1.Influence factors of modified mini-mental state examination evaluation on 6819 elderly people

Objective To explore the main influence factors of ederly people's cognitive function and provide the countermeasures. Methods A cross-sectional study was performed in the elderly volunteers who were sampled from halves of the counties of 22 provinces in China.Modified minimental state examination(MMSE)scale was adopted to investigate the 6819 Chinese elderly people who were investigated in the Chinese elderly health questionnaire in 2002.Simple Chi-square test and multiple logistic regressions were applied in the analysis. Results The results showed that the perfect rate of cognition function was 85.0%in male,and 80.43%in female.The rate was 97.7%in people at age of 60-69 years,94.8%,70-79 years,85.0%,80-89 years,71.11%,90-99 years,55.7%,100-111 years,94.8%.There existed significant cognitive function differences in gender,age,education,born place,marital status,1iving status,smoking history,drinking history,physical exercise,self-satisfaction with life,affective state and healthy status(all P<0.01).Non-conditional logistic regression analysis showed that influence factors of cognitive function of the elderly were age,education level,marital status,physical exercise and optimistic state. Conclusions The risk factors for cognitive function are aging and widowhood.Higher education,regular physical exercise,optimistic state are protective factors for cognitive function.
2.Effects of exercise on dynamic cardiac function and survival after myocardial infarction

Objective To study the effects of exercise after myocardial infarction on the survival rate and the dynamic functioning of the left ventricle ( LV) . Methods Ninety-six male Sprague-Dawley rats were randomly assigned to a sedentary sham operation group ( Sed-Sh) , a previous exercise and sham operation group ( PreE-Sh) , a sedentary myocardial infarction ( MI) group ( Sed-MI) , a previous exercise MI group ( PreE-MI) , a post exercise MI group (PostE-MI) and a combined exercise MI group (ComE-MI), each of 16.All of the rats underwent either MI in-duction or a sham MI operation, the PreE groups after exercising for 5 weeks on a treadmill. The Sed groups did not exercise. The Post E-MI group did not exercise before the operation, but performed treadmill exercises for 4 weeks be-ginning 8 weeks after the operation. The ComE-MI group exercised for 5 weeks before and 4 weeks after the operation. All the exercising rats ran on the treadmill for 60 minutes daily, 5 d/wk. Life situation and spontaneous mortality were recorded, and echocardiographic measurements were performed on the4thday and 2, 4, 8 and 12weeks after the oper-ation. All of the rats were sacrificed at the end of the experiment. Results Compared with the Sed-Sh group, the Sed-MI group had significantly larger average LV dimensions at the end of both the diastol ( LVEDd) and the systol (LVEDs, but it had lower average fractional shortening (FS) and a smaller average ejection fraction(EF) at all of the different measuring times. The PreE-MI group had significantly lower average LVEDd and LVEDs, but a signifi-cantly higher FS and EF than the Sed-MI group on the 4th day. However, 2 weeks after the intervention the difference in LVEDd between the two groups was no longer significant. Significantly higher FS and EF were observed in the PostE-MI group at 12 weeks compared to the Sed-MI group. The ComE-MI group had significantly lower average LVEDd and LVEDs, but significantly higher FS and EF at all the time points than the Sed-MI group. The ComE-MI group had a significantly lower average LVEDs but a higher average FS and EF than the PreE-MI group at 12 weeks after the intervention. Conclusion Pre-MI exercise improves LV function significantly at the early stage, though the difference is temporary. Post-MI exercise improves LV function in the longer term. Combined exercise improves LV function the most effectively.
3.Clinical outcomes of modified Bristow operation with mini-open incision in treatment of recurrent anteroinferior shoulder dislocation

Objective To introduce a new modified Bristow operation with mini-open incision and evaluate jts clinical outcomes in treatment of recurrent anteroinferior shoulder dislocation. Methods A total of 11 patients with recurrent anteroinferior shoulder dislocation were treated with modified Bristow op-eration with mini-open incision.The anterior skin incision with 3-5 mm was made to explore the coracoid process with"moving window"technique.Then.the coracoid process and its short head of biceps were os-teotornjed.transferred and fixed on the anteroinferior potion of scapular neck through the rotator cuff inter-space.All patients were followed up for an average period of 15.8 months(6-48 months)by using modified Rowe score. Results The average operation time was 45 minutes(40-65 minutes).The modified Rowe 8core ranged from 80 to 95 points.excellent in all patients, with SUCCESS rate of 100% and without any re-currence or complication. Conclusions Compared with traditional operation.the modified Bristow oper-ation has smaller incision,less trauma.shorter operation duration and better clinical results.
4.Clinical outcomes of proximal humeral internal locking system self-locking plates in treatment of proximal humeral fractures

