1.Expression of cathepsin B and cystatin C and its potential role in diabetic rats

AIM:To observe the expressions of cathepsin B (CB) and cystatin C (CC) in different stage of diabetic rats and to investigates their potential roles.METHODS:Sixty rats were divided into diabetes mellitus group induced by intravenous injection of streptozotocin (55 mg/kg) and normal group injected with citrate buffer. Ten rats were sacrificed respectively at the end of fourth week,eighth week and sixteenth week in both groups. 24 h urine excretion was collected in rats before sacrifice. The blood and the kidney were also collected. The mRNA and protein expressions of CB and CC in kidney were detected by real time PCR and immunohistochemical staining,respectively.RESULTS:At the end of eighth week,the expression of Ccr,24 h urinary protein excretion,CB,CC in diabetic rats increased significantly,compared to the results at the fourth week (P
2.Compatibility of osteoblasts with shape memory polyurethane before and after the deformation-complex

BACKGROUND:Shape memory polyurethane has good physical and chemical properties and compatibility, but there are relatively few reports on the compatibility of osteoblasts before and after the deformation-complex of the shape memory polyurethane. OBJECTIVE:To observe the compatibility of osteoblasts with shape memory polyurethane before and after the deformation-complex. METHODS: Shape memory polyurethane membranes were prepared, and its stretching-solid-complex was conducted under the experimental environment, to obtain the membrane materials after the deformation-complex. The Sprague-Dawley neonatal rat osteoblasts were inoculated on the shape memory polyurethane membranes before and after the deformation-complex. After 2 hours of culture, the number of adherent cels was counted, and cel spreading was observed; cel proliferation was determined after 1-11 days of culture. RESULTS AND CONCLUSION:The adhesion amount and proliferation activity of osteoblasts on shape memory polyurethane membranes after the deformation-complex were significantly higher than those before the deformation-complex (P < 0.05). The osteoblasts presented fusiform appearance on the shape memory polyurethane membranes after deformation-complex, and cel arrangement showed a clear orientation, but a smal spreading area; while the osteoblasts presented polygonal shape on the shape memory polyurethane membranes before deformation-complex, arranged in no particular direction, and spread largely. These findings show the shape memory polyurethane has better osteoblast compatibility after the deformation-complex.
3.Effect of Omeprazole Combined with Glutamine in the Prevention of Stress Ulcer

Objective:To study the effect of omeprazole combined with glutamine in the prevention of stress ulcer. Methods:To-tally 106 cases of severe patients admitted in ICU of our hospital were divided into the observation group and the control group with 53 ones in each according to the admission order. The patients in the control group were treated with glutamine, and those in the observa-tion group were treated with sodium omeprazole combined with glutamine. The pH change in gastric juice, adverse reactions and inci-dence of stress ulcer after the treatment were observed and compared between the two groups. Results:After the 4-day treatment, the pH value of gastric juice in the observation group was significantly higher than that in the control group (P<0. 05), and after the 7-day treatment, the difference was more notable. After the treatment, the symptoms of upper digestive tract in the observation group were fewer than those in the control group (P<0. 05). After the treatment, the observation group had three stress ulcer patients with the incidence rate of 5. 66%, while that in the control group was higher (24. 53%), and the difference was statistically significant (P<0. 05). There was no statistically significant difference in the adverse reactions between the two groups (P>0. 05). Conclusion:Omeprazole combined with glutamine in the prevention of ulcers stress shows promising clinical efficacy, which can effectively inhibit gastric acid secretion and rapidly increase pH value of gastric juice with low incidence of adverse reactions, mild symptoms and high se-curity, and is worthy of wide use in clinics.
4.Local moderate hypothermia therapy for acute cerebral hemorrhage

