1.The thoracic CT manifestations and pathologic basis of pulmonary Wegener granulomatosis

Objective To explore the thoracic CT manifestations and pathologic basis of pulmonary Wegener granulomatosis (WG),in order to improve the diagnostic accuracy.Methods Eighteen patients with pulmonary WG confirmed by pathology were collected.All the patients underwent CT scan and enhanced scan with 64-slice volume CT.CT features and clinical symptoms were retrospectively reviewed.Results The pathological manifestations of 18 cases with pulmonary WG were miscellaneous,including necrotizing granuloma infiltrated with inflammatory cells,intrapulmonary vasculitis,parenchymal necrosis,hemorrhage of pulmonary alveolus,microabscess and so on.Thoracic CT manifestations:multiple nodules were seen in 7 cases,multiple masses were detected in 4 cases,cavitations showed in 10 cases,peripheral wedge-shaped shadows showed in 2 cases,ground-glass attenuation and consolidation showed in 2 cases,bronchial wall thickening in 2 cases,solitary pulmonary nodule or mass in 3 cases,pleural effusion was seen in 5 cases.All the nodules,masses and cavitations demonstrated inhomogeneous and ring enhancement on contrast-enhanced images.Some patients presented with varied kinds of the above lesions,which could transform each other and became more or less.Conclusions Pulmonary WG has many pathological manifestations,resulting in the variety of the thoracic CT manifestations.The main manifestations are multiple nodules and multiple masses,necrotic cavitations.The feeding vessel sign to a nodule or mass and ring-shade cavity are relatively characteristics to diagnose pulmonary WG.The correct diagnosis could be established on the bases of understanding of the special developments and pathology of the lesions.
2.Role of stress in myocardial protection of ischemic preconditioning

Objective:To determine the role of stress in myocardial protection of ischemic preconditioning (IPC).
Methods:hTirty rabbits were randomly divided into an IPC group, an etomidate (Etom) group, an ischemic/reperfusion (IR) group, a methylprednisolone (MP) group and a sham group. hTe ratio of infarction size versus risk area (infarct/risk) was calculated. hTe elevations of the serum creatine kinase (CK) activity and cardiac troponin I (cTnI) concentrations as well as the serum cortisol concentrations were measured.
Results:hTe percentages of infarct/risk in the IPC group, the MP group, the IR group, and the Etom group were (5.86±2.81)%, (11.28±3.62)%, (26.79±4.53)%, and (18.19±3.72)%, respectively. The elevations of the serum CK activity in the IPC group, the MP group, the IR group, and the Etom group were (255±89), (314±160), (855±371), and (768±404) U/L, respectively. hTe elevations of serum cTnI concentrations in the IPC group, the MP group, the IR group, and the Etom group were (3.6±0.6),(6.1±2.2), (8.1±3.6), and (6.4±1.6)μg/L, respectively. Those indicators among the groups were signiifcantly different (P<0.05). Cortisol reaction was markedly diminished in the Etom group.
Conclusion: A blunted cortisol reaction can markedly reduce the benefit of IPC while methylprednisolone shows cardioprotective effects, suggesting that stress might be involved in the myocardial protection of IPC.
3.Effects of Ginsenoside Rg1 on Structure and Function of Rat Skeletal Muscle with Exercise-induced Fatigue

Objective To observe the effects of ginsenoside Rg1 group on the structure and function of rat skeletal muscle with exercise-induced fatigue.Methods SD rats were randomized into blank control group,model group and ginsenoside Rg1 group.The rats in the model group and ginsenoside Rg1 group received medium-intensity treadmill running to induce exercise-induced fatigue.Ginsenoside Rg1 group was given ginsenoside Rg1 50 mg?kg-1?d-1 for 2 continuous weeks during the modeling.After treatment,malondialdehyde(MDA)content,superoxide dismustase(SOD) activity,mitochondrial membrane potential and free calcium content in the rat skeletal muscle were measured.Meanwhile,ultrastructure of the skeletal muscle was observed under the electron microscope.Results In the model group,SOD activity,mitochondrial membrane potential,and free calcium content were decreased,MDA content was increased,and the difference was significant as compared with the blank control group(P
4. Integrated Pharmaceutical Care on Patients with Coronary Heart Diseaseafter First Percutaneous Coronary Intervention

