1.Controversies in neck dissection of papillary thyroid carcinoma

Parpillary adenocarcinoma is the most common type of thyroid cancer with high probability of cervicel lymph node metastasis. Prophylactic neck dissection is not necessary for patients with clinical negative neck. Modified radical neck dissection is the standard for therapeutic neck dissection. The range of dissection remains controversial. Level Ⅰ is not included routinely, level Ⅱ is recommended. Dissection of level Ⅴ may cause malfunction of the neck and shoulder, clinical negative level Ⅴ for low-risk group can be avoided with close follow-up. Rountine central lymph node dissection tends to reduce the local recurrence. Selective neck dissection for thyroid parpillary adenocarcinoma needs more investigations.
2.The comparison of the treatment effect of continuous intravenous infusion and sequential therapy in pneumonia in children

Objective To investigate the clinical effects and significance of continuous intravenous infusion and sequential therapy in pneumonia in children.Methods 90 patients with pneumonia were randomly divided into the observation group and the control group,with 45 patients in each group,observation group received continuous intravenous infusion,the control group received sequential therapy.The expectoration and rales disappear time,chest X-ray and body temperature return to normal time,clinical efficacy were observed.Results The expectoration and rales disappear time,chest X-ray and body temperature return to normal time were longer than control group,which were significantly different (x2 =6.21,P < 0.05),the total effective rate of observation group was 77.8%,the total effective rate of control group was 97.8%,the differences were statistically significant (t =3.58,4.62,6.18,7.01,all P < 0.05).Conclusion The sequential therapy treatment pneumonia in children has better clinical effect,shorter hospital stay and worthy of clinical application.
3.Basic and Translational Research of Vascular Calcification

Vascular calcification is a common pathology frequently occurred in the process of atherosclerosis ,hypertension , diabetes,chronic kidney disease,vascular injury and aging,resulting in an increased prevalence of cardiovascular events. Along with the social aging process,the incidence becomeshigher,andthis attract more attention. This review summarized a series of our works on vascular calcification in recent years ,including the update concept of vascular calcification ,recent classification and mechanism,and the confusion and hope in experimental and translational researches. This article aiming to promote further studies on the mechanisms and prevention of vascular calcification.
4.Study on alpha-fetoprotein chemiluminescence immunoassay by microfluidic chip

Objective:To establish a microfluidic chip system for immunoassay.Methods:We use direct-writing laser micromachining technology to fabricate microfluidic chip,establish microfluidic chemiluminescence immunoassay system based on superparamagnetic microbeads,and use this system to detect alpha-fetoprotein. Results:We can complete the analysis within 20 minutes with 5?l sample or reagent,the linear range was 1-800 ng/ml with average intra-assay and inter-assay precision at 6.3% and 7.8%,respectively and average recovery at 94.3%. Conclusion:The microfluidic chip system is a time-saving,cost-low and convenient method for AFP analysis.
5.Analysis of the Characteristics between Clinic and Laboratory Examinations in Children with Infectious Mononucleosis

Objective To explore the relationship between the clinical characteristics and laboratory examinations.Methods Retrospective analysis was performed on the clinical data in 172 patients diagnosed as infectious mononucleosis(IM)in our department recently.Results The onset rate was high from 3 to7 years old.The main clinical presentations included fever,neck lymphadenopathy,pharyngitis,hepatomegaly,and splenomegaly.Some patients accompanied with obvious sonorous.The occurring rate of the complications was 74.4%,especially deteriorated liver function was the most common,next came to the lung infection.The proportion of atypical lymphocytes raised in 73.3% cases.The positive rate of EBV-VCA-IGM was 65.1%.Myocardium ferment chart went up in 27.9% cases,Erythrocyte sedimentation rate increased fast in 61.6%,CRP gently got higher in 62.8%.However,the ratio of atypical lymphocyte and EBV-VCA-IGM positive or negative had little to do with the pathologic condition and complications in IM.Conclusions The clinical progress of most IM is benign and multiple organs can be involved with atypical clinical manifestations.Examination for atypical lymphocytes is an easy and effective means to screen IM.
6.Improving the training function of National Natural Science Foundation of China by reforming current loaned personnel model

In order to deal with huge work load of processing and reviewing applications,many personnels have been loaned from universities and research institutes to National Natural Science Foundation of China (NSFC) every year.This phenomenon has received much concern.In this paper,the features of NSFC loaned personnel model were analysed,and the necessity for and feasibility of improving the training function of NSFC by reforming current loaned personnel model were discussed.

8.Color Doppler ultrasound in the evaluation of carotid arteriosclerosis in patients with acute cerebral infarction of anterior circulation

A totel of 120 patients with CT or MRI proved as acute cerebral infarction group of anterior circulation and 90 non-infarction patients as the control group were studied. The diameter,inter median thickness( IMT) ,peak systolic velocity ( PSV ) , end diastolic velocity ( EDV ) , early shrinkage acceleration time ( AT ) , resistance index ( RI) ,and pulsatility index( PI) of carotid artery,and whether there was arteriosclerotic plaque or stenosis,and the properties of plaque were examined with color Doppler ultrasound. In acute cerebral infarction group of anterior cir-culation,the IMT was thicker that in the control group(P<0. 01). The incidence of the film thickness,moderate-se-vere stenosis and instable carotid artery plaque in infaiction group of anterior circulation were obviousely higher than those in the control group(P<0. 01). Arterial complete or incomplete occlusion was observed in severe cases. The blood flow parameters showed that the PSV and EDV of common carotid and internal carotid in acute cerebral in-farction group of anterior circulation were lowerer than those in the control group. RI and PI of internal carotid in a-cute cerebral infarction group of anterior circulation were higher than those in the control group(P<0. 05),while AT was lower than those in the control group.

10.Comparison of the efficacy of duloxetine and gabapentin in the treatment of diabetic peripheral neuritis

Objective To compare the efficacy and safety of duloxetine and gabapentin in the treatment of diabetic peripheral neuritis .Methods 135 patients with diabetic peripheral neuritis were divided into the observation group(71 cases) and the control group (64 cases) according to random number table method ,and all patients received hypoglycemic therapy based on diabetes treatment principle .Furthermore,the observation group received oral duloxe-tine,while the control group was given oral gabapentin .The pain scores of all patients were evaluated using visual ana-log scale(VAS) and medical outcomes study (SF-36),and the adverse reactions were recorded during treatment . Results (1) Compared with before treatment ,both VAS scores and SF-36 scores in observation group and control group declined significantly at the end of 1st,2nd,4th week after treatment(all P<0.05),while there were no statis-tically significant differences in VAS scores and SF-36 scores between the two groups at the same time point ( all P>0.05).(2)The incidence rates of dry mouth ,drowsiness,fatigue and dizziness in observation group were significantly lower than those in control group (18.31% vs.34.38%,4.23% vs.26.56%,0.00% vs.23.44%,0.00% vs. 15.63%,χ2 =4.059,13.308,4.059,4.059,all P<0.05),while there were no statistically significant differences in incidence rates of nausea ,diarrhea and flustered between the two groups (all P<0.05).Conclusion The duloxetine and gabapentin in the treatment of diabetic peripheral neuritis have similar efficacy , but the duloxetine has better security.
