1.Clinical application of acellular biological dressing in burn wound: Outcomes of 106 patients with 150 wounds

OBJECTIVE: To evaluate the application of different types of wound dressing acellular biological effect. METHODS: The acellular biological dressing was used in the 106 cases of 150 burn wounds. There were 84 donor site wounds, 30 Ⅱ degree superficial and 16 deep Ⅱ degree of fresh wounds, 10 Ⅲ degree burn wounds, 5 surgical wounds in.the lack of space in autologous skin grafting, and 5 latter part of the small residual wounds. Acellular biological dressing with the covering of dry gauze bandage was used immediately after the surgery to take skin, debridement, and derrnoplasty clearance separately. RESULTS: Among 150 wounds, 4 non-adherent wounds (2.67%) were Ⅲ degree wounds. Infected 2 wounds (1.3%) were latter part of the small residual wounds. Significant accumulation of exudation required 4 (2.67%) deep Ⅱ degree fresh wounds. Other wounds presented good adherence, no infection, less capillary hemorrhage, and significant relief of pain.CONCLUSION: The acellular biological dressing for skin graft donor site wounds, superficial and deep Ⅱ degree fresh wounds, wounds with autologous skin grafting gap results were satisfactory.

3.Phosphorylation of p38MAPK in hyperoxia-induced lung injury of new-born rats

Objective To investigate the expression and significance of p38 mitogen-activated protein kinase (p38MAPK) in hyperoxia-induced lung injury of new-born rats. Methods Totally 160 rats at the age of 12 h were randomly and equally divided into air control group, hyperoxia-induced lung injury group, hyperoxia-induced lung injury+SB203580 group and hyperoxia-induced lung injury + normal saline group. After the injury was inflicted, SB203580 or normal saline at same volume was given intraperitoneally at 5 mg/kg. After the rats were executed at the time points of 12, 24, 72 h and 1 week after the model establishment, the right upper lungs were resected for histopathology, right below lungs for wet weight/dry weight, and left lungs for detecting the expression of p38MAPK by Western blot analysis. Results Hyperoxia-induced lung injury model were established successfully after 72 h by exposure to hyperoxia. No p38MAPK expression was observed in air control group. In hyperoxia-induced lung injury group p38MAPK was detected from 12 h, reached to the peak at 72 h, and decreased 1 week later, and its expression was significantly higher in this group and normal saline group than in SB203580 group. Conclusion p38MAPK is involved in the process of hyperoxia-induced lung injury, and the injury can be relieved by treatment of SB203580.
4.THE INFLUENCE OF BLEEDING TENDENCY ON EMERGENT MANAGEMENT FOR PATIENTS WITH POSTRENAL RENAL FAILURE

In order to investigate the influence of bleeding tendency on emergent treatment of postrenal renal failure patients, we retrospectively studied the methods and complications of emergent treatment in 86 renal failure patients induced by postrenal obstruction hospitalized from March 1988 to December 2000. Fourteen patients were treated by emergent open operation, in whom one died after operation because of severe bleeding,and 3 with total bleeding volume more than 1000ml. Thirty nine patients were treated with percutaneous nephrostomy, in whom 2 died after operation and 3 had to undergo mephrectomy.32 patients were treated with retrograde indwelling ureteral stent and one with transureteroscopic lithotomy (TUL),without the occurrence of complications. Emergent open operation and percutaneous nephrostomy for renal failure patients induced by postrenal obstruction may induce uncontrolled renal bleeding,resulting in patient death or kidney loss because of bleeding tendency.The retrograde ureteral catheterization for drainge is recommended as the first option for emergent treatment of renal failure patient induced by postrenal obstruction.
5.Analysis on the results of liquid-based thinprep cytologic test in 9 012 patients

Objective To investigate the application value of liquid-based thinprep cytologic test ( TCT) in diagnosis of cervical cancer and precancerous lesion. Methods The results of liquid-based thinprep cytologic test and TBS cytology subdivision from 9 012 patients were retrospectively analyzed, including 12 cases of ASC-US, 10 cases of ASC-H, 35 cases of LSIL, 72 cases of HSIL, 2 cases of SCC, and 6 ca-ses of gland cell abnormal. The abnormal results and the results of biopsy pathological diagnosis underwent colposcopy were compared. Results The liquid-based thinprepcytologic test showed that there were 285 cases of atypical squamous cells of undetermined significance (ASC-US), the positive rate was 3. 2%. The accordance rate of liquid-based thinprep cytologic test and colposcope histopathological diagnosis was 91. 24%. Conclusion TCT and TBS system can accurately reflect cervical intraepithelial lesion. Early detection and treatment of precancer-ous lesions is the key point of preventing cervical cancer.
6.Characteristics and treatment strategy of fresh high-energy fracture dislocation of tarsometatarsal joints

