2.Clinical research of bilevel non-invasive mechanical ventilation in the treatment of chronic obstructive pulmonary disease with respiratory failure

Objective To explore the application of bilevel non-invasive mechanical ventilation in the treatment of chronic obstructive pulmonary disease(COPD) with respiratory failure.Methods 126 COPD patients with type Ⅱ respiratory failure were selected and divided into control group and research group,63 cases in each group.The control group received regular treatment,and the research group received bilevel non-invasive mechanical ventilation on the basis of regular treatment.,The results of blood gas analysis,respiratory frequency(RR),heart rate(HR) and expiratory flow(PEF) were observed and compared between the two groups.Results The indicators of pH,PaCO2,PaO2 in the research group after treatment were (7.41±0.08),(51.26±12.13)mmHg,(77.28±15.34)mmHg,which in the control group were (7.26±0.07),(67.21±13.05)mmHg,(69.26±14.31)mmHg,compared with before treatment,pH and PaO2 of the two groups were increased,PaCO2 was decreased after treatment,and the three indicators of the research group were changed significantly better than those of the control group,the differences were statistically significant(t=11.2,7.11,3.03,all P<0.01).After treatment,HR and PR of the two groups were decreased,PEF was increased.The three indicators of the research group were also changed significantly better than those of the control group,the differences were statistically significant(t=3.13,8.58,5.45,all P<0.01).Conclusion Bilevel non-invasive mechanical ventilation in the treatment of COPD with respiratory failure has remarkable clinical effects,can significantly improve the patients' blood gas analysis index,HR,RR and PEF,and it is worthy of clinical application.
3.Clinical effect of biapenem and levofloxacin in hospital acquired pneumonia caused by extensive drug resistant bacillus

Objective To investigate the clinical effect of biapenem and levofloxacin in the treatment of pulmonary infection caused by extensive drug resistant bacillus.Methods 182 patients with pulmonary infection caused by extensive drug resistant bacillus from the department of respiratory in our hospital were selected and divided into two groups, 91 cases in the control group were given cefoperazone sodium and sulbactam sodium +levofloxacin treatment, 91 cases in the experiment group received biapenem +levofloxacin treatment, serum levels of white blood cell count(WBC), high sensitivity C-reactive protein(hs-CRP),procalcitonin(PCT),serum amyloid A(SAA), recovery time of clinical symptoms, signs and laboratory examination, the clinical effect, bacteriological effect and incidence of adverse reactions were compared after treatment.Results The effective rate in the control group(74.72%)was lower than the experiment group(86.81%), with significant difference (P<0.05); compared with the control group, the bacterial clearance rate was higher, bacterial unclearance rate, partial clearance rate, replacement rate were higher in the experiment group after treatment, serum levels of WBC,hs-CRP,PCT,SAA were lower after treatment, recovery time of body temperature, pulmonary signs, abnormal shadow of chest X-ray, WBC, hs-CRP were shorter, with significant difference ( P<0.05 ); there was no significant difference in the incidence of adverse reactions between the two groups.Conclusion The clinical effect of biapenem and levofloxacin in the treatment of pulmonary infection caused by extensive drug resistant bacillus was exactly, can effectively remove bacteria, control infection, shorten the treatment time, and the safety was high.
4.Application of SELDI proteomic patterns in serological diagnosis of breast cancer

Objective To explore the application of serum SELDI proteomic patterns to distinguish breast cancer patients from healthy individuals.Methods All serum samples from 101 breast cancer patients and 45 healthy individuals were analyzed by surface-enhanced laser desorption/ionization time-of-fight mass spectrometry (SELDI-TOF-MS).The spectra were generated on weak cation exchange (WCX2) chips,and protein peaks clustering and classification analysis were made using Biomaker Wizard software and Biomarker Pattern software (BPS).Then the pattern was evaluated by blinded test.Results 49 different proteins were found to have statistically differential expression levels between breast cancer and normal control sera (P < 0.05).A diagnostic model consisting of three protein peaks (M/Z 5627,8124 and 2864) could do the best in the diagnosis between breast cancer and healthy individual.When the diagnostic model was tested with the blinded test set,it yielded a positive value of 95 % (139/146),a sensitivity of 97 % (98/101) and a specificity of 91% (41/45).Conclusion These results suggested that serum SELDI protein profiling can distinguish breast cancer patients from normal subjects with relatively high sensitivity and specificity.SELDI-TOF-MS plays a valuable role in the diagnosis of breast cancer and the discovery of new tumor-specific protein biomarkers.
5.Establishment of the sweep pattern visual evoked potential system and its application.

OBJECTIVE:
To establish an acuity inspection system with sweep pattern visual evoked potential (SPVEP) so as to provide the evidence for acuity objective inspection.
METHODS:
Based on the domestic sweep pattern visual evoked apparatus, sections of hardware were reformed and a manipulation program possessing false random control software was compiled. The SPVEP acuity for the 78 eyes (10 normal eyes, 10 ametropia eyes, 48 prevalence eyes, 10 false ametropia eyes) was estimated with our acuity objective inspection system, then compared with the E visual acuity of those eyes by statistical procedure.
RESULTS:
There was a close correlation between the SPVEP acuity and E visual acuity for 78 eyes (r2 = 0.946).
CONCLUSION
SPVEP acuity inspection system can be applied to estimate objective acuity.