OBJECTIVE: To explore the effect of integrated pharmaceutical intervention on medication compliance, risk factors control, and 6 months and 2 years prognosis in patients with coronary artery disease(CAD) after first percutaneous coronary intervention (PCI) in china. METHODS: 200 cases of patients diagnosed with CAD and underwent PCI successfully firstly in Zhoupu hospital during January 2015 to December were enrolled. The control group was given routine treatment and nursing care; pharmacist intervention group, on this basis, was given pharmaceutical intervention by pharmacists, including optimizing medication program, formulating pharmaceutical care plan, medication education, health education and regular pharmacists outpatient follow-up. The two groups were followed up for two years. Comparison of the Readmission(Readm) rate, incidence rate of major adverse cardiac events (MACEs), Readm expenses, awareness rate of commonly used two-grade preventive drugs for CAD, medication compliance and emotion management after 6 months and two years in the two groups. The control of risk factors in the two groups after 2 years were compared. RESULTS: After six months, the Readm rate, the incidence of MACEs and Readm expenses in pharmacist intervention group were lower than those in the control group, and the medication compliance was slightly higher than the control group (94.9% vs 87.4%); two years later, the standard rate of risk factors in pharmacist intervention group was significantly better than the control group(smoking3.1% vs. 12.6%, P=0.012; blood glucose standard rate79.6% vs. 62.1%, P=0.007; blood pressure standard rate74.5% vs. 53.7%, P=0.003; blood lipid standard rate84.7% vs. 57.9%, P<0.001); and the Readm rate, the incidence of MACEs and the cost of Readm were significantly lower than those of the control group after six months and two years. CONCLUSION: Pharmacists′ active pharmaceutical intervention on patients after PCI can effectively improve the medication compliance of the patients, reduce the postoperative risk factors and the incidence of MACEs, reduce medical expenses, improve the clinical prognosis, which were more significant when the follow-up time was longer.
5.Effect of Social Support, Coping Styles and Self-efficacy on Depression of Adults with Epilepsy

ObjectiveTo explore the effect of the social support, coping styles and self-efficacy on depression of adults with epilepsy. Methods84 patients with epilepsy were evaluated with Zung's self rating depression scale (SDS), general conditions questionnaire, Social Support Scale, Coping Styles Questionnaire, General Self-efficacy Scale. Based on the SDS, they were divided into the depression group and the epileptic group.Results52 patients suffered from depression (61.2%). There were significant differences in general self-efficacy, coping styles and social support between the depression group and the epileptic group (P<0.05). The score of SDS was positively correlated with the frequency of seizure, duration of epilepsy, fantasy, self-accusation and avoiding (P<0.05), and negatively correlated with the economic status, total support, objective support, subjective support, utilization of support, problem solving, help seeking, rationalizing and self-efficacy (P<0.05). Multiple regression analysis showed that the score of SDS was correlated with the self-efficacy, help seeking, self-accusation, economic status and frequency of seizure.ConclusionThe depression of adults with epilepsy are affected by the social support,coping styles and self-efficacy.

Objective:Discussion MKK4 protein expression in nasopharyngeal carcinoma and -1044 A/T polymorphism correlation.Methods:90 patients with nasopharyngeal carcinoma , MKK4 protein expression was determined by immunohistochemical staining,polymerase chain reaction-restriction fragment length polymorphism ( PCR-RFLP ) to detect the gene -1044A/T sites monocytes nucleotide polymorphism.Results:MKK4 protein expression in nasopharyngeal carcinoma (-) was 24.4%(22/90),(+) was 15.6%(14/90),(++) was 34.4%(31/90),(+++) was 25.6% (23/90).Low expression (-/+) patients with a total of 36 cases,-1044AA genotype accounted for 22 cases (61.11%),AT genotype accounted for 12 cases (33.33%),TT genotype accounted for two cases (5.56%),AT+TT gene type accounted for 14 cases (38.89%).The patients with high MKK4 expression of 54 cases,of which accounted for 38 cases of AA genotype (70.37%),AT genotype accounted with 15 cases (27.78%),TT genotype accounted for one case (1.85%),AT +TT genotype accounted for 16 cases (29.63%).Low expression and high expression of T gene mutation occurs no significant ( Z=0.323 , P=0.747 ) .Conclusion: MKK4 protein expression correlated with -1044 A/T gene promoter polymorphisms was no significant correlation .

