1.Analysis of medical assistance to low-income inpatients

Objective To investigate the medical assistance to inpatients from low-income families for solving their difficulties in seeing doctors and affordability.Methods Random sampling was made to five ongoing medical assistance projects in the hospital for statistics and analysis of the data so collected.Results Medical expenses of such inpatients are beyond their affordability.Despite the coverage of their basic medical insurance,the reimbursement rate is too low to alleviate their financial burden.Conclusion A“4-party payment”model is recommended,combining the government support for basic medical insurance,social welfare assistance,philanthropic assistance and that paid by the inpatients.This model is expected to effectively alleviate the financial burden of such inpatients.
2.Negative modulation of NO for diaphragmatic contractile reduction induced by sepsis and restraint position.

In practice of forensic medicine, potential disease can be associated with fatal asphyxia in restraint position. Research has demonstrated that nitric oxide (NO) and nitric oxide synthase (NOS) are plentifully distributed in skeletal muscle, contributing to the regulation of contractile and relaxation. In the current study, respiratory functions, indices of diaphragmatic biomechanical functions ex vivo, as well as NO levels in serum, the expressions of diaphragmatic inducible NOS (iNOS) mRNA, and the effects of L-NNA on contractility of the diaphragm were observed in sepsis induced by cecal ligation and puncture (CLP) under the condition of restraint position. The results showed that in the CLP12-18h rats, respiratory dysfunctions; indices of diaphragmatic biomechanical functions (Pt, +dT/dt(max), -dT/dt(max), CT, Po, force over the full range of the force-frequency relationship and fatigue resistance) declined progressively; the NO level in serum, and iNOS mRNA expression in the diaphragm increased progressively; force increased significantly at all stimulation frequencies after L-NNA pre-incubation. Restraint position 1 h in CLP12 h rats resulted in severe respiratory dysfunctions after relative stable respiratory functions, almost all the indices of diaphragmatic biomechanical functions declined further, whereas little change took place in NO level in serum and diaphragmatic iNOS mRNA expression; and the effects of L-NNA were lack of statistical significance compared with those of CLP12 h, but differed from CLP18 h group. These results suggest that restraint position and sepsis act together in a synergistic manner to aggravate the great reduction of diaphragmatic contractility via, at least in part, the negative modulation of NO, which may contribute to the pathogenesis of positional asphyxia.

3.The epidemiological characteristics and correlated factors of daily hassles for thermal power plant workers.

OBJECTIVEThis study aimed to investigate the epidemiological characteristics and correlated factors of daily hassles among thermal power plant workers.
METHODSA mass screening of daily hassles and correlated factors was conducted on 498 workers from a thermal power plant in Zhengzhou in July, 2008. The questionnaires included Daily Hassles Questionnaires, Work Roles Questionnaires, Job Content Questionnaires (Chinese version), Effort-Reward Imbalance (Chinese version), Work Locus of Control Scale and Type A Behavior Scale, with content covering demographic characters and occupational stress correlated factors among subjects. The daily hassles was divided into lower level and higher level according to scores, and the epidemiological characteristics and correlated factors of daily hassles were analyzed.
RESULTSA total of 446 qualified questionnaires were obtained, effective response rate was 89.6% (446/498). For respondents, the age was (36.96 ± 6.49) years old, working length of the current job was (12.05 ± 7.54) years, the daily hassles scores was (9.01 ± 2.50), and the prevalence rate of the higher level of daily hassles was 34.1% (152/446). The multiple non-conditional logistic regression analysis showed 5-14 years' working length of current job (OR = 0.451, 95%CI: 0.225 - 0.904), average income > 3000 yuan(OR = 0.372, 95%CI: 0.202 - 0.684), reward (OR = 0.557, 95%CI: 0.325 - 0.954) and coping strategy (OR = 0.552, 95%CI: 0.330 - 0.925) were negatively correlated with daily hassles, and shift-work (OR = 1.887, 95%CI: 1.108 - 3.215), effort (OR = 2.053, 95%CI: 1.198 - 3.519), psychological demand (OR = 1.797, 95%CI: 1.049 - 3.078), negative affectivity (OR = 3.421, 95%CI: 2.065 - 5.668) were positively correlated with daily hassles.
CONCLUSIONThe prevalence rate of the higher level of daily hassles was considerable high for thermal power plant workers. Its negative correlated factors included 5 - 14 years' working length of the current job, average income > 3000 yuan, reward and coping strategy and its positive corelated factors included shift-work, effort, psychological demand and negative affectivity.