Objective To evaluate local moderate hypothermia technology on acute brain hemorrhage cases.Methods 100 patients of cerebral hemorrhage were randomly divided into two groups(moderate hypothermia groupand control group). The moderate hypothermia group was 50 cases treated with moderate hypothermia after routine treatment,the control group was 50 cases with routine treatment. The two groups were assessed after treatment for 21 days. Results The group treated via moderate hypothermia therapy showed the nerve function damages relatively lower than the control group(P < 0.05), and the surviving probability and curative ratio were significant higher than the control group(P< 0.05 ). Conclusion The local moderate hypothermia therapy for acute cerebral hemorrhage patient in clinical can significantly improve the never cell function and reduce mortality and disablement.
5.A study on care-giving competence of the primary caregivers influence on nurse-social worker-college student volunteer team guide for the long-term bedridden elderly

Objective To explore the effect of nurse-social worker-college student volunteer team guide on care-giving competence of the primary caregivers for the long-term bedridden elderly. Methods The research was conveniently focused on a total of 60 long-term bedridden elderly and 30 primary caregivers, pension caregiver in welfare home from March to May 2015. The nurse-social worker-college student volunteer team guide for the primary caregivers included distributing health manual, training skill, psychological support, respite care and so on. The intervention lasted for 3 months, twice a month, 150 minutes each time. This was a self comparison study. The care competence for the primary caregivers and self-care ability of daily activities for the long-term bedridden elderly were conducted before and after intervention. Results Before intervention, the total score of care-giving competence was 59.07 ± 13.42. After intervention, the total score was 66.64 ± 14.16. Before intervention, the scores of caring knowledge, operation technique, behavior and attitudes, decision-making and self-efficacy were as follows:18.48±3.46, 17.95±3.98, 12.16±3.08, 7.98±2.56. Whereas, after intervention, the scores were as follows:20.32±3.58, 20.48±2.74, 14.29±3.03, 9.65±2.07. Each dimension score after intervention was significantly higher than that before intervention (t=-2.87--2.02, P<0.01). For bathing, dressing, indoor transferring and eating in self-care ability of daily activities, the numbers of people in 60 long-term bedridden elderly were 8, 13, 21, 18 before intervention and 17, 24, 43, 29 after intervention. The self-care ability of daily activities of the primary caregivers such as bathing, dressing, indoor transferring and eating after intervention were significantly higher than that before intervention (χ2=4.093-4.857, P<0.05). Conclusions Nurse-social worker-college student volunteer team guide can enhance the care-giving competence of the primary caregivers as well as improve the self-care ability of daily activities for the long-term bedridden elderly.

7.Effect of rt-PA thrombolysis combined with antiplatelet therapy on recurrent cardiovascular and cerebrovascular events in patients with mild acute ischemic stroke

Objective To explore the effect of recombinant human tissue plasminogen activator(rt-PA) intravenous thrombolytic therapy combined with antiplatelet therapy on the recurrence of cardio cerebral vascular adverse events in patients with mild acute ischemic stroke (AIS).Methods From December 2015 to December 2017,74 patients with mild AIS treated in Shaoxing Central Hospital were selected in the research.According to the random number table method,the patients were divided into two groups,with 37 cases in each group.The control group received rt-PA intravenous thrombolysis,and the combined group was treated with antiplatelet therapy on the basis of the control group.The serum creatinine(SCr),alanine transaminase(ALT) and platelet count(PLT) were compared before and after treatment in the two groups,and the scores of the Barthel index scale and the modified Rankin scale were compared between the two groups.The incidence of cardiovascular adverse events in 3 months of the two groups was also recorded.Results There were no statistically significant differences in liver and kidney function indicators between the two groups before and after treatment (t =0.30,0.27,0.20,0.77,0.03,0.64,all P > 0.05).Before treatment,the Barthel index scale scores in the combined group and control group[(74.97 ±4.74)points,(61.95 ± 4.24) points] were significantly increased,and the increasing degree in the combined group was more significant than that in the control group(t =12.45,P < 0.05).The modified Rankin scale scores in the combined group and control group[(1.16 ± 0.37) points,(2.05 ± 0.45) points] were significantly decreased compared with those before treatment,and the reduction degree of the combined group was more significant than that in the control group(t =9.29,P < 0.05).The total incidence rate of bleeding events,cardiovascular events,cerebrovascular events and death events in the combined group(5.41%) was significantly lower than that of the control group(27.03%) (x2 =4.87,P < 0.05).Conclusion rt-PA intravenous thrombolytic therapy combined with antiplatelet therapy can effectively improve the nerve function and daily activity of patients with mild AIS.It can also effectively reduce the incidence of cardiovascular and cerebrovascular adverse events,and thus help to improve the prognosis and improve the quality of life.Therefore,it has good clinical application value.
8.Study on the factors impacting on early cochlear implantation between the eastern and western region of China.