9.A reappraisal of the relationship between metatarsus adductus and hallux valgus.

BACKGROUNDA causal link between the metatarsus adductus and hallux valgus is not clear. The aim of this study was to investigate the configurations of the metatarsus adductus deformity by radiological measurements and reappraise the relationship between hallux valgus and metatarsus adductus.
METHODSThe first step was evaluation of the relationship between metatarsus adductus and hallux valgus on 143 dorsoplantar weight-bearing radiographs diagnosed as hallux valgus which was also known as bunions. Measurements including the hallux valgus angle (HVA), the intermetatarsal angle (IMA), the Kilmartin angle (KA), the tibial sesamoid position (TSP), and metatarsus adductus angle were taken. The metatarsus adductus angle is defined by Sgarlato's angle (SMA) and Engel's angle (EMA) respectively.
RESULTSThe metatarsus adductus angle positively correlates with the HVA (r = 0.590, P = 0.000) and KA (r = 0.601, P = 0.000), yet negatively correlates with the grade of TSP, (r = -0.348, P = 0.000). Contradiction of diagnosis existed in 22 (22/100) subjects diagnosed as metatarsus adductus by SMA yet normal by EMA. In this group, the correlation between HVA and metatarsus adductus angle was negative (r = -0.472, P = 0.027).
CONCLUSIONSEMA and SMA defined metatarsus adductus by different deformity apexes. Metatarsus adductus configurations in that the apex of the deformity lay in either the base of metatarsals or tarsus. They respectively correlate positively or negatively to the HVA.

10.Establishment of Primary Adult MDS Nested Case-Control Study Cohort and Study of Risk Factors Associated with MDS Evolution to Leukemia.

OBJECTIVETo establish a nested case-control study cohort in myelodysplastic syndrome (MDS) patients and investigate the clinical characteristics, WHO subtype and risk factors associated with MDS evolution to leukemia of this cohort.
METHODSAll patients, ≥18 years of age, provided by 24 Shanghai hospitals with initial clinical findings consistent with a hematopoietic abnormality between June 2003 and April 2007, were the candidates for inclusion in this study. The blood and bone marrow samples of every patient should be provided at baseline. Diagnosis was made by incorporating morphologic, immunophenotypic, cytogenetic and molecular features according to WHO classification criteria. Cytogenetic analysis was performed using conventional G-banding karyotyping and fluorescence in situ hybridization (FISH) techniques. Cumulative risk of evolution was estimated by Kaplan-Meier method. Prognostic factors were evaluated by univariate Log-rank method and multivariate Cox proportional hazard models.
RESULTSA total of 435 patients were diagnosed as MDS. The median age of MDS onset was 58(18-90) years, with 248 male patients and 187 female patients (male: female 1.33: 1). The percentage of cases with refractory cytopenia with multilineage dysplasia (RCMD) was the highest (65.5%), while that of refraetory anemia (RA) (2.3%), refractory anenia with ring sideroblast (RARS) (1.1%) and 5q-syndrome (0.5%) was lower. Trisomy 8 (+8) was the most common chromosome abnormalities (71 cases, 12.7%). The mean follow-up time was 20.3 (4.2-57.1) months. Cases were patients with evolution by the end of follow-up, while controls were patients without evolution by that time. Case group included 41 patients and control group included 342 patients. Univariate analysis showed that the age, sex, WHO subtype, WBC count, absolute neutrophil count (ANC), IPSS cytogenetic subgroup, IPSS group and bone marrow blast percentage were significant risk factors for leukemia-free survival (LFS). Multivariate analysis of COX model showed that the age, sex, WHO subtype, IPSS cytogenetic subgroup and bone marrow blast were independent risk factors for LFS.
CONCLUSIONA nested case-control study cohort of MDS patients is established. The clinical characteristics and WHO subtype of MDS patients in Chinese Shanghai are different from that in Western countries. The independent risk factors for MDS evolution are age, sex, WHO subtype, IPSS cytogenetic subgroup and bone marrow blast percentage.
