BACKGROUND:Silver sulfadiazine water gel dressings are a combination of two antibacterial drugs, which can play a local broad-spectrum, potent, long-lasting antibacterial effect to effectively control and prevent wound infection. OBJECTIVE:To observe the clinical efficacy of silver sulfadiazine water gel dressings on skin defects with wound infection and to explore its biocompatibility. METHODS:Thirty patients with skin defects combined with wound infection were randomized into observation and control groups. Folowing conventional debridement and dressing, the observation group was subject to wound rinse with normal saline, silver sulfadiazine water gel dressings and gauze bandage; in the control group, Vaseline gauze was used folowed by gauze bandage. Visual analog scale scores, dressing adhesion to wound, wound healing rate, and healing time were observed in the two groups at 0, 1, 2 weeks after dressing. RESULTS AND CONCLUSION:The visual analog scale scores in the observation group were significantly lower than those in the control group at 1 and 2 weeks after dressing (P < 0.01), and the wound healing rate was also higher in the observation group than the control group (P < 0.05); wound adhesions were milder in the observation group than the control group when dressing; the healing time was significantly shorter in the observation group than the control group (P < 0.05). These findings suggest that the silver sulfadiazine water gel dressing has good effects on the treatment of skin defects with wound infection, which can reduce pain, effectively control wound infections and promote wound healing.

The metabolic properties of leukemia-initiating cells (LICs) in distinct bone marrow niches and their relationships to cellfate determinations remain largely unknown. Using a metabolic imaging system with a highly responsive genetically encoded metabolic sensor, SoNar, we reveal that SoNar-high cells are more glycolytic, enriched for higher LIC frequency, and develop leukemia much faster than SoNar-low counterparts in an MLL-AF9-induced murine acute myeloid leukemia model. SoNar-high cells mainly home to and locate in the hypoxic endosteal niche and maintain their activities through efficient symmetric division. SoNar can indicate the dynamics of metabolic changes of LICs in the endosteal niche. SoNar-high human leukemia cells or primary samples have enhanced clonogenic capacities in vitro or leukemogenesis in vivo. PDK2 fine-tunes glycolysis, homing, and symmetric division of LICs. These findings provide a unique angle for the study of metabolisms in stem cells, and may lead to development of novel strategies for cancer treatment.

Objective:To evaluate the role of miR-124 in breast cancer and its underlying mechanism. Methods:Quantitative re-verse transcription-polymerase chain reaction (qRT-PCR) was employed to quantify the expression level of miR-124 in the breast can-cer cell lines and matched tissues of 52 patients. Cell proliferation, invasion, and migration of MDA-MB-231 and T-47D were deter-mined by miR-124 overexpression in vitro. Luciferase vectors (pMIR-SP1 3'UTR) were also constructed. The predicted target gene of miR-124 was identified via luciferase activation assay. The mRNA and protein expression of SP1 was detected via qRT-PCR and West-ern blot, respectively. Results:MiR-124 was decreased in breast cancer tissues and cell lines. This result is correlated with metastatic capacity, TMN stages, and prognosis in breast cancer tissues. In breast cancer cell lines, ectopic overexpression of miR-124 inhibited cell proliferation, invasion, and migration in vitro. MiR-124 mimics significantly inhibited luciferase activation (P<0.05) in HEK293 cells and could significantly decrease the mRNA (P<0.05) and protein expression levels of SP1 in MDA-MB-231 and T-47D cells. Con-clusion:MiR-124 could be inhibited in breast cancer. The low miR-124 expression is associated with poor prognosis. In addition, miR-124 could inhibit cell proliferation, invasion, and migration by targeting SP1. These findings confirm that miR-124 downregulation may be a key mechanism for breast cancer carcinogenesis.

Objective To analyze the coincidence of the patients with capillary electrophoresis hemoglobin A 2 increase with defi-nitely diagnosed beta thalassaemia and to investigate its application value in the diagnosis of beta thalassaemia.Methods Two hundreds and sixty outpatients and inpatients with hemoglobin A 2 increase in our hospital from May 2014 to May 2015 were per-formed the genetic testing.Results Among 260 patients with hemoglobin A2 increase ,beta thalassemia gene mutations were detec-ted in 257 cases ,the coincidence rate reached 98.85% ,and the common 17 beta thalassemia gene mutations were not detected in the other 3 cases ,follow-up further detection of rare beta thalassaemia gene and beta globin gene sequencing was performed ,1 case of SEA-HPFH βdeletion type was found ,1 case was Taiwaneseβdeletion type and 1 case was Codon 89-93(-AGT GAG CTG CAC TG) heterozygous mutation ,it was verified that 3 cases of hemoglobin A2 increase without detecting 17 kinds of common beta thalassemia gene mutations were still beta chain mutation occurrence or big fragment deletion.At the same time ,among 257 speci-mens of beta thalassemia gene mutations ,42 cases were compound alpha thalassaemia ,accounting for 16.34% of beta thalassaemia. Conclusion Capillary electrophoresis hemoglobin A2 increase can provide a fast and accurate basis for beta thalassemia diagnosis , but which can not rule out the possibility of compoound alpha thalassaemia ,when the patient's hemoglobin A2 is increased ,alpha and beta thalassaemia genetic diagnosis should be simultaneously carried out.