5.Relationship between job satisfaction and occupational stress in the workers of a thermal power plant.

OBJECTIVETo explore the relationship between job satisfaction and occupational stress in the workers of a thermal power plant.
METHODSThe cluster sampling method was used to investigate 875 workers in a thermal power plant. The job satisfaction, occupational stressors, strains, personalities, meeting strategy and social support were measured using occupational stress instruments, job content questionnaire and effort-reward imbalance questionnaire.
RESULTSThere were no significant differences of job satisfaction scores between different groups according to sex, educational level, marriage status, smoking and drinking (P > 0.05). But there were significant differences of job satisfaction scores between different age groups or between different service length groups (P < 0.01). The correlation analysis revealed that job satisfaction scores were related positively to responsibility for persons and things, promotion opportunity, job control, job stabilization, rewards, mental health, positive affectivity, self-esteem, superior support and coworker support scores (P < 0.05 or P < 0.01), negatively to role ambiguity and conflict, job future ambiguity, job demands, negative affectivity, depressive symptoms, patience, and mental locus of work control(P < 0.05 or P < 0.01). The results of variance analysis indicated that the job relationship, responsibility for persons and things, promotion opportunity, job control, job stabilization, reward, mental health, positive affectivity, self-esteem and buffer scores of the workers with high job satisfaction scores were significantly higher than those of workers with moderate and lower job satisfaction scores (P < 0.01), but the role ambiguity and conflict, job future ambiguity, job demands, and depressive symptoms scores and mental locus of work control of the workers with high job satisfaction scores were significantly lower than those of workers with moderate and lower job satisfaction scores (P < 0.05 or P < 0.01). The results of multivariate logistic regression analysis showed that the risk of job dissatisfaction for workers with low reward was about four times as high as that for workers with high reward (OR = 3.773), the risks of job dissatisfaction for workers with low social support and mental locus of external work control were about two times as high as that for workers with high social support or mental locus of internal work control (OR = 2.419 and 2.219, respectively). The daily life stress, low control strategy, low support strategy, low job control and negative affectivity were risk factors of job dissatisfaction (OR = 1.125 approximately 1.790), but the self-esteem and positive affectivity could reduce the risk of job dissatisfaction.
CONCLUSIONIncreasing the decision level, social support, meeting strategy and reward or decreasing the role conflict and ambiguity, demands and negative affectivity could improve worker's job satisfaction level.

6.The clinical significance of CT angiography in the diagnosis of arterioportal shunts associated with hepatocellular carcinoma

Objective To evaluate the clinical significance of CT angiography(CTA)in the diagnosis of arterioportal shunts(APS)associated with hepatocellular carcinoma(HCC).Methods One hundred and twenty-seven consecutive HCC patients accepted both dynamic enhancement CT and DSA examinations.The interval between CT and DSA exam was from 3 to 15 days.Based on transverse CT images in hepatic artery phase,CTA was performed for all the patients.By contrast with DSA results,the capabilities of transverse CT and transverse images combined with CTA in APS diagnosis were analyzed. Results In all 127 HCC cases,52 cases with APS were confirmed by DSA(40.94%),33 with central type of APS and 19 with peripheral type.Diagnostic sensitivity of APS based on transverse CT and combined CTA with transverse CT images were both 94.23%(49/52).However,specificity was 84.00%(63/75) and 97.33%(73/75),respectively,accuracy was 88.19%(112/127)and 96.06%(122/127),the predictive value of positive cases was 80.33%(49/61)and 96.08%(49/51),and the predictive value of negative cases was 95.45%(63/66)and 96.05%(73/76).Combined with CTA,false positive cases of 4 central type of APS and 6 peripheral type of APS were excluded which were demonstrated by transverse CT images.By contrast with DSA,the coincidence rate of the type of APS diagnosed by transverse images combined with CTA was 88.46%(46/52),including 90.91%(30/33)of central type of APS and 84.21%(16/19)of peripheral type.The supplying arteries of central type of APS were intuitively displayed by CTA in 23 cases,19 from proper hepatic artery and 4 from gastro-duodenal artery.Conclusion CTA techniques based on the dynamic enhancement CT exams could effectively promote the specificity and the accuracy of APS diagnosis.
7.Influence of affectivity trait on relationships between occupational stress and subjective physical health and job satisfaction.

