Objective To investigate the effect of initial fluid resuscitation with different liquid crystal glue ratio on prognosis of patients with severe acute pancreatitis(SAP).Methods Ninety-four cases of SAP patients were selected,with the first 24 h liquid crystal glue ratio of 1.5 and 3.0 for the sector,were divided into low crystal glue ratio group(crystal glue ratio ＜ 1.5,26 cases),microtek glue ratio group (crystal glue ratio 1.5-3.0,30 cases)and high crystal glue ratio group(crystal glue ratio ＞3.0,38cases).24 h mechanical ventilation rate,oxygenation index,intra-abdominal pressure(IAP),third interstitial fluid retention,the parameters of fluid resuscitation and 2 week survival rate were observed in three groups.Results 24 h crystal solution volume,crystal glue ratio,third interstitial fluid retention and 72 h infusion volume in high crystal glue ratio group was higher than that in microtek glue ratio group and low crystal glue ratio group [(3612 ± 799)ml vs.(2309 ±417),(2125 ±478)ml,(3.66 ± 0.14)vs.(2.44 ±0.15),(1.42 ±0.09),(2835 ±632)ml vs.(1889 ±283),(1866 ±305)ml,(11942 ± 1162)ml vs.(9037 ± 983),(9401 ± 1052)ml],and there were significant differences(P ＜ 0.05).24 h crystal glue ratio in microtek glue ratio group was higher than that in low crystal glue ratio group,and there was significant difference(P＜ 0.05).24 h glue volume in low crystal glue ratio group was higher than that in high crystal glue ratio group and microtek glue ratio group [(1507 ± 335)ml vs.(994 ± 234),(949 ± 141)ml],and there were significant differences(P＜0.05).There was no significant difference in 24 h crystal volume,third interstitial fluid retention and 72 h infusion volume between microtek glue ratio group and low crystal glue ratio group(P＞ 0.05).24 h mechanical ventilation rate,IAP in high crystal glue ratio group was higher than that in microtek glue ratio group and low crystal glue ratio group [65.8％(25/38)vs.23.3％(7/30),26.9％(7/26),(16.9 ± 3.7)cm H2O(1 cm H2O =0.098 kPa)vs.(13.5 ± 3.6),(13.2 ± 3.4)cm H2O],24 h oxygenation index was lower than that in microtek glue ratio group and low crystal glue ratio group [(180.8 ±26.4)mm Hg(1 mm Hg =0.133 kPa)vs.(280.7 ±24.9),(260.4 ±25.8)mm Hg],and there was significant difference(P ＜ 0.05).2 week survival rate in high crystal glue ratio group was lower than that in microtek glue ratio group and low crystal glue ratio group [34.2％(13/38)vs.86.7％ (26/30),57.7％(15/26)],there was significant difference between high crystal glue ratio group and microtek glue ratio group (P ＜ 0.05),but there was no significant difference between high crystal glue ratio group and low crystal glue ratio group(P ＞ 0.05).There was no significant difference in 2 week survival rate between low crystal glue ratio group and microtek glue ratio group(P ＞ 0.05).Conclusion In early SAP patients should follow the appropriate crystal glue ratio controlled fluid resuscitation,reduce fluid retention and improve the cure rate,and the use of microtek glue ratio controlled fluid resuscitation strategy is best.

A complete Electrical Impedance Tomography system that can monitor lung function is introduced in this paper. The master PC is responsible for visual control and image reconstruction, while DSP carries out tasks such as electrode selection, data acquisition and data communication with the master. Based on the DDS technique, the exciting signal source has high accuracy, and the data can be acquired by phase-sensibility demodulation. The vivo experiment proves that gravity exerts an effect on the distribution of intrapulmonary ventilation, and shows that the Electrical Impedance Tomography is able to monitor lung circulation and check pathological changes.

Objective To investigate the clinical efficacy and safety of low-dose rituximab (RTX) for patients in primary Sj(..o)gren's syndrome (pSS) with thrombocytopenia.Methods Four pSS patients,2 with refractory thrombocytopenia and 2 with glucocorticoid-dependent thrombocytopenia,were treated with rituximab at 100 mg,intravenous,weekly for a total of two cycles,together with prednisone 1-2 mg · kg-1 ·d-1,and the counts of platelets and B-cells were evaluated.Results Efficacy of treatment was observed in all patients.The counts of platelets,at (3-39) x 109/L baseline,incleased in 1-2 weeks,and went up to ( 107-241 ) × 109/L in 3-8 weeks.Sustained remission had been achieved for 27-52 weeks.The doses of prednisone were tappered to 3.75-7.50 mg/day in 12 weeks.One patient who relapsed at the 27th week (platelet count 47 x 109/L),was retreated with 100 mg of RTX and still had good efficacy.The counts of B-cells reduced to (0.007-0.010) × 109/L,but they did not achieved the depletion.There were no severe adverse events during RTX therapy.Conclusions Our study has shown good efficacy and tolerability of lowdose RTX for pSS with thrombocytopenia.Low-dose RTX allows for reduction in corticosteroid doses and B-cells,while large-scale randomized double-blind controlled trials are needed to confirm the results.

Obiective To realize real-time data processing in lung ventilation reconstruction based on electrical impedance tomography(EIT). Methods The image reconstruction algorithms, plotting functions and matrix calculations are dynamic transferred with Matlab C++, Mathtools and VC++ to meet the requirements of data processing and imaging in real time.Results Image reconstruction and real-time data processing were realized successfully in lung ventilation based on EIT. Conclusion It provides a new strategy for real-time data processing of image reconstruction by EIT.