[Objective]To investigate the characteristics and treatment strategy for fresh high energy fracture dislocation of tarsometatarsal joints in a retrospective study.[Method]From September 2002 to April 2008,43 patients with fresh high energy fracture dislocation of tarsometatarsal joints were treated with cannulated screw combined with Kirschner wire fixation or Kirschner wire fixation alone.There were 35 males and 8 females,with a mean age of 37.5 years(range,18 to 55 years).The injury was caused by a road accident in 15 patients,by a heavy crash in 21 patients,and by a fall from a height in 7 patients.According to the Myerson damage typing,there were 9 cases of type A,2 cases of type of B1,25 cases of type B2,4 cases of type CI and 3 cases of type C2.Eight were open injuries(6 of Gustilo-Anderson type I and 2 of type Ⅱ)and 35 closed injuries.Internal fixation were undertaken at an average of 4.6 days after injury(range,2.5 hours to 21 days).At 4-6 weeks all the patients began functional exercises after removal of the plaster casts.At 16-18 weeks all the patients began to walk with partial weigh loading after removal of the internal fixators.[Result]All the 43 patients were followed up for 26 months on average(range,13 to 38 months).No infection,breakage,loosening of internal fixator,nonunion or longitudinal arch collapse was found.According to ZHOU'S assay standard.excellent result was achieved in 26 cases,good in 14 cases,and fair in 3 cases,with excellent to good results of 95.3%.Six patients had post-traumatic arthritis,which was complicated with injured articular surface.[Conclusion]Fresh high-energy fracture dislocation of tarsometatarsal joints can be diagnozed correctly.Early diagnosis and correct classification,proper operation timing,anatomical reduction,rational internal and external fixation and timely functional training are essential for patients to achieve good clinical result.
7.Blood lead level and related factors in children from 2 kindergartens in Chongqing

Objective To investigate the blood lead level and its related factors in children in Chongqing and to provide evidence for the prevention and treatment of lead contamination. Methods In June 2001, the blood lead levels in 173 children(aged 2~7 2 years old) selected from 2 kindergartens representing the children in urban district and suburb were detected. At the same time, the related factors were investigated by using questionnaires. Results The highest, the lowest and the mean blood lead levels were 270 ?g/L, 18 ?g/L and 77 73 ?g/L, respectively. The blood lead level of 23 70% children investigated surpassed 100 ?g/L(the international diagnostic criterion of lead poisoning in children). Significant regional difference in blood lead level was found. The mean blood lead levels in children in Yuzhong District and Dadukou District were 111 23 ?g/L and 57 57 ?g/L, respectively. The percentages of children in the two districts with blood lead levels higher than 100 ?g/L were 52 31% and 6 48%, respectively. Blood lead level was closely correlated with the dwelling condition, the dwelling location where factories were nearby or the number of factories was large, health habits, the frequency of eating canned food and popcorn, and culture levels of the mothers. Conclusion Blood lead level of children in the urban distict is significantly higher than that in the suburb, to which more attention should be paid.
8.STUDY ON ANTI-MICROBIAL ACTIVITY OF GENISTEIN AND ITS MECHANISM

Objective To investigate the anti-microbial activity of genistein and its mechanism. Method The anti-microbial experiment was carried out by utilizing scanning and transmission electron microscope(SEM and TEM) ,and further analyzing the respiratory metabolism and change of SDS-PAGE protein spectra. Results Genistein could inhibit several kinds of bacteria obviously.The erose structures such as rugae and bubbles were observed on the surface of cells by SEM after 24h. Moreover,with TEM,we detected the shrinkage of cytoplasm,the plasmolyses,and then the breach of wall and membrane along with the outflow of protoplasm in Staphylococcus aureus treated with genistein for 4h,14h and 24h respectively. Notablely the respiratory inhibition experiment revealed that the genistein mainly inhibits the TCA cycle of bacteria. Besides,the SDS-PAGE elucidated that the total expression of proteins was decreased in the cell treated with genistein,and especially the larger proteins were reduced with 90.1%. Conclusion Genistein showed obvious anti-microbial activity to Staphylococcus aureus. It could destroy the integrity of cell wall and membrane,prevent the respiratory metabolism and protein synthesis of the bacteria.

10.Literature review on treatment of type 2 diabetic foot cases with umbilical cord blood mesenchymal stem cell transplantation

Objective To observe the clinical effect and safety of umbilical cord blood mesenchymal stem cell(UCB-MSC) transplantation in the treatment of diabetic foot. Methods UCB-MSC suspension (cell concentration (3 -7) × 107/L,0.3 -0.5 ml per point) was injected into multiple spots on affected lower limb with a 3 cm × 3 cm istance among each point. Demixing injection could be performed in regions with multilayer muscles. Clinical symptoms and related index were routinely observed from the first day to three months after operation. Results After three months of UCB-MSC transplantation, pain of patients was relieved, skin temperature increased, intermittent claudication ameliorated, ulcer healed, ankle-brachial index and transcutaneous partial pressure of oxygen increased. The lower extremity lesions showed an abundant collateral vessel formation after the treatment in 2 patients by angiography. Both patients had no severe complications and adverse reactions, none underwent amputation. Conclusions Umbilical cord blood mesenchymal stem cell transplantation is a safe and effective treatment of diabetic lower limb ischemic disease, which can exempt the patient from amputation and improve their quality of life.