OBJECTIVE:
To describe tne regional different factors which impact on early cochlear implantation in prelingual deaf children between eastern and western regions of China.
METHOD:
The charts of 113 children who received the cochlear implantation after 24 months old were reviewed and analyzed. Forty-five of them came from the eastern region (Jiangsu, Zhejiang or Shanghai) while 68 of them came from the western region (Ningxia or Guizhou). Parental interviews were conducted to collect information regarding the factors that impact on early cochlear implantation. Result:Based on the univariate logistic regression analysis, the odds ratio (OR) value of universal newborn hearing screening (UNHS) was 5. 481, which indicated the correlation of UNHS with early cochlear implantation is significant. There was statistical difference between the 2 groups (P<0. 01). For the financial burden, the OR value was 3. 521(strong correlation) and there was statistical difference between the 2 groups (P<0. 01). For the communication barriers and community location, the OR value was 0. 566 and 1. 128 respectively, and there was no statistical difference between the 2 groups (P>0. 05). The multivariate analysis indicated that the UNHS and financial burden are statistically different between the eastern and western regions (P=0. 00 and 0. 040 respectively).
CONCLUSION
The UNHS and financial burden are statistically different between the eastern reinforced in the western region. In addition, the government and society should provide powerful policy and more financial support in the western region of China. The innovation of management system is also helpful to the early cochlear implantation.

9.Qualitative study on self-experience of patients with early psychological aberration on RAIDS ;management model

Objective To improve interventional factors and clinical practice by exploring the effect of comprehensive weight management on reviewing, aerobic, intervention, diet guidance and sleeping management ( RAIDS) model in patients with early psychological aberration. Methods This study adopted phenomenological methods of qualitative study. 13 psychological aberration patients′ views on RAIDS model were collected. Results Four concepts including full support, regret, achievement, and positive expectation, were concluded and analyzed from interviews. Conclusions RAIDS weight management model has the function of controlling weight of psychological aberration patients, and it helps patients rebuilt their confidence. It is worth of being spread. Patients should be advised to strengthen oxidant activities. Over expectation of the effect of weight control needs a correct guide from nurses and professional staff. In addition, aimed social support and clearing negative emotion are effective in nursing intervention.
10. Association of etheno-DNA adduct and DNA methylation level among workers exposed to diesel engine exhaust

Objective:
To investigate the association between etheno-DNA adduct and the promoter of DNA methylation levels of cyclin dependent kinase inhibitor 2A (P16), Ras association domain family 1 (RASSF1A) and O-6-methylguanine-DNA methyltransferase (MGMT) in workers with occupational exposure to diesel engine exhaust (DEE).
Methods:
We recruited 124 diesel engine testing workers as DEE exposure group and 112 water pump operator in the same area as control group in Henan province in 2012 using cluster sampling. The demographic data were obtained by questionnaire survey; urine after work and venous blood samples were collected from each subject. The urinary etheno-DNA adducts were detected using UPLC-MS/MS, including 1,N6-etheno-2'-deoxyadenosine (εdA) and 3,N4-etheno-2'-deoxycytidine(εdC). The DNA methylation levels of P16, RASSF1A, and MGMT were evaluated using bisulfite-pyrosequencing assay. The percentage of methylation was expressed as the 5-methylcytosine (5mC) over the sum of cytosines (%5mC). Spearman correlation and multiple linear regression were applied to analyze the association between etheno-DNA adducts and DNA methylation of P16, RASSF1A, and MGMT.
