5.Clinical Features and Treatment of Eosinophilic Lymphoid Granuloma in Children

Objective To explore the clinical features and treatment of eosinophilic lymphoid granuloma(ELG) in children,in order to increase understanding of the disease.Methods Clinical features,laboratory examination,pathological examination,treatment and the-(rapeutic) effect of 5 patients with ELG were retrospectively studied,and the related literature were reviewed.Results Five cases were all boys.They all presented many subcutaneous swellings and lymphadenopathy.A case among them was associated with nephrotic syndrome,and 2 cases had the complication of eczema repeatedly,and 1 case ever had 2 times episodes of asthmatiform bronchitis.All of them had no hepatomegaly and splenomegaly,physically was well developed.Only chemotherapy was supplied to all the patients.During the course of therapy,4 cases among them recurred.After that,they were successfully treated with irradiation of lesion or cyclosporine(CsA).Conclusions ELG in children is as same as adult in clinicopathological features.It tends recur only with chemotherapy.We can get better curative effect with the synthetic treatment of chemotherapy,radiotherapy,operation or oral CsA and so on.
6.Distribution and Drug Resistance of Pathogenic Bacteria of Urinary Tract Infection in Children

Objective To explore the pathogenic bacteria′s distribution and their drug resistance of urinary tract infection in children.Methods A total of 555 pathogen strains in urinary tract infection for children from inpatients and outpatients from Jan.2005 to Dec 2006 were identified and the drug resistance test was preformed.Results In the 555 strains of bacteria,80.7% were Gram-negative,17.7%were Gram-positive and 1.6%were fungi.Most of Gram-negative bacteria were E.coli,among 300 strains of identified Escherichia,69.3% of them(208 strains) produced extended speetrum ? lactamases(ESBLs);among 41 strains of identified Klebsiella pneumoniae,78.1% of them(32 strains) produced ESBLs.Most of Gram-positive bacteria were Enterococcus,there were 70 strains(12.6%),sensitivity rates of nitrofurantion and vancomycin were 100%.Conclusion Gram-negative is the main infection bacterium in urinary tract infection for children and the most of them are very serious drug resistant.The clinicians should pay more attentions to idstream urin bacteriology culture and choose the suitable antibiotic according to the results of antibiotic susceptibility tests.
7.Clinical study of femoral peel-off lesions in acute posterolateral corner injuries

Objective To determine the incidence of clinically common acute avulsions of the popliteus tendon (PT) and the lateral collateral ligament (LCL) of the femur (peel-off lesion) in knees with multiple injured ligaments.Methods A retrospective review of 48 acute grade 3 posterolateral corner (PLC) knee surgeries was conducted.All office evaluations were reviewed to determine the demographic data,tear description,preoperative MRI examination,physical examination under anesthesia,intraoperative arthroscopic findings and surgical techniques.Results In total,19 patients were surgically verified as having femoral peel-off lesions as a portion of a series of multiple pathologic findings.Tear patterns were typically categorized into 3 patterns:isolated PT tear (pattern 1,4 patients),combined PT and LCL tears (pattern 2,8 patients) and complex tears (associated intrasubstance and/or fibular-based injuries,pattern 3,7 patients).Seventeen of the 19 patients underwent preoperative MRI examination,which demonstrated conclusive signs of femoral insertion separation and discontinuity in 13 patients (76%) and inconclusive signs of femoral insertion separation.Examination under anesthesia (EUA) revealed that for the 4 patients with pattern 1 injuries,the tibial external rotation increases were all greater than 10°.For the 15 patients with pattern 2 and 3 injuries,the yarus instability was as follows:grade 3 in 8 patients,grade 2 in 2,grade 1 in 1,and negative in 4.External rotation increases were greater than 10° in 12 patients and less than 10° in 3.Seventeen of the 19 patients were inspected with an arthroscope prior to PLC surgery:of these patients,94% had acute avulsion findings and positive lateral gutter drive-through sign;8 patients were repaired with recess or reattachment procedures;6 patients were primarily reconstructed;4 patients were treated with combined repair and reconstruction;and 1 patient was left untreated for secondary reconstruction due to priority repair for concurrent patellar tendon rupture.Conclusion For adequate clinical identification,we recommend a comprehensive diagnostic protocol,including MRI,physical examination and arthroscopic inspection,to avoid underdiagnosis of the lesion in multiple-ligament injured knees.The lateral gutter arthroscopic evaluation is particularly sensitive for detecting the lesion.
8.Primed in situ labeling technique for subtelomeric rearrangements in 70 children with idiopathic mental retardation.

Subtelomeric rearrangements contribute to idiopathic mental retardation (MR), but most children with idiopathic MR do not show any chromosome abnormalities with standard cytogenetic analysis. The primed in situ labeling (PRINS) technique, using an oligonucleotide primer complementary to the telemetric repeat sequences (TTAGGG), can identify chromosome telomeric abnormality (deletion) in idiopathic MR children. In this study, seventy children with idiopathic MR were enrolled and subjected to PRINS. The results showed normal karyotype in all the children, subtelomeric rearrangements (1q del and 4q del) in 2 cases, which was confirmed by fluorescence in situ hybridization (FISH). It was concluded that PRINS is effective for the detection of subtelomeric rearrangements and may become a routine technique for cytogenetical abnormality screening.
9.Early-pathologic Changes of Gastric and Duodenal Mucosa in Children Infected by Different Types of Helicobacter Pylori

Objective To investigate the early-pathologic changes in children′s antrum infected with different types of Hp and study the Hp isolate′s pathogenic.Methods The serum types of CagA and VacA from Hp were determined by Westen-Blot in 70 patients with Hp positive and 36 patients with Hp negative.The standard of gastritis pathologic classification was accordance with that of international made in Sydney. The pathogenic of Hp affecting was evaluated by the degree of inflammation, severity of active gastritis,lymph follicles and atrophy.Results The detection rate of type Ⅰwith high virulence in Hp isolates was 68.1%,mid-type isolates was 27.7% and type Ⅱ with low virulence isolates was 4.2%.To observe the pathologic distinction in 49 patients with type Ⅰisolate,20 patients with mid-type isolate and 3 patients with type Ⅱ isolate,the type Ⅰ and mid-type isolates had significant difference in inflammation and their activity in either antrum or duodenal ampulla.Three patients with type Ⅱ isolate have not active gastritis.Type Ⅰand mid-type isolates had significant difference in lymph follicles,and the lymph follicles caused by type Ⅰwere significant higher than those caused by mid-type.But there were no significant differences in intestinal metaplasia and atrophy.Conclusions TypeⅠisolate with high virulence is the main detection isolate of children infected by Hp in our district.There is inflammation occurrence in antrum specimens in childhood who infected with Hp.