Objective To evaluate the clinical results of proximal humeral internal locking sys-tem(PHILOS)self-locking plates in treatment of proximal humeral fractures. Methods There were 35 patients including 19 males and 16 females(at mean age of 53.5 years,ranging from 29 to 92 years)with proximal humeral fractures treated with PHILOS plate.According to the Neer's classification sys-tem,there were two-part fractures in 19 patients,three-part fractures in 15 and four-part fractures in 1.Surgery was performed with PHILOS serf-locking plate via dehopectoral approach.All patients were fol-lowed up for mean 18.9 months(6-46 months)and evaluated by using Constant score and X-rays. Re-suits X-rays proved that all fractures got good reduction and complete healing during 8-12 weeks.The latest Constant scores was mean 89.5 points(82-94 points).There were no complications including plate loosing.humenral head necrosis or gleno-humeral ioint osteoarthritis. Conclusion PHILOS plate is a good choice for treatment of proximal humeral fractures because of its advantages including easy operation,stable fixation,good clinical results and few complications.
5.The characteristic of fractional amplitude of low frequency fluctuation in depression patients with suicidal ideation:a resting-state functional magnetic resonance imaging study

Objective To explore the characteristic of fractional amplitude of low frequency fluctuation (fALFF) and the relationship with the severity of depression, suicidal ideation and suicide risk in depression patients with suicidal ideation with resting-state functional magnetic resonance imaging (rs-fMRI). Methods Resting state functional magnetic resonance imaging maps were conducted using fractional amplitude of low frequency fluctuation (fALFF) in 52 depression patients (30 with suicidal ideation and 22 without) and 21 healthy controls (HCs). The severity of depression was evaluat-ed by using Hamilton Depression scale(HAMD). The suicidal ideation, the suicide risk in depression patients with sui-cidal ideation were both assessed by the Beck Scale for Suicide Ideation. The correlation between the fALFF value and the score of HAMD and the Beck Scale for Suicide Ideation was analyzed. Results MRI revealed significant differences in fALFF in the left superior/middle occipital gyrus and the right middle/inferior occipital gyrus (P<0.05, AlphaSim cor-rected)between depression patients with suicidal ideation and the HCs. Compared to the HCs, depression patients with-out suicidal ideation showed a higher fALFF in the left middle occipital gyrus (P<0.05, AlphaSim corrected). MRI re-vealed significant differences in fALFF in the left middle occipital gyrus (P<0.01, AlphaSim corrected)and the right mid-dle occipital gyrus (P<0.01, AlphaSim corrected) between depression patients with suicidal ideation and without. The fALFF of left middle occipital gyrus (r=0.366, P=0.046) and right middle occipital gyrus (r=0.513, P=0.004) were posi-tively correlated with the scores of HAMD, respectively whereas were not correlated with suicidal ideation and suicide risk. Conclusions Depression patients with suicidal ideation have an abnormal spontaneous activity in their left and right middle occipital gyrus. The increased activity in these brain areas are probably associated with the severity of de-pression whereas are not associated with suicidal ideation or suicide risk.
6.The effects of combined pre- and post-myocardial infarction exercise on microvessel density and vascular endothelial growth factor signaling

Objective:To investigate the effect of exercise both before and after myocardial infarction (MI) on microvessel density in the left ventricle, and on the mRNA and protein expression of vascular endothelial growth factor (VEGF) and its receptors.Methods:Ninety-six male Sprague-Dawley rats were randomly divided into a sedentary sham operation group (Sed-Sh), a pre-MI exercise sham operation group (PreE-Sh), a sedentary MI group (Sed-MI), a pre-MI exercise group (PreE-MI), a post-MI exercise group (PostE-MI) and a combined exercise group (ComE-MI). The rats of the exercise groups underwent 60min of treadmill training daily five times a week for five weeks, while those of the Sed-Sh and Sed-MI groups remained sedentary. Then all of the rats underwent either an operation to induce an acute MI (Sed-MI, PreE-MI, PostE-MI and ComE-MI groups) or a sham MI operation (Sed-Sh and PreE-Sh groups). Four weeks after the operation, the exercise groups underwent five days adaptive training, followed by 8 weeks of 60min of treadmill training daily, five times a week. All of the rats were then sacrificed. The microvessel densities in the infarcted and uninfarcted volumes were detected using Factor VIII-related antigen staining, while the mRNA expressions of VEGF and its receptors were examined using a real-time polymerase chain reaction. The protein expressions of VEGF and its receptors were measured using western blotting.Results:Compared with Sed-MI group, the average microvessel densities of the PostE-MI and ComE-MI groups had increased significantly, with the average density of the PostE-MI group significantly higher than that of the PreE-MI group, but significantly lower than the ComE-MI group′s average. Compared with the Sed-MI group, the average mRNA expression of VEGF and its receptors in the PostE-MI and ComE-MI groups was significantly higher, without significant differences between the two groups.Conclusions:Pre-MI exercise does not significantly improve left ventricular function or microvessel density, while post-MI exercise does, as it can upregulate the expression of VEGF and its receptors to activate VEGF signaling. The increase in microvessel density may benefit left ventricular remodeling and cardiac function.
7.Advances in tumor targeted nanocrystals

