVE: To probe into the effect of bushen yiqi huoxue formula on pathological process of the cartilage of knee joint by setting up ostoarthritis of knee in rabbt.DESIGN: Randomized controlled study by observation in which the experimental animals were taken as the objects.SETTING: Animal experimental center of a university.MATERIALS: Totally 36 New Zealand normal male rabbits, weighted varied from 2.0 to 3.0 kg.METHODS: The experiment was performed in Animal Experimental Center of Guangzhou University of Chinese Medicine from January 2002 to January 2003. Thirty-six New Zealand rabbits were involved to set up Hulth knee osteoarthritis model. After operation, they were randomized into 3 groups,named as model group, acetaminophen group and bushen yiqi huoxue formula group (formula group). Adopted corresponding managements in each group,the materials were collected at the 4th, 8th and 12th week after operation for histological pathological assess.MAIN OUTCOME MEASURES: Primary results: integrals of pathological changes in cartilage of joint in every group. Secondary results: ①Naked-eye observation of cartilage of joint in every group. ② Optic microscopic observation of cartilage of joint in every group.RESULTS: The total integrals of pathological changes in cartilage of joint in model group, acetaminophen group and bushen yiqi huoxue formula grcup (formula group) at the 4th week were 18.50±1.00, 14.25±1.26,11.75±2.22 respectively, and 30.75±1.26, 22.00±3.16, 17.75±2. 21 at the 12th week respectively. The total integrals of pathological changes in cartilage of joint, in chondrocyte and in cartilage surface layer are in the sequence of model group ＞ acetaminophen group ＞ formula group successively. The significant or very significant differences present in the comparisons between groups.CONCLUSION: Bushen yiqi huoxue formula delays pathological progression of osteoarthritis of knee so that it can treat and prevent the disease.

Objective To explore the feasibility,efficacy and safety of endoscopic submucosal dissection (ESD) for tumors originating from gastric muscularis propria.Methods A total of 20 patients with tumors originating from gastric muscularis propria confirmed by EUS and CT scan underwent ESD.Laparoscopic intervention was applied in 3 cases when difficulties in en bloc removal or perforation occurred.Results The mean ESD procedure time was 74.8 min (40-120 min),and the mean resected tumor size was 2.6 cm (1.5-3.5 cm).No severe complication occurred.Pathological findings were 17 cases of gastrointestinal stromal tumors and 3 cases of leiomyoma.Conclusion ESD is an effective method for treating the tumors originating from gastric muscularis propria,and laparoscopic intervention is necessary for en bloc resection in some cases.

Aim To screen and identify anti-CCR7 single chain fragments variable(scFv)from lager phage library and to detect the scFv efficiency.Methods The insert ratio of ScFv antibodies library was identified by PCR.The products digested by Sfi I/Not I double enzyme.Panning against breast cancer cell and CCR7 were performed four and three rounds respectively.Positive clones were transformed to E.coli HB2151, and their dissolvability was assayed.The soluble scFv was purified by affinity chromatography, and its relative molecular mass was determined by Western blot.The ELISA assay was used to identify the immunocompetence of the antibody.Immunocytochemical staining and radioimmunoimaging were employed to determine the affinities of scFv binding with CCR7 in cell line and in nude mice.Results The insert ratio of ScFv gene was 90%(18/20), enzyme digest reaction showed the aim products on 1% agarose gel.ScFvs were obviously enriched after 7 round panning.Western blot result showed soluble scFv's molecular mass was about 34 KD.ELISA analysis showed dissolved antibody had high immunocompetence to MDA-MB-435 s cells.Immunocytochemical staining and radioimmunoimaging indicated that ScFvs were bound efficiently to MDA-MB-435 s cells which expressed CCR7.Conclusions ScFvs against CCR7 are successfully acquired by screening the phage antibody library.The soluble ScFvs have high affinity and specifical binding to human breast cancer cells.

Objective To evaluate multi-slice spiral computed tomography(MSCT)in the diagnosis and preoperative staging of gastrointestinal cancer.Methods (1)Three hundred and thirty-two gastric cancer patients underwent preoperative MSCT examination.The staging of gastric cancer by MSCT was compared with histological examination.(2)Seventy-three small intestinal cancer patients underwent preoperative MSCT,and 70 healthy adults were selected as normal contr01.The MSCT images of small intestinal lymphoma and small intestinal cancer were compared with those of normal small intestine.(3)Two hundred and twenty-eight colorectal cancer patients underwent preoperative MSCT,and the features of the MSCT images of colorectal cancer were analyzed by chi-square test.Results (1)The overall accuracy of MSCT in the assessment of gastric wall invasion,lymph node metastasis and distal metastasis were 78.2%(244/312),67.3%(210/312)and 93.8%(30/32),respectively.More information such as the location of lesions and the vessels around the stomach couldbe provided by MSCT image.(2)Abdominal masses,diffuse thickening of intestinal wall and intestinal obstruction were the main features of small intestinal cancer,while mesenterie and retroperitoneal lymph nodes metastases were rarely detected.The presentation of small intestinal lymphoma detected by MSCT were completely the opposite.(3)Signs such as"coarse surface"and"heterogeneous enhancement"were important to the diagnosis of colorectal cancer.The measurement of the MSCT value is helpful in the differential diagnosis of colorectal cancer and colonic inflammation.Conclusions MSCT can provide correct imaging information on the diagnosis and preoperative staging of gastrointestinal cancer.The combination of MSCT and endoscopy can improve the accuracy of diagnosis.