Objective To evaluate anatomic hepatectomy (AH) vs lithotomy of liver parenchyma (LLP) for regional hepatolithiasis.Methods From January 2012 to June 2015,fifty-nine cases underwent LLP and 47 received AH.Clinical end-points included residual stones,infection of the raw surface of liver remnant,time to restoration of liver function,bile leakage,recurrent stones,morbidity and mortality.Results The time of liver function restoration was not statistically different between the two groups (2.96 d vs.2.82 d,P ＞0.05).Patients in AH group suffered from less residual stones (2 cases vs.13 cases,P ＜0.01),lower infection rate of the raw surface of liver remnant (1 case vs.21 cases,P ＜ 0.01),and less bile leakage rate (0 case vs.7 cases,P ＜ 0.05).With a median follow-up of 31 months (range 3-48 mon).AH group suffered from less recurrent hepatolithiasis (0 case vs.12 cases,P ＜ 0.01).There was no mortality in both groups.Conclusions Anatomic hepatectomy is a safe and effective treatment for hepatolithiasis,with a high stone clearance rate and low surgical complications.

Objective To analyze the value of color Doppler ultrasound in the diagnosis of lower extremity arterial disease in patients with type 2 diabetes mellitus.Methods A total of 800 patients with type 2 diabetes mellitus underwent color Doppler ultrasonography to examine anterior tibial artery (ATA),dorsalis pedis artery (DPA) and posterior tibial artery (PTA).Ultrasonic findings including vascular diameter,stenosis ratio and hemodynamics of lower extremity arterial disease were analyzed retrospectively.Results ATA and DPA had more plaques and stenosis than PTA.There was no statistical difference of vascular diameter,stenosis ratio and hemodynamics between left and right lower extremity artery in patients with type 2 diabetes mellitus including diabetic foot.Conclusion Color Doppler ultrasound is a useful method in the diagnosis of lower extremity arterial disease in patients with type 2 diabetes mellitus,providing information of stenosis ratio and hemodynamics of lower extremity artery,so as contributing to the clinical therapy of this disease.

Objective To induce adult adult adipose-derived stem cells(ADSCs) in vitro to differentiate into neuronal-like cells,and to analyze the features of their cell morphology and ultrastructure. Methods Adipose stromal cells were obtained and amplified in vitro. Then make use of chemical induction to induce them. Observed ADSC and differentiation of cells in morphology and ultrastructure under inverted microscope and transmission electron microscopy. Immunocytochemistry to detection of Nestin, Neuron Specific Endolase( NSE) ,and glial fibrillary acidic protein(GFAP) expression in cells. Used Real-time PCR to detection of Nestin,GFAP gene mRNA expression before and after induction in ADSC. To observe the morphology and ultrastructure of the cells prior to and after induction under microscope and electron microscope. Results The morphology of ADSC was similar to fibroblasts ,and could be amplified stability within 10 passages in vitro. Some of the cells induced display a typical astrocyte-like cells in ultrastructure. Followed neuronal induction,astrocyte-like cells began to stain brightly for CFAP, Nestin. GFAP stained in both the cytoplasm and nuclei of astrocyte-like cells, but Nestin only stained in the cytoplasm. The peak positive expression rate within 14d following neuronal induction. The rate of positive expression cells was( 14.4 ± 3. 6) % for Nestin, (87. 3 ± 5. 3 ) % for GFAP. Then two kinds of protein expression remained the similar rate. The average relative concentration of GFAP and Nestin gene mRNA have significant statistical difference between ADSC and differented cells analyzed by Real-time PCR (P<0.05).The peak concentration of GFAP was within 20 d after induction,and GFAP was within 14 d after induction. Conclusion In the cytoplasm of adult adipose-derived cells possess Nestin genetic material,which is the marker of neural stem cell. The differential astrocyte-like cells have the typical morphology, ultrastructure and GFAP phenotype of mature astrocytes.

Objective To compare the diagnostic value of sound velocity(SV) and real-time ultrasonic elastography in differentiating benign or malignant of breast lesions.Methods 75 patients with 99 lesions were examined with the zone speed index technique and real-time ultrasonic elastography respectively.Then the SVs of the lesions were calculated and elastography of the lesions were scored with 5-scoring method.The ROC curves were constructed with histology as the golden standard.Results There was significant difference between the SVs of benign and malignant breast lesions(P =0.0001).1561 m/s was the best cufoff point.And the areas under the curves (AUC) of SV and real-time ultrasonic elastography were 0.842 and 0.968,respectively.There was significant difference between the two methods of AUC (P =0.023).The sensitivity,specificity and accuracy for SV were 81.5％,91.7％,88.9％,respectively.There was no significant difference between sensitivity,specificity and accuracy of the two methods(P =1.0,P =0.125,P =0.146).Conclusions Both SV method and real-time ultrasonic elastography are helpful in the evaluation of benign or malignant of breast lesions.

ObjectiveTo analyze the stress contribution of different cement volume to the adjacent vertebral endplates in percutaneous kyphoplasty (PKP) so as to explore the possible mechanism of adjacent vertebral fractures after PKP.Methods The three-dimensional finite element model of osteoporotic thoracolumbar vertebral compression fractures was established to simulate vertebral body partial restoration (80％) with PKP.During the process,two doses of bone cement ( polymethylmethacrylate,PMMA) were filled in the vertebral body (4.0 ml bone cement filling 15％ of the vertebral body volume and 8.0 ml bone cement filling 30％ of the vertebral body volume).Endplate stress under conditions of axial compression,flexion and extension was analyzed. ResultsRegarding the two filling doses in PKP,the adjacent vertebral endplate stress under the above-mentioned conditions was all increased at T11 and L1 vertebral body compared with that before operation.Meanwhile,endplate stress had positive correlation with the cement volume and the stress concentrated largely in the anterior and middle parts of endplate.ConclusionsThe stress of adjacent vertebral endplate is positively correlated with cement volume,with anterior and middle parts of endplate as the stress concentration.The probability of adjacent vertebral fractures shows a rising trend with the increase of cement volume in PKP.
