1.Clinical characteristics and treatment of primary ovarian malignant lymphoma

Objective:To investigate the clinical features and prognostic factors of primary ovarian lymphoma(POL). Methods:A retrospective review was performed based on the clinical records of 14 POL cases treated at Tianjin Medical University Oncology in-stitute Hospital verity from 2000-6 to 2010-5. Results:The median age of patients was 47 years at presentation(range 28~62years). Ab-dominal pain was the most common initial symptom. The majority of histological subtype was B cell lymphoma, above 50%of which is diffuse large B-cell non-Hodgkin's lymphoma. A R0 resection was carried out in 2 patients who were lost to follow-up soon after sur-gery. 11 patients were treated by CHOP, FC or CHOPE chemotherapy. Chemotherapy combined with rituximab were given to 5 pa-tients. Until now, 5 patients were alive, 6 patients died, and only one patient was lost to follow up after therapy. Conclusion:POL is an extremely rare lymphoma. The ultimate diagnosis depends on histopathologic examination. Primary ovarian non-Hodgkin lymphoma should be treated with multi-modality strategies. Treatment with doxorubicin-based chemotherapy after oophorotomy is recommended. B cell lymphoma preferred to use rituximab combination chemotherapy. Abdominal cavity chemotherapy prophylaxis and irradiation can decrease the probability for recurrence .
2.Acute toxicity of potassium bichromate and 3,4-dichloroaniline in Chinese rare minnow (Gobiocypris rarus)

Objective and Methods As a local species, Chinese rare minnow (Gobiocypris rarus) has been one of the standardized test fish for chemical toxicity tests in China .By optimal screening , the acute toxicity of potassium bichromate and 3,4-dichloroaniline (3,4-DCA) was determined to evaluate repeatability and accuracy for Gobiocypris rarus in one laboratory and between different laboratories .Result Based on a proper data analysis , for the two chemicals per-formed with the same fish, all 96 h LC50 values were within mean ( x-) and upper and lower control limits ( ±2s) in both inner test ( in one laboratory ) and outer test ( between different laboratories ) .Conclusions From these results , a valid database could also be established to evaluate one single test .In addition, Gobiocypris rarus will not only be a potential test species for ecotoxicity tests , but also recommended as a standard laboratory animal .
3.The Disposal Medical Wastes Sorting in Second-level Hospitals in Zunyi City

OBJECTIVE To understand the status of medical wastes sorting disposal and the use of sharp containers in the second-level hospitals. METHODS The status of medical wastes sorting disposal and the use of sharp containers in some of the second-level hospitals in Zunyi City,Guizhou Province were investigated. RESULTS It was 77.8% where the medical wastes were sorted,packed and signed,It was 55.6% and 22.2%,respectively,where the plastic infusion bottles and the glass seal bottles were sorting and placed as recycle wastes. It was 33.3% where the hospitals used sharp containers according to the requirement of the People's Republic of China(PRC). CONCLUSIONS Partial second-level hospitals lack of enough recognition to the medical wastes management,and do not perform strictly these rules; therefore,all of the hospitals should strengthen the study of medical policies and regulations,the management of medical wastes should be standardized. The hospitals should use the wrapping sack and sharp containers making of regular factories and conforming to the national requirement for decreasing the environmental pollution of medical wastes and the sharps injury in the health care workers,and the management of recycle medical waste should be strengthened.
4.Sequence analysis of 16S rDNA and pmoCAB gene cluster of trichloroethylene-degrading methanotroph.