Objective To investigate the efficacy and toxicity of neoadjuvant regional arterial chemotherapy in the treatment of advanced gastric cancer. Methods The clinical data of 158 patients with advanced gastric cancer and with the same clinical stages who were admitted to Renji Hospital of Shanghai Jiaotong University from February 2002 to May 2005 were retrospectively analysed. Preoperative regional arterial chemotherapy was applied to 76 patients (test group) and the remaining 82 patients only received surgical treatment (control group). The chemotherapy regimen was epirubicin (50 mg/m2) + cisplatin (60 mg/m2) + 5-fluorouracil (1000 mg/m2).This regimen was modified to oxaliplatin (130 mg/m2) + 5-fluorouracil (1000 mg/m2) since 2003, and surgery was performed 6-11 days after the chemotherapy. All patients received postoperative intravenous chemotherapy.The clinical effects, radical resection rate, operative complications and long-term survival of the two treatment methods were evaluated. All data were analysed using the chi-square test and Kaplan-Meier analysis. Results The radical resection rate was significantly higher at 86% (65/76) in the test group compared with 71% (58/82)in the control group ( x2 = 5.01, P ＜ 0. 05 ). The toxicity of the chemotherapy in the test group was mild. The postoperative complication rate was 20% (15/76) in the test group and 16% (13/82) in the control group, with no significant difference between the two groups (x2 = 0.41, P＞0.05). The median survival time was 41 months in the test group and 23 months in the control group. The 5-year overall survival rate was higher in the test group (44.6%) than that in the control group (29.1%) (x2 =3.95, P＜0. 05). Conclusions Neoadjuvant regional arterial chemotherapy is well tolerated by patients with advanced gastric cancer. It is also effective for increasing the radical resection rate and improving the long-term survival.

Objective To observe the changes of serum potassium concentration in rectal cancer patients from the admission to before the operation,to provide clinical evidence for preventing serum potassium disorder during and after their operation.Methods Monitoring of the changes of serum potassium concentration of 40 cases of rectal cancer patients according to three time points including the first day of admission,before taking of cathartics and the morning before operation.They were divided into different groups according to their age(young,middle age and elderly group),gender(male and female group),diet(normal,decreased slightly and serious decline group) and course(long,middle and short course group) of disease.The serum potassium concentration was observed between each time point of each group and SPSS13.0 statistical software was used to analyze the changes above.Results The mean of all patients' serum potassium concentration was (4.09 ± 0.62) mmol/L on the first day of admission,was (3.83 ± 0.46) mmoL/L before taking of cathartics that was decreased compared to the former and there was significant difference (P ＜ 0.01),it was (3.36 ±0.40) mmol/L on the morning of operation and had significant difference with the above two groups (P ＜ 0.01).It hadn't significant difference between different gender and age groups at different time points (P ＞ 0.05) except between the young group and the elderly group in the morning of the operation (P ＜ 0.05).There were statistically significant differences between the different diet condition and different course at anytime point (P ＜ 0.01).Conclusions The rectal cancer patients were possiblely in hypokalemia before operation,the causes might be relative to the long course of disease,the decreased diet,the aged and infirm,the diet structure change after admission and taking cathartics cleaning intestinal tract.

Objective In order to provide clinical evidence of reducing postoperative complications of procedure for prolapse and hemorrhoids and observe the effects of constipation on postoperative complications of procedure for prolapse and hemorrhoids.Methods One hundred and sixty cases of hemorrhoid were divided into two groups according to whether suffering from constipation:there were eihgty cases who were suffered from constipation in the experimental group and eighty cases who were not suffered from constipation in the control group.All patients' postoperative bleeding,anal edema,stool impaction and satisfaction were observed within 30 days and these data were compared and analysed by using SPSS 13.0 software.Results In the experimental group,the postoperative bleeding incidence was 37.5 ％ (30/80),the anal edema incidence was 31.3 ％ (25/80),the stool impaction inci dence was 8.6％ (7/80),and the patients' satisfaction rate was 88.6％ (71/80).In the control group,the postoperative bleeding incidence was 18.6％ (15/80),the anal edema's incidence was 15.0％ (12/80),the stool impaction incidence was 1.3％ (1/80),and the patients' satisfaction rate was 97.5％ (78/80).All the indexes above were different statistically in the two groups (P ＜ 0.05).Conclusion constipation could increase complications and reduce the degree of satisfaction after procedure for prolapse and hemorrhoids.

Batch and continuous fermentation were adopted to investigate the effect of specific growth rate and amino acid components on RNA accumulation in Candida tropicalis ATCC 20408 in fermentation medium ( FM), yeast peptone dextrose medium (YPD), molasses fermentation medium ( MFM) and FM without corn steep liquor. The data showed that obvious differences in intracellular RNA accumulation were observed at different cell growth phases in bath fermentation prosess, and RNA level reached 11. 8% (g-RNA /g-DCW) during exponential phase, and only 6.9% during stationary phases. It was also found that intracellular RNA accumulation increased with the increase of specific growth rate in continue fermentation prosess, and the highest RNA level reached 15. 6% with the glucose conversion rate of 42. 8% at the dilution rate of 0. 5 h-1. Furthermore, the data showed that RNA lever was notably increased in batch fermentation process when amino acids or peptone was added into the fermentation medium containing no corn steep liquor. Taken together, it was reported for the first time that specific growth rate and amino acid components plays a leading role on the intracellular RNA accumulation in C. tropica lis, and specific growth rate is more important.