Objective To investigate the clinical characteristics of bronchopulmonary amyloidosis secondmy to primary Sj(0)gren's syndrome (pSS).Methods One patient in our series and 42 patients in the literatures were analyzed.The clinical manifestations,imaging features,diagnosis,treatments and prognosis of these patients were described respectively.Results Among the 43 cases,42 patients were female (98％).The median age was 57 (range 29-79) years.The diagnosis of bronchopulmonary amyloidosis was made subsequently to that of pSS with a median delay of 8.9 (range 0-30) years.Thirty-eight cases (88％) were localized amyloidosis.Most cases were related to AL amyloidosis (21/28,75％ ).The main clinical manifestations included cough (18/38,47％),short of breath (13/38,34％ ),sputum (9/38,24％) and hemoptysis (5/38,13％ ).Nine patients (9/38,24％ ) had no clinical symptoms.The most common patterns of radiological manifestations included multiple nodules (40/43,93％),multiple cysts or bullae (16/43,37％),interstitial lung disease (16/43,37％ ),irregular luminal narrowing and airway wall thickening (8/43,19％ ).The pulmonary function test was done in 17 patients,which revealed moderate to severe reduced diffusion capacity for carbon monoxide (8/17,47％ ).The diagnosis of amyloidosis was made based on pathological findings in all cases.Pathologic examination showed diffuse deposits of amorphous,eosinophilic,Congo-red positive staining material.The treatments were symptomatic.The prognosis of most patients was good.The median follow-up time was 26.5 (range 2-96) months,only 2 patients died during the follow-up.Conclusion The bronchopulmonary amyloidosis secondary to pSS is localized amyloidosis in most cases.Clinical manifestations depend on the location and extent of airway lesions.The appearance of multiple lung nodules with calcified or cysts in chest images should be considered as secondary amyloidosis.No specific therapy is available for these cases,however,this condition in majority of patients progresses slowly.

Objective To determine health related quality of life (QOL) of patients with systemic lupus erythematosus (SLE) and to identify the factors affecting QOL. Methods One hundred and fifty-one consecutive patients with SLE were separated into different groups according to the type or stage of disease. Health status were measured by 1 health transition (HT) and 8 composite scales of the medical outcomes study short form-36 (SF-36). The mean scores for each subscale of the SF-36 for patients with SLE in China were compared to those of a health population in China and of foreign SLE patients.The effect of patients′age,sex,education level,disease duration,disease activity (assessed by SLEDAI score),and organ damage(assessed by SLICC/ACR DI) and treatment were analyzed. ANOVA, rank sum test and correlation analysis were applied for statistical analysis. Results Patients with SLE had significantly lower HT and mean scores in each subscale of the SF-36 compared to those of a health population in China,while the scores in such subscales as physical functioning (PF), bodily pain (BP), vitality (VT), role-emotional (RE) and mental health (MH) were higher than those of foreign SLE patients. The mean scores of those patients with lupus nephritis and center neural lupus or in active state were lower than those not or those in steady-going. The SLEDAI score and SLICC/ACR DI were the most important elements that affected the QOL in SLE patients and showed obviously negative correlated with one′s scores in SF-36. The glucocorticoid dose of patients was closely negatively correlated with one′s scores. The latter were also associated with age, education level and disease duration. Conclusion The health status of patients with SLE is mainly associated with disease activity,damage and therapy. The SF-36 is a valid instrument for measuring the health related QOL in SLE.

To investigate whether HIV3B and HIV Ada-M can infect cultured human dorsal root ganglion (DRG) neurons, organotypic and dissociated human fetal DRG cell culture models were established. On the 14th day, organotypic cultured DRG explants were exposed to HIV3B or HIV Ada-M for another 14 days. Outgrowth and morphology of neurites were observed with phase contrast microscope at different time of cultured age. On the 3rd day, dissociated cultured DRG cells were exposed to HIV3B or HIV Ada-M for another 3 days. After that, dissociated DRG cells were processed for microtubule-associated protein 2 (MAP2) labeling and observed with fluorescent microscopy. DRG explants on the 28th day and dissociated DRG cells on the 6th day, the samples were processed for electronic microscopic observation. Both organotypic and dissociated DRG cultures were cultured continuously in culture media as controls. Immature HIV-like particles were found in organotypic cultured DRG neurons. Many HIV-like particles were found in dissociated cultured DRG neurons. HIV infection could not cause morphological and ultrastruc( )l alterations on both organotypic and dissociated cultured DRG neurons. These discoveries will be valuable for studies on pathogenic (mee)hanisms of HIV infection and/or HIV associated peripheral neuropathies.

To investigate the effects of didannsine(ddI)on the morphological alterations of dorsal root ganglion(DRG)neurons,dissoci-ated DRG cells from rat embryo were studied.DRG cells were cultured for 3 days and then treated with ddI for additional 3 claysin differ-ent concentrations(1μg/ml,5 μg/ml,10μg/ml and 20 μg/ml,respectively).Afarthat,DRG cells were processedformicrotubule as-soeiated protein 2(MAP2)labeling and observed under confocal laser scanning microscopy(CLSM).The results showed that both thenumber and length of neurites of the DRG cells after exposed to ddl significantly down-regulated in a dose-dependentmanner compared withcontrol group,thus suggesting that ddI may have inhibitory effects on neufite regeneration and outgrowth in dissociated DRG cultures.