6.Comparison between auditory steady-state response and 40 Hz auditory event related potential in the evaluation of hearing thresholds.

OBJECTIVE:
To compare the results between auditory steady-state response (ASSR) and 40 Hz auditory event related potential (AERP), and explore the accuracy of hearing thresholds by using ASSR and AERP and the clinic forensic value.
METHODS:
Thirty seven ears were tested with pure-tone audiometer, 40Hz AERP and ASSR, respectively. All the volunteers in our study were awake during 40 Hz AERP test and ASSR test.
RESULTS:
Thresholds acquired with ASSR and 40Hz AERP test had a close correlativity and showed higher than those acquired with PTA test. There was no significant difference between the accuracy of ASSR and 40Hz AERP in estimating pure-tone thresholds.
CONCLUSION
After determining the correct value, ASSR can be used directly to evaluate hearing loss objectively.

7.High-fat intake alleviates lung injury induced by Paragonimus proliferus infection in rats through up-regulating CYP 4A1 expression in lung tissues

Objective To explore the improvements of high-fat intake on lung injury induced by Paragonimus proliferus infection in rats, and to preliminarily explore the mechanisms underlying the role of cytochrome P450 4A1 (CYP 4A1) in the improve ments. Methods SD rats were randomly assigned into three groups, including the normal control group (n = 10), the infection and normal diet group (n = 12) and the infection and high-fat diet group (n = 12). Rats in the normal control group were fed with normal diet and without any other treatments, and animals in the infection and normal diet group were subcutaneously injected with 8 excysted metacercariae of P. proliferus via the abdominal wall, followed by feeding with normal diet, while rats in the infection and high-fat diet group were subcutaneously injected with 8 excysted metacercariae of P. proliferus via the abdominal wall, followed by feeding with high-fat diet. All rats were sacrificed 28 weeks post-infection, and serum samples and lung specimens were collected. Following hematoxylin-eosin (HE) staining of rat lung specimens, the rat lung injury was observed under an optical microscope, and alveolitis was evaluated using semi-quantitative scoring. Serum interleukin-1β (IL-1β) and tumor necrosis factor alpha (TNF-α) levels were measured using enzyme-linked immunosorbent assay (ELISA), and the cytochrome P450 4A1 (CYP 4A1) expression was quantified in rat lung specimens at transcriptional and translational levels using quantitative real-time PCR (qPCR) and Western blotting assays. Results Alveolar wall thickening, edema and inflammatory cell infiltration were alleviated 28 weeks post-infection with P. proliferus in rats in the infection and high-fat diet group relative to the infection and normal diet group, and no alveolar consolidation was seen in the infection and high-fat diet group. The semi-quantitative score of alveolitis was significantly higher in the infection and normal diet group [(2.200 ± 0.289) points] than in the normal control group [(0.300 ± 0.083) points] and the infection and high-fat diet group [(1.300 ± 0.475) points] (both P values < 0.05), and higher serum IL-1β [(151.586 ± 20.492)] pg/mL and TNF-α levels [(180.207 ± 23.379) pg/mL] were detected in the infection and normal diet group than in the normal control group [IL-1β: (103.226 ± 3.366) pg/mL; TNF-α: (144.807 ± 1.348) pg/mL] and the infection and high-fat diet group [IL-1β: (110.131 ± 12.946) pg/mL; TNF-α: (131.764 ± 27.831) pg/mL] (all P values < 0.05). In addition, lower CYP 4A1 mRNA (3.00 ± 0.81) and protein expression (0.40 ± 0.02) was quantified in lung specimens in the infection and normal diet group than in the normal control group [(5.03 ± 2.05) and (0.84 ± 0.14)] and the infection and high-fat diet group [(11.19 ± 3.51) and (0.68 ± 0.18)] (all P values < 0.05). Conclusion High-fat intake may alleviate lung injuries caused by P. proliferus infection in rats through up-regulating CYP 4A1 expression in lung tissues at both translational and transcriptional levels.
8.Neutrophil-to-lymphocyte ratio may help predict pediatric testicular torsion in chlidren with acute scrotal pain before surgery

【Objective】 To explore the feasibility and accuracy of neutrophil-to-lymphocyte ratio (NLR) in the prediction of testicular torsion (TT) in children with acute scrotal pain. 【Methods】 A retrospective case-control study was performed on 158 pediatric patients with ultrasound suspicion of TT who underwent surgical testicular examination during Jan.2017 and Jan.2024.The patients were divided into TT group and non-TT group.Clinical data and laboratory data at admission were analyzed.Sensitivity and specificity of NLR to TT were determined with the area under the curve (AUC) represented on the receiver operating characteristic (ROC) curves. 【Results】 There were with no statistically significant differences in clinical data between the two groups (P>0.05).The NLR was significantly higher in the TT group than in the non-TT group [ (4.82±2.37) vs.(2.85±0.75), P<0.05] .The optimal cut-off value of TT predicted by NLR was 2.07, the AUC was 0.809 (95%CI:0.709-0.909), and the sensitivity and specificity were 97.9% and 93.3%, respectively, which were significantly higher than other factors. 【Conclusion】 For suspicious ultrasound diagnosis of pediatric acute scrotal pain cases, NLR can be used to predict the possibility of TT and may help to evaluate the urgent surgical treatment in these patients.