Methanotrophs could degrade methane and various chlorinated hydrocarbons. The analysis on methane monooxygenase gene cluster sequence would help to understand its catalytic mechanism and enhance the application in pollutants biodegradation. The methanotrophs was enriched and isolated with methane as the sole carbon source in the nitrate mineral salt medium. Then, five chlorinated hydrocarbons were selected as cometabolic substrates to study the biodegradation. The phylogenetic tree of 16S rDNA using MEGE5.05 software was constructed to identify the methanotroph strain. The pmoCAB gene cluster encoding particulate methane monooxygenase (pMMO) was amplified by semi-nested PCR in segments. ExPASy was performed to analyze theoretical molecular weight of the three pMMO subunits. As a result, a strain of methanotroph was isolated. The phylogenetic analysis indicated that the strain belongs to a species of Methylocystis, and it was named as Methylocystis sp. JTC3. The degradation rate of trichloroethylene (TCE) reached 93.79% when its initial concentration was 15.64 μmol/L after 5 days. We obtained the pmoCAB gene cluster of 3 227 bp including pmoC gene of 771 bp, pmoA gene of 759 bp, pmoB gene of 1 260 bp and two noncoding sequences in the middle by semi-nested PCR, T-A cloning and sequencing. The theoretical molecular weight of their corresponding gamma, beta and alpha subunit were 29.1 kDa, 28.6 kDa and 45.6 kDa respectively analyzed using ExPASy tool. The pmoCAB gene cluster of JTC3 was highly identical with that of Methylocystis sp. strain M analyzed by Blast, and pmoA sequences is more conservative than pmoC and pmoB. Finally, Methylocystis sp. JTC3 could degrade TCE efficiently. And the detailed analysis of pmoCAB from Methylocystis sp. JTC3 laid a solid foundation to further study its active sites features and its selectivity to chlorinated hydrocarbon.

5.Diagnosis, therapy and follow-up in infants with subclincal hypothyroidism--A clinical analysis of 107 cases

The serum TSH levels of 107 infants with subclinical hypothyroidism (SH) were > 20 mIU/L after 1-8 check-up, along with FT4, TT4, within low normal range. After given small dosage L-T4, for 4 weeks, blood TSH level obviously descended while FT4, TT4, ascended (all P <0.01). Seven cases of thyroid hypogenesis and 7 strumas were found by ultrasonography. It seems appropriate to use dosage of 3-4 μg·kg·-1·d-1 L-T4 in treating SH.
6.Exploration of cut-off value of TSH during screening of congenital hypothyroidism in the newborns

Positive linear correlation was found between TSH level and detection rate of congenital hypothyroidism (CH). When measured with a cut-off point at 9, 10, 15 and 20 mU/L of TSH, the sensitivity of positive detection rate for CH was 99.77%, 96.80%, 81.25% and 71.88%, respectively. TSH > 9 mU/L seems to be a reasonable cut-off value.
7.Clinical characteristics and prognostic analysis of primary testicular lymphoma

Objective To discuss the clinical features and prognostic factors of primary testicular lymphoma (PTL). Methods A retrospective review was performed based on the clinical records of 33 PTL cases treated at Tianjin Medical University Cancer Hospital from June 1977 to May 2009.Drawing survival curves by Kaplan-Meier method, using Log-rank test to the univariate analysis, and multivariate analysis by COX regression model to evaluate independent prognostic factors. Results The median age of patients was 62 years at presentation(range 33-81 years). Painless testicular swelling was the initial symptom. The majority of histological subtype was B cell lymphoma, 48% of which was diffuse large B-cell non-Hodgkin's lymphoma. By postoperative chemotherapy and/or radiotherapy, 23 patients achieved complete remission and 7 achieved partial remission, with a median follow-up of 23 months (4-231 months). The 5 years and 10 years of overall survival was 39.1% and 19.5%. The effect of patients who received chemotherapy≥4 cycles and B cell lymphoma patients combination with Rituximab were better. In multivariate analysis, Ann Arbor stage, B symptoms, age of patient and number of extranode site were independent prognostic factors of survival. Conclusions Primary non-Hodgkin lymphoma should be treated with multi-modality strategies. Treatment with doxorubicin-based chemotherapy after orchiectomy should be recommended. Ann Arbor stage, B symptoms, age of patient and number of extranode site were independent prognostic factors of survival in PTL.
8.Clinical characteristics and treatment of primary hepatic lymphoma