OBJECTIVETo explore the influence of affectivity trait on relationships between occupational stress and subjective physical health and job satisfaction.
METHODSSelf-reports for psychosocial work conditions and health and well-being in a sample of 878 workers from a thermal power plant in China were conducted using the job demand-control model, the effort-reward imbalance model, job satisfaction, depression symptom, and physical health complaints questionnaires. Logistic regression analysis was used by controlling for age, sex, and educational level.
RESULTSNegative and positive affectivity were correlated with occupational stressors or strains (P<0.01). In the most equation of job dissatisfaction, psychosomatic complaints and depressive symptoms as dependent variables respectively, odds ratios decreased greatly when negative affectivity or positive affectivity was controlled; Odds ratios decreased greater when negative affectivity and positive affectivity were controlled simultaneously. Furthermore in some equation no variable entered.
CONCLUSIONAffectivity trait has effect on relationships between occupational stress and subjective physical health and job satisfaction. According to the outcome category of study, the possible confounding role of affectivity should be controlled in the study of the relationships between occupational stress and health self-reports.

8.A novel full-length gene of human ribosomal protein L14.22 related to human glioma.

BACKGROUNDThis study was undertaken to obtain differentially expressed genes related to human glioma by cDNA microarray and the characterization of a novel full-length gene.
METHODSTotal RNA was extracted form human glioma and normal brain tissue, and mRNA was used as a probe. The results of hybridization procedure were scanned with the computer system. The gene named 507E08 cone was subsequently analyzed by northern blot, bioinformatic approach, and protein expression.
RESULTSFifteen differentially expressed genes were obtained from human glioma by hybridization and scanning for four times. Northern blot analysis confirmed that the 507E08 clone was low expressed in human brain tissue and over expressed in human glioma tissues. The analysis of BLASTn and BLASTx showed that the 507E08 clone was a novel full-length gene, which codes 203 amino acid of protein and is called human ribosomal protein 14.22 gene. The nucleotide sequence had been submitted to the GenBank with the accession number of AF329277. After expression in E. coli., protein yielded a major band of apparent molecular mass 22 kDa on an SDS-PAGE gel.
CONCLUSIONScDNA microarray technology can be successfully used to identify differentially expressed genes. The novel full-length gene of human ribosomal protein 13.22 may be correlated with the development of human glioma.

9.DNA genotyping of oral epithelial cells by laser capture microdissection.

OBJECTIVE:
The STR genotypping of trace oral epithelial cells which are microdissected by laser capture microdissection system (LCM) is explored.
METHODS:
The oral epithelial cells are microdissected using a low-power infrared laser by VERITAS Microdissection Instrument. STR loci of Profiler Plus are detected by multiplex PCR procesures.
RESULTS:
DNA genotyping of 7-8 oral epithelial cells are succeeded, and DNA genotyping of 3-4 oral epithelial cells are failed.
CONCLUSION
It is viable in genotyping of trace oral epithelial cells by Laser Capture Microdissection as a new technology of seperating single cell.

10.Co-effect of Demand-control-support model and effort-reward imbalance model on depression risk estimation in humans: findings from Henan Province of China.

OBJECTIVETo investigate the co-effect of Demand-control-support (DCS) model and Effort-reward Imbalance (ERI) model on the risk estimation of depression in humans in comparison with the effects when they are used respectively.
METHODSA total of 3 632 males and 1 706 females from 13 factories and companies in Henan province were recruited in this cross-sectional study. Perceived job stress was evaluated with the Job Content Questionnaire and Effort-Reward Imbalance Questionnaire (Chinese version). Depressive symptoms were assessed by using the Center for Epidemiological Studies Depression Scale (CES-D).
RESULTSDC (demands/job control ratio) and ERI were shown to be independently associated with depressive symptoms. The outcome of low social support and overcommitment were similar. High DC and low social support (SS), high ERI and high overcommitment, and high DC and high ERI posed greater risks of depressive symptoms than each of them did alone. ERI model and SS model seem to be effective in estimating the risk of depressive symptoms if they are used respectively.
CONCLUSIONThe DC had better performance when it was used in combination with low SS. The effect on physical demands was better than on psychological demands. The combination of DCS and ERI models could improve the risk estimate of depressive symptoms in humans.