Nanocrystals are nanoscale (1-1000 nm) dispersion systems in which small numbers of surfactants or polymers are used as stabilizers to disperse insoluble drug particles in water or oil. Nanocrystals enjoy not only high drug content, but also a simple and mature preparation process. At present, 24 nanocrystals products that have been marketed mainly focus on enhancing the solubility and bioavailability of poorly soluble drugs. And recent years have witnessed an increasing number of research reports on target drug delivery of nanocrystals through particle size control and surface modification. This paper mainly introduces three targeting strategies for prolonging the in vivo circulation time of nanocrystals, increasing the affinity for tumor cells and achieving the response to internal and external stimuli, and discusses the current challenges in the application of nanocrystal technology to targeted anti-tumor drugs.
9.Bayesian Network Meta-analysis of Therapeutic Efficacy of Xianling Gubao Capsules Combined with Routine Therapy for Postmenopausal Osteoporosis

OBJECTIVE: To systematically evaluate the difference of therapeutic efficacy of Xianling gubao capsules combined with routine therapy for postmenopausal osteoporosis (PMOP). METHODS: Retrieved from Cochrane Library, PubMed, Embase, ClinicalTrials, CNKI, VIP, Wanfang database and CBM, clinical randomized controlled trials (RCTs) about Xianling gubao capsules combined with placebo and routine treatment (trial group) versus placebo and routine treatment or Xianling gubao capsules (control group) alone in the treatment of PMOP were collected. After literature screening and data extraction, the quality of included literatures were evaluated with Cochrane system evaluator manual 5.3 recommend bias risk evaluation tool. Bayesian Network Meta-analysis was performed with Stata 14.0 software and Bayesian Markov Chain Monte Carlo. RESULTS: A total of 16 RCTs were included, involving a total of 1 360 patients and 3 intervention measures as Xianling gubao capsules alone, Xianling gubao capsules combined with routine treatment, routine treatment. Results of Meta-analysis showed that in respect of improving total response rate, compared with routine treatment, Xianling gubao capsules combined with routine treatment [OR=0.28,95%CI(0.12, 0.64),P<0.05]could significantly improve total response rate; network Meta-analysis ranking showed that Xianling gubao capsules combined with routine treatment>Xianling gubao capsules alone>routine treatment. In terms of increasing bone mineral density (BMD), compared with routine treatment, Xianling gubao capsules combined with routine treatment [OR=0.45,95%CI(0.24, 0.84),P<0.05] and Xianling gubao capsules alone [OR=0.78, 95%CI(0.32, 0.84),P<0.05] could significantly improve BMD; network Meta-analysis ranking showed that Xianling gubao capsules combined with routine treatment>Xianling gubao capsules alone>routine treatment. In the term of improving serum calcium level, compared with Xianling gubao capsules alone [OR=4.76,95%CI(2.14, 10.59),P<0.05] and routine treatment [OR=0.45, 95%CI(0.21, 0.99),P<0.05], Xianling gubao capsules combined with routine treatment could significantly improve serum calcium level; network Meta-analysis ranking showed that Xianling gubao capsules combined with routine treatment>routine treatment>Xianling gubao capsules alone. In the term of improving serum phosphorus level, compared with Xianling gubao capsules alone, Xianling gubao capsules combined with routine treatment [OR=2.85,95%CI(1.81, 4.48),P<0.05] and routine treatment [OR=2.93,95%CI(1.76, 4.86),P<0.05] could significantly improve serum phosphorus level; network Meta-analysis ranking showed that routine treatment>Xianling gubao capsules combined with routine treatment>Xianling gubao capsules alone. CONCLUSIONS: Xianling gubao capsules combined with routine treatment can significantly improve therapeutic efficacy of PP patients, increase BMD and serum calcium level after treatment, but routine treatment is the best in improving serum phosphorous level.
10. Effect of radiotherapy for primary orbital lymphoma: analyses of 28 cases

Objective:
To analyze the effect and adverse reactions of radiotherapy in patients with primary orbital lymphoma.
Methods:
A total of 28 patients with primary orbital lymphoma from Tianjin Union Medical Center, the First Central Hospital of Tianjin and Tianjin Dagang Oil Company Hospital between March 2006 and August 2012 were retrospectively analyzed. All the patients received orbital tumor dissection or biopsy, then received radiation therapy. Three patients received chemotherapy with CHOP protocol before radiotherapy. Radiotherapy was delivered routinely with 3D-conformal radiation therapy (CRT) technique with daily 2-2.5 Gy for 5 times per week and 3-5 fields. Total dosage was 20-50 Gy. There were 18 cases of 30 Gy, 1 case of 22 Gy, 1 case of 25 Gy, 1 case of 20 Gy, 1 case of 28.8 Gy, 3 cases of 40 Gy, 1 case of 42 Gy, 1 case of 46 Gy and 1 case of 50 Gy.
Results:
All the patients had complete remission (CR) after radiotherapy during follow-up. One patient recurred after radiotherapy of 4 months and received CR with radiotherapy again. One patient died of lung infiltration. Acute complications during radiotherapy were conjunctivitis (28 cases, 100.0%) and keratitis (1 case, 3.6%), long-term complications with ophthalmoxerosis (10 cases, 35.7%) and decreased visual acuity (1 case, 3.6%).
Conclusion
Radiotherapy is an effective method for primary orbital lymphoma, and the adverse reactions can be tolerated for most patients.