Objectives To investigate the clinical features,diagnosis,treatment and prognosis of primary hepatic lymphoma (PHL).Methods A retrospective study was carried out on the clinical records of 6 patients with primary hepatic lymphoma (PHL) treated at the Tianjin Medical University Cancer Hospital from April 2005 to September 2010.The domestic and foreign medical literatures were reviewed.Results For the 6 patients with PHL,the median age was 57 years (range 31-78 years).The male-to-female ratio was 2: 1.The most common initial symptoms were abdominal pain and fever.Serum transaminase,lactate dehydrogenase and β2-microglobulin levels were elevated in 4 of 6 patients.For the 3 patients who were tested for alpha-fetoprotein and carcinoembryonic antigen levels,the results were normal.None of the patients had a history of hepatitis or cirrhosis.The diagnosis was non-Hodgkin's lymphoma.A R0 resection was carried out in 1 patient who was lost to follow-up soon after surgery.A R2 resection was carried out in another patient.The remaining 4 patients received liver biopsy.Five patients were treated by CHOP or CHOP-like chemotherapy.A patient died of brain metastases after 8 cycles of chemotherapy,and another patient was lost to follow-up after 1 cycle of chemotherapy.Chemotherapy combined with rituximab were given to the other 3 patients.There was a complete remission in 2 patients after chemotherapy and biotherapy,and these patients were still alive at the last follow-up.Partial remission was achieved in the remaining patient after chemotherapy.The patient was given radiotherapy,but he died finally of tumor progression.Conclusions PHL is an extremely rare lymphoma.Its clinical presentations and imaging manifestations are non-specific.PHL should be considered when the patient has abdominal pain or fever,with a mass in the liver.The ultimate diagnosis depends on histopathologic examination.The pathological type is mainly non Hodgkin's lymphoma,with diffuse large B cell lymphoma.There is still no standard treatment.Surgery,chemotherapy,radiotherapy,targeted therapy and biotherapy can be used.
9.In vitro studies of the effects of high-fat dieton the expression of pancreatic acinar cells' IP3 and amylase release in rats

Objective To investigate the effects of high-fat diet on pancreatic acinar cells' IP3 expression and CCK-induced amylase release in rats.Methods Male Wistar rats were divided into high-fat diet group and normal diet group,they were fed for 4 weeks.Blood triglycerides,cholesterol,amylase and glucose levels were determined by automatic biochemical analyzer.Pancreatic tissues were taken for histopathological observations.Pancreatic acinar cells were isolated and cultured,and intracellular inositol 1,4,5-trisphosphate (IP3) was detected using a commercial kit.Amylase release rates were measured after CCK-8 stimulation.Results The rats in high-fat diet group appeared hyperlipidemia,vacuolization of acinar cells and the lymphocytes appeared around the acinar cells can be seen on the pancreatic tissue pathology staining.The levels of IP3 in acinar cells of rats fed a high-fat diet were higher than that of normal rats [(31.807 ± 3.448) pmol/106 cells vs (24.632 ± 3.649) pmol/106 cells,t=7.479,P<0.001];and amylase release rate in these rats'acinar cells were also higher than those of normal rats [when CCK-8=0.01 nmol/L:( 11.056 ±3.369)% vs (7.354 ± 2.181) %,t=3.912,P<0.001;when CCK-8=1 nmol/L:( 13.854 ± 4.087 ) % vs (9.432 ±2.477) %,t=3.939,P<0.001 ) after CCK-8 stimulation in different concentrations.Additionally,there was a positive co-relationship between acinar cell's IP3 level and amylase release (r=0.896,P<0.001 ).Conclusion Chronic high-fat diet induces hypersensitivity for pancreatic acinar cells' exocrine function,and IP3 as a signal molecule may play an important role in this process.
10.Clinical efficacy of pegaspargase plus GEMOX regimen for initial treatment of nasal NK/T-cell lymphoma

Objective:To evaluate the efficacy and safety of pegaspargase plus GEMOX (pegaspargase, gemcitabine, oxaliplatin) regimen in the initial treatment of nasal NK/T-cell lymphoma. Methods: Twelve preliminarily diagnosed nasal NK/T-cell lymphoma patients in Tianjin Medical University Cancer Institute and Hospital from June 2011 to March 2012 were analyzed. All patients took the pegaspargase plus GEMOX regimen (gemcitabine 800-1 000 mg/m2 on days 1 and 8, oxaliplatin 130 mg/m2 on day 1, and pegaspargase 2 500 IU/m2 on day 2), every three weeks for one cycle. The efficacy and toxicity of the regimen were evaluated in the follow-up treat-ment. Results:After two cycle treatments, 1 patient dropped out of treatment because of acute pancreatitis;the remaining 11 patients had response, in which 1 achieved complete response , 7 had partial response, 2 had stable disease, and 1 had progressive disease. The objective response rate was 72.7%, and the disease control rate was 90.9%. The 2-year overall survival rate was 90.9%. With median 6-cycle P-GEMOX regimen treatment, 81.8% of 11 patients presented side effects, primarily myelosuppression and hepatic dysfunc-tion. Conclusion:Pegaspargase plus GEMOX regimen showed high efficacy on the initial treatment of nasal NK/T-cell lymphoma pa-tients, but the incidence of adverse effect was still high.
